Strategies to attenuate pathological remodeling in heart failure.

PURPOSE OF REVIEW: The incidence of heart failure is increasing due to an aging population and improved management of diseases that are precursors to ventricular dysfunction. The success of therapeutic advances has created a challenge for the next generation of investigational heart failure treatments because the mortality rate has decreased to such a degree that larger trials will be needed to demonstrate mortality advantage. Prior work has linked favorable changes in ventricular geometry to improved survival, suggesting that remodeling may be a suitable surrogate endpoint. RECENT FINDINGS:

Spectrum of congenital heart diseases in Kashmir, India.

A retrospective analysis of case-records data of 53,653 patients (0-18 years) over a two and half year period was conducted to ascertain the spectrum of congenital heart diseases. Two hundred and twenty one patients were found having congenital heart diseases; a prevalence of 4.1/1000. Ventricular septal defect (VSD) was the most frequent lesion seen in 69 (31.2%), followed by patent ductus arteriosus (PDA) in 36 (16.3%) children. Tetralogy of Fallot (TOF) was the most frequent cyanotic heart disease seen in 17 (7.8%) patients.

Cheyne-stokes respiration in patients with heart failure.

Cheyne-Stokes respiration (CSR) is a form of central sleep-disordered breathing (SDB) in which there are cyclical fluctuations in breathing that lead to periods of central apneas/hypopnea, which alternate with periods of hyperpnea. The crescendo-decrescendo pattern of respiration in CSR is a compensation for the changing levels of blood oxygen and carbon dioxide. Severe congestive heart failure seems to be the most important risk factor for the development of CSR. A number of pathophysiologic changes, such as sleep disruption, arousals, hypoxemia-reoxygenation, hypercapnia/hypocapnia, and chan

[Monitoring and adjustment of immunosuppression after heart transplantation].

Heart transplantation remains the best therapeutic option for the treatment of end-stage heart failure. However, good survival rates can be obtained only if patients are closely monitored, particularly for their immunosuppressive regimens. Currently, a triple-drug regimen usually based on calcineurin-inhibitors (cyclosporin A or tacrolimus), anti-proliferative agents and steroids is used in most recipients. New agents such as the mTOR inhibitors, a more recently developed class of immunosuppressive drugs, can also be used in some patients. The aim of this article is to review currently used im

A rare cause of reversible complete heart block.

Lyme disease is a recognised cause of atrioventricular heart block. In the majority of cases conduction disturbances are reversed with antibiotic treatment. The diagnosis of Lyme disease is not always obvious and its recognition is important if unnecessary implantation of a permanent pacemaker is to be avoided. A 34-year-old man presented to our hospital with complete heart block due to Lyme carditis. This resolved completely within a few days after treatment with appropriate antibiotics.

Changing trends in infectious disease in heart transplantation.

BACKGROUND: During the past 25 years, advances in immunosuppression and the use of selective anti-microbial prophylaxis have progressively reduced the risk of infection after heart transplantation. This study presents a historical perspective of the changing trends of infectious disease after heart transplantation. METHODS: Infectious complications in 4 representative eras of immunosuppression and anti-microbial prophylaxis were analyzed: (1) 38 in the pre-cyclosporine era (1978-1980), (2) 72 in the early cyclosporine era (1982-1984), where maintenance immunosuppression included high-dose cyc

Listeriosis in pregnancy complicated by postpartum heart block.

BACKGROUND: Listeria monocytogenes is a food-borne pathogen that primarily affects pregnant women. Cardiac involvement is an uncommon complication of infection. We present a case of a gravida with Listeria bacteremia at 36 weeks of gestation. CASE: Two of a patient's blood cultures grew L monocytogenes after she experienced chills, headache, myalgia, and diarrhea. The patient was treated with antibiotics for 48 hours, and then labor was induced, resulting in a normal delivery with a healthy neonate. On day 5 postpartum, the patient developed progressive heart block, resulting in a third-degre

Testicular mixed germ cell tumor metastasizing to heart.

A 21 years old male presented with low grade fever, hemoptysis and progressively increasing dyspnoea of four month duration followed by acutely developing dizziness, hypotension, convulsion and altered sensorium. He had been operated (left inguinal orchidectomy) for left testicular swelling two years back with high alpha-fetoprotein and normal beta-human chorionic gonadotropin (beta-hCG). In view of this a possibility of metastasis secondary to a malignant testicular tumor was considered. Echocardiography demonstrated a large intracardiac mass, chest computed tomography (CT) revealed intracard

Cardiorenal syndrome in heart failure: a cardiologist's perspective.

One of the most important comorbidities in heart failure is renal dysfunction. Diminished estimated glomerular filtration rate is a potent predictor of cardiovascular mortality and complications. On the other hand, worsening heart failure or acute decompensated heart failure can accelerate worsening of renal function--the so-called cardiorenal syndrome. Risk factors include hypertension, diabetes, elderly age, and prior history of heart or renal failure. The pathophysiology of the cardiorenal syndrome involves intrarenal hemodynamics, transrenal perfusion pressure and systemic neurohormonal fa

Transseptal left heart catheterization a 50-year odyssey.

Development in the 1950s of the transseptal technique for left heart catheterization is described. Initial studies in animals and human cadavers were followed up by left atrial puncture with measurements of left atrial and left ventricular (LV) pressure (the latter using a small plastic catheter) in patients with cardiac disease. Many such procedures were performed safely without complications. Subsequent modification of the original technique for percutaneous catheter insertion allowed placement of a larger taper-tipped catheter in the LV chamber for selective LV angiography. Early clinical r

Recorded heart sounds for identification of ventricular tachycardia.

BACKGROUND: The ECG discrimination of ventricular tachycardia (VT) vs. supraventricular tachycardia (SVT) is both important and often difficult. In this study, we tested the hypothesis that recorded digital cardiac acoustical data reflect hemodynamic changes that can be used for VT detection. METHODS: We studied 57 subjects (42 males, mean age 57, range 24-83 years) who had undergone electrophysiological testing for known and suspected cardiac arrhythmias. Acoustic cardiography (Audicor, Inovise Medical, Inc.) was performed during each subject's electrophysiological study. We evaluated the ab

Antithrombotic therapy for patients with valvular heart disease.

To avoid the devastating consequences of thromboembolism and bleeding in patients with valve disease, in particular those with prosthetic valves, it is of utmost importance to correctly manage anticoagulation.Patients with mitral stenosis and either permanent or paroxysmal atrial fibrillation should receive anticoagulation, whereas those in sinus rhythm should only receive it in selected cases. In patients with prosthetic valves, anticoagulation management should be governed by the risk-benefit ratio taking factors related both to the patient and the prosthesis into account. Aspirin should be

Renal consequences of prostaglandin inhibition in heart failure.

Prostaglandins are products of intracellular arachidonate metabolism via the cyclooxygenase pathways. In the kidney, prostaglandins are modulators of afferent arteriole autoregulation. In diminished effective intravascular volume, vasodilatory prostaglandins reduce afferent arteriolar resistance, helping to maintain overall glomerular filtration. In subjects who have a low perfusion state, as in congestive heart failure, arthrosclerosis, hepatorenal syndrome, and cirrhosis, impaired renal autoregulation occurs, predisposing to kidney injury. Adverse effects of prostaglandin inhibition include

Retinoic acid deficiency alters second heart field formation.

Retinoic acid (RA), the active derivative of vitamin A, has been implicated in various steps of cardiovascular development. The retinaldehyde dehydrogenase 2 (RALDH2) enzyme catalyzes the second oxidative step in RA biosynthesis and its loss of function creates a severe embryonic RA deficiency. Raldh2(-/-) knockout embryos fail to undergo heart looping and have impaired atrial and sinus venosus development. To understand the mechanism(s) producing these changes, we examined the contribution of the second heart field (SHF) to pharyngeal mesoderm, atria, and outflow tract in Raldh2(-/-) embryos.

Genes in congenital heart disease: atrioventricular valve formation.

Through the use of animal studies, many candidate genes (mainly encoding transcriptional factors and receptors) have been implicated in the development of congenital heart disease. Thus far, only a minority of these genes have been shown to carry mutations associated with congenital disease in humans, e.g., GATA 4, TBX-5, NOTCH1 and NKX2-5. Mutations in these genes can cause a variety of cardiac defects even within the same family. Conversely, similar phenotypes are observed for different gene mutations suggesting a common pathway. Multiple genes and genetic pathways have been related to atrio

Heart transplantation: who should be considered and when?

Heart transplantation is now firmly established as the most effective therapy for selected patients with end-stage heart disease who have exhausted all other treatment options. The excellent long-term survival and quality of life achieved by the large majority of heart transplant recipients has led to considerable broadening of the recipient eligibility criteria over the past 24 years of continuous transplant activity in Australia and New Zealand. Although these changes in recipient eligibility criteria have resulted in a marked increase in the potential pool of transplant recipients, heart tr
