OBJECTIVE: According to Association Research Circulation Osseous (ARCO) staging, patients with ARCO III osteonecrosis of femoral head (ONFH) have joint pain and dysfunction due to the collapse of femoral head. Current hip preservation surgeries are not satisfying in the treatment of ARCO III ONFH according to the long-term follow-up. In our practice, we focused on reconstructing femoral head by restoring the round shape of femoral head and subsequently preventing the development of hip arthritis. To lift the collapsed femoral head and postpone the potential total hip arthroplasty, we practiced

OBJECTIVE: To explore the feasibility and effectiveness of total hip arthroplasty (THA) with acetabulum structural bone grafting using autogenous femoral head through direct anterior approach (DAA) in lateral decubitus position in the treatment of Crowe type Ⅲ and Ⅳ developmental dysplasia of the hip (DDH).
METHODS: Between June 2016 and July 2020, 12 patients with Crowe type Ⅲ and Ⅳ DDH were treated with THA with acetabulum structural bone grafting using autogenous femoral head through DAA in lateral decubitus position. There were 2 males and 10 females with an average age of 60.2 years (ran

PURPOSE OF THE STUDY The literature is conflicting as to whether the high hip center (HHC) reconstruction in total hip arthroplasty (THA) is an ideal option for patients with severe developmental dysplasia of the hip (DDH). This study aimed to compare the mid-term functional and radiographic outcomes of THA using HHC versus anatomical hip center (AHC) technique in Crowe types II and III DDH. Our hypothesis was that there may be no differences in terms of functional and radiographic outcomes between patients who underwent THA using HHC or AHC. MATERIAL AND METHODS Fifty-seven patients who under

OBJECTIVE: To investigate revision reasons and prosthesis selection of Crowe Ⅳ developmental dysplasia of the hip (DDH) after total hip arthroplasty (THA).
METHODS: A clinical data of 14 patients (15 hips) with Crowe Ⅳ DDH, who underwent a revision hip arthroplasty between January 2008 and May 2018, was retrospectively reviewed. There were 1 male (1 hip) and 13 females (14 hips). The age ranged from 27 to 63 years (mean, 45.0 years). There were 7 cases of left hip, 6 cases of right hip, and 1 case of bilateral hips. The prosthetic interfaces of primary THA were metal-on-polyethylene (MOP) in

BACKGROUND: Femurs of dysplastic hips exhibit specific abnormalities, and use of modular or specially designed components is recommended. An anatomic short stem was previously designed specifically for dysplastic hips using 3-dimensional data acquired from dysplastic patients. To investigate effects of stem geometry on bone remodeling, we undertook a prospective, randomized study of patients who had undergone 1-stage bilateral total hip arthroplasty (THA) with the anatomic short stem on one side and a conventional straight stem on the other.
METHODS: The study included 36 patients who underwe

BACKGROUND: The goal of periacetabular osteotomy (PAO) is to delay or prevent osteoarthritic development in dysplastic hips. However, it is unclear whether the surgical goals are achieved and if so in which patients. This information is essential to select appropriate patients for a durable PAO that achieves its goals.
QUESTIONS/PURPOSES: We therefore (1) determined hip survival rates; (2) determined how many preserved hips were functionally unsuccessful after PAO; and (3) identified demographic, clinical, and radiographic factors predicting failure after PAO.
METHODS: We retrospectively rev

We evaluated the clinical and radiographic results of primary total hip arthroplasty for 81 dysplastic hips (71 patients) using cementless Spongiosa Metal II cups (ESKA Implants, Lübeck, Germany). The mean follow-up period was 6.4 years (minimum 5 years), and the preoperative mean Japanese Orthopaedic Association hip score had improved from 45.2 to 87.4 points at the latest follow-up. The radiographic outcome was no aseptic loosening in all 81 hips. The hip center was located significantly more superior than in the contralateral normal hip in 45 patients, but the difference was less than 10 mm

BACKGROUND: We previously reported the outcomes at a minimum of twelve years after eighty-four Charnley low-friction arthroplasties performed in patients with osteoarthritis who were less than fifty-six years old at the time of the surgery. We now update the results of that cohort at a minimum of twenty-two years postoperatively.
METHODS: Eighty-four hips (in sixty-nine patients) with osteoarthritis, which was secondary to congenital hip disease in sixty-four (76%) of them, were followed prospectively with use of the Merle D'Aubigné and Postel scoring system as modified by Charnley and with s

BACKGROUND: Operative strategies to overcome the anatomical anomalies in patients with osteoarthritis secondary to developmental dysplasia of the hip remain controversial. The objective of this study was to determine the outcomes of total hip replacement with a grit-blasted cementless threaded cup and a cementless straight stem in patients with developmental dysplasia.
METHODS: Ninety-three patients with developmental hip dysplasia who had been treated with a total of 121 cementless total hip arthroplasties were clinically assessed at a mean of 9.3 years. The acetabular reconstruction was don

This article describes the technique for performance and interpretation of magnetic resonance arthrography of the hip. A description of normal anatomy of the hip is presented, and the appearance of the abnormal labrum is discussed. Labral detachments and tears are the most common clinically significant abnormalities to be identified. These abnormalities are recognized on the basis of the presence of contrast material at the acetabular-labral interface or within the substance of the labrum. Many varied appearances of the labrum have been identified within the asymptomatic population, and the co

BACKGROUND: Conversion total hip arthroplasty (THA) in ankylosed hips is technically challenging because of distorted anatomy, soft-tissue contractures, and compromised bone quality. Flexion, abduction, and external rotation deformities, common in inflammatory conditions, further restrict visualization of the femoral neck and pubofemoral arch when using the posterior approach. This study outlines technical modifications to overcome these challenges and reports the clinical and radiological outcomes of conversion THA performed through a posterior approach in patients with ankylosed hips.

BACKGROUND: This study aimed to evaluate the outcome of surgical debridement combined with postoperative hip spica immobilization in the treatment of hip joint tuberculosis in children.
METHODS: This retrospective study enrolled 87 children with stage I to III hip joint tuberculosis treated surgically at our hospital from January 2010 to January 2016. All patients considering hip tuberculosis routinely received a 4-week conservative treatment first, including rest, protected weight-bearing, and 4-drugs antitubercular chemotherapy. Surgical debridement was indicated when there was no relief af

BACKGROUND: Hip arthroscopy is increasingly used to treat femoroacetabular impingement, labral tears, and related disorders in young and active patients. As its indications have expanded, more patients subsequently undergo total hip arthroplasty (THA). Whether prior ipsilateral hip arthroscopy adversely affects the results of THA remains unclear. This systematic review assessed whether previous hip arthroscopy influences (1) perioperative findings, (2) postoperative complications and revision-related events, and (3) postoperative functional outcomes after THA.
METHODS: A systematic review was

BACKGROUND: Hip microinstability-subtle, symptomatic femoral head translation without dislocation-compromises the labral suction seal and accelerates chondrolabral wear. Clinical overlap with femoroacetabular impingement syndrome (FAIS) and borderline dysplasia delays recognition.
PURPOSE: To synthesize contemporary evidence on definitions, diagnosis, imaging, management, and outcomes in hip microinstability and to propose a phenotype-guided framework grounded in the labrum-capsule-ligamentum teres (LT) stability continuum.
STUDY DESIGN: Narrative review.
METHODS: Peer-reviewed literature (

BACKGROUND: Chronic hip pain due to osteoarthritis or hip dysplasia has been shown to negatively affect many daily life aspects. One aspect, however, which persists underestimated is sexual health. The number of total hip arthroplasties (THA) are increasing, especially in young patients who have high functional expectations, not only to pain relief, but to an increase in hip mobility and quality of life as well as sexual activity.
AIM: (1) to report the demographic factors, (2) the sexual activity before and after THA, as well as the concerns related to sexual activity after THA and (3) the p
