Estimated prevalence of orthopaedic fractures in Taiwan--A cross-sectional study based on nationwide insurance data.

BACKGROUND: Injury is the most common diagnostic category in the emergency unit, but no survey of epidemiological data for trauma or orthopaedic fractures has been made to date in Taiwan. A nationwide study of orthopaedic injuries is therefore necessary and would be of benefit to the Taiwanese population. METHODS: A qualified dataset, provided by a governmental authority, containing the original claims data of 1,000,000 randomly-sampled claimants from the year 2005 in Taiwan was analyzed, and a survey was made of 12 categories of orthopaedic fracture based on the ICD9-CM codes using two sets

Manifestations of Ollier's disease in a 21-year-old man: a case report.

INTRODUCTION: Ollier's disease is a rare nonhereditary disorder characterized by multiple enchondromas with a predilection for unilateral distribution. Malignant changes in Ollier's disease may occur in adult patients. Radionuclide bone scanning is one method used to assess lesions depicted on radiographs or magnetic resonance images that are presumed to be enchondromas. Also, a bone scan may give a clue to the multifocal nature of the disease and it has been recommended that scintigraphy is useful in the monitoring of lesions and the development of any malignant transformation. CASE PRESENTA

Effects of shoulder position on axillary nerve positions during the split lateral deltoid approach.

BACKGROUND: The axillary nerve may be injured during percutaneous fixation of proximal humerus fractures. This study investigated the kinematic behavior of the superior and inferior borders of the axillary nerve under varying shoulder positions. This information may reduce iatrogenic neurologic injury during fracture reduction and hardware placement. METHODS: The lateral deltoid approach was performed on 7 fresh frozen shoulders. The inferior and superior borders of the axillary nerve were tagged. Screws were placed in the anterior, middle, and posterior acromion as landmarks. Three-dimension

Shoulder hemiarthroplasty for fracture with a conservative rehabilitation regime.

INTRODUCTION: Standard rehabilitation regime following hemiarthroplasty for trauma is early mobilisation to prevent the development of a stiff shoulder. However, an aggressive early rehabilitation may lead to non-union of the greater tuberosity. We hypothesise that a delayed rehabilitation will result in a good union rate without undue risk of shoulder stiffness. MATERIALS AND METHODS: Between December 1996 and June 2003, 40 patients with three or four part fracture of proximal humerus with or without dislocation, not amenable to open reduction and internal fixation underwent hemiarthroplasty

Sural nerve autografts for high radial nerve injury with nine centimeter or greater defects.

PURPOSE: The purpose of this study is to present the outcome of sural nerve grafting that focuses on motor fascicles in 6 patients with high radial nerve injuries with defects of more than 9 cm. METHODS: Six patients with a high radial nerve complete injury were treated with a sural nerve autograft during a 6-year interval from June 1994 to June 2000. The gap of the radial nerve, which was measured in the surgical field, was 9 cm in 1 patient, 10 cm in 4 patients, and 11 cm in 1 patient. There were 4 men and 2 women with an average age of 32 years (range, 14-49 years). The causes of the injur

Influence of osteoporosis on fracture fixation--a systematic literature review.

UNLABELLED: The goal of our systematic literature search was to prove whether the experimentally shown influence of osteoporosis on fracture fixation could be confirmed in clinical studies. Despite significant effects in several studies, this is not supported by pooled data due to lack of accurate osteoporosis assessment and complication definitions. INTRODUCTION: The fact that osteoporosis causes fractures is well-known; the assumption that it aggravates their orthopaedic treatment has not been proven. The goal of our systematic literature search was to find out whether the experimentally pr

Early results with the new internal fixator systems LCP and LISS: a prospective study.

The Locking Compression Plate (LCP) and the Less Invasive Stabilisation System (LISS) are new implants with angular stability developed by the AO/ASIF. They offer an alternative for internal fixation in complex intra-articular fractures and high-energy diaphyseal fractures of the long bones. The new system was used in a prospective study to treat 113 patients (76 women, 37 men; average age: 53.1 years) with 122 injuries (multifragment shaft fractures or complex intra-articular, delayed/ non-union, malunion) from October 2002. Of these, 13 affected the humerus, 20 the radius, 2 the ulna, 21 the

Prosthetic joint replacement for long bone metastases: analysis of 154 cases.

INTRODUCTION: Metastatic bone disease is the most common cause of malignancies to the skeleton in adults. The treatment of bone metastases is frequently palliative aiming to achieve a satisfactory control of pain and to prevent or to treat pathological fractures. In selected cases the resection of a single bone metastasis may improve the survival of the patients. Our experience with bone metastases located in the appendicular skeleton, between 1992 and 2004, is retrospectively reviewed here. MATERIALS AND METHODS: We report a series of 154 patients (95 females and 59 males) treated with prost

Prenatal diagnosis for detection of aneuploidy: the options.

The value of all noninvasive prenatal tests must be viewed with the perspective of the consequences of invasive testing. Regarding second trimester noninvasive testing, biochemical screening is more accurate in establishing risk than maternal age alone. One or more major ultrasound abnormalities, nuchal thickening, or a shortened humerus should raise concern for Down syndrome regardless of the patient's a priori risk based on age or biochemical markers. Isolated minor ultrasound markers should not be used in calculating risk in low-risk patients regarding Down syndrome unless the biochemical p

The fracture liaison service: success of a program for the evaluation and management of patients with osteoporotic fracture.

INTRODUCTION: Fracture care often represents the first opportunity for clinical management of osteoporosis; however, many patients do not receive any evaluation after a fracture. In Glasgow, Scotland, fewer than 10% of fracture patients underwent bone mineral density (BMD) testing. In an effort to better meet the needs of fracture patients by providing routine assessment and, where necessary, treatment for osteoporosis after their fracture, a novel service (The Fracture Liaison Service) was designed and implemented in two separate National Health Service trusts in Glasgow. METHODS: An agreed-

Magnetic resonance imaging in the evaluation of treatment response of lateral epicondylitis of the elbow.

The purpose of this study was to investigate the treatment response in lateral epicondylitis (tennis elbow) by MRI. Magnetic resonance imaging was obtained in 30 patients with clinical symptoms of lateral epicondylitis of the elbow using T1-, T2- and T2-weighted fat-saturated (FS) sequences. The patients were randomised to either i.m. corticosteroid injection ( n=16) or immobilisation in a wrist splint ( n=14). Magnetic resonance imaging of the elbow was performed on a 1.5-T MR system at baseline and after 6 weeks. The extensor carpi radialis (ECRB) tendon, the radial collateral ligament, late

Isolated ruptures of the supraspinatus muscle.

We rarely encounter isolated ruptures of the supraspinatus muscle. At the Clinic of Orthopedics at the Faculty Hospital in Olomouc, we encountered only 21 cases out of 385 arthroscopic operation cases from October 1998 to October 2003. The patients were examined by USG, 5 patients were examined arthrographically and 3 patients underwent MRI examination. Of these 21 patients, only 3 were operated for acute post-injury haemarthrosis of the shoulder joint. During arthroscopic operation, an isolated rupture of the supraspinatus muscle was discovered in all these patients. The remaining 40 patients

The Upper Paleolithic triple burial of Dolní Vestonice: pathology and funerary behavior.

This work focuses on paleopathological analysis of one of the skeletons from the Gravettian triple burial of Dolní Vestonice (Moravia) and addresses issues of Upper Paleolithic funerary behavior. The burial includes the well-preserved skeletons of three young individuals. The skeleton in the middle (DV 15) is pathological and very problematic to sex; the other two (DV 13 and DV 14) are males and lie in an unusual position. The young age, the possibility of a simultaneous interment, and the position of the three specimens have given rise to speculations about the symbolic significance of this s
