Crohn's disease is an inflammatory disorder of the gastrointestinal tract, the cause of which remains unknown. Since the first description by Dalziel in 1913 (1), similarities between Crohn's disease and intestinal mycobacterial infection, particularly Johne's disease in ruminants, have been widely recognized (2, 3). After Mitchell and Rees demonstrated the transmission of granulomata from Crohn's disease by injecting intestinal homogenates into the footpads of mice (4), there followed many studies attempting to identify infective agents within the bowel of patients with Crohn's disease. The first and second phases of the human microbiome project provided a view of mucosal surfaces and the skin of humans that mapped an abundant and complex ecosystem (microbiota) that is composed of bacteria (bacteriobiota), fungi (mycobiota), viruses (virobiota), enteric phages, archaea, protists, and helminths. Intestinal dysbiosis describes an adverse shift in microbial homeostasis in the gut that enhances intestinal epithelial permeability, translocating toxins that may lead to endotoxemia. Numerous intestinal and extra-intestinal illnesses have been linked to gut dysbiosis, including infla

BACKGROUND: Intestinal organoids provide physiologically relevant in vitro models that bridge the gap between conventional cell culture and animal studies. Although these systems have been developed for adult cattle, their use in neonatal calves-who are particularly vulnerable to enteric disease-has not been well established. Neonatal diarrhea remains a major health concern in modern agriculture, yet age-appropriate models for studying its pathogenesis are lacking. Given that host-pathogen interactions vary with developmental stage, there is a need for culture systems that reflect the distinct

Intestinal failure (IF) is a state in which the nutritional demands are not met by the gastrointestinal absorptive surface. A majority of IF cases are associated with short-bowel syndrome, which is a result of malabsorption after significant intestinal resection for numerous reasons, some of which include Crohn's disease, vascular thrombosis, and radiation enteritis. IF can also be caused by obstruction, dysmotility, and congenital defects. Recognition and management of IF can be challenging, given the complex nature of this condition. This review discusses the management of IF with a focus on

Interleukin-17 (IL-17) is associated with nonalcoholic fatty liver disease (NAFLD) and gut microbiota, and how IL-17 mediates the NAFLD/nonalcoholic steatohepatitis (NASH) process depending on the gut microbiota is unclear. We found that T helper 17 (TH17) cells were decreased in the small intestine in a methionine choline-deficient (MCD) diet-induced NASH model. IL-17-deficient (Il17-/-) mice showed alterations in intestinal microbiota, including the inhibition of probiotic growth and the overgrowth of certain pathogenic bacteria, and were prone to higher endotoxemia levels and more severe ga

Small intestinal medullary carcinoma (MC) is a newly recognized subclass of small intestinal carcinomas and is an exceptional entity for this site. A search of the literature for similar cases arising in the small intestine revealed only six previously reported cases. Here we present a case of MC arising in the jejunum of a 65-year-old male. The patient presented to the emergency with features of perforation peritonitis with liver metastasis and no known predisposing factors like inflammatory bowel disease and celiac disease. Studies conducted on this tumor's colonic counterpart have shown mic

A clinical case of Neuronal Intestinal Dysplasia (NID) is reported. This is an important pathology which is part of congenital alterations of bowel's innervation. This disease consists of two forms (A and B) which occur during different ages causing problems in intestinal motility with consequent various symptoms. Actually NID is diagnosed easier than in the past because histopathological techniques are very refined and permit to distinguish NID from cases once classified as Hirschsprung's disease. Histology can show an increase of Ach and Ldh activity in cholinergic fibers of lamina propria a

An adolescent female with long-standing, difficult-to-control ulcerative colitis developed leukocytoclastic vasculitis, a rare cutaneous extra-intestinal manifestation of the inflammatory bowel disease. The authors provide a literature review on leukocytoclastic vasculitis complicating ulcerative colitis. Furthermore, the clinical features of leukocytoclastic vasculitis are compared and contrasted with the more common cutaneous extra-intestinal manifestations of inflammatory bowel disease, erythema nodosum, and pyoderma gangrenosum.

The increasing number of publications about the incidence of cancer in small intestine with Crohn's disease shows Ileitis terminalis as an affection promoting cancer. Main problems are early diagnosis and differential diagnosis concerning intestinal stenosis caused by Crohn's recidivation or blind loop after resection. Regular scrutiny of patients with Crohn's disease is of special significance; because of the few present case reports early resection as prophylaxis is not justified.

BACKGROUND & AIMS: Antibiotic (ABx) therapy is associated with increased risk for Crohn's disease but underlying mechanisms are unknown. We observed high fecal serine protease activity (PA) to be a frequent side effect of ABx therapy. The aim of the present study was to unravel whether this rise in large intestinal PA may promote colitis development via detrimental effects on the large intestinal barrier.
METHODS: Transwell experiments were used to assess the impact of high PA in ABx-treated patients or vancomycin/metronidazole-treated mice on the epithelial barrier. Serine protease profiling

We read the comments by Nylund K et al. regarding our paper "Ultrasonographic scores for ileal Crohn's disease assessment: Better, worse or the same as contrast‑enhanced ultrasound?". Intestinal ultrasound has become one of the most valuable developments in the past decade, a non-invasive, well-tolerated exam, with an easy repeatability, and absence of sedation, ionizing radiation, or preparation. Particularly for inflammatory bowel disease, where there is a lack of agreement of patient's symptoms with disease activity, in an era where the paradigm of mucosal healing is changing to transmural

Inflammatory bowel disease is a chronic inflammatory disease and has periods of recurrence and remission. Improper immune responses to gut flora bacteria, along with genetic susceptibility, appear to be involved in causing this complex disease. It seems dysbiosis and oxidative stress may also be involved in IBD pathogenesis. A significant number of clinical studies have shown an interesting association between sleep disturbances and IBD. Studies in animal models have also shown that sleep deprivation has a significant effect on the pathogenesis of IBD and can aggravate inflammation. These inte

OBJECTIVE: There is a strong evidence for association between type 1 diabetes and Celiac Disease. Up to 8% of patients with type1 diabetes have characteristic features of CD on small intestinal biopsy. Type 1 diabetics who have HLA DQ2 or DQ8 are at risk for CD. However most of this data is from studies conducted in Europe, with mostly Caucasian population. This study aims to identify the prevalence of celiac disease in African American children with Type1 diabetes in inner city Brooklyn, New York.
METHODS: IgA and IgG Antigliadin antibodies, IgA tissue transglutaminase and HLA typing was mea

Direct investigation of intestinal humoral immunity requires collection of intestinal secretions or mucosal biopsy specimens, or both. A non-invasive technique of gut lavage, with a polyethyleneglycol electrolyte lavage solution as a means of collecting intestinal secretions for immunoglobulin and antibody studies, was evaluated. Fifty patients were studied--25 immunologically normal patients or volunteers, 15 patients with untreated coeliac disease, and 10 patients with active Crohn's disease. Protease inhibitors were added promptly to samples to prevent proteolysis of immunoglobulin content.

INTRODUCTION: sickle cell disease is a genetic disease with autosomal inheritance associated with haemoglobin structure abnormality which causes the formation of hemoglobin S. The purpose of our study was to collect data on digestive diseases in patients with sickle cell disease in Lubumbashi and to highlight their epidemiological and clinical features.
METHODS: We conducted a retrospective, descriptive, cross-sectional study at the Research Center for Sickle Cell Disease in Lubumbashi. All the records of patients on follow-up for sickle cell disease with digestive disease during our 3-year p

Although the hallmark of intestinal failure (IF) management - also known as intestinal rehabilitation - is parenteral nutrition, many of the complications that arise from this complex condition require additional pharmacologic interventions to prevent more serious consequences such as intestinal failure associated liver disease and central line associated bloodstream infections. This brief review highlights some of the more commonly used treatment strategies in pediatric patients with IF along with a review how different types of short bowel anatomy impact drug administration.

Ascariasis is a prevalent intestinal helminthic infection caused by Ascaris lumbricoides, commonly known as roundworm. While the disease may remain asymptomatic when the parasites are confined to the intestinal lumen of the small bowel, symptoms and complications can arise when the helminths invade the biliary tree or pancreatic ducts. In cases of severe infection, a mass of roundworms can lead to intestinal complications, such as bowel obstruction. The morphological identification of Ascaris by multimodality imaging techniques plays a crucial role in the diagnosis, as well as in the monitorin

Familial Mediterranean Fever (FMF) is a recurrent disease characterized by inflammatory process effecting synovial membranes such as peritoneum, pericardium and joints. It usually presents with acute abdominal pain. Intestinal obstruction secondary to adhesions may be observed in FMF patients. Sometimes diagnosing intestinal obstruction can be a challenging problem. We were presented a patient with FMF and adhesive intestinal obstruction. He was operated on after 10 days of symptoms. Delay in diagnosis and treatment of the case discussed with literature review.

We report a patient who presented with small intestinal ischaemia due to metastatic mesenteric disease from an oesophageal carcinoma. Enteroclysis suggested the diagnosis of small intestinal ischaemia, which was found at laparotomy to be due to compression of mesenteric veins by a nodal mass at the root of the small intestinal mesentery. Although this phenomenon has been described particularly in patients with carcinoid tumours, we believe that mesenteric venous ischaemia resulting from spread to the mesentery from an extra-abdominal primary neoplasm is extremely rare.
