Commentary on the regulatory implications of noise-induced cochlear neuropathy.

OBJECTIVE: A discussion on whether recent research on noise-induced cochlear neuropathy in rodents justifies changes in current regulation of occupational noise exposure. DESIGN: Informal literature review and commentary, relying on literature found in the authors' files. No formal literature search was performed. STUDY SAMPLE: Published literature on temporary threshold shift (TTS) and cochlear pathology, in humans and experimental animals, as well as the regulations of the US Occupational Safety and Health Administration (OSHA). RESULTS: Humans are less susceptible to TTS, and probably to

Noise exposure immediately activates cochlear mitogen-activated protein kinase signaling.

Noise-induced hearing loss (NIHL) is a major public health issue worldwide. Uncovering the early molecular events associated with NIHL would reveal mechanisms leading to the hearing loss. Our aim is to investigate the immediate molecular responses after different levels of noise exposure and identify the common and distinct pathways that mediate NIHL. Previous work showed mice exposed to 116 decibels sound pressure level (dB SPL) broadband noise for 1 h had greater threshold shifts than the mice exposed to 110 dB SPL broadband noise, hence we used these two noise levels in this study. An Src-protein tyrosine kinase inhibitor to reduce cisplatin ototoxicity while preserving its antitumor effect.

Ototoxicity remains a major dose-limiting side effect of cisplatin. The current studies were carried out to evaluate the effectiveness of a novel Src-protein tyrosine kinase inhibitor in protecting the ear from cisplatin ototoxicity without compromising cisplatin's antitumor effects. The Src inhibitor has been shown to be effective in protecting the ear from noise-induced hearing loss. Three studies were carried out to determine whether this compound has otoprotective activity in rats treated with cisplatin. The first two studies used the Src inhibitor as a cotreatment with single doses of cis

Third windows as a cause of failure in hearing gain after exploratory tympanotomy.

OBJECTIVE: Exploratory tympanotomy (ET) can be performed selectively on patients who have air-bone gaps (ABGs) without any apparent external or middle ear abnormalities for diagnosis and treatment of their conductive hearing loss. However, surgeons cannot always find middle ear problems. It has recently been reported that pathologic third windows in inner ears can cause ABGs. Therefore, the authors conducted this study to evaluate the inner ear anatomical problems of patients who had undergone ET. STUDY DESIGN: Case series with chart review. SETTING: Tertiary center. SUBJECTS AND METHODS: T

Methylenetetrahydrofolate reductase C677T gene mutation as risk factor for sudden sensorineural hearing loss: association with plasma homocysteine, folate and cholesterol concentrations.

OBJECTIVE: Impaired cochlear perfusion appears to be the most important event in the development of sudden sensorineural hearing loss. Methylenetetrahydrofolate reductase gene mutations at nucleotide 677 cause reduced methylenetetrahydrofolate reductase enzyme activity, resulting in vascular impairment. METHODS: Thirty-three patients and 68 control subjects underwent audiological and haematological investigation. RESULTS: No statistically significant association was found between sudden sensorineural hearing loss and the methylenetetrahydrofolate reductase C677T gene mutation. Mean homocyste

Age-related hearing loss in the Fischer 344/NHsd rat substrain.

Studies of the F344 rat have shown a variety of age-related auditory anatomy and physiology changes. The current study was undertaken to clarify the ARHL in the F344 rat, by examining the auditory pathway of the F344/NHsd substrain that is distributed by Harlan Laboratories for research in the United States. The F344/NHsd rat begins to lose its hearing at about 12 months, and by 24 months, there are 50-60 dB auditory brainstem response threshold shifts at 20 and 40 kHz and 20 dB losses at 5-10 kHz. Distortion product otoacoustic emissions (DPOAE) amplitudes at 1.8-12 kHz stimuli were depressed

Cellular correlates of progressive hearing loss in 129S6/SvEv mice.

Several strains of mice hear well initially but show progressive sensorineural hearing loss. Affected cochlear cell types include all those known to be affected in human age-related hearing loss (ARHL), or presbycusis. Thus these mice have been offered as models of human ARHL. At present, however, few mouse ARHL models are sufficiently well described to serve as the basis for specific hypotheses about human ARHL. We examined 1-month-old and 15-month-old 129S6/SvEv (129S6) mice and compared them with BALB/cJ and CBA/J mice. Age-related elevation of compound action potential thresholds was inter

[Cochlear hearing loss following Mycoplasma pneumoniae infection].

BACKGROUND: A sudden unilateral hearing loss in childhood is commonly regarded as an unusual event. CASE-REPORT: In this paper, we report of a 10 year old girl suffering from a unilateral right-sided sudden hearing loss following an acute infection with Mycoplasma pneumoniae, which is also rarely observed. Maximum hearing loss was estimated around 30 dB (0.25-6 kHz), which resolved after infusions using plasma expanders. Both transitory and distortion product otoacoustic emissions remained absent, while latencies of acoustically evoked potentials were normal. DISCUSSION: This suggests a dama

Evolution of transient evoked otoacoustic emissions in preterm newborns: a preliminary study.

Transient evoked otoacoustic emissions (TEOEs) were recorded in a cohort study of preterm neonates in order to study their basic properties as a function of gestational age. Their main properties were: (1) TEOEs spectrum did not vary with age; it was analogous to those of full term; (2) there was no statistically significant variation of the TEOEs amplitude with age. The maturation of outer hair cell properties appears to be complete at a gestational age of 29 weeks. Because a number of infants at risk for hearing loss are preterm babies, screening for TEOEs has to be performed in the neonatal

Temporal bone histopathological findings in trisomy 13 syndrome.

This study reports the histopathological findings of 14 temporal bones from infants with trisomy 13 syndrome. The most primitive anomalies in the structures of the inner and middle ears in the present series are those of the semicircular canals, particularly of the horizontal canals: flattened horizontal canal cristae, absence or opening of the utricular endolymphatic valve, small facial nerve, and obtuse angle of the geniculate area of the facial nerve. Each ear demonstrated more than one of those anomalies. The anomalies present features similar to those found in the structures of the normal

Clinical high-resolution imaging and grading of endolymphatic hydrops in Hydropic Ear Disease at 1.5 T using the two-slice grading for vestibular endolymphatic hydrops in less than 10 min.

BACKGROUND: Hydropic Ear Disease (Menière) is one of the most common inner ear disorders and one of the most common causes of vertigo attacks. The underlying pathology is a distension of the endolymphatic space of the inner ear, termed endolymphatic hydrops. However, the unequivocal morphologic confirmation of ELH has been restricted to post-mortem histologic analysis until 2007, when the first clinical MR imaging report demonstrated ELH in living patients with Menière's disease at 3 T combined with intratympanic application of contrast. Imaging techniques have since then evolved further. Howe

A Fully Automated Analytic System for Measuring Endolymphatic Hydrops Ratios in Patients With Ménière Disease via Magnetic Resonance Imaging: Deep Learning Model Development Study.

BACKGROUND: Recently, the analysis of endolymphatic hydropses (EHs) via inner ear magnetic resonance imaging (MRI) for patients with Ménière disease has been attempted in various studies. In addition, artificial intelligence has rapidly been incorporated into the medical field. In our previous studies, an automated algorithm for EH analysis was developed by using a convolutional neural network. However, several limitations existed, and further studies were conducted to compensate for these limitations. OBJECTIVE: The aim of this study is to develop a fully automated analytic system for measur

Traditional oriental medicine for sensorineural hearing loss: Can ethnopharmacology contribute to potential drug discovery?

ETHNOPHARMACOLOGICAL RELEVANCE: In Traditional Oriental Medicine (TOM), the development of hearing pathologies is related to an inadequate nourishment of the ears by the kidney and other organs involved in regulation of bodily fluids and nutrients. Several herbal species have historically been prescribed for promoting the production of bodily fluids or as antiaging agents to treat deficiencies in hearing. AIM OF REVIEW: The prevalence of hearing loss has been increasing in the last decade and is projected to grow considerably in the coming years. Recently, several herbal-derived products pres

Auditory outcomes in tumor vs. nontumor patients fitted with auditory brainstem implants.

Auditory brainstem implants (ABIs) are currently indicated for patients older than 12 years with neurofibromatosis type 2 (NF2) who had bilateral schwannoma removed. Over the last 10 years, we have extended the indications for ABIs to nontumor children and adult patients with cochlear or cochlear nerve injuries or malfunctions who would not benefit from a cochlear implant. We have provided ABIs for patients with cochlear nerve aplasia and other injuries, and patients in whom any benefit was, or would be, severely compromised as in extensive cochlear ossification. In the present chapter we repo
