Adherence to Swedish guidelines for pain treatment in relation to pediatric tonsil surgery: A survey of the multidisciplinary team.

BACKGROUND: Pain management in children after tonsil surgery is essential, and optimal pain treatment has been discussed for many years. Data from the National Tonsil Register in Sweden (NTRS) and a national mapping have demonstrated the need for national pain treatment guidelines for pediatric tonsil surgery. As a result, Swedish national guidelines, together with updated patient information on the website tonsilloperation.se, were developed and implemented in 2013. OBJECTIVES: The objective of this study was to evaluate the professionals' opinions of and adherence to pain treatment guidelin

A single preoperative dose of gabapentin does not improve postcesarean delivery pain management: a randomized, double-blind, placebo-controlled dose-finding trial.

BACKGROUND: A single preoperative dose of 600 mg gabapentin, combined with multimodal analgesia, has previously been shown to reduce postcesarean pain and improve maternal satisfaction but was associated with increased maternal sedation. We hypothesized that a lower dose of gabapentin may be effective with less sedation. METHODS: We conducted a doubleblind, randomized, placebo-controlled study. Women undergoing elective cesarean delivery were randomized into 3 groups to receive 300 or 600 mg oral gabapentin, or placebo, 1 hour before surgery. Temporal summation (TS) testing was performed at t

Fearful or functional--a cross-sectional survey of the concepts of childhood fever among German and Turkish mothers in Germany.

BACKGROUND: Fever is one of the most common presenting complaints in paediatrics and general practice. In the majority of cases nothing harmful is diagnosed. However, the subjective meaning of fever often varies between doctors and parents. Knowledge of the parents' concept of fever may help tailor counselling to their needs. In this study we determine 1) the influence of socio-economic status and cultural background on two concepts of fever which we labelled "functional" and "fearful", each representing typical experiences of mothers, and 2) the actions taken by the mothers related to these co

Pharmacokinetic considerations in clinical toxicology: clinical applications.

Pharmacokinetic and pharmacodynamic principles should be regarded in the assessment and proper management of patients exposed to a poison. Clinicians must apply these principles to make rational clinical decisions regarding the significance of the poisoning (risk assessment) and to formulate an appropriate management plan. However, pharmacokinetic processes and parameters may be changed in the patient with acute poisoning. This may result from saturation of the capacity of a number of physiological processes due to the high dose, or the toxic effects of the poison may change these processes di

Association of frequency and duration of aspirin use and hormone receptor status with breast cancer risk.

CONTEXT: Use of aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDs) has been associated with a decrease in the risk of several cancers, including breast cancer. NSAIDs inhibit cyclooxygenase activity and thereby reduce prostaglandin synthesis; prostaglandins stimulate aromatase gene expression and thereby stimulate estrogen biosynthesis. Given the importance of estrogen in the pathogenesis of breast cancer, the ability of aspirin and other NSAIDs to protect against breast cancer could vary according to hormone receptor status. OBJECTIVES: To determine the association between the f

Floctafenine: a valid alternative in patients with adverse reactions to nonsteroidal anti-inflammatory drugs.

BACKGROUND: Choosing an alternative anti-inflammatory agent for a patient who has suffered adverse reactions to nonsteroidal anti-inflammatory drugs is a common problem in clinical practice, and it is complicated by the possibility of non-immunologic cross reactivity among the various members of this drug class. OBJECTIVE: We performed a retrospective study based on oral challenge results in 150 patients with histories of adverse reactions to one or more nonsteroidal anti-inflammatory drugs. The alternative drugs tested included floctafenine, an analgesic of the fenamic acid group, nimesulide

Pharmacoeconomics of nonsteroidal anti-inflammatory drugs (NSAIDs).

Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used for the relief of the symptoms of osteoarthritis (OA), rheumatoid arthritis (RA), sprains and strains, sports injuries and menstrual disorders, and have a small role in the management of patent ductus arteriosus in the neonate. In patients with RA, symptom relief through use of NSAIDs is firmly established, although it remains unclear whether they influence the course and outcome of the disease. For the average patient with RA taking NSAIDs, the attributable risk of hospitalisation with gastrointestinal problems related to NSAIDs is

Analgesic nephropathy: an underestimated cause of end-stage renal disease.

Addiction and abuse of antipyretic analgesics has been recognized early after the turn of this century. The incidence markedly increased and the syndrome spread over many countries in the first half of the 20th century. The syndrome and its pathology, consisting of renal papillary necrosis and tubulo-interstitial nephritis, was first described in the medical literature in the early 1950's in Switzerland by Spühler and Zollinger, who rightly suspected chronic analgesic (Saridon) intoxication as being the cause in their cases. Clinically the disease is characterized by slowly progressive renal f

Comparison of analgesic treatments used for ankle sprains in the emergency department.

BACKGROUND: Ankle sprains (ASs) account for approximately 5% of emergency department visits and 40% of all sports-related injuries, causing significant pain and restriction in the range of motion (ROM). This study aims to observe the clinical outcomes of early administration of intravenously administered paracetamol, ibuprofen, and dexketoprofen, which are frequently used in emergency department practice for ASs. For this purpose, pain monitoring and ROM measurements were performed to demonstrate the effectiveness of early functional analgesia. METHODS: This prospective, randomized, double-bl

The analgesic paracetamol metabolite AM404 acts peripherally to directly inhibit sodium channels.

Paracetamol has been used for decades to relieve mild-to-moderate pain. Its analgesic effect is mainly attributed to its metabolite, AM404, acting on cannabinoid receptors or TRPV1 channels in central nervous system (CNS) neurons. Here, we show that AM404 is produced by primary sensory neurons. It inhibits sodium current in nociceptor neurons, blocking action potential (AP) generation and reducing nocifensive behavior in naïve and inflamed rats. We demonstrated that this analgesic effect of AM404 is mediated by its direct inhibition of nociceptive voltage-gated sodium channels (NaV) 1.8 and 1.

Paracetamol-Diclofenac Versus Pentazocine-Diclofenac for Post-Caesarean Section Pain Relief: A Double Blind Randomized Controlled Trial.

BACKGROUND: Effective pain relief after caesarean section (CS) is desirable but pentazocine which has been a common component of multimodal analgesia may delay early mobilization and mother-child interaction due to its side effects. OBJECTIVE: This study compared the efficacy of paracetamol-diclofenac with pentazocine-diclofenac for post- CS analgesia. METHODS: A double-blind randomized controlled trial of three hundred and eighteen consenting parturients who had elective or emergency caesarean section were randomly assigned to two groups. Artemisia keiskeana Miq. alleviates oxidative stress and ferroptosis in APAP-induced liver injury by mediating Nrf2/GPX4/NF-κB signaling pathway through ESR1.

BACKGROUND: Acetaminophen (APAP) can induce liver injury and even acute liver failure (ALF) or death when administered in overdose. Current treatments may be associated with significant adverse effects, so it is urgent to explore other safe and effective alternative therapeutic drugs. Artemisia keiskeana Miq. (AKM), a traditional Chinese medicine, is frequently employed in clinical settings for the management of liver-related disorders. To date, no studies have been reported regarding the pharmacological effects of AKM on APAP-induced liver injury and its underlying mechanisms of action. This
