Beware the syndrome in neonatal hip instability: follow up assessment is required after apparent resolution.

Developmental dysplasia of the hip is one of the most common congenital musculoskeletal disorders of childhood, affecting 1-3% of newborns. An early diagnosis and prompt treatment is essential to avoid complex treatments and achieve improved results. Since 1992, we have undertaken a screening programme for clinical instability and at risk patients. During this time, there have been only two cases which have been normal on both clinical, and, static and dynamic ultrasound assessment, that have subsequently deteriorated. In these two cases there was an underlying syndrome associated with hyperla

Treatment strategies for severe C1C2 luxation due to congenital os odontoideum causing tetraplegia.

PURPOSE: High-grade C1C2 luxation is a rare pathology. There is no clear evidence as to how to treat this deformity. There is only limited evidence about the different surgical techniques and possible approaches including advantages, disadvantages, and complications. METHODS: This is an uncommon case of a 13-year-old child with progressive, tetraplegia due to congenital os odontoideum with translational instability between C1 and C2, and progressive luxation of C2. An irreducible dislocation of the C0/C1 complex caused significant compression at the cervicomedullary junction and neurologic de

Use of a 1.5 mm butterfly locking plate for stabilization of atlantoaxial pathology in three toy breed dogs.

The objective of this study was to describe the use of a titanium 1.5 mm locking plate in the stabilization of atlantoaxial pathology in three toy breed dogs. Two dogs with atlantoaxial subluxation and another dog with an axial fracture, confirmed by diagnostic imaging, were stabilized via a ventral approach with a 1.5 mm titanium 5-hole locking butterfly-plate. Surgical reduction and stabilization were assessed by computed tomography and radiography after surgery. Follow-up evaluation for resolution of neurological signs and possible complications was performed in all three dogs. For long-ter

Orthopedic pitfalls in the ED: neurovascular injury associated with posterior elbow dislocations.

Posterior elbow dislocations are the most common type of elbow dislocation and are usually caused by a fall on an outstretched hand. Although the incidence of elbow dislocation complications is rare, the emergency physician is responsible for evaluation and identification of concomitant neurovascular injuries. Failure to identify neurovascular compromise after elbow dislocation or reduction can potentially lead to severe morbidity with limb ischemia, neurologic changes, compartment syndrome, and potential loss of limb. Cyanosis, pallor, pulselessness, and marked pain should suggest vascular in

The sulcus deepening trochleoplasty-the Lyon's procedure.

Sulcus deepening trochleoplasty is a technically demanding procedure with precise indications: high grade trochlear dysplasia with patellar instability and/or abnormal tracking. The main goal is to decrease the prominence of the trochlea and to create a new groove with normal depth, thus optimising patellar tracking. Associated abnormalities should be specifically treated. Recurrence of instability is very rare after this procedure and is more likely to result from missed associated abnormalities. Although results seem very good in terms of instability, further evidence is still needed since t

Multidirectional instability accompanying an inferior labral cyst.

Paralabral cyst of the shoulder joint can be observed in 2% to 4% of the general population, particularly in men during the third and fourth decade. On average, these cysts measure 10 mm to 20 mm in diameter and are located preferentially on the postero-superior aspect of the glenoid. The MRI has increased the frequency of the diagnosis of paralabral cysts of the shoulder joint. Paralabral cysts of the shoulder joint usually develop in the proximity of the labrum. The relationship between shoulder instability and labral tears is well known, however, the association of shoulder instability with

Effect of an anterior glenoid defect on anterior shoulder stability: a cadaveric study.

BACKGROUND: There have been few biomechanical studies to clarify which size of a glenoid defect is critical. However, those studies have assumed that the defect occurred anteroinferiorly. Recent studies have reported that the defect is located anteriorly rather than anteroinferiorly. Therefore, the effect of the anterior, not anteroinferior, glenoid defect on shoulder stability needs to be investigated. HYPOTHESIS: The anterior glenoid defect would have a similar effect on anterior shoulder stability as that of the anteroinferior glenoid defect. STUDY DESIGN: Controlled laboratory study. ME

Extended retinacular soft-tissue flap for intra-articular hip surgery: surgical technique, indications, and results of application.

Osteotomies of the proximal femur for hip joint conditions are normally done at the intertrochanteric or subtrochanteric level. Intra-articular osteotomies would be more direct and therefore allow a more powerful correction with no or very little undesired side correction. However, concerns about the risk of vascular damage and osteonecrosis of the femoral head have so far basically excluded this technique from practical use. Based on detailed knowledge of the vascular anatomy of the proximal femur, an approach to safely dislocate the femoral head has been described and successfully performed.

Evaluation of TMJ articular eminence morphology and disc patterns in patients with disc displacement in MRI.

The aim of this study was to assess the shape of the temporomandibular joint (TMJ) articular eminence and the articular disc configuration and position in patients with disc displacement. TMJ magnetic resonance images (MRI) of 14 patients with bilateral disc displacement without unilateral reduction were analyzed. Articular eminence morphology was characterized as box, sigmoid, flattened, or deformed. Articular disc configuration was divided into biconcave, biplanar, biconvex, hemiconvex or folded, and its position, as "a" (superior), "b" (anterosuperior), "c" (anterior) or "d" (anteroinferior

Emergency department management of selected orthopedic injuries.

Rather than providing an encyclopedic review of extremity injuries, this article reviews selected serious injuries of the extremities that can be missed in the emergency department, either because they are relatively uncommon or because they are subtle in their clinical and radiographic presentation. They include injuries to the scapula, the shoulder, the forearm, the femur and hip, the knee, the tibia (which is the most common long bone fracture), and the ankle and foot. Their various causes include sports injuries, falls, and motor vehicle accidents. Several of these injuries can result in e

Reconstruction of the brachial artery in supracondylar humerus fractures and forearm dislocations.

Presented herein is an analysis of the short- and long-term results of the treatment of brachial artery injuries in 25 patients with extension supracondylar humerus fractures and anterior forearm dislocations. The cause of arterial injury was a blunt trauma in 11 patients and contused-crushed wounds in 12 cases. In two patients, iatrogenic injury of the brachial artery occurred after reposition of humerus fractures in other medical institutions. Fourteen patients were admitted to the Department for the first 6 hours from the time of injury, 5 after up to 12 hours, and 6 patients after 12 hours

The forearm complex: anatomy, biomechanics and clinical considerations.

The forearm complex is comprised of the proximal radioulnar joint, middle radioulnar joint/interosseous membrane, and the distal radioulnar joint. These three areas function in a coordinated manner to rotate the hand in space and allow performance of functional tasks. If a structure or structures in one of these three areas is disrupted, this can adversely affect the function at any of the other two remaining areas. Surgical intervention focuses on restoring anatomical alignment to preserve the function of the forearm complex. Rehabilitation is guided by the relationships between the three are

Dynamic lumbrical muscle transfer for correction of posttraumatic extensor tendon subluxation.

Posttraumatic ulnar subluxation of the extensor tendon over the metacarpal head results from rupture of the radial sagittal fibers. The patient will complain of limited digital extension and pain. Various techniques have been described to correct the disorder by either reefing the sagittal fibers or using a slip of extensor tendon around the radial collateral ligament. Unfortunately, these techniques are either technically not feasible, reefing of the sagittal fibers, or result in significant stiffness, using a strip of extensor tendon. The author describes an extraarticular technique which us

[Limitation in subtalar motion in a patient nine years after treatment for medial subtalar dislocation].

Subtalar dislocations are rare injuries. They may cause serious complications in the long-term. A 27-year-old professional soldier developed medial subtalar dislocation of the right foot due to fall from height and was treated with closed reduction under general anesthesia and below-knee cast fixation for six weeks. At the end of a nine-year follow-up, he began complaining of occasional pain around his ankle. There was minimal loss in subtalar movements. He had no stiffness or instability. The ankle-hindfoot score was 90 according to the American Orthopaedic Foot and Ankle Society clinical rat

[Sequentially occurred bilateral atlantoaxial rotatory subluxation following craniotomy: a case report].

Atlanto-axial rotatory subluxation (AARS) often presents in childhood as a stiff, painful neck with associated typical cock-robin type of torticollis. It can occur spontaneously or following minor trauma or an inflammatory process in the cervical tissues or following otorhinolaryngeal surgery. We report a case of bilateral AARS which occurred sequentially following craniotomy. To the best of our knowledge, no case of AARS after craniotomy has been reported previously and only one case in which bilateral AARS was observed has been reported to date. Failure to recognize the condition may result

Sesamoid and toe fractures.

Injuries of the toes and sesamoids of the first metatarsophalangeal joint comprise a wide spectrum of traumatic entities. Despite the fact that a majority of lesions may well respond to nonsurgical treatment and exhibit an excellent prognosis, appropriate clinical and imaging analysis is mandatory to select those injuries that require specific therapeutic and surgical measures to avoid long-term sequelae of functional disability.

Atraumatic retrosternal dislocation of the clavicle.

Atraumatic retrosternal dislocation of the clavicle is an exceedingly rare event and three out of four previously reported cases lack any radiological evidence. We report the case of a 30-year-old male patient who presented an atraumatic retrosternal dislocation of the clavicle without a history of previous injury and underlying pathology. The diagnosis was delayed and established by a CT scan ten days after and initial presentation of the symptoms. A successful, stable, closed reduction under general anaesthesia was performed ten days after the initial presentation, having a cardiothoracic su

[Anatomy of the superior labrum complex of the shoulder].

The outstanding progress achieved in MRI and arthroscopic techniques has forced orthopedic surgeons to consider the anatomy of the shoulder joint in detail. The superior labrum complex shows a broad range of variations, which is the reason for some difficulties regarding diagnostic conclusions as well as therapeutic considerations. In particular, the differentiation between the normal and pathological adherence of the labrum is a serious clinical problem. The glenoid labrum consists of circularly arranged collagenous fibers attached to the osseous margin of the glenoid by a fibrocartilaginous
