Pathology Image of the Month:Cough and Shortness of Breath in a Noncompliant Patient with HIV/AIDS.

A 37- year-old man with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) was admitted to the intensive care unit following a four month history of progressive shortness of breath, productive cough, and flu-like symptoms. His HIV/AIDS was diagnosed at the age of 19 (CD4 count =15; viral load = 294,436 copies/ mL) and was complicated by hemodialysis-dependent, HIV-associated nephropathy, prior Pneumocystis pneumonia and known noncompliance with prescribed antiretroviral therapy. Chest film at admission was interpreted as diffuse bilateral interstitial and airspace opaci

Nodular trachea: tracheobronchopathia osteochondroplastica.

Tracheobronchopathia osteochondroplastica (TPO) is a rare disorder characterised by development of cartilaginous and bony nodules in the trachea. It usually affects elderly males with total prevalence of disease reported to be around 0.1%. Even though multiple theories and associations have been described, the exact cause of the disease remains unknown. Its presentation is non-specific with many cases being diagnosed incidentally. Chronic cough and dyspnoea are the most common presenting symptoms, but some cases may present with severe airway compromise. We present a case of a 70-year-old man

Extra-esophageal gastroesophageal reflux disease and asthma: understanding this interplay.

Gastroesophageal reflux disease (GERD) is a condition that develops when there is reflux of stomach contents, which typically manifests as heartburn and regurgitation. These esophageal symptoms are well recognized; however, there are extra-esophageal manifestations of GERD, which include asthma, chronic cough, laryngitis and sinusitis. With the rising incidence of asthma, there is increasing interest in identifying how GERD impacts asthma development and therapy. Due to the poor sensitivity of endoscopy and pH monitoring, empiric therapy with proton pump inhibitors (PPIs) is now considered the

A light bulb moment: an unusual cause of foreign body aspiration in children.

A 15-month-old girl developed persistent cough with no associated history of foreign body aspiration. Chest X-ray showed a U-shaped radiopaque foreign body, which was initially thought to be a hairpin, in the right main bronchus. Rigid bronchoscopy was performed and the foreign body turned out to be a light-emitting diode (LED) bulb. In this article, we report our experience of LED bulb aspiration in children, with the view to raise the awareness of clinicians about this potentially life-threatening emergency.

The relationship between psychological symptoms, lung function and quality of life in children and adolescents with non-cystic fibrosis bronchiectasis.

OBJECTIVES: The aim of this study was to evaluate the relationship between psychological symptoms and quality of life (QOL) and clinical variables in a cohort of children and adolescents with non-cystic fibrosis (non-CF) bronchiectasis. METHODS: Seventy-six patients (aged 8-17years) participated in this study. Questionnaires were used to evaluate the psychological status and QOL of the patients and healthy controls. The patient and control groups were divided into child and adolescent groups to exclude the effect of puberty on psychological status. RESULTS: No significant difference was foun

Kaposiform lymphangiomatosis, a newly characterized vascular anomaly presenting with hemoptysis in an adult woman.

Disorders of the pulmonary lymphatic system include macro- and microcystic lymphatic malformations, primary or secondary lymphangiectasias, generalized lymphatic anomalies, diffuse pulmonary lymphangiomatosis, and combinations of lymphatic and other tissue anomalies, including lymphangioleiomyomatosis (LAM). We report a case of a patient with a newly defined entity classified as kaposiform lymphangiomatosis (KLA). This 50-year-old nonsmoking Hispanic woman presented with a 20-year history of cough, hemoptysis, chyloptysis, and pleuritic chest pain. Laboratory evaluation demonstrated a low norm

Strangulation of chronic transdiaphragmatic intercostal hernia.

Transdiaphragmatic intercostal hernia (TIH) caused by violent coughing is a rare clinical diagnosis. Most patients diagnosed with TIH have a chronic condition consisting of a hernia that can be reduced completely by surgical intervention. Our patient presented with acute abdomen resulting from mechanical bowel obstruction secondary to an incarcerated hernia. Acute TIH presents a diagnostic challenge because of its rarity and lack of specific signs or symptoms in the differential diagnosis of acute abdomen. We recommend performing diagnostic computed tomography (CT) early if there is suspicion

Changing pertussis epidemiology: everything old is new again.

Before vaccination, pertussis was a universal disease of early childhood. Although apparent control of the disease in the United States and other countries was achieved through vaccination, pertussis is resurgent. Though acellular vaccines have been in use for 20 years, new data are emerging on their effectiveness and durability of protection and the contribution of these characteristics to the resurgence of pertussis.

Progressive granulomatous pneumonitis in response to cosmetic subcutaneous silicone injections in a patient with HIV-1 infection: case report and review of the literature.

Silicone, commonly used in cosmetic procedures owing to its presumed inertness, can yield serious sequelae including acute embolization and pneumonitis. Chronic pulmonary sequelae in response to silicone injection have not been previously described. We report a case of chronic progressive granulomatous pneumonitis in response to subcutaneous silicone injections in a transgender male-to-female patient infected with human immunodeficiency virus 1 (HIV-1). After receiving silicone injections to the buttock, the patient developed rapid onset dyspnea, pleuritic chest pain, fever, and chills. Chest

Bronchogenic cysts as a cause of infantile stridor: case report and literature review.

INTRODUCTION: Cystic lesions related to the upper airway are an unusual cause of infantile stridor. Such a lesion may exert a mass effect, with subsequent airway compromise. CASE REPORT: A six-month-old boy was transferred to our unit with a right-sided, level IV neck lump and a three-month history of chronic cough and, latterly, inspiratory stridor. Computed tomography revealed a large, unilocular, cystic, cervicothoracic lesion causing marked compression of the trachea. Airway endoscopy subsequently revealed the larynx to be displaced to the left, with severe external compression of the tra

[Practical aspects of vaccination in elderly subjects].

The elderly subjects are at high risk of infection due to decreased immune responses and institutionalization. Studies show the effectiveness of influenza vaccination every year in subjects over 65 years, and pneumococcal vaccination every 5 years in patients with risk factors. Vaccinations against diphtheria low dose, tetanus, and polio should be renewed every 10 years. Pertussis should be catched-up in every adult not vaccinated since more than 10 years. Admission in institution, a pre-travel consultation and hospitalization for an infectious disease must be an opportunity to offer vaccinati

Primary pulmonary coccidioidomycosis in China.

Coccidioidomycosis is a fungal infection endemic to south-west USA, north Mexico and parts of Central and South America. We report here a case of primary pulmonary coccidioidomycosis in a previously healthy 14-year-old boy in China, which is considered a non-endemic country. The patient had non-specific symptoms of pulmonary infection, including fever, non-productive cough and night sweats. Both spherules and endospores of Coccidioides immitis were seen histologically following transbronchial biopsy of a cavitary lesion. The patient was treated with amphotericin B and fluconazole. Follow up 6

[Laryngeal tuberculosis in the patients of otholaryngology department--case reports].

Tubercular infection is still one of the most serious health and social problems. It's been estimated that one-third of the population is infected with Koch's bacillus. More than 90% of overall morbidity of tuberculosis in Poland pose pulmonary tuberculosis, in the worldwide level the percentage reaches 80. The most popular causes that spread the disease are famine, malnutrition, homelessness, limited availability to medical care, alcohol abuse, drug addiction, ageing of the society and more intensive migration. Among the cases of extrapulmonary tuberculosis the most common were certified as t

Assessment and validation of bronchodilation using the interrupter technique in preschool children.

OBJECTIVE: To determine and validate a cut-off value for bronchodilation using the interrupter resistance (Rint) in preschool children. PATIENTS AND METHODS: Rint was measured in 60 healthy children (age range 2.7-6.4 years) before and after salbutamol inhalation (200 microg). Four potential methods for assessing BDR were evaluated: percent change from baseline, percent change of predicted values, absolute change in Rint, and change in Z-score. These cut-off values, determined as the fifth percentile of the healthy group, were applied to children referred for the assessment of recurrent wheez

Organising pneumonia and lung cancer - case report and review of the literature.

Organising pneumonia (OP) is a distinct clinicopathological entity resulting from pulmonary reaction to noxious environmental or endogenous factors, but also idiopathic cases have been noted. Frequently, small foci of OP accompany lung cancer infiltrations. Also OP is sometimes a reaction to radio- or chemotherapy, but it is rarely a predominant lesion in the course of lung cancer. We present the case of 65-year-old patient who presented with fever, dry cough, exertional dyspnoea and pneumonic consolidation in the right lower lobe. Bronchoscopy revealed squamous carcinoma obstructing the right

Increased risk of chronic suppurative lung disease after measles or pertussis in non-vaccinated children.

We retrospectively analysed 23 children originating from Campania--Southern Italy--with recurrent pneumonia and chest high-resolution computed tomography (HRCT) proven bronchiectasis. In six patients (26%) who had not undergone measles or pertussis vaccination, recurrent pneumonia started after an episode of lower respiratory tract infection complicating pertussis (n=2) or measles (n=4), contracted at a mean age of 3.6 years. Thirty-three percent (2/6) and 67% (4/6) of the patients had bilateral or monolobar disease, respectively, while in two subjects (33%) bronchiectasis were found in all lo

The HOPE TIPS: the HOPE study translated into practices.

The HOPE TIPS study assessed the practicality and tolerability of ramipril titration to a target dose of 10 mg (as achieved in definitive efficacy studies) in a clinical practice setting. 3881 patients at high cardiovascular risk (HOPE study criteria) were recruited in primary and specialist care settings in 9 countries by 439 investigators. Dose titration of ramipril from 2.5 mg to 10 mg daily took place over 9-12 weeks. The mean age of the patients was 64 years, 60% were male and 79% Asian. The target dosage of 10 mg daily was reached in 73% of patients with 96% of patients achieving 5 mg or
