The effects of Okra supplementation on blood pressure, lipid profile, and inflammation in patients with prediabetes, type 2 diabetes, and diabetic nephropathy: a meta-analysis and systematic review.

Cardiovascular disease (CVD) is a leading cause of morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Okra (Abelmoschus esculentus), a traditional plant rich in fiber, flavonoids, and antioxidants, has been suggested to improve glycemic control, lipid metabolism, and inflammation, but clinical evidence remains inconsistent. The major objective of our review was to elucidate the impact of okra supplementation on blood pressure, lipid profile, and inflammation in patients with prediabetes, T2DM, and diabetic nephropathy. Eligible studies were identified through PubMed, Emb

Magel2 deficiency promotes cardiac remodeling and increases arrhythmogenic susceptibility in a mouse model relevant to Prader-Willi and Schaaf-Yang syndromes.

Prader-Willi syndrome (PWS) and Schaaf-Yang syndrome (SYS) share overlapping phenotypic features, but potential cardiac involvement in both conditions remains poorly understood. Here, we investigated cardiac function in Magel2 knockout (KO) mice, a model relevant to PWS and SYS, to assess the impact of Magel2 deficiency on the heart. Echocardiographic analysis of 20-week-old Magel2-KO mice revealed concentric remodeling of the left ventricle together with reduced left ventricular end-diastolic volume and stroke volume, as well as a modest but significant reduction in ejection fraction. Maternal cardiometabolic health and the role of GLP-1 receptor agonists from preconception to postpartum: A review of evidence and opportunities.

BACKGROUND: Cardiovascular disease (CVD) is a leading cause of maternal mortality. Adverse pregnancy outcomes (APOs), including preeclampsia and gestational diabetes, confer significant long-term cardiometabolic risk, yet prevention strategies during the preconception and postpartum periods are limited. Glucagon-like-peptide 1 receptor agonists and/or combined therapies (GLP-1 agonists) have demonstrated substantial benefits in weight reduction, glycemic and lipid control, and cardiovascular risk reduction in nonpregnant populations, but their impact on cardiometabolic health in women with inc

High-value laboratory testing in erectile dysfunction, reduced sexual desire, and suspected hypogonadism: analytical pitfalls and selective biomarker use.

In men presenting with erectile dysfunction, reduced sexual desire, or suspected hypogonadism, laboratory work-up frequently oscillates between indiscriminate hormone panels and underuse of high-yield metabolic testing. From a clinical biochemistry perspective, the key question is not how many biomarkers can be ordered, but which tests are analytically fit for purpose and likely to change management. This evidence-informed narrative review examines biomarkers commonly used in erectile dysfunction and androgen-deficiency phenotypes, with emphasis on glycemic biomarkers, lipids, testosterone, se

Sodium-Glucose Cotransporter 2 Inhibitors in Lupus Nephritis and ANCA-Associated Vasculitis: Unmet Needs to be Addressed.

BACKGROUND: Current treatment of severe autoimmune glomerulonephritis such as lupus nephritis (LN) and anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) relies primarily on immunosuppression to reduce kidney inflammation and chronic damage. However, many patients exhibit persistent proteinuria despite treatment, which serves as a prognostic biomarker for long-term outcomes. The residual proteinuria after 12 months might reflect hyperfiltration in remaining nephrons due to chronic damage rather than active inflammation, contributing to progressive kidney function deteriora

Acute Small Bowel Obstruction Induced by Tirzepatide (Mounjaro).

Tirzepatide (Mounjaro), a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, has gained widespread adoption for the management of type 2 diabetes mellitus and obesity owing to its robust efficacy in glycemic control and weight reduction. While gastrointestinal adverse effects, including nausea, vomiting, constipation, and delayed gastric emptying, are well recognized, acute small bowel obstruction represents a rare and poorly characterized complication. We report a case of a 50-year-old woman with insulin-dependent type 2 diabetes mell

The ApoA-IV-LRP1 Signaling Axis: A Novel Insulin-Independent Pathway for the Suppression of Diabetic Hyperglucagonemia.

Apolipoprotein A-IV (ApoA-IV) is a glycoprotein secreted by the small intestine to regulate lipid metabolism and satiety. Its role in insulin-independent glucose homeostasis remains largely unknown. In this study, we demonstrate that intestinal ApoA-IV overexpression significantly attenuates diet-induced obesity and hyperglycemia following severe β-cell loss. Over a 20-week high-fat diet challenge, ApoA-IV transgenic (ApoA-IV-Tg) mice maintained significantly lower adiposity than wild-type controls, driven by elevated energy expenditure and fatty acid oxidation rather than reduced caloric inta

Ketogenic diet, cardiometabolic diseases and aging.

The ketogenic diet (KD), characterized by a high-fat, moderate-protein, and low-carbohydrate macronutrient composition, has gained growing interest as a potential nutritional approach to cardiometabolic diseases and aging. Emerging evidence suggests that ketone bodies, particularly β-hydroxybutyrate, act not only as alternative energy substrates but also as signaling molecules that influence vascular, metabolic, and epigenetic pathways. This review summarizes current knowledge on the cardiovascular and metabolic implications of KD, emphasizing endothelial function, cardiac energy metabolism, l

Meta-analysis of sodium glucose transport 2 inhibitors versus other glucose-lowering agents on the progression of diabetic retinopathy and diabetic macular edema.

OBJECTIVE: While glycemic control reduces the progression of diabetic retinopathy (DR), the effects of specific glucose-lowering drugs (GLD) remain incompletely defined. Emerging evidence suggests that sodium-glucose cotransporter‑2 inhibitors (SGLT2i) may confer protective effects on DR and diabetic macular edema (DME). RESEARCH DESIGN AND METHODS: We reviewed human studies comparing SGLT2i versus other GLD with primary outcomes of DR or DME. Random effects meta-analyses and meta-regression were performed to estimate relative risks (rr) and risk differences (rd) for retinopathy progression.

Improving Patient Outcomes by Bridging Bariatric and Body Contouring Surgery with Comprehensive Prehabilitation.

BACKGROUND: Rates of obesity and diabetes are increasing in the United States, accompanied by greater use of bariatric surgery and pharmacologic therapies, including glucagon-like peptide-1 receptor agonists and glucose-dependent insulinotropic polypeptide-based agents. Although massive weight loss may improve glycemic control and blood pressure, patients seeking body contouring surgery remain medically complex. Reduced fat-free mass associated with weight-loss pharmacotherapy and the high incidence of wound healing complications after excess skin resection present additional perioperative cha

Connecting sugar and fibrosis: Diabetes as a hidden player in rheumatoid arthritis-associated interstitial lung disease.

Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease that extends beyond joint inflammation, affecting pulmonary and metabolic pathways. Interstitial lung disease (ILD) is one of its most serious extra-articular complications, while type 2 diabetes mellitus (T2DM) frequently coexists with RA and may exacerbate inflammatory and fibrotic processes. This editorial discusses the study by Sutton et al, the largest population-based analysis to date exploring the link between T2DM and ILD in patients with RA, and reflects on its mechanistic and clinical implications. Where the Future Lies: GLP-1 Receptor Agonists as Therapeutics for Dermatologic Comorbidities.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have transformed the management of obesity, type 2 diabetes mellitus, and cardiometabolic disease through effects on weight reduction, glycemic control, and systemic inflammation. Their varied immunometabolic actions have generated increasing interest in dermatology, where chronic inflammatory skin diseases frequently coexist with obesity, insulin resistance, metabolic syndrome, and cardiovascular disease. Emerging evidence from clinical trials, observational studies, and real-world analyses suggests that GLP-1RAs may improve outcomes in pso

Gut Microbiota and Diabetic Complications: Potential Mechanisms, Microbial Signatures, and Clinical Implications.

Type 2 diabetes mellitus is a systemic metabolic disorder with an extensive spectrum of complications, which still persist despite improvements in glycemic control. Emerging evidence suggests that gut dysbiosis may be an underpinning factor in the pathogenesis of both microvascular and macrovascular complications associated with diabetes. This narrative review explores the relationship between gut microbiota and the development of diabetes complications, including nephropathy, retinopathy, neuropathy, cardiovascular, cerebrovascular, peripheral vascular, and reproductive system disorders.
