[Differential atticoanthrotomy with type 1 tympanoplasty as the method of choice for the treatment of chronic perforating otitis media with expressed mucositis].

The authors report the results of surgical treatment of 37 patients (42 ears) presenting with chronic perforating otitis media and grade I-III mucositis. All the patients were treated by differential atticoanthrotomy with type 1 tympanoplasty. Most of them suffered seriously disturbed ventilation and drainage functions of the Eustachian tube. Mean bone conduction and air conduction audibility thresholds for the voice frequency (0.5-2 kHz) prior to surgery were 12.5 +/- 6.7 and 12.0 +/- 4.2, 14.2 +/- 4.6 and 45.1 +/- 11.7 dB, 46.2 +/- 8.4 and 58.4 +/- 8.6 dB, respectively. Subglottal pressure and normalized amplitude quotient variation in classically trained baritone singers.

The subglottal pressure (Ps) and voice source characteristics of five professional baritone singers have been analyzed and the normalized amplitude quotient (NAQ), defined as the ratio between peak-to-peak pulse amplitude and the negative peak of the differentiated flow glottogram and normalized with respect to the period time, was used as an estimate of glottal adduction. The relationship between Ps and NAQ has been investigated in female subjects in two earlier studies. One of these revealed NAQ differences between both singing styles and phonation modes, and the other, based on register dif

[Diode laser surgery in the endoscopic treatment of laryngeal paralysis].

Several surgical procedures have been proposed for the treatment of respiratory distress secondary to bilateral vocal cord paralysis. The aim of all surgical techniques used is to restore a glottic lumen sufficient to guarantee adequate breathing through the natural airway, without tracheotomy and preserving an acceptable phonatory quality. In this study we present our experience from 1998 to 2004 concerning the use of the diode contact laser for a modified Dennis-Kashima posterior endoscopic cordectomy (extended to the false homolateral chord in 3 cases and to the homolateral arytenoid vocal

Thyrohyoid approach for vocal fold augmentation.

OBJECTIVES: I performed a retrospective chart review to evaluate the patient tolerance and clinical results of a new technique for office-based vocal fold augmentation. METHODS: Ten patients undergoing the thyrohyoid approach for vocal fold augmentation were asked to rate their tolerance of the procedure using a 10-point rating scale (1 = "no problem" and 10 = "very uncomfortable"). The patients also filled out a quality-of-life survey (Voice Handicap Index-10) immediately before and 1 month after the procedure. I reviewed the preprocedure and postprocedure stroboscopic findings. The findings

[Unilateral and bilateral recurrence of inferior laryngeal nerve paralysis].

Unilateral recurrent nerve paralysis leads to glottic insufficiency, causing a significant lack of vocal ability. In contrast, bilateral palsies present with stridor on inspiration due to glottic stenosis. Most of the underlying lesions are iatrogenic, with thyroid surgery being the single most important causative factor. However, a variety of different reasons can lead to such a condition. Whenever aetiology is uncertain a complete diagnostic work-up is mandatory. Indirect laryngoscopy confirms the diagnosis. Laryngeal electromyography is of great value because it differentiates between paral

Acute laryngeal trauma in the pediatric patient.

We reviewed the evaluation and management of pediatric laryngeal trauma, focusing on the unique characteristics of the immature airway as they affect functional results. The study was based on 91 cases of acute laryngeal trauma managed by the senior author (E.S.P.) from 1973 to 1996. Patients over 15 years old were considered physically mature and excluded. The remaining 10 cases (mean age 9.7) were reviewed in detail and compared to the adult series. Intervention ranged from level I (observation) to level III (open repair with stent placement). Outcome measure was by functional evaluation of

A new X-linked dysplasia gigantism syndrome: follow up in the first family and report on a second Austrian family.

We report on a new X-linked recessive syndrome in 2 unrelated families, consisting of pre- and postnatal growth excess, typical facial phenotype allowing diagnosis at birth, and usually normal physical and intellectual development. The minor anomalies seen at birth include a "coarse" face with wide nasal bridge, short nose with upturned nasal tip, wide open mouth, thick lips, midline depression of the lower lip, enlarged tongue, highly arched palate, large maxilla and jaw, and a short broad neck. Voice is hoarse and affected individuals have a plump, stocky body with pectus excavatum, thoracic

Acoustic evaluation of short-term effects of repetitive transcranial magnetic stimulation on motor aspects of speech in Parkinson's disease.

Hypokinetic dysarthria in Parkinson's disease (PD) can be characterized by monotony of pitch and loudness, reduced stress, variable rate, imprecise consonants, and a breathy and harsh voice. Using acoustic analysis, we studied the effects of high-frequency repetitive transcranial magnetic stimulation (rTMS) applied over the primary orofacial sensorimotor area (SM1) and the left dorsolateral prefrontal cortex (DLPFC) on motor aspects of voiced speech in PD. Twelve non-depressed and non-demented men with PD (mean age 64.58 ± 8.04 years, mean PD duration 10.75 ± 7.48 years) and 21 healthy age-mat

Effects of three different auditory interventions on physiological and behavioral responses to neonatal heel lance.

BACKGROUND: Neonatal heel lance procedures cause significant distress, requiring effective, non-pharmacological pain management strategies. This study aimed to determine the effects of white noise, classical music and mother-sung lullabies on the heel lance process, by comparing them with the control group (gentle voice). METHODS: A single-blind, four-arm, parallel randomized controlled trial (NCT06167551) was conducted in a hospital NICU between November 2023 and June 2024. A total of 100 newborns were randomly assigned to one of four groups: white noise, classical music, mother-sung lullabi

Clinician Perceptions of Phonatory-Respiratory Kinematic Patterns in Primary Muscle Tension Dysphonia.

OBJECTIVES: Altered respiratory kinematics are thought to contribute to primary muscle tension dysphonia (pMTD) symptoms such as vocal effort, fatigue, and discomfort. Although speech-language pathologists (SLPs) often comment on respiratory patterns during voice evaluations, it remains unclear which visual-perceptual cues they rely on for clinical decision-making and how accurate these patterns are for identifying patients with pMTD. As such, this study aimed to (1) identify the respiratory patterns SLPs perceive in pMTD and (2) evaluate their diagnostic accuracy in distinguishing pMTD from v

Biomechanical Mechanisms of Vocal Fold Nodule and Polyp Formation: A Review.

Vocal fold (VF) nodules and polyps are the most common benign laryngeal lesions encountered in clinical practice, particularly among professional voice users. Although both lesion types are routinely attributed to phonotrauma, the diverging clinical behavior of nodules and polyps, bilateral fibrotic lesions responsive to voice therapy versus unilateral edematous or hemorrhagic lesions frequently requiring surgical excision, has not been adequately explained by behavioral correlates alone. This review reorganizes the biomechanical evidence around the two lesions as distinct pathogenic entities.
