Subregional effects of meniscal tears on cartilage loss over 2 years in knee osteoarthritis.

OBJECTIVES: Meniscal tears have been linked to knee osteoarthritis progression, presumably by impaired load attenuation. How meniscal tears affect osteoarthritis is unclear; subregional examination may help to elucidate whether the impact is local. This study examined the association between a tear within a specific meniscal segment and subsequent 2-year cartilage loss in subregions that the torn segment overlies. METHODS: Participants with knee osteoarthritis underwent bilateral knee MRI at baseline and 2 years. Mean cartilage thickness within each subregion was quantified. Logistic regressi

[Observations of normal and ACL-deficient knee joints after stress MRI].

BACKGROUND: The Lachman test is the most reliable clinical test for diagnosing rupture of the anterior cruciate ligament (ACL). Previous X-ray studies have presented a "radiologic Lachman test". Recently anterior tibial translation was demonstrated using open access MRI. Two methods were developed to transfer a similar technique to a more widely available closed MRI. METHODS: Using closed MRI we investigated 22 knees in 21 patients with pure rupture of the ACL. Anteriorly and posteriorly directed shear forces were applied to the tibiofemoral joint at 20 degrees flexion either by positioning a

Arthroscopic osteochondral autograft transplantation for chondral lesion of the tibial plateau of the knee.

Arthroscopic osteochondral autograft transplantation is often used to treat chondral/osteochondral lesions of the femoral condyle of the knee. However, arthroscopic autologous osteochondral grafting to the tibial plateau has not been reported. We report the surgical technique and the clinical course of a patient who underwent engraftment by this method. A 26-year-old man developed symptoms of pain and catching in his knee. Arthroscopy revealed a deep chondral lesion, 10 x 15 mm in size, down to the subchondral bone on the posterocentral area of the lateral tibial plateau. The injured cartilage

Patellofemoral pain syndrome in athletes: a 5.7-year retrospective follow-up study of 250 athletes.

Two hundred fifty athletes from a sports clinic, 122 (49%) females and 128 (51%) males (mean age 21.1 years), who were diagnosed as having patellofemoral pain syndrome (PFPS) and were instructed to practice vastus medialis training, returned a completed questionnaire after an average of 5.7 years' follow-up. Sixty-eight (27%) were pain-free for an average of 8.1 months: 17 (25%) women and 51 (75%) men. As to the remaining 182 (73%) who were still suffering, 95 (52%) had experienced a decrease in pain, 64 (35%) were unchanged and 23 (13%) had experienced an increase in pain. Sixty-eight (37%) d

Some clinical aspects of reconstruction for chronic anterior cruciate ligament deficiency.

A total of 250 patients was reviewed 71.8 months (range 49-105 months) after anterior cruciate ligament (ACL) reconstruction for disabling instability that had not responded to conservative treatment or correction of internal derangements. Knees that had undergone previous operation or had damage to other ligaments were excluded. Four techniques were used; MacIntosh extra-articular lateral substitution alone (n = 18), extra-articular reconstruction plus intra-articular carbon fibre (n = 29), extra-articular reconstruction plus a free graft from the medial third of the patellar tendon (n = 74),

[Instrumented measurement of ruptures of the anterior cruciate ligament].

The KT-1000 arthrometer was used to measure anterior tibial displacement in patients with acute unilateral anterior cruciate ligament (ACL) disruptions. Measurement revealed 11.5 mm absolute displacement on the injured side and 3.6 mm side-to-side difference with the 89 N test, 15.2 mm absolute displacement and 6.1 mm side-to-side difference with the manual maximum displacement test, and 8.5 mm displacement and 2.0 mm side-to-side difference with the active quadriceps test. Chronic anterior instabilities measured 13.0 mm absolute displacement on the involved side and 5.4 mm side-to-side differ

[Reattachment of the meniscus. Techniques, long-term results and recommendation for individual treatment].

To prevent the joint degeneration which commonly occurs after meniscectomy, meniscal repair has become the treatment of choice. With more advanced arthroscopic techniques, including application of fibrin clot and synovial flaps, almost all types of meniscal ruptures are technically accessible and have been found to heal. However, from a biomechanical point of view only a meniscus with a peripheral longitudinal rupture functions as a normal meniscus after repair and, therefore, minimizes cartilage degeneration; this statement was confirmed by long-term follow-up evaluations. In this respect, th

Aberrant anterior tibial artery on knee MRI: A case report advocating for routine search of this anatomical variant.

An aberrant anterior tibial artery is an anatomical variant in which the anterior tibial artery arises anomalously from the popliteal artery and takes an atypical anterior course relative to the popliteus muscle. We report a case of a 56-year-old male who underwent a routine knee MRI following trauma to evaluate for a suspected meniscal tear. MRI revealed an incidental finding of an aberrant anterior tibial artery coursing anterior to the popliteus muscle and near the posterior tibial cortex. Knowledge of this variant is crucial to avoid potential iatrogenic vascular injury during orthopedic s

Percutaneous Spinal Needle-Assisted Suture Passing in Medial Meniscal Posterior Root Tear Repair.

The medial meniscal posterior root (MMPR) is essential for absorbing hoop stress and preventing meniscal extrusion. MMPR tears lead to significant alterations in knee joint kinematics, increasing tibiofemoral contact pressure and accelerating knee osteoarthritis. Given the poor prognosis of untreated meniscal root tears, there has been growing interest in surgical repair techniques. The percutaneous spinal needle-assisted suture-passing technique offers a cost-effective, minimally invasive alternative that reduces iatrogenic damage to articular cartilage and eliminates the need for medial coll

Subarachnoid Hemorrhage as a Presentation of Hemophilia: A Case Report.

A young boy presented with features of non-traumatic Subarachnoid hemorrhage (SAH) with hematemesis and melaena. He has had past history of prolonged bleeding following cut injury even requiring blood transfusion after circumcision. On examination, he was found confused, severely anemic, with presence of neck rigidity and painful swelling of right knee joint. But no positive family history was found. Non-contrast CT scan showed SAH. Cerebral angiography showed no aneurysm but knee joint had features of hemarthrosis. He was resuscitated and hemophilia was diagnosed on the basis of clinical susp

How Many Bundles Does the Anterior Cruciate Ligament Consist of? A Case Report.

In some cases with anterior cruciate ligament (ACL) injury, physical examination and magnetic resolution imaging cannot clearly identify whether the ACL is intact or partially or completely ruptured. A 40-year-old female patient was admitted to our clinic with complaints of knee pain. After the requested examinations, we could not clearly identify whether the ACL was intact or partially or completely ruptured. Arthroscopic knee surgery was planned for the patient. In diagnostic arthroscopy, it was also determined that there was a multibundle ACL that was not surrounded by the synovium and was

Surgical repair of sciatic nerve traumatic rupture: technical considerations and approaches.

Traumatic proximal sciatic nerve rupture poses surgical repair dilemmas. Disruption often causes a large nerve gap after proximal neuroma and distal scar removal. Also, autologous graft material to bridge the segmental defect may be insufficient, given the sciatic nerve diameter. The authors utilized knee flexion to allow single neurorrhaphy repair of a large sciatic nerve defect, bringing healthy proximal stump to healthy distal segment. To avoid aberrant regeneration, the authors split the sciatic nerve into common peroneal and tibial divisions. After 3 months, the patient can fully extend t

Empty Sunrise Sign: An Arthroscopic Finding in Recurrent Patellar Dislocation.

Patellar dislocation is a relatively common knee injury and can be challenging to diagnose. We describe an arthroscopic finding we noted in recurrent patellar dislocation. Arthroscopic photos were evaluated from 50 knees with a history patellar dislocation and 100 control patients. We quantified the amount of patellar subluxation seen on a single arthroscopic view and coined it Empty Sunrise Sign if the patella did not overlap the trochlea. Empty Sunrise Sign was found in 82% of 50 cases with recurrent patellar dislocation and none of the 100 control cases. The finding was 100% specific for id

Surgical treatment for osteochondritis dessicans of the knee.

Osteochondritis dissecans (OCD) of the knee is a disease of the subchondral bone with secondary injury to the overlying articular cartilage. OCD lesions are generally categorized as juvenile-growth plates open-or adult-growth plates closed. This maturity-based classification scheme has a prognostic value in that many juvenile OCD lesions will heal with conservative care while most symptomatic adult OCD lesions need surgical intervention. OCD can result in pain, knee joint effusions, loose body formation, and arthritis. Short-term treatment goals include pain and symptom resolution while the lo

Management of the Patient with an ACL/MCL Injured Knee.

The knee is a mobile functional anatomical unit which plays a key role in recreational function. In the last three decades, the knee has received a great deal of attention in the sports medicine literature, particularly in respect to isolated ligament pathology and management. In reference to combined multiple ligament pathology, a more limited number of articles exist, and indeed lead to confusing management. Although hundreds of publications address the topic of surgical correction of the anterior cruciate ligament (ACL), debate continues regarding clinical intervention for the patient with

Arthroscopic use of the Herbert screw in osteochondritis dissecans of the knee.

This article describes repair of osteochondritis dissecans of the knee by arthroscopic Herbert screw fixation and reports the healing response and clinical results. Eleven patients were subjected to surgery using arthroscopy and all were affected by symptomatic osteochondritis dissecans of the medial femoral condyle. Average follow-up time was 16.3 months (13-29). One to four Herbert screws were used, either as the only fixation method or in conjunction with absorbable pins [poly (p-dioxanone)], depending on size and fragmentation of the injury. The patients were non-weight bearing for 10 week

The Clinical Significance of Using PASS Thresholds When Administering Patient-Reported Outcome Instruments After Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Patient-reported outcome (PROs) instruments of knee function quality of life are routinely administered to patients after anterior cruciate ligament reconstruction (ACLR). The Patient Acceptable Symptom State (PASS), an evidence-based threshold defining perceived outcomes, may be a useful indicator of strength and functional performance. PURPOSE: To compare strength and functional performance between patients recovering from ACLR who did and did not meet PASS thresholds on associated PROs. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A total of 223 patient
