Initiating Preventive Care for Hyperlipidemia in the Emergency Department: The Emergency Medicine Cardiovascular Risk Assessment for Lipid Disorders Trial.

BACKGROUND: Hyperlipidemia (HLD) is a major contributor to atherosclerotic cardiovascular disease (ASCVD). Nearly 30% of emergency department (ED) patients with chest pain have undiagnosed and/or unmanaged HLD, putting them at an increased risk of ASCVD. Although safe and effective HLD treatments exist, the ED traditionally focuses on acute care and does not offer preventive cardiovascular care services. This represents a large, missed opportunity to improve cardiovascular health for the millions of Americans evaluated in the ED each year who are not receiving appropriate preventive care in th

Contextual and culturally adapted interventions to improve HIV outcomes: a scoping review.

BACKGROUND: Antiretroviral therapy is widely available, yet structural, social, and cultural barriers still limit human immunodeficiency virus prevention and care. Prevention tools such as consistent condom use and pre-exposure prophylaxis (PrEP) remain essential. Interventions that ignore local context and culture often have low uptake, especially among adolescents, Indigenous communities, and sexual minorities. OBJECTIVE: To map and synthesize contextually and culturally adapted interventions that aim to improve outcomes related to human immunodeficiency virus. METHOD: This scoping review

Short and long-term follow-up and clinical outcomes in patients with celiac disease in a large private practice setting.

BACKGROUND: Celiac disease (CD) is caused by an immune response to gluten and treatment is adherence to a gluten-free diet. Guidelines from studies in large academic settings recommend registered dietitian (RD) referrals at time of diagnosis and periodic testing for micronutrient deficiencies. There is limited data to guide follow-up parameters in a large, community-based practice. The purpose of this study was to evaluate guideline adherence in this setting. METHODS: This retrospective study conducted in 2019 assessed CD care based on follow-up rates, micronutrient testing, symptoms, and ser

Developing a novel tool to assess the ability to self-administer medication in non-demented in-hospital patients: ABLYMED study protocol.

BACKGROUND: Older people often suffer from multimorbidity resulting in polypharmacy. The correct administration of medication is a crucial factor influencing treatment efficacy. However, tools for evaluating the ability to self-administer different dosage forms of medications are lacking. The objectives of the ABLYMED study are to 1) assess the ability to self-administer different dosage forms of medication in older non-demented in-hospital patients who report autonomous management of medication, 2) identify factors influencing the ability to self-administer medication, and 3) develop a standa

A Summary of the Evidence for Managing Exercise Phobia in Patients After Anterior Cruciate Ligament Reconstruction Surgery.

BACKGROUND: In recent years, the incidence of anterior cruciate ligament (ACL) injuries has risen persistently. Notably, some patients develop movement fear after ACL reconstruction surgery. This psychological issue significantly reduces patients' exercise rehabilitation adherence and active participation willingness. It also exacerbates family caregiving pressures and healthcare systems' socioeconomic burden. Thus, it has become a critical factor affecting postoperative rehabilitation outcomes and public health resource allocation. METHODS: Using the "6S" evidence resource pyramid model, we

A Multicenter, Mixed Methods Study of United States Heart Teams and Health Policy: The EVOLVE Study Design and Protocol.

BACKGROUND: The management of patients with aortic stenosis is rapidly evolving, along with the "heart team," a multidisciplinary group collaborating in program management. There is scant research providing rigorous data on the variability in heart teams' approach to optimal patient care delivery. This investigation aimed to describe the current health care delivery strategies and attitudes of contemporary heart teams at diverse hospitals across the United States and provide key insight into future health policy and guideline directions. METHODS: We will conduct a convergent parallel design m

Korean consensus-based recommendations for preanesthetic gastric ultrasound and anesthetic considerations for GLP-1 receptor agonist-based therapy: a consensus statement from Clinical Practice Guideline Committee of the Korean Society of Anesthesiologists.

Pulmonary aspiration during anesthesia remains an uncommon but clinically significant perioperative complication associated with substantial morbidity and mortality. Although contemporary fasting guidelines recommend minimum fasting intervals to reduce aspiration risk, adherence does not guarantee an empty stomach, particularly in patients with delayed gastric emptying or those receiving medications that impair gastrointestinal motility, such as glucagon-like peptide-1 receptor agonist (GLP-1 RA)-based therapies. Preanesthetic gastric ultrasound has emerged as a practical point-of-care modalit

Long-term outcomes in recurrent diverticulitis: a systematic review of treatment strategies and recurrence rates.

BACKGROUND: Diverticulitis prevalence is on the rise, with recurrences occurring in up to one-third following an uncomplicated initial episode. While sigmoid resection was historically routine after a second episode, current guidelines recommend individualization. This review evaluates long-term outcomes for recurrent diverticulitis, comparing different treatment approaches. METHODS: A systematic review (PROSPERO ID CRD42024600674) adhering to PRISMA guidelines searched several electronic databases. Clinical trials and observational studies published in English over the past decade that repor

Cystic Fibrosis-Related Bone Disease in Youth: High-Risk Population With Low Screening Adherence.

OBJECTIVES: Cystic fibrosis (CF)-related bone disease (CFBD) is characterized by low bone mineral density and increased fracture risk. The U.S. CF Foundation recommends using dual-energy x-ray absorptiometry (DXA) to screen all adults and at-risk children 8 to <18 years. We aimed to establish the proportions of children who meet CFBD screening criteria, are ordered a DXA, and complete DXA screening. METHODS: The medical charts of children seen in our pediatric CF clinic in 2022 were reviewed to identify who met criteria for CFBD screening by DXA per CF Foundation guidelines: prior fracture, d

Lumpectomy Patients are at Highest Risk for Opioid Overprescription: A Comparison Between Practice Patterns and OPEN National Guidelines.

BACKGROUND: Nearly 25% of opioid-related deaths are from prescribed opioids, and the exacerbation of the opioid epidemic by the coronavirus disease 2019 (COVID-19) pandemic underscores the urgent need to address superfluous prescribing. Therefore, we sought to align local opioid prescribing practices with national guidelines in postoperative non-metastatic breast cancer patients. METHODS: A single-institution analysis included non-metastatic breast surgery patients treated between April 2020 and July 2021. 'Overprescription' was defined as a discharge prescription quantity of oral morphine eq

Compliance to recommended intra-partum management protocols of high-risk pregnancies among mothers in the Sunderbans: A mixed method study.

BACKGROUND: During the intra-partum period, high-risk pregnancies are particularly susceptible, and without proper management, they can escalate into maternal deaths or severe complications known as maternal near-misses. Despite the availability of various national guidelines aimed at managing these high-risk situations, adherence to these protocols is notably low among mothers with high-risk intra-partum conditions. OBJECTIVES: This study aims to evaluate the compliance with recommended guidelines when referring high-risk pregnancies during labour, particularly in the challenging regions of

Determining the impact of an artificial intelligence tool on the management of pulmonary nodules detected incidentally on CT (DOLCE) study protocol: a prospective, non-interventional multicentre UK study.

INTRODUCTION: In a small percentage of patients, pulmonary nodules found on CT scans are early lung cancers. Lung cancer detected at an early stage has a much better prognosis. The British Thoracic Society guideline on managing pulmonary nodules recommends using multivariable malignancy risk prediction models to assist in management. While these guidelines seem to be effective in clinical practice, recent data suggest that artificial intelligence (AI)-based malignant-nodule prediction solutions might outperform existing models. METHODS AND ANALYSIS: This study is a prospective, observational
