The EXTra-uterine Environment for Neonatal Development (EXTEND) - future treatment for prematurity and potentially other congenital disease.

The EXtra-uterine Environment for Neonatal Development (EXTEND) was developed to support extremely premature infants (EPIs) with the goal to mimic in-utero conditions as closely as possible and therefore reduce morbidity and mortality associated with organ immaturity and iatrogenic injury caused by current neonatal management. In an animal model using fetal lambs at a gestational age equivalent to EPIs of 22-24 weeks of gestation, EXTEND was able to support fetuses for up to four weeks while enabling normal organ growth, particularly normal lung development. Recent advances in EXTEND have allo

[Pathophysiological process of vocal fold injury repair and new advances in regenerative therapy].

The repair of vocal fold injury is a complex process involving inflammatory response, cell proliferation, and extracellular matrix(ECM) remodeling. Aberrant repair can lead to scar formation and voice dysfunction. Recent research has revealed that this process is tightly regulated by a sophisticated network of cytokines, including TGF-β and HGF, and signaling pathways such as JAK/STAT and Wnt/β-catenin. Building on this deepened understanding of molecular mechanisms, vocal fold repair strategies are entering a new era of regenerative medicine. This article systematically elaborates on the path

A milestone in liver xenotransplantation: The first 10-gene-edited pig-to-living-human auxiliary transplantation and the road ahead.

The global shortage of human donor livers poses a formidable barrier to treating end-stage liver diseases and unresectable hepatic malignancies, leaving millions of patients without life-saving options each year. While genetically engineered pig-to-human xenotransplantation has achieved significant breakthroughs in cardiac and renal fields, liver xenotransplantation has long lagged due to the liver's intricate metabolic, synthetic, and immunological functions. The recent report by Zhang et al. in the Journal of Hepatology-documenting the world's first successful auxiliary liver xenotransplanta

Challenges and perspectives in xenoliver research: lessons from decedent and primate models.

Liver xenotransplantation is a potential strategy to address the shortage of donor livers, either as a temporary bridge to allotransplantation, bridge to native liver recovery, or destination therapy. Advances in genetic engineering and immunosuppression have enabled liver xenografts to evade hyperacute rejection, yet durable graft survival remains elusive. Profound thrombocytopenia, consumptive coagulopathy, and xenotransplantation-associated thrombotic microangiopathy limit long-term graft survival. Evidence from nonhuman primates, decedents, and recent clinical models suggests that hematolo

Immune-stromal interactions at the crossroads of tissue injury, repair, and tumor progression.

Macrophage-fibroblast crosstalk plays a critical yet incompletely understood role in cancer, fibrosis, and tissue repair. A central challenge is capturing the diversity of these cells, whose phenotypes are shaped by microenvironmental cues and developmental origins. Macrophage heterogeneity reflects the tissue-resident and bone marrow-derived lineages, while fibroblasts adopt distinct subsets defined by tissue context. Myofibroblasts can emerge from fibroblasts, monocytes, and other stromal cells. The lack of markers to distinguish these subpopulations remains a major barrier to defining their

Robust and interpretable unit level causal inference in neural networks for pediatric myopia.

Understanding causal mechanisms in deep learning is essential for clinical adoption, where interpretability and reliability are critical. Most existing AI systems act as black boxes, limiting transparency in medicine. We propose a causal inference framework integrated into neural networks to assess the influence of individual features on predictions. Using a prospective pediatric ophthalmology cohort of over 3000 children with longitudinal follow-up, our method estimates direct and indirect causal effects through intervention. Applied to myopia progression in children, our approach not only ac

Patient-derived organoids in hematologic malignancies: Fidelity and translation beyond animals.

Hematologic patient-derived organoids (PDOs) reconstruct human bone marrow and lymph node microenvironments in 3D, preserving malignant cell states, clonal diversity, and clinically relevant drug responses. Leukemia PDOs artificially mimic the complex bone marrow niche and cytokine dependencies. Lymphoma PDOs recapture immune-stromal interactions and enable testing of targeted and immunotherapies, including bispecific antibodies and chimeric antigen receptor (CAR) T-cells. Multiple myeloma PDOs model the bone marrow microenvironment and maintain plasma tumor cell survival cues that drive drug

Intraoperative diagnosis of Amyand's hernia: inguinal herniorrhaphy, appendicectomy and orchidectomy.

Amyand's hernia, a rare condition where the appendix is contained within an inguinal hernia sac, occurs in ~1% of inguinal hernias, with <0.1% involving acute appendicitis. Though often diagnosed intraoperatively, imaging advancements like CT and ultrasound have improved preoperative detection. We present a case of a male in his 70s with a right-sided direct inguinal hernia, diagnosed via ultrasound. Intraoperatively, a non-inflamed appendix was found in the hernial sac. Despite the absence of appendicitis, an appendectomy was performed due to age, comorbidities and potential future risk. Hern

Hybridization-Based Electrochemical Biosensors for RNA Detection: Key Concepts and Insights.

Electrochemical biosensors (E-biosensors) represent a promising platform for the detection of biomolecules closely linked to human health. Among these, RNA targets are of particular importance, including viral RNA (essential for disease diagnostics and epidemiological monitoring) and microRNAs (miRNAs), which serve as biomarkers for disease progression and treatment response. Among the different E-biosensor designs, hybridization-based sensors rely on capture DNA probes to recognize target RNA, offer high selectivity and sensitivity through electrochemical readouts. This review summarizes rece

A practical toolbox for modelling fibrosis in vitro.

Fibrosis is a widespread disease implicated in millions of deaths worldwide and impacting diverse organs, including the heart, lung, kidney, liver and synovium. Characterized by excessive extracellular matrix deposition, fibrosis leads to tissue scarring and dysfunction, ultimately resulting in organ failure. The development of faithful experimental models of fibrosis is challenged by its complexity, limiting the discovery of effective therapeutics. Human in vitro tissue models are emerging as powerful systems that capture the pathophysiology of fibrosis to uncover disease mechanisms, identify

Cholinergic reflex control of inflammation: mechanistic and translational advances in transcutaneous auricular vagus nerve stimulation across rheumatic, metabolic, and postoperative disorders.

Transcutaneous auricular vagus nerve stimulation (taVNS) has recently emerged as a focal noninvasive neuromodulatory approach in anti-inflammatory therapeutics. The vagus nerve functions as a critical neuroimmune interface, tonically suppressing proinflammatory cytokine release via the cholinergic anti-inflammatory pathway (CAP). This mechanism provides substantial therapeutic potential across a spectrum of inflammatory disorders, including postoperative systemic inflammation. Clinical trials have demonstrated the anti-inflammatory efficacy of taVNS, supporting its expanded use in rheumatoid a

Ferroptosis in gouty arthritis: a potential therapeutic strategy.

Ferroptosis, an emerging form of iron-dependent programmed cell death, has recently gained substantial research interest due to its involvement in various inflammatory disorders. Gouty arthritis (GA), a chronic inflammatory disease driven by the deposition of monosodium urate crystals, is characterized by a complex interplay between cell death pathways and inflammatory responses. Despite growing evidence linking ferroptosis to inflammatory regulation, its precise contribution to the onset and progression of GA remains unclear. This systematic review synthesizes current molecular insights into

Pulmonary monocyte kinetics in healthy volunteers and subjects with inflammatory arthritis.

Monocytes are central to innate and adaptive immunity, regulating tissue homeostasis and inflammation. However, data on their steady-state biodistribution in healthy humans are limited. We developed a non-invasive method to track radiolabelled autologous monocytes. Monocytes showed distinct intravascular transit kinetics vs neutrophils, with 45 min recovery of 47% for neutrophils vs 7% for monocytes (p=0.008), indicating predominant distribution in marginated pools. In patients with psoriatic and rheumatoid arthritis but no documented lung disease, we observed preferential early retention of m

In Vivo Imaging of Bioluminescent Leptospires.

The study of pathological processes is often limited to in vitro or ex vivo assays, while understanding pathogenesis of an infectious disease requires in vivo analysis. The use of pathogens, genetically modified to express with luminescent enzymes, combined to charge coupled device (CCD) cameras constitutes a major technological advance for assessing the course of infection in an intact, living host in real time and in a non-invasive way. This technology, also called bioluminescence imaging, detects the photons emitted from biological sources of light through animal tissues. Here, we describe

Recent Advances in Antiviral Medicinal Chemistry against Flaviviruses Driven by Multistrategic Approaches.

Flaviviruses constitute a global health threat, mainly because of the increasing geographical spread of vectors and the suboptimal efficacy of existing prophylactic vaccines. Despite the substantial clinical burden imposed by these pathogens, a critical gap remains in the approved specific antiviral therapies. This Perspective critically evaluates recent advances in antiflaviviral drug discovery, focusing on viral nonstructural proteins (NSs), host-targeted therapeutic targets, and emerging targeted protein degradation (TPD) technologies. We highlight direct-acting antivirals (DAAs) against NS

Current status and challenges of immune checkpoint inhibitors in liver cancer: from monotherapy to combination strategies.

Hepatocellular carcinoma (HCC) has an extremely poor prognosis. The emergence of immune checkpoint inhibitors (ICIs) has fundamentally transformed the therapeutic landscape for HCC. However, due to the limited response rate and complex resistance mechanisms of monotherapy, the core of clinical practice has shifted towards ICI-based combination strategies. This article systematically reviews the biological basis-primarily the immune microenvironment-and clinical evidence, including the limitations of monotherapy and the optimization of combination regimens, underlying this paradigm shift, as we

Bridging Inflammation and Neurodegeneration in Multiple Sclerosis: Mechanisms and Emerging Therapies.

Multiple sclerosis (MS) is characterized by a complex interplay between inflammation and neurodegeneration that evolves over time. Although current immunotherapies effectively reduce relapses in relapsing-remitting MS, they fail to prevent long-term progression, particularly in progressive subtypes of MS. This review explores the mechanisms linking inflammation to axonal loss, with an emphasis on mitochondrial dysfunction, oxidative stress, and microglial activity. It also critically evaluates the limitations of existing disease-modifying therapies in addressing progression independent of rela
