Beyond age: Frailty-guided care in hematologic malignancies.

Frailty, distinct from chronological age, reflects physiologic reserve and predicts toxicity, treatment discontinuation, and survival across hematologic malignancies. The authors synthesize contemporary frailty tools and summarize disease-specific evidence in multiple myeloma, acute myeloid leukemia, and aggressive lymphomas. Integrating frailty measures can inform shared decision-making and guide the management of vulnerable populations to enhance therapeutic benefit and optimize quality of life.

Potential for compensatory exercise engagement among individuals seeking weight loss: implications for eating disorder prevention and healthy activity promotion.

Although weight loss programmes encourage exercise alongside dietary changes, some patients use exercise to compensate for perceived overeating and/or binge eating. Unlike non-compensatory exercise (which is not tied to eating or calories), this type of compensatory exercise can precipitate disordered eating, weight cycling and weight gain. Research is needed to understand the risks of compensatory exercise in weight management, and to inform interventions promoting non-compensatory exercise.

Is deafness mutation screening required in cystic fibrosis patients?

Aminoglycosides are widely used in cystic fibrosis management. The m.1555A>G mutation predisposes to aminoglycoside ototoxicity. It may cause later onset hearing loss in the absence of aminoglycosides use and gradual hearing loss may be an inevitable consequence of the mutation. Given that aminoglycoside therapy forms the backbone of IV protocols in CF, this article recommends screening for this mutation to allow informed decision-making prior to aminoglycoside administration, to avoid preventable deafness.

Alopecia areata: what to expect from current treatments.

Alopecia areata is relatively benign and often resolves on its own, although its psychosocial impact on children and young adults can be severe. Some form of treatment is usually required. Because current treatments may not show results for 3 to 6 months, reassuring the patient and the parents and informing them about the results that can be expected are an essential part of management. The choice of treatment depends on the patient's age and the extent of alopecia activity.

Perinatal care at the threshold of viability. American Academy of Pediatrics Committee on Fetus and Newborn. American College of Obstetricians and Gynecologists Committee on Obstetric Practice.

The survival rate for infants at the threshold of viability has been improving. However, there are insufficient data regarding the cost(s) of initial and ongoing care of these infants and the long-term outcome of survivors. Furthermore, there has been little study of the impact of obstetric management on the survival rates of extremely low birth weight infants and on long-term morbidities. Continued research on these issues is imperative, and physicians need to remain informed of changing statistics.

Physician Approaches to Determining Goals of Stroke Care for Patients Living With Disability or Dementia: Results from the SEED Mixed-Methods Study.

BACKGROUND: Despite remarkable advances in stroke management, there is a continued lack of evidence to guide care for the 1 in 3 stroke patients living with disability or dementia (PLWD). To help inform best practices, this study sought to understand how physicians approach the complex issue of determining goals of care for PLWD and what challenges they encounter. METHODS: In a mixed-methods investigation, we invited physicians involved in stroke care to participate in semistructured interviews and an online survey, enquiring into perspectives on stroke management in PLWD. Interviews were ana

Randomized Controlled Trials on Thoracolumbar Burst Fracture Management Are Statistically Fragile: A Systematic Review.

STUDY DESIGN: Statistical fragility analysis. OBJECTIVE: The management of thoracolumbar burst fractures (TLBFs) is challenging, and many surgeons rely on randomized controlled trials (RCTs) to inform decision-making. RCTs often report P-values, which may not fully reflect statistical robustness. This study utilizes fragility statistics to assess the reliability of RCTs evaluating TLBF outcomes. SUMMARY OF BACKGROUND DATA: Thoracolumbar burst fracture management remains controversial. Surgeons often rely on RCTs, though P-values alone may not reflect the reliability of the results. Fragility

Patient perspectives and barriers to effective home-based care in lymphatic filariasis: A mixed methods study from Puducherry, India.

Morbidity Management and Disability Prevention (MMDP) remain an under-addressed component of lymphatic filariasis elimination. Despite the simplicity of the hygiene-based regimen, adherence to these practices remains low due to multiple barriers, exacerbating chronic suffering. As the primary role remains vested in the patients, this study explores their perspectives and barriers to home-based care to inform strategies to improve outcomes. This study employed a mixed-methods approach using an explanatory sequential design. The quantitative phase involved a cross-sectional survey of 321 adult p

Survivorship preparedness and activation among survivors of lymphoma.

OBJECTIVES: Taking an active role in managing post-treatment care has emerged as a key aspect of promoting a successful transition into survivorship and is associated with better patient outcomes. In this study, we focus on two key aspects of active self-management, activation and preparedness. Activation was defined as understanding one's role in the care process and having the knowledge, skill, and confidence to take on a role in managing self-care. Preparedness was defined as the extent to which individuals perceived they had sufficient information about what to expect after cancer treatmen
