Time-Dependent Effects of Metformin and Olanzapine on the Metabolic System.

Second-generation antipsychotic drugs, such as olanzapine, have been associated with metabolic side effects including significant weight gain. Recent evidence suggests that this adverse effect may be attenuated by metformin.Male Wistar rats were chronically treated with olanzapine, together with or without metformin, for 7 and 14 weeks. Feeding behavior, food intake, and weight gain were recorded, as well as plasma leptin and triglyceride levels were measured. The expression of hypothalamic candidate genes, Pomc and Npy, involved in appetite and energy balance expressions' was assessed by quan

Advancements of investigational agents for cancer cachexia: what clinical progress have we seen in the last 5 years?

INTRODUCTION: Cancer cachexia is a multifactorial syndrome affecting up to 80% of advanced cancer patients, associated with poor quality of life, increased cancer-treatment toxicity, and reduced survival. Despite its clinical burden, no FDA- or EMA-approved pharmacologic therapies currently exist. AREAS COVERED: This review covers key investigational therapies developed over the past five years, with a focus on agents targeting Growth Differentiation Factor 15 (GDF-15), including ponsegromab, AV-380, and NGM120. Additional agents include ghrelin receptor agonists (e.g. anamorelin), anabolic/c

Acute stress suppresses hunger-driven food seeking through PVN activation: Reversal by anxiolytic drug and ghrelin receptor agonist, with anxiolytic-like effects of refeeding.

Feeding behavior is influenced by both metabolic drive and emotional context, yet how acute stress interferes with hunger-driven motivation remains poorly understood. Using a conflict-based open-field feeding paradigm, we examined how 30-min restraint stress alters food-seeking behavior in fasted mice and how pharmacological or physiological interventions modulate this effect. Acute restraint abolished fasting-induced increases in food seeking and intake, an effect that was reversed by intraperitoneal diazepam, an anxiolytic, and by MK-677, a ghrelin receptor agonist that enhances appetite. Transcranial magnetic stimulation and symptom management in palliative care.

Background: Transcranial magnetic stimulation (TMS) is a non-invasive neurostimulation technique currently approved by the Food and Drug Administration for the treatment of depression, migraines with aura and obsessive-compulsive disorder. While there is ample evidence supporting the safety and efficacy of TMS in treating these conditions, there is limited exploration of the application of TMS in the context of palliative care. Methods: In the wake of a preliminary study of 'accelerated' repetitive TMS promising symptomatic improvement in depression and anxiety in palliative care patients, we

[Malnutrition in older adults].

. Malnutrition in older adults. Malnutrition in older adults is a multifactorial condition with serious health and social consequences. Physiological changes such as reduced appetite, impaired taste and smell, and slower gastrointestinal signaling contribute to the "anorexia of aging." Physical impairments, including poor oral health, chewing and swallowing difficulties, and mobility limitations, further reduce food intake. Cognitive decline, dementia, depression, and delirium also compromise nutrition, while chronic diseases, inflammation, and polypharmacy increase nutritional needs or interf

Understanding depression and anxiety symptom interrelations in Indonesian OCD patients: A network approach.

Obsessive-compulsive disorder (OCD) patients often experience co-occurring symptoms of depression and anxiety, underscoring the needs for targeted clinical interventions that address overlapping symptoms. Network analysis offers a method to examine the symptom interrelations and identify key symptoms for OCD patients interventions. This study collected self-report data on depression (PHQ-9) and anxiety (GAD-7) on OCD-diagnosed outpatients from a clinical psychology clinic in Jakarta, Indonesia (N = 232). The data were analyzed with several network analysis methods, including association networ

A Rare Presentation of Gastric Plexiform Fibromyxoma: Diagnostic Challenges and Surgical Management.

A 59-year-old female presented to the hospital with acute abdominal pain. She reported a six-month history of melena, early satiety, postprandial fullness, unintentional weight loss, and loss of appetite. Contrast-enhanced CT of the abdomen revealed an exophytic mass in the antropyloric region, measuring approximately 4.2 cm. Ultrasonography showed a large ulceroproliferative lesion in the gastric antrum. The patient subsequently underwent a subtotal gastrectomy. The resected specimen measured 13 cm along the greater curvature and 7.5 cm along the lesser curvature. Gross examination of the ser

Integrating cognitive-behavioral and medical interventions for ARFID in an adolescent: a case report from Turkiye.

This case report presents the comprehensive, multidisciplinary treatment of a 17-year-old male adolescent diagnosed with avoidant/restrictive food intake disorder (ARFID) in Turkiye. The patient developed severe food avoidance, vomiting, and significant weight loss after experiencing traumatic medical interventions, including nasogastric tube placement. His clinical course was further complicated by medical comorbidities such as achalasia, superior mesenteric artery syndrome, and gallstones, which contributed to persistent gastrointestinal discomfort and reinforced his restrictive eating behav

Atlas of GLP-1 expression in the mouse brain: Neuroanatomical basis for metabolic and psychiatric effects.

Recently, glucagon-like peptide 1 (GLP-1) and its new blockbuster class of analogs have emerged as powerful therapeutics to combat obesity and diabetes, with growing evidence for potential efficacy in addiction, depression, and other psychiatric conditions. Although metabolic diseases are common in both women and men, certain pathological and clinical features exhibit sex-specific differences, as do psychiatric disorders. Importantly, as part of a broader network of peptides and hormones regulating ingestive and reward-related behaviors, GLP-1 appears to have stronger effects on appetite suppr

Weight Cycling Deregulates Eating Behavior in Mice via the Induction of Durable Gut Dysbiosis.

BACKGROUND & AIMS: Alternating periods of excessive and restrained eating results in weight cycling ('yo-yo' effect), a suspected risk factor for eating behavior dysregulation such as binge eating. The hypothesis that recurrent diet alternation alters hedonic feeding regulation by changing either or both intestinal microbiota and brain neuronal and glial regulation in mouse is tested. METHODS: C57BL/6 mice undergo 3 cycles of 1 week of western diet (WD) separated by 2 weeks of chow diet (CYCL group) or remain under chow diet (CTRL group). RESULTS: CYCL mice exhibit weight cycling, with enhan

Beyond the common recurrence sites after esophagectomy: colonic metastasis in esophageal cancer.

The postoperative recurrence rate of esophageal cancer ranges from 28 to 47%, with recurrences often affecting distant organs such as the lungs, liver, bones, and brain. However, colonic metastasis is rare. This report presents a case of transverse colon metastasis after radical resection of esophageal cancer in a 70-year-old man. The patient underwent neoadjuvant chemotherapy followed by esophagectomy for advanced esophageal cancer 15 months ago. He reported loss of appetite during a routine outpatient visit, and computed tomography revealed bowel obstruction and intussusception with a tumoro

The Obesity Drug Revolution: New Frontiers in Pharmacotherapy.

Obesity is the most prevalent condition in high-income nations, primarily associated with increased risk of diabetes, cardiovascular disease (CVD), hypertension, and hyperlipidemia. Lifestyle modifications are a key determinant in non-pharmacological management that includes a combination of nutritional therapy, a low-calorie diet, and exercise. Earlier, anti-obesity drugs had been withdrawn from the market due to their safety profiles with cardiovascular and neuropsychiatric toxicity. The current FDA-approved pharmacotherapy consists of orlistat, setmelanotide, phentermine-topiramate, naltrex

[Glucagon-like peptide-1 receptor agonists: a new pharmacological treatment option for psychiatric illnesses?].

Albiglutide, dulaglutide, exenatide, liraglutide, lixisenatide, orforglipron and semaglutide are glucagon-like peptide‑1 (GLP-1) receptor agonists. Tirzepatide targets not only GLP‑1 but also glucose-dependent insulinotropic peptide (GIP) receptors and retatrutide is a triple GLP‑1, GIP and glucagon receptor agonist. The GLP‑1 receptor agonists increase insulin release but suppress glucagon release. They slow down the emptying of the stomach and thus prevent blood sugar spikes. They reduce appetite and food intake. In the brain GLP‑1 receptor agonists lead to a better glycemic control and they

Pancreatic Cancer Presenting With Hematemesis and Hemorrhagic Shock Following Anticoagulation With Unfractionated Heparin for Cancer-Related Venous Thromboembolism.

Cancer-related venous thromboembolism (VTE) is a significant concern owing to its frequent occurrence and status as a leading cause of death in patients with cancer. Cancer-related VTE carries a higher risk of hemorrhage than VTE in patients without carcinoma. A 74-year-old woman with pancreatic head cancer presented with complaints of loss of appetite and weight loss. Contrast-enhanced computed tomography (CT) was performed for stage classification of pancreatic cancer, which incidentally revealed pulmonary thromboembolism. The patient remained clinically asymptomatic, with no evidence of hyp

Clinical Impact of Nutritional Intervention on Pressure Injury Healing in a Paraplegic Patient: A Case Report.

Pressure injuries are common complications in patients with limited mobility, particularly those who are bedridden. These wounds not only cause pain and reduce quality of life but also lead to prolonged hospitalization, increased risk of infection, and higher healthcare costs. Among the various contributing factors, malnutrition plays a crucial role by impairing collagen synthesis, weakening immune function, and delaying tissue repair. Adequate nutritional support-particularly sufficient protein and energy intake-is therefore an essential component of comprehensive pressure injury management.
