A 39-year-old man presented with a 10-year history of psoriatic arthritis, and more recently a progressive high cervical myelopathy. The definite diagnosis of osteoid osteoma relies on the demonstration of the nidus, best shown by CT, which also provides precise preoperative localization of the nidus. While bone-block excision to remove the nidus is feasible in the long bones, there may be unacceptable sequelae in the vertebral column and small bones of the hands. By precisely localizing the nidus, radionuclide scintimetry permits excellent therapeutic results, with minimal morbidity.

Fracture of the occipital condyle is a rare problem. Diagnosis requires a high index of suspicion in a patient with a head injury and abnormalities of function of the lower cranial nerves. The diagnosis is best made by high resolution computed tomographic scanning with sagittal and coronal reconstructions. Three unusual and rare cases are reported with progressive anterior dislocation of the cervical spine of unknown etiology causing severe myelopathy. Spinal pathology was confined to a single level and at different locations in each case. Narrowing of the spinal canal resulted in three different neurological syndromes, each requiring a different operative approach: one anterior, one posterior, and one both. Two patients made complete functional recovery. One patient died 3 days after operation of aspiration pneumonia.

Extracapsular hemorrhage from spontaneous rupture of a hyperplastic parathyroid gland is exceedingly rare. Rupture of an intrathoracic parathyroid adenoma mimicking a dissecting thoracic aortic aneurysm has been reported in two cases. Hypercalcemic crisis associated with contained hemorrhage within a parathyroid adenoma has been well documented. We present the first reported case of spontaneous rupture and extracapsular hemorrhage of a cervical parathyroid adenoma. Serum calcium levels should be obtained when spontaneous cervical or mediastinal hemorrhage develops without obvious cause.

An analysis of 167 deaths occurring among 1000 patients with cervical spine trauma, treated in the early post-traumatic period in the Rehabilitation Institute at Konstancin, has been carried out in this report. The cervical spine radiographs of 100 patients with early rheumatoid disease were studied annually, on a prospective basis, for a mean follow-up period of 7 years 2 months. Atlantoaxial subluxation developed in 12 patients. The clinical and radiological findings of Larsen's syndrome are illustrated by one case and the differential diagnosis is discussed. Benign tumours of the spine are infrequent. Among the bony lesions occurring in children and adolescents are osteochondroma, benign osteoblastoma, and aneurysmal bone cyst.
