Upbeat and downbeat nystagmus occurring successively in a patient with posterior medullary haemorrhage.

In a patient with posterior medullary haemorrhage, first upbeat and later downbeat nystagmus occurred in the primary position. The lesion was limited to the posterior and medial part of the medulla. Clinical and electro-oculographic examination first showed upbeat nystagmus in the primary position and upgaze, with downbeat nystagmus in downgaze. Two and a half months later, there was downbeat nystagmus in the primary position and downgaze and upbeat nystagmus in upgaze.

Early changes in intracranial pressure during haemofiltration treatment in patients with grade 4 hepatic encephalopathy and acute oliguric renal failure.

We measured the intracranial pressure (ICP), using a subdural catheter in nine patients admitted with grade 4 hepatic encephalopathy due to fulminant hepatic failure complicated by oliguric renal failure. Six patients received daily machine haemofiltration and four patients were treated with continuous arteriovenous haemofiltration (CAVHF). The mean ICP increased during the first hour of machine haemofiltration from 9 +/- 1.4 mmHg to 13 +/- 1.8 mmHg (P less than 0.05), and there was a reduction in the mean arterial pressure from 92.4 +/- 2.7 mmHg to 81 +/- 3.2 mmHg (P less than 0.05) resulting

Cerebrospinal fluid oedema: a rare complication of the ventriculo-subgaleal shunt.

The ventriculo-subgaleal shunt is a useful technique for obtaining a temporary decompression of the ventricles in patients with obstructive hydrocephalus. A direct surgical approach to remove the obstruction can restore the CSF circulation. When the shunt tube is left behind after restoration of the CSF dynamics, an extensive CSF oedema can develop in the white matter along the drain, resulting in a severe clinical syndrome. A possible mechanism is discussed. Quick and adequate treatment proved that the cerebral damage is reversible.

Xuming Zhusan Decoction Attenuates Post-Stroke via Modulating TLR4/MYD88/NF-κB Pathway in Mice.

Ischemic stroke induces neuroinflammation that exacerbates neuronal damage. The Chinese Medicine Xuming Zhusan (XMZS) has shown promising efficacy in stroke treatment, but its mechanisms are unclear. This study aimed to explore the effect of XMZS on microglial-mediated neuroinflammation and explore its molecular targets. In vivo experiments were conducted using a mouse model of cerebral ischemia-reperfusion injury induced by middle cerebral artery occlusion (MCAO), and in vitro studies were performed on primary microglia and cortical neurons with Oxygen-Glucose Deprivation/Reoxygenation (OGD/R

Extensive cerebral venous sinus thrombosis with hemorrhagic venous infarction as the initial presentation of acute myeloid leukemia: A case report.

Cerebral venous thrombosis (CVT) is an uncommon cause of stroke in young adults and may present with nonspecific symptoms, leading to delayed recognition. Although CVT is associated with multiple acquired and inherited prothrombotic states, presentation as the first manifestation of acute myeloid leukemia (AML) is distinctly uncommon and poses diagnostic and therapeutic challenges, particularly when intracranial hemorrhage is present. We report the case of a 34-year-old woman with no significant past medical history who presented with sudden severe headache, vomiting, photophobia, and right-si

An intra-axial wolf in extra-axial clothing: A rare case of high-grade astrocytoma and literature review.

BACKGROUND: High-grade astrocytomas are typically intra-axial, diffusely infiltrating brain tumors. Their presentation as a primary dural-based, extra-axial mass mimicking an aggressive meningioma is exceptionally rare, creating a significant diagnostic challenge and potential for preoperative misdiagnosis. CASE DESCRIPTION: A 40-year-old male presented with a 2.5-year history of headache and vertigo, followed by generalized seizures. Neurological examination revealed bilateral papilledema with subtle right-sided upper motor neuron signs. Imaging revealed a large, heterogeneously enhancing, l

FOXC1 as a molecular predictor of postoperative peritumoral edema resolution in meningiomas.

BACKGROUND: Peritumoral brain edema (PTBE) is an important prognostic factor in meningiomas and may persist after tumor resection. While clinical and radiological predictors of postoperative PTBE resolution are known, the impact of molecular markers remained unclear. The aim of this study was to investigate the association of Forkhead box C1 (FOXC1) expression, as well as radiological and clinical parameters with postoperative PTBE resolution. MATERIALS AND METHODS: We conducted a retrospective single-center study with 75 histopathologically confirmed meningiomas. Pre- and postoperative PTBE

Visual Epilepsy Mimicking Migraine Aura Leading to Delayed Diagnosis of an Occipital Cavernoma: A Case Report.

Cerebral cavernous malformations, also known as cavernomas, are uncommon vascular malformations that may present with seizures, headache, or focal neurologic deficits depending on lesion location. Occipital cavernomas are rare and may manifest with positive visual phenomena, thereby mimicking migraine aura and delaying diagnosis. We report the case of a 54-year-old woman who presented in March 2018 with recurrent brief visual episodes described as flashing lights, colored shapes, scotomas, and distortion of object contours, associated with a persistent headache. She was initially treated for m

Case Report: DICER1-mutant primary intracranial sarcoma with concurrent TP53, PDGFRA, and KEAP1 somatic mutations in a 5.5-year-old boy.

DICER1-mutant primary intracranial sarcoma (DICER1-mutant PIS) is a rare and aggressive central nervous system (CNS) malignancy primarily affecting pediatric patients. We report the case of a 5.5-year-old boy who presented with non-specific prodromal symptoms, including recurrent dizziness and progressive vomiting. Brain neuroimaging revealed a cystic-solid mass in the frontal lobe, characterized by heterogeneous contrast enhancement, significant perilesional edema, and intratumoral hemorrhage. Histopathological examination of the tumor demonstrated marked cellular pleomorphism, atypical mitos

Carotid artery dissection and brain herniation following traditional carotid massage: a case report.

Carotid artery dissection is an important cause of ischemic stroke in young adults and has been associated with minor cervical trauma, including neck manipulation. Traditional carotid massage, commonly practiced in low-resource settings, is often perceived as harmless despite potential vascular risks. We report the case of an 18-year-old woman with no known comorbidities who developed acute neurological deterioration shortly after undergoing carotid massage by a traditional healer. Brain computed tomography revealed a large left hemispheric infarction involving the malignant middle cerebral ar

cRGD-functionalized exosome-Mn₃O₄ nanoplatform enhanced ischemic lesion accumulation and ros scavenging-mediated neuroprotection in ischemic stroke.

The therapeutic efficacy of ischemic stroke (IS) treatment is severely limited by insufficient accumulation of therapeutic agents within ischemic lesions and persistent secondary injury after ischemia-reperfusion. Herein, we report a cRGD-functionalized exosome-based nanoplatform that enhances ischemic lesion-associated accumulation and antioxidative neuroprotection for the treatment of IS. Neural stem cell-derived exosomes were functionalized with cyclic RGD peptides (cRGD) and subsequently loaded with Mn₃O₄ nanoparticles to construct a hybrid nanosystem (cRGD-Exo@Mn₃O₄). The engineered exoso

Brainstem tuberculoma mimicking brainstem stroke: Crossed syndrome in a young female.

INTRODUCTION: Tuberculosis (TB) involving the central nervous system (CNS) can present as tuberculoma and may mimic neoplasms or vascular lesions, particularly when the brainstem is involved. Early recognition is critical in endemic settings such as India. CASE PRESENTATION: A previously healthy female in late adolescence presented with a one-month history of headache followed by progressive left-sided weakness and multiple cranial nerve deficits, producing a crossed brainstem syndrome. Magnetic resonance imaging (MRI) of the brain revealed conglomerated ring-enhancing lesions in the midbrain

Cisternal Contrast-Enhanced MRI Reveals Post-Stroke Glymphatic Impairment and Compensatory Metabolic Waste Clearance via Microglia/Macrophages.

Recent studies have shown that the glymphatic system plays a crucial role in driving hyperacute edema after ischemic stroke. This has sparked interest in understanding how this system changes in later phases of ischemic stroke. In this study, we utilized cisternal contrast-enhanced magnetic resonance imaging (CE-MRI) and immunofluorescence staining to investigate glymphatic system alterations at subacute and chronic phases of ischemic stroke. Middle cerebral artery occlusion (MCAO) for 90 min in Sprague-Dawley rats was used to mimic ischemic stroke. A total of 20 rats were randomly divided int
