Obesity Is Associated With More Complications and Longer Hospital Stays After Orthopaedic Trauma.

OBJECTIVE: The objective of this study was to characterize relationships between obesity and initial hospital stay, including complications, in patients with multiple system trauma and surgically treated fractures. DESIGN: Prospective, observational. SETTING: Level 1 trauma center. PATIENTS: Three hundred seventy-six patients with an Injury Severity Score greater than 16 and mechanically unstable high-energy fractures of the femur, pelvic ring, acetabulum, or spine requiring stabilization. MAIN OUTCOME MEASUREMENTS: Data for obese (body mass index ≥ 30) versus nonobese patients included pr

Modified application of a wound retractor for surgery in chest trauma.

Wound retraction is useful for chest wall surgery involving surgical rib fixation in patients with rib fractures. However, blunt chest trauma with rib fractures frequently involves lung injury, requiring simultaneous pulmonary repairs. In intrapleural surgery for chest trauma involving rib fractures, a rib spreader could cause additional rib fractures. Therefore, we describe the modified application of a second wound retractor for surgery in the thorax and chest wall, and discuss its advantages and disadvantages. We call this method double-wound retraction.

MRI findings of bilateral proximal radial physeal injury in a gymnast.

Recent advances in imaging techniques shed light on anatomical variants considered normal historically in medical literature. We present an 11-year-old gymnast with unilateral left elbow pain and unusual symmetrical radiographic findings of both elbows that initially raised the question of normal variant cleft epiphysis of the proximal radius. Further imaging with MRI demonstrated bilateral elbow injury that is likely repetitive and chronic rather than an anatomical variant. Post-treatment MRI showed improved changes with rest and non-steroidal anti-inflammatory medications. These findings emp

Fracture dislocation of carpometacarpal joints: a missed injury.

Fracture dislocation of the carpometacarpal joints on the ulnar side of the hand is an uncommon injury. These are high-energy injuries seen in motorcyclists and boxers. The mechanism of injury involves violent, forceful dorsiflexion of the wrist combined with longitudinal impact on the closed hand. This article reports a case of fracture of the base of the middle finger with dislocation of the ring and little finger carpometacarpal joints. On first examination, a diagnosis of isolated, minimally-displaced, middle-metacarpal base fracture was made and deemed suitable for nonoperative management

Bilateral low-energy simultaneous or sequential femoral fractures in patients on long-term alendronate therapy.

BACKGROUND: While alendronate therapy has been shown to decrease the risk of vertebral and femoral neck fractures in postmenopausal osteoporotic patients, recent reports have associated long-term alendronate therapy with unilateral low-energy subtrochanteric and diaphyseal femoral fractures in a small number of patients. To our knowledge, there has been only one report of sequential bilateral femoral fractures in patients on long-term bisphosphonate therapy. METHODS: We retrospectively reviewed the case log of the senior author over the last four years to identify patients who presented with

Bilateral musculocutaneous nerve palsy. A case report.

A case of bilateral, isolated, proximal musculocutaneous nerve palsy is reported. Initial physical and electromyographic examinations demonstrated complete denervation of the biceps brachii, brachialis, and coracobrachialis muscles bilaterally. Nerve conduction studies of the musculocutaneous nerves initially revealed no evoked potentials. The patient underwent resection with end-to-end anastomosis on the left and neurolysis without resection on the right. Significant functional recovery was noted bilaterally but was more rapid and complete on the side that underwent neurolysis without resecti

Lumbosacral monolateral dislocation.

The authors describe a case of traumatic monolateral dislocation of L5 on S1. It is a very rare lesion which involves serious diagnostic and therapeutic problems. Surgical treatment constituted by reduction and stabilization alone can obtain stable correction of the deformity and complete functional recovery.

Pediatric pelvic fractures combined with an additional skeletal injury is an indicator of significant injury.

Traumatic fractures of the adult pelvic area have high rates of associated injuries and hemorrhagic complications resulting in significant morbidity and mortality. We studied whether or not the plain roentgenographic identification of other fractures is a reliable marker of increased morbidity and mortality in children with pelvic fractures. The records of 79 children admitted with the diagnosis of pelvic fracture from January 1986 through December 1990 were reviewed. Associated injuries were categorized as head, chest, abdomen or skeletal. The pelvic fractures were grouped using the classific

[The diagnostic and treatment characteristics of combined injuries to the spine and chest cavity].

The author has analysed the combined injuries of the spinal column and the chest in 46 (2.19%) patients. In the treatment of the spinal injuries conservative (combined and functional) and surgical (in the posterior and in the anterior compartments) methods were employed differentially which allowed to reduce the period of treatment of the patients and to restore the working capacity.

Three-dimensional computed tomography in evaluation of occipital condyle fracture.

Fracture of the occipital condyle is a rare sequela of craniocervical trauma. The diagnosis can be suspected in an injured patient with disproportionate torticollis, uncertain evidence of subluxation, and lower cranial nerve symptoms. Standard radiographic diagnosis of this fracture is difficult owing to its superimposition on other bony structures. Film tomography or computed tomography (CT) is usually required to identify the fracture. We report a case of occipital condyle fracture initially masked by atlantoaxial subluxation, but then best demonstrated by three-dimensional CT.

Prevalence and Patterns of Multiple Injuries Affecting a Single Permanent Tooth (Combination Injuries): A Systematic Review and Meta-Analysis.

BACKGROUND: Combination injuries or multiple traumatic dental injuries (TDI) affecting a single permanent tooth are clinically significant but remain underrecognized. Although the global prevalence of TDI is well documented, the epidemiology of combination injuries has not been clearly established. This systematic review and meta-analysis aimed to assess the prevalence and patterns of combination injuries in traumatized permanent teeth. METHODS: A review protocol was registered in PROSPERO (CRD1245967). Comprehensive searches of PubMed, EMBASE, Web of Science, and Scopus were conducted withou

Osteoporotic vertebral compression fracture in patients with childhood-onset lupus nephritis: prevalence and risk factors.

UNLABELLED: Vertebral fractures (VF) can occur due to childhood-onset lupus nephritis (cLN), which needs long-term use of immunosuppressants that affect bone health. This study found VF in 26.7% of cLN patients. Risk factors included low lumbar spine BMD Z-scores and high disease activity (SLEDAI-2K ≥ 20). Bone-strengthening and disease management strategies are important. PURPOSE: Pediatric vertebral fracture (VF) may occur under conditions that lead to decreased bone mass. Childhood-onset lupus nephritis (cLN) requires treatment with glucocorticoids and immunosuppressants. This study aimed

Inferior vena cava syndrome following percutaneous vertebroplasty with polymethylmethacrylate.

STUDY DESIGN: A case of inferior vena cava syndrome following percutaneous vertebroplasty is described herein. OBJECTIVE: To alert clinicians to the potential occurrence of inferior vena cava syndrome following percutaneous vertebroplasty. SUMMARY OF BACKGROUND DATA: Vertebroplasty is a less invasive treatment solution for the osteoporotic compression fracture. There complications of the cement leakage would appear to have been rather infrequent. We report a case of inferior vena cava syndrome related to the cement leakage. METHODS: A 59-year-old woman underwent percutaneous vertebroplasty

It's Just a Fingertip! Yet Controversy Exists: Standardizing a Treatment Pathway for Pediatric Fingertip Injuries.

BACKGROUND: Pediatric fingertip injuries are very common; however, treatment varies greatly depending on geographic location and surgeon. A paucity of data exists on how emergency department practitioners and hand surgeons should treat these injuries. We sought to develop consensus around treatment guidelines for these injuries. METHODS: We conducted a Delphi survey of self-identified pediatric hand surgeons. Participants answered care delivery questions about 11 scenarios of fingertip injuries. These included minor (e.g., <50% subungual hematomas without fractures) to severe injuries (e.g.,

Compartment-specific effects of muscle strength on bone microarchitecture in women at high risk of osteoporosis.

BACKGROUND: It is well known that skeletal integrity is influenced by the musculature. Poor muscle strength (i.e. sarcopenia) is considered a major predictor of fragility fractures. While this observation appears particularly relevant for older women with increased risk of osteoporosis, there has been no comprehensive investigation to determine the influence of muscle performance on compartment-specific bone microarchitecture in multiple body regions. METHODS: We retrospectively analysed data from different muscle performance and bone microarchitecture assessments in 230 women (aged 21 to 87

Early surgical site infection in adult appendicular skeleton trauma surgery: a multicenter prospective series.

INTRODUCTION: Surgical site infections (SSI) studies rely on an imprecise and debatable definition. The term "wound healing problems" (WHP), not necessarily septic, is also frequently cited. This study had the objectives of determining the frequency of early SSIs in traumatology, these terms eventual correlation, and the factors influencing onset. PATIENTS AND METHODS: A multicenter prospective observational study was conducted in 12 centers. The exclusion criteria were open lesions as well as multiple injuries and multiple fractures (more than two fractures treated surgically). All patients
