The legacy of MeNZB and possible implications for COVID-19 vaccination.

It is now over a decade since the meningococcal B vaccine, MeNZB, was in routine use in New Zealand. From July 2004 until June 2008 it was administered in a three-dose schedule to over a million individuals, aged six weeks to 20 years, to provide protection against the epidemic strain of group B Meningococci. The cost of the campaign, including the development of the vaccine was substantial, in excess of $200M, but it contributed to a reduced incidence of meningococcal infections along with a reduction in morbidity and mortality. The campaign led to the development of a national immunisation r

Comparison of risk-based versus random sampling in the monitoring of antimicrobial residues in Danish finishing pigs.

In Denmark, a monitoring program for residues of antimicrobials in pork is in place involving annual testing of around 20,000 samples from finishing pigs corresponding to 0.1% of the animals slaughtered. Annually, zero to two samples are found above the maximum residue limit. Both authorities and industry have expressed interest in adjusting the monitoring to a risk-based system. The objective of this study was to assess the opportunities and consequences of the monitoring considering: 1) replacing the current bioassay with high-performance liquid chromatography-mass spectrometry (HPLC LC-MS/M

Hydroxyurea-induced foot ulcer in a case of essential thrombocythaemia.

OBJECTIVE: To illustrate the association between hydroxyurea and the development of ulcers. METHOD: A case study is presented, in which histological changes, mean corpuscular volume (MCV) and transcutaneous oxygen tension (TcPO2) were all measured and analysed, both during hydroxyurea treatment and following it's discontinuation. RESULTS: Two months following the cessation of hydroxyurea therapy, the patient's ulcer had healed completely. Biopsy specimens taken before and after its discontinuation showed a considerable improvement in vascularity, with a capillary density 6.28 times higher af

An attempt to estimate the global burden of disease due to fluoride in drinking water.

A study was conducted to examine the feasibility of estimating the global burden of disease due to fluoride in drinking water. Skeletal fluorosis is a serious and debilitating disease which, with the exception of one area in China, is overwhelmingly due to the presence of elevated fluoride levels in drinking water. The global burden of disease due to fluoride in drinking water was estimated by combining exposure-response curves for dental and skeletal fluorosis (derived from published data) with model-derived predicted drinking water fluoride concentrations and an estimate of the percentage po

Toward a National System of Expanded Testing of Existing Urine Specimens: The Drug Outbreak Testing Service (DOTS).

BACKGROUND: NIDA's National Drug Early Warning System (NDEWS) was established in 2014 with the mission of identifying and monitoring emerging drugs in the United States. Identification of emerging drugs has been complicated, however, by the rapid development of novel psychoactive substances such that users often cannot identify and report the drugs they have ingested. Biologic testing of urine, hair or blood is the only way to reliably identify the substances recently used. Unfortunately, the large number of up-to-date tests required is beyond the resources available to most organizations. ME

Non-invasive detection of drug toxicity in rats by solid-phase extraction and MALDI-TOF analysis of urine samples.

An access to fast and non-invasive techniques to infer or predict the drug-induced injury caused by newly developed drugs and to monitor therapeutic efficacy of established drugs during treatment are of the outmost importance in pharmaceutical industry and clinical diagnosis. Peptidome and low molecular weight proteome profiling is an emerging technique that allows the recognition of distinctive patterns and differentiation among diverse physiopathological conditions. In this article, we evaluated the utility of peptide/small protein profiling using matrix-assisted laser desorption/ionization

Drug-induced parkinsonism.

Drug-induced parkinsonism (DIP) is the second cause of akinetic rigid syndrome in the Western world and its prevalence is increasing and approaching that of idiopathic Parkinson's disease due to the ageing of the population and to the rising of polypharmacotherapy. DIP was initially reported as a complication of neuroleptics in psychiatric patients, but it has also been described with a great diversity of compounds such as antiemetics, drugs used for the treatment of vertigo, antidepressants, calcium channel antagonists, antiarrythmics, antiepileptics, cholinomimetics and other drugs. Controlled hypotension for spinal surgery.

Controlled, deliberate hypotension during anesthesia for major spinal surgery reduces intraoperative blood loss and transfusion requirement. Hypotension may be achieved with increased doses of volatile anesthetic agents or by continuous infusion of vasodilating drugs. Safe application of this technique requires knowledge of the physiology of hemorrhagic shock and close intraoperative monitoring to avoid vasoconstriction and end-organ ischemia.

Smart Additive-Manufactured Humanoid Electronic-Skin Model Based One-Drop-Touch Electrochemical Sensing of the Alprazolam Drug.

This research article analyzes the state-of-the-art technologies of bio-inspired, next-generation humanoid electronic skin (e-skin) electrochemical aptasensors developed for the detection of alprazolam. Current wearable devices suffer from poor flexibility, low durability, limited sensitivity, and discomfort during long-term use, thereby necessitating the development of sensors for flexible, sensitive, and real-time health monitoring applications. Here, 3D-printed flexible humanoid e-skin (silicon sheet) mimics human skin and holds the potential to replicate realistic skin for wearable and art

Prevention of symptomatic recurrences of paroxysmal atrial fibrillation in patients initially tolerating antiarrhythmic therapy. A multicenter, double-blind, crossover study of flecainide and placebo with transtelephonic monitoring. Flecainide Supraventricular Tachycardia Study Group.

Paroxysmal atrial fibrillation (PAF) is a problematic clinical arrhythmia that is usually symptomatic. Unfortunately, few adequate trials and trial methods are available for assessment of the value of therapy, and traditional treatment has often been ineffective or associated with unacceptable side effects. Transtelephonic monitoring is a new method that allows evaluation of paroxysmal arrhythmias and arrhythmia-related symptoms in outpatients. We used a patient-initiated transtelephonic monitor system to evaluate the potential of flecainide, a class 1C antiarrhythmic, in prevention of symptom

The Case for Prescribing Metformin Off-Label for a Child in Remission from Ependymoma.

Pediatrics often faces ethical challenges when families request unproven therapies. We respond to a clinical case described in "A Parental Request for an Unproven Treatment for a Rare Pediatric Cancer: Sound Reasons for Not Going Off-Label," by Margot Hedlin and Louis Voigt, in the same issue of this journal. The case concerns Natalie, a three-year-old in remission from ependymoma, whose parents requested metformin to prevent recurrence. Although Natalie's neuro-oncologist opposed the use of the drug, its favorable safety profile, pediatric Food and Drug Administration approval for other indic

An efficient natural product-derived theranostic fluorescent probe for carboxylesterase bioimaging and activatable therapy of colon tumors.

Natural products have demonstrated significant potential in the treatment of many diseases. However, designing a diagnostic and therapeutic integrated system derived from natural products still poses significant challenges. This work proposes a drug delivery strategy based on natural products, which involves linking natural products to reactive moieties of tumor markers. Herein, a novel fluorescent probe (S-CES) for carboxylesterase 2 (CES2) imaging and therapy of colon tumor has been formulated and put to successful use both in vitro and in vivo. The probe was designed by combining trimethyla

Pharmacological Management and Longitudinal Cognitive, Psychiatric, and Behavioral Changes During Stem Cell Therapy in a Patient With Mesial Temporal Sclerosis: A Case Report.

Polypharmacy is common in epilepsy, particularly in patients with psychiatric comorbidities requiring concomitant pharmacological treatment. Long-term treatment often increases the risk of adverse drug reactions and pharmacological interactions. Stem cell therapy has emerged as a topic of growing interest in neurology because of its immunomodulatory and neuromodulatory properties. We report the case of a 15-year-old female patient with mesial temporal sclerosis characterized by marked cognitive and psychiatric comorbidities requiring a complex five-drug pharmacological regimen. The patient und

[Assessing the risk-benefit balance of medicines: Some lessons from Covid-19].

The efficacy of medications is evaluated by clinical trials. However, such trials are not designed to assess adverse effects, particularly when those are rare. A robust pharmacovigilance system is therefore required to assess risks, supplemented as requested by pharmacoepidemiology studies. The benefits of medications are expressed in either relative or absolute terms and they vary depending on the baseline risk of the disease (its severity, potential complications, progression, and its incidence for the population-level benefits). This is not the case for adverse effects, which are influenced

Decoding the Neural Circuit Underlying Context-Cue Association in Retrieval of Morphine Reward Memory.

Context-induced relapse is a major challenge in treating drug use disorders, yet the neural circuits underlying drug memory retrieval remain unclear. A glutamatergic projection from the ventral CA1 (vCA1) to the dorsomedial prefrontal cortex (dmPFC) is identified as a critical pathway for morphine reward memory retrieval. In vivo monitoring and ex vivo electrophysiology show that vCA1 inputs engage distinct dmPFC neuronal populations and differentially regulate local parvalbumin (PV+) and somatostatin (SST+) interneurons, thereby reshaping cortical output during memory retrieval. Pathway-speci
