Modification of the stages in total reconstruction of the auricle: Part I. Grafting the three-dimensional costal cartilage framework for lobule-type microtia.

Total auricular reconstruction for microtia is dependent on the fabricated three-dimensional costal cartilage framework, with all the auricular features and the skin flaps formed and utilized to cover the three-dimensional framework, in which extreme caution is required. Furthermore, other factors, such as contraction of the skin graft, use of the remnant ear cartilage in construction of the tragus, and the number of surgeries required, all affect the final results of total auricular reconstruction for microtia, especially with reference to the contour of the constructed ear, morphologic locat

[Rectocele: a sign of the relaxed pelvic floor syndrome].

Routine physical examination, anorectal dynamics, electromyography of the pelvic floor, defecography and bowel transit were made in 45 patients with rectocele. The results showed that the disorder occurred mainly postpartum and always coexisted with other relaxed pelvic floor disorder. The patients had abnormal descent of pelvic floor, which was positively related to the depth of rectocele. Those with rectocele and spastic pelvic floor syndrome showed less descent of pelvic floor and depth of rectocele compared with those without SPFS. Thess suggest that the relationship between rectocele and

[Effects of various histamine H2-receptor antagonists on gastrointestinal motility and gastric emptying].

Effects of various histamine H2-receptor antagonists (H2-RA) on gastrointestinal motility and gastric emptying were investigated. Ranitidine, nizatidine and cimetidine dose dependently induced contractions over the entire gastrointestinal tract, and the order of their effects was ranitidine > nizatidine > cimetidine. Roxatidine or famotidine had no effects on motility of the gastrointestinal tract. Ranitidine and nizatidine also induced contractions during the postprandial period and facilitated gastric emptying, while cimetidine, roxatidine or famotidine did not influence gastric emptying. Si

Rupture of membranes before the onset of spontaneous labour increases the likelihood of instrumental delivery.

We studied obstetric outcome in 350 consecutive nulliparous women in spontaneous labour and term pregnancy. Women who presented with rupture of membranes before the onset of contractions were more likely to deliver by forceps compared with those in whom contractions preceded rupture of membranes. This increased likelihood of instrumental delivery was significant with and without the use of extradural analgesia in labour (P less than 0.05 and P less than 0.001, respectively). Furthermore, significantly more women with premature rupture of membranes received extradural analgesia in labour (P les

[Clinical evaluation of pseudosinusoidal fetal heart rate pattern associated with abruption placentae].

From retrospective studies of fetal heart rate records on abruption placentae, six cases with the pseudosinusoidal FHR pattern (pseudosinusoidal pattern) are reported. The pseudosinusoidal pattern has the following characteristics: 1) oscillation frequency synchronized with the frequency of uterine contractions, 2) an amplitude of 19bpm or more, which is positively correlated with the area of placental separation, 3) uniform frequency and amplitude, 4) a frequency of 1.3 cycles/min or less, which is clearly different from the true sinusoidal pattern. It seems that this characteristic pseudosin

Effect of porcine relaxin on spontaneous, oxytocin-driven and prostaglandin-driven pig myometrial activity in vitro.

The effect of porcine relaxin on myometrial activity was studied in 18 nonpregnant and 54 pregnant pigs. Relaxin inhibited spontaneous contractility in all luteal phase and pregnant pigs. However, similar doses of relaxin had no effect on oxytocin-, prostaglandin E2- or prostaglandin F2 alpha-induced contractions. Only when the doses were increased fivefold to sixfold, to nonphysiologic levels, did relaxin inhibit the action of these oxytocic agents. The effect of relaxin was the same throughout pregnancy. This finding helps explain the two paradoxical roles of relaxin: it inhibits spontaneous

Endoscopic manometry of the sphincter of Oddi in dogs. Pharmacological effects of anticholinergic and selective antimuscarinic agents.

A dog model was used for direct measurement of the sphincter of Oddi using endoscopy. The effects of anticholinergic agents and a selective antimuscarinic agent on the canine sphincter of Oddi motility were studied. Manometric tracing at the canine sphincter showed rhythmical phasic contractions which were not affected by respiration. The mean values of the basal motility were as follows: the peak pressure was 61.5 +/- 2.2 (m +/- SE) cmH2O, trough pressure was 2.18 +/- 1.2 cmH2O, and the frequency was 17.4 +/- 0.2 waves/min. Intravenous administration of atropine sulfate, butropium bromide, bu

Rumination, heartburn, and daytime gastroesophageal reflux. A case study with mechanisms defined and successfully treated with biofeedback therapy.

A 31-year-old man with a 19-year history of rumination developed frequent episodes of heartburn and regurgitation associated with acid gastroesophageal reflux that occurred predominantly during the day. This reflux and its attendant symptoms resulted from abdominal muscle contractions at the time of gastroesophageal pressure equilibration (i.e., common cavity phenomena) consistent with the egress of air from the stomach to the esophagus. A voluntary pharyngeal maneuver unassociated with swallowing but simultaneous with the abdominal contraction resulted in a decrease in upper esophageal sphinc

Hepatitis. A review of the disease and its significance to dentistry.

The report is a review of the literature related to hepatitis B and its implications for dental practitioners. Hepatitis B represents a serious health risk among dentists and simple preventive measures can greatly reduce the risk of transmission or contraction of the disease. Certain positive findings in a healthy history should alert the dentist that he may be treating a carrier of hepatitis B. If so, the wearing of gloves, a mask and protective eyewear will almost totally protect one from contracting the disease. To guard against transmission of hepatitis B to other patients in the dental pr

G-protein coupled receptor activity mediates detrusor smooth muscle phasic contractility through regulation of membrane potential.

The urinary bladder functions to store and release urine, yet how the sensation of bladder fullness is conveyed and perceived to the central nervous system is not understood. During bladder filling, the detrusor smooth muscle (DSM) generates phasic contractions, resulting in pressure fluctuations within the bladder. These transient pressure events drive bursts of afferent nerve activity, yet the underlying mechanism leading to rhythmic contractions remains unclear. Here, we examined the role of Gq protein-coupled receptor (GqPCR) activity on DSM excitability and contractility. Using ex vivo pr

Effects of Visual and Auditory Feedback on the Mixed-Nerve Silent Period During Isometric Knee Extension Torque Control in Female University Basketball Players.

Sensory feedback plays an important role in isometric force regulation by modulating neural control during voluntary contraction. The mixed-nerve silent period (MnSP) is an electrophysiological index reflecting inhibitory neural mechanisms during voluntary muscle contraction. This study investigated the effects of visual and auditory feedback on MnSP during an isometric knee extension torque maintenance task in trained athletes. Twelve female collegiate basketball players performed a sustained knee extension torque task at 25% of maximal voluntary isometric contraction. The task was performed

Volitional EMG Control of a Novel Powered Ankle Prosthesis: A Case Series on Muscle Selectivity and Biomechanical Consequences.

This study investigated the feasibility and biomechanical effects of volitional electromyography (EMG)-based control of a powered transtibial ankle prosthesis. Four male participants completed static and dynamic EMG assessments and gait analysis while using both their prescribed passive prosthesis and an EMG-controlled powered prototype during level walking at self-selected and fast speeds, as well as ramp ascent and descent. Selective activation of residual tibialis anterior and gastrocnemius muscles was quantified using a co-contraction index, and lower-limb kinematics and kinetics were comp

Effect of ASP5633, a novel peripherally restricted MT1/MT2 receptor agonist, on urethral function in female rats.

Melatonin is a hormone that regulates numerous physiological processes through activation of specific G protein-coupled receptors, termed MT1 and MT2 receptors. We previously reported that stimulation of MT1 receptors in female rat urethra activated intracellular Ca2+ mobilization via a Gq protein-dependent pathway, resulting in urethral smooth muscle contraction. This study aimed to investigate the potential of MT1 receptor-related urethral contraction in the treatment of urological disorders using ASP5633 (3-(2-acetamidoethyl)-6-fluoro-5-methoxy-N-methyl-1H-indole-2-carboxamide), a novel per

Brain lesions associated with abnormal muscle tone during the swing phase in post-stroke hemiparesis.

BACKGROUND: Post-stroke spasticity may involve dynamic abnormalities that impair voluntary movement, such as muscle co-contraction during gait, which may be present even in patients without apparent spasticity at rest. This study aimed to identify brain lesion locations associated with abnormal lower-limb muscle co-contraction during walking using support vector regression lesion-symptom mapping. METHODS: Thirty-two ambulatory patients with first-ever supratentorial stroke were included. Lower-limb motor impairment was defined as a Fugl-Meyer Assessment lower-extremity score of < 34. Surface

Motor Unit Behaviour in the Biceps Brachii During Asymmetric Bilateral Versus Unilateral Dynamic Force Tracking Across Force Levels and Rates of Force Development: A Preliminary Study.

Most bilateral actions in daily life are asymmetric: one limb sustains a steady postural load while the other performs a continuously adjusted force task. Whether the influence of such a sustained contraction on the contralateral limb is a fixed property of bilateral coordination or a strategy that higher centres rescale with task demand is unknown. In this exploratory study, we examined motor unit recruitment and common neural drive in the biceps brachii during asymmetric bilateral versus unilateral isometric force tracking, tested whether the interlimb effect depends on force level and rate

Distortions of lip size bias perceived facial attractiveness.

Perceiving faces as attractive or not guides decisions to approach or date a person and can sway opinions in recruiting and legal proceedings. However, the mechanisms underlying facial attractiveness are not fully understood. While popular models of face recognition emphasize holistic processing, individuals often attempt to enhance their own attractiveness in feature-centric ways (cosmetic surgery, make-up, injectables). Here, we use a local feature manipulation (lip expansion/contraction) and show that it alters the perceived attractiveness of male and female faces. Females showed peak prefe

Does a counterforce brace reduce common extensor tendon loading during a wrist extension task? An in vivo study.

This study assessed the biomechanical effect of a counterforce brace on the common extensor origin (CEO) tendon at the elbow via the measurement of shear wave velocity (SWV) using ultrasound. The counterforce brace was hypothesised to reduce SWV, which is a proxy measure of tendon stiffness, whilst the wrist and finger extensors were contracting at different levels of maximum voluntary contraction (MVC). In this cross-sectional study, nineteen healthy participants (age±SD: 30±9) were included in the study. The counterforce brace was applied with either 0 or 80 mmHg pressure to the forearm. The

Adjuvant-induced arthritis promotes vascular hyporesponsiveness to phenylephrine through a nitric oxide-related mechanism.

Arthritis has important cardiovascular repercussions. Phenylephrine-induced vasoconstriction is impaired in rat aortas in the early phase of the adjuvant-induced arthritis (AIA), around the 15th day post-induction. Therefore, the present study aimed to verify the effects of AIA on hyporesponsiveness to phenylephrine in rat aortas. AIA was induced by intradermal injection of Mycobacterium tuberculosis (3.8 mg/dL) in the right hind paw of male Wistar rats (n=27). Functional experiments in isolated aortas were carried out 15 days after AIA induction. Morphometric and stereological analyses of the

Ultradynamic Isoreticularly Expanded Porous Organic Crystals.

Porous organic materials showcasing large framework dynamics present new paths for adsorption and separation with enhanced capacity and selectivity beyond the size-sieving limits, which is attributed to their guest-responsive sorption behaviors. Porous hydrogen-bonded crosslinked organic frameworks (HCOFs) are attractive for their remarkable ability to undergo guest-triggered expansion and contraction facilitated by their flexible covalent crosslinkages. However, the voids of HCOFs remain limited, which restrains the extent of the framework dynamics. In this work, we synthesized a series of HC

Commonality and heterogeneity of pacemaker mechanisms in the male reproductive organs.

During emission, the first phase of ejaculation, smooth muscle in organs of the male reproductive tract (MRT) vigorously contract upon sympathetic nerve excitation to expel semen consisting of sperm and seminal plasma. During inter-ejaculation phases, the epididymis, seminal vesicles and prostate undergo spontaneous phasic contractions (SPCs), this transporting and maintaining the quality of sperm and seminal plasma. Recent studies have revealed platelet-derived growth factor receptor α-expressing (PDGFRα+) subepithelial interstitial cells in seminal vesicles subserve the role of pacemaker cel
