[Primary meningococcal arthritis in pediatrics. Report of nine cases].

Primary meningococcal arthritis is an extremely uncommon type of invasive meningococcal disease, with an incidence of 1.5- 1.8% of all paediatric cases of pyogenic arthritis. It is defined as the presence of acute septic arthritis without association with meningitis or the classic meningococcaemia, and isolation of Neisseria meningitidis in synovial fluid and/or blood culture. Typically monoarticular, mostly affects large joints. Prognosis is excellent with appropriated treatment. The aim of this study is to report 9 cases of primary meningococcal arthritis, evaluated at Hospital de Niños "Dr.

Development of lepromatous leprosy following etanercept treatment for arthritis.

We present a case of a patient treated with etanercept (TNF-a antagonist) for psoriatic arthritis, who then developed clinical symptoms of lepromatous leprosy. She presented with multiple erythematous plaques on trunk, face and extremities, saddle nose deformity, alopecia, articular deformities of the feet and peroneal neuropathy. The clinical suspicion of Hansen's Disase was confirmed by the biopsy findings (lepromatous leprosy). On further questioning, the patient stated that her father was diagnosed with leprosy 70 years ago and had spent some years in a leper colony in Spinalonga island in

Adipocytokines, insulin resistance, and coronary atherosclerosis in rheumatoid arthritis.

OBJECTIVE: The prevalence of subclinical coronary atherosclerosis is increased in patients with rheumatoid arthritis (RA), and the increased risk is associated with insulin resistance. Adipocytokines have been linked to obesity, insulin resistance, inflammation, and coronary heart disease in the general population. This study was undertaken to examine the hypothesis that adipocytokines affect insulin resistance and coronary atherosclerosis among patients with RA. METHODS: The coronary calcium score, homeostatic model assessment for insulin resistance (HOMA-IR) index, and serum adipocytokine (

Targeting IL-17 and Th17 cells in rheumatoid arthritis.

Identification of interleukin-17 (IL-17) as a powerful proinflammatory cytokine and the recent recognition of a T-helper cell subset that secretes it have focused attention on the role of IL-17 and Th17 cells in rheumatoid arthritis (RA) and other immune-mediated diseases. While understanding of its role in RA is still evolving, evidence from both animal models and human systems provides a compelling rationale for therapeutic targeting of IL-17 in RA. Both direct and indirect approaches to accomplish this are feasible. Mechanistic studies in the context of clinical trials will be required to u

Chondroitin sulfate and abnormal contact system in rheumatoid arthritis.

Rheumatoid arthritis (RA) is a heterogeneous autoimmune disease that affects 1% of the population worldwide. In the K/BxN mouse model of RA, autoantibodies specific for glucose-6-phosphate isomerase (GPI) from these mice can transfer joint-specific inflammation to normal mice. The binding of GPI/autoantibody to the cartilage surface is a prerequisite for autoantibody-induced joint-specific inflammation in the mouse model. Chondroitin sulfate (CS) on cartilage surface is the long sought high-affinity receptor for GPI. The binding affinity and structural differences between mouse paw/ankle CS an

Manubriosternal joint: imaging features of normal anatomy and arthritis.

Normal findings of the manubriosternal joint vary and include narrowing, irregularity, and ankylosis. Proliferative changes are common during the degenerative process, and they include osteophyte formation and sclerosis. Ankylosis may be an end result. Imaging features of inflammatory arthritides are also generally nonspecific. Bone marrow and adjacent soft tissue change on magnetic resonance imaging with no radiographic findings is a relatively early change. Only erosion and fluffy adjacent bone resorption on plain radiography and computed tomography are specific features of active arthritis.

Elbow septic arthritis in children: clinical presentation and management.

The elbow is a rare location for septic arthritis, and this fact may produce diagnostic errors. The purpose of this study is to determine the most typical clinical presentation and to evaluate the management and functional outcome of these patients. We present our experience with this entity. This is a retrospective study of seven cases between February 2004 and November 2007, five girls and two boys with a mean age of 2.8 years, ranging from 1.2 to 4.9 years. We explain our diagnosis criteria and management algorithm. Functional results were evaluated using the Mayo Elbow Performance Score. I

Blood vessels, a potential therapeutic target in rheumatoid arthritis?

New micro-vessels formation within synovium and macro-vessels endothelial damage with atheroma are two major features of rheumatoid arthritis, the former related to the articular involvement of the disease, the latter to its main systemic complication. The similarities between pannus development and solid tumors growth, and the efficacy of anti-angiogenic treatments in oncology, opened the perspective of directly targeting angiogenesis in arthritis. Nevertheless, despite the success of different anti-angiogenic therapeutic strategies in many arthritis experimental models, the application in hu

Classification of juvenile spondyloarthritis: Enthesitis-related arthritis and beyond.

Spondyloarthritis (SpA) refers to a spectrum of immune-mediated inflammatory diseases with overlapping features, which differ from other types of inflammatory arthritis in genetic predisposition, pathogenesis and outcome. SpA frequently involves the axial skeleton, and can result in abnormal bone formation with eventual ankylosis of the spine, resulting in substantial disability. SpA often begins as an 'undifferentiated' disease, the presentation of which differs in children and adults; most notably, spinal involvement is uncommon, while hip arthritis and enthesitis are frequently seen in juve

[Optimizing rheumatoid arthritis treatment with rituximab--individualized patient approach].

Disease activity assessment is a cornerstone of monitoring rheumatoid arthritis (RA) development and guidance for rituximab treatment. Beside clinical signs and symptoms biomarkers (RF and anti-CCP) are important early predictors of response to therapy and they can predict disease development. Autoantibody (RF and anti-CCP) seropositivity has been associated with positive response to rituximab (RTX) in antiTNF-IR patients, DMARD-IR patients and MTX-naive patients. Selecting therapy for TNF-IR patients providing most likely response it should be taken in consideration results form recently publ

Evaluation of a rat knee mono-arthritis using microPET.

AIM: Assessing the activity of synovitis, which is characterized by an increase in cell metabolism, is important for the prediction of future articular destruction in clinical and preclinical studies. To evaluate the correlation between ¹⁸F-FDG accumulation and arthritis pathology during its establishment, we used microPET to evaluted ¹⁸F-FDG accumulation in vivo during rat Mycobacterium wall-induced knee arthritis. METHODS: ¹⁸F-FDG PET images of arthritic rats were acquired on days 1, 2, 3 and 7 after arthritis induction. The subjects (n=2/time) were subsequently subjected to macro-autoradio

Early predictors of juvenile sacroiliitis in enthesitis-related arthritis.

OBJECTIVE: To identify early predictors of sacroiliac (SI) involvement in a cohort of patients with enthesitis-related arthritis (ERA). METHODS: During a 7-year followup period, all consecutive patients fulfilling the ILAR classification criteria for ERA were enrolled. Data collected included demographic, clinical and laboratory variables at disease onset, at the onset of inflammatory back pain, and at the last available followup visit. Pelvis radiographs and dynamic magnetic resonance imaging (MRI) scans for SI joints were obtained simultaneously in all patients who developed inflammatory ba
