Cytokines, Signaling and Epigenetic Mechanisms: Shaping the Acute Lymphoblastic Leukemia Microenvironment.

Acute Lymphoblastic Leukemia (ALL) is a heterogeneous hematological malignancy in which disease progression and response to therapy are influenced by a complex network of molecular alterations, interactions with the bone marrow microenvironment, and epigenetic modulation mechanisms. Crosstalk between oncogenic, inflammatory, and immunoregulatory signaling pathways, together with epigenetic modifications, contributes to the maintenance of leukemic survival and the development of therapeutic resistance. This review analyzes the role of cytokines and chemokines such as IL-6, TNF-α, and CXCL12, wh

Long non-coding RNAs and survival outcomes in chronic lymphocytic leukemia.

Chronic lymphocytic leukemia (CLL) is a highly heterogeneous hematological malignancy characterized by pronounced differences in clinical course and genetic background. While research has historically focused on alterations in protein-coding genes, it is increasingly evident that the non-coding transcriptome plays a pivotal role in CLL pathogenesis and progression. This review explores the emerging field of long non-coding RNAs (lncRNAs) in CLL, and their association with survival outcomes. We discuss how these pleiotropic transcripts act as master regulators of gene expression, orchestrating

Postoperative complications in patients with chemotherapy-induced acute myeloid leukemia (Review).

The present comprehensive review examines the latest clinical and translational research on postoperative complications in adult patients with chemotherapy-treated acute myeloid leukemia (AML) undergoing elective and emergency surgery. Patients with chemotherapy-treated AML face a markedly elevated risk of postoperative complications, with nearly one-half of patients experiencing complications such as severe infections and bleeding, with associated postoperative mortality rates of 10-20%. These risks are primarily driven by profound neutropenia and thrombocytopenia, impairing immune defense an

Host-microbiome interactions in leukemia: mechanisms, treatment response, and clinical implications.

Host-microbiome interactions regulate immune function, epithelial barrier integrity, and hematopoietic homeostasis. Intestinal microbial communities show consistent disruption in leukemia, particularly during intensive chemotherapy and hematopoietic stem cell transplantation. Reduced microbial diversity, depletion of short-chain fatty acid (SCFA)-producing commensals, and expansion of opportunistic taxa are recurrent findings across cohorts. Such patterns correlate with inflammatory signaling, impaired barrier function, and shifts in immune responses affecting treatment tolerance and hematopoi

Overcoming resistance to immune checkpoint inhibitor therapy in acute myeloid leukemia.

Acute myeloid leukemia (AML) is characterized by molecular, immunological, and clinical heterogeneity and this has been reflected with changing paradigms in the treatment landscape of AML in recent years. Despite high remission rates with conventional chemotherapy regimens, most patients eventually relapse and have disappointing outcomes in the relapsed setting, with estimated 5-year overall survival rates ranging from 10-15%. Immune checkpoint inhibitors (ICIs) restrain T-cell suppressive signals delivered through cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) or programmed cell death p

Does stringent cytoreduction improve survival in advanced proliferative chronic myelomonocytic leukemia?

Whether mitigation of myeloproliferation improves prognosis of CMML independently of bone marrow response is unknown. Flow-defined classical monocytes (cMo) and immature granulocytes (iGRAN) have not yet been studied as biomarkers of response. We inspected the prognostic value of WBC, circulating monocytes, cMo and iGRANs in the 120 DACOTA (NCT02214407) patients randomized to decitabine (n = 63) or hydroxyurea (n = 57) evaluated after 3 cycles with BM aspiration and complete blood count. Across arms, 59% and 56% patients had monocytes > 1 × 109/L or WBC > 10 × 109/L at the 3- and 6-cycle evalu

A STAT3 degrader demonstrates efficacy in venetoclax resistant acute myeloid leukemia.

Acute myeloid leukemia (AML) is an aggressive myeloid malignancy with a poor prognosis. Venetoclax (Ven), a BCL2 inhibitor, has shown promising results but often leads to relapse due to mitochondrial dysregulation, particularly due to upregulation of the anti-apoptotic protein MCL1. Overexpression of the transcription factor STAT3 has been linked to poor survival in AML patients. Overexpression of STAT3 in a transgenic murine model induces a myeloid malignancy with a short latency period and inflammatory upregulation. The current study identifies STAT3 upregulation as a key mechanism of Ven re

Fibrinogen promotes acute myelogenous leukemia progression via miR-486/GPR153 axis.

Acute myeloid leukemia (AML) is a frequently fatal malignancy of bone marrow stem/progenitor cells. Fibrinogen (Fg), the predominant coagulation factor in plasma, has been reported to have a negative correlation with the prognosis of AML patients. However, the underlying mechanisms through which Fg exerts its effects on AML remain unclear. In this study, we developed a Fg-deficient AML mouse model and utilized AML cell lines and primary human AML cells to explore the role of Fg in AML progression both in vivo and in vitro. Our findings demonstrate that Fg significantly accelerates AML progress

Cervical epidural plasmacytoma causing recurrent radiculopathy and revealing plasma cell leukemia.

BACKGROUND: Cervical epidural plasmacytomas are an uncommon manifestation of plasma cell dyscrasias and may mimic degenerative cervical spondylosis. Here, we report an epidural plasmacytoma discovered during reoperation for recurrent radiculopathy that led to the diagnosis of plasma cell leukemia (PCL). CASE DESCRIPTION: A 76-year-old female underwent a C5-C7 anterior cervical discectomy and fusion for cervical spondylotic radiculopathy. After initial improvement, severe radiculopathy recurred 6 weeks later. During posterior C6-T1 decompression, a gray, friable epidural mass encasing the left

Pulmonary Leukemic Infiltration in Relapsed Acute Myeloid Leukemia: A Case Report.

Pulmonary leukemic infiltration (PLI) is a rare complication of acute myeloid leukemia often misdiagnosed as pneumonia. This case highlights a unique presentation of PLI in a relapsed patient without hyperleukocytosis, necessitating a high index of suspicion. Aggressive salvage chemotherapy followed by stem cell transplantation was effective in resolving the infiltration and achieving remission.

Ectopic expression of BEX genes in T-cell acute lymphoblastic leukemia.

T-cell acute lymphoblastic leukemia (T-ALL) is a malignant proliferation of T-cell progenitors originating in the thymus. T-ALL is a heterogenous disease involving the dysregulation of various oncogenes/tumor-suppressor (TS) genes. Loss of the TS gene phosphatase and TENsin homolog (PTEN) is a recurrent alteration, which is often associated with a mature T-ALL subgroup expressing a T-cell receptor. Herein, we used a single-cell RNA-sequencing approach to investigate the impact of the absence of PTEN on pathological development of mouse thymocytes. First, our differential gene expression analys

Epigenetic Regulation in Acute Myeloid Leukemia: Molecular Mechanisms and Clinical Implications.

Acute myeloid leukemia (AML) is a heterogeneous hematologic malignancy characterized by a block of differentiation and uncontrolled expansion of myeloid progenitor cells. Standard treatment includes intensive induction chemotherapy, typically with cytarabine and anthracycline, followed by consolidation chemotherapy or allogeneic hematopoietic stem cell transplantation. However, these approaches are often associated with relapse and treatment-related toxicity. Accumulating evidence highlights a critical role for epigenetic dysregulation in driving disease initiation, progression, and therapeuti

Bioactive venetoclax-lipid nanomedicine enhances therapeutic efficacy in acute myeloid leukemia.

Acute myeloid leukemia (AML) remains a highly aggressive hematological malignancy with frequent relapse and limited therapeutic durability. Although venetoclax has provided an important therapeutic option for AML, suboptimal response and treatment resistance continue to restrict its clinical benefit. Here, we developed a venetoclax-loaded lipid nanomedicine, Vene-lipo, to enhance anti-leukemic efficacy. Vene-lipo displayed uniform nanoscale morphology, favorable colloidal stability, and good preliminary biosafety. Compared with free venetoclax, Vene-lipo more effectively suppressed AML cell gr

Ribonucleotide Reductase Inhibition Overcomes FLT3 Inhibitor Resistance in Acute Myeloid Leukemia.

Internal tandem duplication mutations in FLT3 (FLT3 ITD) occur in approximately 30% of patients with acute myeloid leukemia (AML) and are among the most common genetic alterations in this disease. FLT3 ITD is a major driver of AML and is associated with poor clinical outcomes. Although FLT3 inhibitors (FLT3is) have significantly improved outcomes for patients with FLT3 ITD+ AML, acquired resistance remains a major barrier to durable clinical benefit. Reactivation of RAS/MAPK signaling, often driven by activating NRAS mutations, is a major mechanism of FLT3i resistance in AML; however, effectiv

KMT2A-Partial Tandem Duplication Associates With Leukemia Transformation in Myelodysplastic Syndrome.

BACKGROUND: Partial tandem duplication of KMT2A (KMT2A-PTD) is observed not only in acute myeloid leukemia (AML) but also in myelodysplastic syndrome (MDS), while the prognostic value of KMT2A-PTD in MDS needs to be clarified and the optimal therapies for these patients remains unclear. METHODS: In this study, we analyzed 137 MDS patients with KMT2A-PTD from our hospital (PKUPH cohort) and incorporated 2493 MDS patients from the International Working Group for Prognosis in MDS (IWG-PM) cohort, among whom 65 patients had KMT2A-PTD. RESULTS: MDS patients with KMT2A-PTD in IWG-PM cohort had sig
