Cortical stimulation mapping and Wada results demonstrate a normal variant of right hemisphere language organization.

PURPOSE: Exclusive right hemisphere language lateralization is rarely observed in the Wada angiography results of epilepsy surgery patients. Cortical stimulation mapping (CSM) is infrequently performed in such patients, as most undergo nondominant left hemisphere resections, which are presumed not to pose any risk to language. Early language reorganization is typically assumed in such individuals, taking left hemisphere epileptiform activity as confirmation of change resulting from a pathologic process. We present data from CSM and Wada studies demonstrating that right hemisphere language occu

[Schizencephaly--clinical and radiological presentation of pediatric patients].

UNLABELLED: Schizencephaly is a rare central nervous system malformation. The anomaly is characterized by uni- or bilateral clefts in the brain's cerebral hemispheres. There are 2 types of the anomaly distinguished: type I ("closed lips") if there are fused clefts in cerebral mantle and type II ("open lips") if the clefts are separated. The etiology of this malformation is not clear; both environmental (intrauterine cytomegaloviral infection) and genetic risk factors are considered (mutations in EMX2, Lhx2 genes). The aim of the study was the analysis of clinical presentation, neurodevelopment

Development of a de novo cerebral arteriovenous malformation in a child with sickle cell disease and moyamoya arteriopathy. Case report.

The authors report the unique case of a 6-year-old African-American girl with sickle cell disease (SCD) and an associated moyamoya arteriopathy who developed a de novo arteriovenous malformation (AVM) of the cerebral circulation. Based on preoperative cerebral angiography, computerized tomography angiography, and magnetic resonance imaging, the incidentally discovered lesion was originally thought to be a direct arteriovenous fistula with an associated varix. At surgery, however, a 1.5-cm AVM was identified adjacent to the deep surface of the varix, and it was successfully resected. The diagno

Arteries of the insula.

OBJECT: The insula is located at the base of the sylvian fissure and is a potential site for pathological processes such as tumors and vascular malformations. Knowledge of insular anatomy and vascularization is essential to perform accurate microsurgical procedures in this region. METHODS: Arterial vascularization of the insula was studied in 20 human cadaver brains (40 hemispheres). The cerebral arteries were perfused with red latex to enhance their visibility, and they were dissected with the aid of an operating microscope. Arteries supplying the insula numbered an average of 96 (range 77-1

Salvianolic Acid B Attenuates Excitotoxic Neuronal Injury After Transient Cerebral Ischemia.

BACKGROUND/AIM: Excitotoxic neuronal injury plays a central role in the pathological cascade which follows cerebral ischemia. Excessive glutamatergic signaling leads to calcium overload and the activation of apoptotic pathways, ultimately triggering neuronal death. Salvianolic acid B (Sal B), a water-soluble polyphenolic compound derived from Salvia miltiorrhiza, exhibits neuroprotective properties; however, its effects on excitotoxic neuronal injury following cerebral ischemia remain unclear. The present study used both in vitro and in vivo models of ischemic injury to investigate whether Sal

Electroacupuncture alleviates ferroptosis through miR-145-5p/ACSL4 axis to reduce post-reperfusion brain injury in ischemic stroke.

BACKGROUND: Ferroptosis induced by reperfusion in ischemic stroke (IS) represent core mechanisms that exacerbate brain injury and neurological dysfunction. Electroacupuncture (EA) is widely used in neurological rehabilitation, but its specific mechanism for improving reperfusion injury in ischemic stroke remains unclear. METHODS: We established a middle cerebral artery occlusion (MCAO) mouse model, the impacts of EA on infarction volume, cerebral cortex damage, and neuronal apoptosis were observed using pathological staining. Additionally, an oxygen-glucose deprivation/reoxygenation (OGD/R) S

Effect of encephalomyosynangiosis procedure on post stroke recovery in a permanent model of ischemic stroke.

Acute ischemic stroke (AIS) remains a leading cause of disability worldwide. Limited treatments for AIS with thrombolytics are further constrained by narrow therapeutic windows and no neuroprotective effects. Damaged or underdeveloped vasculature and poor angiogenesis after AIS remains a major cause of failed neuroprotective therapy after AIS. There is a critical need for alternative approaches that enhance angiogenesis and neuroprotection to improve post-stroke recovery. Here, we use an indirect intracranial bypass technique via encephalomyosynangiosis (EMS), which involves the transposition

Efficacy of puerarin in rats with focal cerebral ischemia through modulation of the SIRT1/HIF-1α/VEGF signaling pathway and its effect on synaptic plasticity.

This study aimed to evaluate the efficacy of puerarin and its effect on synaptic plasticity in rats with focal cerebral ischemia (FCI) by modulating the silent mating type information regulation 2 homolog (SIRT1)/hypoxia-inducible factor-1α (HIF-1α)/vascular endothelial growth factor (VEGF) signaling pathway. Fifty specific pathogen-free-grade healthy male rats were randomly divided into sham operation group (SOG), model group, low-dose group, medium-dose group, and high-dose group, with 10 rats in each group. The SOG group received sham operation and saline treatment, while the other four gro

Neuropilin-1 promotes mitochondrial structural repair and functional recovery in rats with cerebral ischemia.

OBJECTIVES: Available literature documents that ischemic stroke can disrupt the morphology and function of mitochondria and that the latter in other disease models can be preserved by neuropilin-1 (NRP-1) via oxidative stress suppression. However, whether NRP-1 can repair mitochondrial structure and promote functional recovery after cerebral ischemia is still unknown. This study tackled this very issue and explored the underlying mechanism. METHODS: Adeno-associated viral (AAV)-NRP-1 was stereotaxically inoculated into the cortex and ipsilateral striatum posterior of adult male Sprague-Dawley

Changes in Cortical Activation During Dual-Task Walking in Individuals With and Without Visual Vertigo.

BACKGROUND AND PURPOSE: Persons with vestibular disorders are known to have slower gait speed with greater imbalance and veering during dual-task walking than healthy individuals, but the cerebral mechanisms are unknown. The purpose of this study was to determine whether individuals with visual vertigo (VV) have different cerebral activation during dual-task walking compared with control subjects. METHODS: Fourteen individuals with VV and 14 healthy controls (CON) were included (mean 39 years old, 85% women). A cross-sectional experimental study consisting of 4 combinations of 2 surfaces (eve

Recent Progress in Therapeutic Strategies for Ischemic Stroke.

Possible strategies for treating stroke include neuroprotection in the acute phase of cerebral ischemia and stem cell therapy in the chronic phase of cerebral ischemia. Previously, we have studied the temporal and spatial expression patterns of c-fos, hypoxia inducible factor-1α (HIF-1α), heat shock protein 70 (HSP70), and annexin V after 90 min of transient middle cerebral occlusion in rats and concluded that there is a time window for neuroprotection from 12 to 48 h after ischemia. In addition, we have estimated the neuroprotective effect of glial cell line-derived neurotrophic factor (GDNF)

An autopsied case of MM1 + MM2-cortical with thalamic-type sporadic Creutzfeldt-Jakob disease presenting with hyperintensities on diffusion-weighted MRI before clinical onset.

A 78-year-old Japanese man presented with rapidly progressive dementia and gait disturbances. Eight months before the onset of clinical symptoms, diffusion-weighted magnetic resonance imaging (DWI) demonstrated hyperintensities in the right temporal, right parietal and left medial occipital cortices. Two weeks after symptom onset, DWI showed extensive hyperintensity in the bilateral cerebral cortex, with regions of higher brightness that existed prior to symptom onset still present. Four weeks after clinical onset, periodic sharp wave complexes were identified on an electroencephalogram. Myocl

Influence of age on brain edema formation, secondary brain damage and inflammatory response after brain trauma in mice.

After traumatic brain injury (TBI) elderly patients suffer from higher mortality rate and worse functional outcome compared to young patients. However, experimental TBI research is primarily performed in young animals. Aim of the present study was to clarify whether age affects functional outcome, neuroinflammation and secondary brain damage after brain trauma in mice. Young (2 months) and old (21 months) male C57Bl6N mice were anesthetized and subjected to a controlled cortical impact injury (CCI) on the right parietal cortex. Animals of both ages were randomly assigned to 15 min, 24 h, and 7

Use of 2-channel bedside electroencephalogram monitoring in term-born encephalopathic infants related to cerebral injury defined by magnetic resonance imaging.

OBJECTIVE: Single-channel amplitude-integrated electroencephalography has been shown to be predictive of neurodevelopmental outcome in term infants with hypoxic-ischemic encephalopathy. We describe the relationship of quantifiable electroencephalogram (EEG) measures, obtained using a 2-channel digital bedside EEG monitor from term newborn infants with encephalopathy and/or seizures, to cerebral injury defined qualitatively by MRI. METHODS: Median values of minimum, mean, and maximum EEG amplitude were obtained from term-born encephalopathic infants during a 2-hour seizure-free period obtained

Brain Accumulation of Amyloid-beta in Non-Alzheimer Neurodegeneration.

We report an unusual case of amyotrophic lateral sclerosis (ALS) marked by extensive cerebral amyloid-beta deposition in small and medium-size vessels, capillaries, and perivascular plaques in the cerebral cortex, and in most leptomeningeal vessels. Despite considerable cerebral amyloidosis, the patient remained cognitively intact until death. For comparison with other neuro-degenerative diseases and normal aging, we assessed the densities of amyloid-beta-immunoreactive cortical vessels and plaques in matched frontal and temporal lobe sections from archival uncomplicated cases of Alzheimer's d

Cortical information flow during flexible sensorimotor decisions.

During flexible behavior, multiple brain regions encode sensory inputs, the current task, and choices. It remains unclear how these signals evolve. We simultaneously recorded neuronal activity from six cortical regions [middle temporal area (MT), visual area four (V4), inferior temporal cortex (IT), lateral intraparietal area (LIP), prefrontal cortex (PFC), and frontal eye fields (FEF)] of monkeys reporting the color or motion of stimuli. After a transient bottom-up sweep, there was a top-down flow of sustained task information from frontoparietal to visual cortex. Sensory information flowed f
