Changes in intracranial venous blood flow and pulsatility in Alzheimer's disease: A 4D flow MRI study.

Cerebral blood flow, arterial pulsation, and vasomotion may be important indicators of cerebrovascular health in aging and diseases of aging such as Alzheimer's disease. Noninvasive markers that assess these characteristics may be helpful in the study of co-occurrence of these diseases and potential additive and interacting effects. In this study, 4D flow MRI was used to measure intra-cranial flow features with cardiac-gated phase contrast MRI in cranial arteries and veins. Mean blood flow and pulsatility index as well as the transit time of the peak flow from the middle cerebral artery to the

Factors influencing time-location patterns and their impact on estimates of exposure: the Multi-Ethnic Study of Atherosclerosis and Air Pollution (MESA Air).

We assessed time-location patterns and the role of individual- and residential-level characteristics on these patterns within the Multi-Ethnic Study of Atherosclerosis and Air Pollution (MESA Air) cohort and also investigated the impact of individual-level time-location patterns on individual-level estimates of exposure to outdoor air pollution. Reported time-location patterns varied significantly by demographic factors such as age, gender, race/ethnicity, income, education, and employment status. On average, Chinese participants reported spending significantly more time indoors and less time

"Is That A Method of Birth Control?" A Qualitative Exploration of Young Women's Use of Withdrawal.

Despite its ubiquity, withdrawal is understudied as a family planning method. We investigated the context of and decision making around withdrawal use, drawing on in-depth, qualitative interviews with 38 Black and Latina women (ages 18 to 24). We examined contraceptive use histories to understand when and why participants used withdrawal. The majority of participants (n = 29; 76%) had used withdrawal in their lifetimes, though two-thirds of users mentioned withdrawal in their contraceptive histories only after interviewer prompts. Withdrawal was primarily used during transitions between contra

The Evolving Medical and Veterinary Importance of the Gulf Coast tick (Acari: Ixodidae).

Amblyomma maculatum Koch (the Gulf Coast tick) is a three-host, ixodid tick that is distributed throughout much of the southeastern and south-central United States, as well as several countries throughout Central and South America. A considerable amount of scientific literature followed the original description of A. maculatum in 1844; nonetheless, the Gulf Coast tick was not recognized as a vector of any known pathogen of animals or humans for >150 years. It is now identified as the principal vector of Hepatozoon americanum, the agent responsible for American canine hepatozoonosis, and Ricket

Crystal structures of Plasmodium falciparum cytosolic tryptophanyl-tRNA synthetase and its potential as a target for structure-guided drug design.

Malaria, most commonly caused by the parasite Plasmodium falciparum, is a devastating disease that remains a large global health burden. Lack of vaccines and drug resistance necessitate the continual development of new drugs and exploration of new drug targets. Due to their essential role in protein synthesis, aminoacyl-tRNA synthetases are potential anti-malaria drug targets. Here we report the crystal structures of P. falciparum cytosolic tryptophanyl-tRNA synthetase (Pf-cTrpRS) in its ligand-free state and tryptophanyl-adenylate (WAMP)-bound state at 2.34 Å and 2.40 Å resolutions, respectiv

Homeless aging veterans in transition: a life-span perspective.

The need for counseling and career/educational services for homeless veterans has captured political and economic venues for more than 25 years. Veterans are three times more likely to become homeless than the general population if veterans live in poverty or are minority veterans. This mixed methods study emphasized a life-span perspective approach for exploring factors influencing normative aging and life-quality of 39 homeless veterans in Alabama and Florida. Seven descriptive quantitative and qualitative research questions framed the investigation. Study participants completed a quantitati

The after discharge care management of low income frail elderly (AD-LIFE) randomized trial: theoretical framework and study design.

Interdisciplinary care management is advocated for optimal care of patients with many types of chronic illnesses; however, few models exist that have been tested using randomized trials. The purpose of this report is to describe the theoretical basis for the After Discharge Management of Low Income Frail Elderly (AD-LIFE) trial, which is an ongoing 2-group randomized trial (total n = 530) to test a chronic illness management and transitional care intervention. The intervention is based on Wagner's chronic illness care model and involves comprehensive posthospitalization nurse-led interdiscipli

Structural mechanism for the carriage and release of thyroxine in the blood.

The hormones that most directly control tissue activities in health and disease are delivered by two noninhibitory members of the serpin family of protease inhibitors, thyroxine-binding globulin (TBG) and corticosteroid-binding globulin. The structure of TBG bound to tetra-iodo thyroxine, solved here at 2.8 A, shows how the thyroxine is carried in a surface pocket on the molecule. This unexpected binding site is confirmed by mutations associated with a loss of hormone binding in both TBG and also homologously in corticosteroid-binding globulin. TBG strikingly differs from other serpins in havi

Transdermal progesterone cream as an alternative progestin in hormone therapy.

OBJECTIVE: To evaluate the endometrial effects and determine patients' acceptance of transdermal progresterone cream compared to standard hormone therapy. METHODS: Healthy menopausal women were recruited and received a pretreatment endometrial biopsy (EMB). They were randomized to 0.625 mg conjugated equine estrogen (CEE) daily and 2.5 mg medroxyprogesterone acetate (MPA) (Prempro, Wyeth USA) or daily 0.625 mg CEE and twice daily 20 mg transdermal PC (Pro-gest, Transitions for Health USA). At the end of 6 months, a repeat EMB was obtained, and the women were crossed over to other treatment. A

Care of adults with congenital heart disease--a challenge for the new millennium.

The care of complex congenital heart defect (CHD) patients should be continuous through life. Medium- and high-risk patients should be seen in special facilities for grown-up congenital heart (GUCH) patients, and followed for life. CHD in the adult is different than CHD in the child. Transitional programs should be available to prepare the adolescent patient to take charge of his/her own health. The patient should be transferred smoothly from pediatric to adult care. GUCH care should be regionally anchored in special facilities (groups or sometimes individuals) willing to make a commitment to

Wearables for Physical Activity Guideline Development and Health Surveillance: Converging Research and Practice Agendas.

OBJECTIVE: Etiological epidemiology and health surveillance are interdependent foundations of physical activity research, yet wearable devices have been adopted disproportionately across these 2 pillars of policy and practice. As physical activity research transitions from mostly self-reports toward a combination of mostly wearables and some self-reports, alignment between etiological evidence generation and surveillance systems is becoming increasingly important. We examine the goals, methods, and emerging convergence between wearable-based policy, surveillance, and etiological epidemiology.

Gaps in public health surveillance of elective surgery: a national analysis of inpatient data, COVID-19 impact, and coding validity in Germany (2006-2023).

BACKGROUND: Administrative inpatient datasets are cornerstone tools for health surveillance, yet their validity in tracking elective, self-pay surgical markets remain poorly defined. This study evaluates the utility of nationwide inpatient data for monitoring aesthetic surgery by quantifying 18-year trends, assessing the COVID-19 pandemic's impact, and benchmarking hospital capture rates against national estimates to identify critical public health surveillance gaps. METHODS: We conducted a descriptive longitudinal epidemiologic analysis of mandatory German hospital quality reports (2006-2023

Examining Multimorbidity in Older Adults Living in Naturally Occurring Retirement Communities, in Ontario, Canada, Using Latent Class Analysis.

BACKGROUND: Multimorbidity, ≥ 2 chronic conditions as per the World Health Organization, can complicate aging in place and lead to earlier transitions to nursing homes. To inform supportive service intervention planning in naturally occurring retirement communities (NORCs), which are areas where large proportions of older adults reside, we examined clustering patterns of specific chronic conditions among NORC residents. METHODS: We conducted a cross-sectional analysis of a population-based cohort of community-dwelling NORC residents aged ≥ 66 years as of January 1, 2022, in Ontario, Canada. A

Joint latent profiles of death anxiety and treatment adherence in HCC patients.

BACKGROUND: Hepatocellular carcinoma (HCC), one of the leading contributors to the global cancer burden, often places patients in a dual predicament of pronounced death anxiety and suboptimal treatment adherence. Prior research has largely treated death anxiety and adherence as independent, homogeneous constructs, thereby overlooking potential within-population heterogeneity and their co-occurring patterns. This study adopted a person-centered approach to identify joint latent profiles of death anxiety and treatment adherence among patients with HCC and to examine factors associated with profi

Diabetes Medication Regimen Complexity and Glycemic Outcomes among Medically Underserved Latino Adults: Implications for Pharmacoequity.

Latino adults in South Los Angeles living with type 2 diabetes mellitus (T2DM) experience disproportionate cardiometabolic complications alongside structural barriers that influence medication regimen complexity (MRC) and medication access. During 2017-2022, diabetes treatment guidelines increasingly emphasized cardioprotective antihyperglycemic medications while concurrent federal and state policy changes influenced health coverage continuity and pharmacy access for underserved populations. The present study evaluated the association between diabetes specific MRC (DM MRC) and cardiometabolic
