Type II mixed cryoglobulinaemia, hepatitis C virus infection, and glomerulonephritis.

Mixed cryoglobulins (MC) are immunoglobulins which precipitate reversibly in the cold. Type II mixed cryoglobulins are composed of a monoclonal component (usually IgMk) with rheumatoid factor (RF) activity against polyclonal IgG. In type III MC, all the components are polyclonal. The majority of MC are secondary to connective tissue diseases, infectious or lymphoproliferative disorders, hepatobiliary diseases, or immunologically mediated glomerular diseases. The aetiology of MC is not clear and cryoglobulinaemia was considered 'essential' until an association between hepatitis C virus (HCV) in

Pneumococcus-induced thrombocytopenia in rabbits.

Thrombocytopenia without other hemostatic changes is the most common coagulopathy associated with sepsis. We studied pneumococcus (PNC)-induced hemostatic changes, including thrombocytopenia, in rabbits. Nonviable PNC or saline solution was injected into rabbits preinfused with chromium 51-labeled platelets or iodine 125-fibrinogen. Blood was serially obtained for determination of platelet counts, 51Cr activity or 125I activity, and fibrinogen and fibrin degradation products. Lung, liver, and spleen tissues were counted for 51Cr or 125I activities per gram of wet tissue. PNC-challenged animals

Blood and tissue branched-chain amino and alpha-keto acid concentrations: effect of diet, starvation, and disease.

Branched-chain alpha-keto and amino acid (BCKA, BCAA) concentrations were measured in blood, plasma, and tissues of rats fed low protein (8% casein) or high protein (60% casein) diets; and in rats fed a stock diet and subjected to 3 days of starvation of chemically-induced diabetes. Concentrations of these amino and ketoacids were also measured in blood from patients with maple syrup urine disease. Valine, isoleucine, and leucine concentrations in blood from rats fed the stock diet were 124 +/- 7, 58 +/- 4 and 99 +/- 5 microM, respectively. Blood BCAA concentrations of rats fed the high protei

Rho-kinase inhibitor targeting the liver prevents ischemia/reperfusion injury in the steatotic liver without major systemic adversity in rats.

Rho-kinase (ROCK) inhibitors improve liver blood flow after ischemia/reperfusion (IR) injury, especially in the setting of steatosis, by decreasing the resistance of intrahepatic microcirculation through hepatic stellate cell (HSC) relaxation. However, the systemic administration of ROCK inhibitors causes severe hypotension; therefore, liver-specific ROCK inhibition is required. Here, we tested vitamin A (VA)-coupled liposomes carrying the ROCK inhibitor Y-27632 for targeted HSCs in steatotic rats. Rat livers with steatosis induced by a choline-deficient diet were subjected to IR injury. The d

A Case of Intravenous Leiomyomatosis with Involvement of a Renal Vein.

Our patient is a 44-year-old woman who underwent exploratory laparotomy with total abdominal hysterectomy and bilateral salpingo-oophorectomy by gynecologic oncology for a suspected malignancy. The diagnosis of intravenous leiomyomatosis (IVL) was made instead. She was followed by her gynecologist and remained asymptomatic until 2 years later when she complained to the GYN of leg heaviness, and 2 months later also complained of palpitations. She was treated with a short course of megestrol without any clinical response. She was then referred to Vascular Surgery. A computed tomography scan of t

Effects of Aerobic Exercise on Autonomic and Endothelial Function and Splanchnic Hemodynamics in Compensated Cirrhosis: RCT Study.

BACKGROUND: Patients with cirrhosis often develop autonomic dysfunction (AD) and endothelial dysfunction (ED), which correlate with higher mortality. Aerobic exercise (AE) is a potential intervention, yet its effects on the autonomic nervous system (ANS) and hemodynamics remain underinvestigated in cirrhosis. METHODS: We conducted a parallel-group randomized controlled trial including 50 sedentary patients with compensated cirrhosis. Participants were randomized to a 16-week moderate-intensity AE plus nutritional consultation (AE group; n = 25) or nutritional consultation alone (control group

Circulating miR-184 and miR-206 as predictive biomarkers for early recurrence in HBV-related hepatocellular carcinoma: a prospective study.

BACKGROUND: Early recurrence after curative resection remains a major challenge in hepatocellular carcinoma (HCC), particularly in hepatitis B virus (HBV)-related cases. Liquid biopsy using circulating microRNAs (miRNAs) offers a non-invasive approach to identify molecular markers predictive of recurrence. METHODS: We prospectively enrolled 30 patients with HBV-related HCC presenting with a single tumor (< 5 cm) and no vascular invasion or metastasis. Blood samples were collected preoperatively and on postoperative day 7. Expression of 20 selected miRNAs from circulating cell-free DNA/RNA and

Correlation of Cerebral Oxygen Saturation Change with Perioperative Complications in Elderly Patients Who Underwent Laparoscopic Partial Hepatectomy.

BACKGROUND: Laparoscopic partial hepatectomy is a crucial treatment for primary liver cancer but is associated with risks such as bleeding and organ hypoperfusion due to controlled low central venous pressure (CLCVP). Regional cerebral oxygen saturation (rSO2), measured using near-infrared spectroscopy (NIRS), serves as a reliable indicator of cerebral perfusion. This study aims to investigate the relationship between intraoperative rSO2 changes, perioperative outcomes, and neurological complications. METHODS: This retrospective study analyzed 218 elderly patients undergoing laparoscopic part

Robot-assisted hepatic mobilization and control of suprahepatic infradiaphragmatic inferior vena cava for level 3 vena caval thrombectomy: An IDEAL stage 0 study.

Robot-assisted level 1/2 inferior vena caval (IVC) thrombectomy techniques have been recently described for clinical use. Following the IDEAL recommendations on safe surgical innovation, we here describe a robotic approach for level 3 IVC thrombectomy in fresh frozen cadavers (IDEAL stage 0). In all cadavers (n = 3), hepatic mobilization with control of short hepatic veins, contralateral renal vein, infrarenal IVC and suprahepatic-infradiaphragmatic IVC could be achieved successfully, without converting to open surgery. Clinical feasibility of our technique remains to be tested.

Molecular mechanisms of circulatory dysfunction in cirrhotic portal hypertension.

Acute or chronic insults to the liver are usually followed by a tissue repairing process. Unfortunately, this action, in most cases, is not effective enough to restore the normal hepatic structure and function. Instead, fibrogenesis and regenerative nodules formation ensue, which are relatively nonfunctioning. The common final stage of the process is liver cirrhosis with increased intrahepatic resistance to portal venous blood flow. Throughout the entire course, the extrahepatic circulatory dysfunction, including increased splanchnic blood flow, elevated portal venous blood flow and pressure,

In vivo cardiac diffusion-weighted magnetic resonance imaging: quantification of normal perfusion and diffusion coefficients with intravoxel incoherent motion imaging.

OBJECTIVES: Diffusion-weighted imaging (DWI) and the introduction of the intravoxel incoherent motion (IVIM) model have provided a unique method for evaluating perfusion and diffusion within a tissue without the need for a contrast agent. Despite its relevance, cardiac DWI has thus far been limited by low b values because of signal loss induced by physiological motion. The goal of this study was to develop a methodology for estimating IVIM parameters of in vivo cardiac magnetic resonance imaging using an efficient DWI acquisition framework. This was achieved by investigating various acquisitio

[Portal venography with 64-slice MDCT in evaluation of the anatomic distribution of compensatory circulation resulting from posthepatitic cirrhosis].

This study sought to determine the value of portal venography with 64-slice MDCT in the evaluation of compensatory circulation resulting from decompensated posthepatitic cirrhosis (PHC), and in the clarification of its anatomic distribution. Thirty-six patients with clinically confirmed compensatory circulations resulting from PHC were enrolled in this study. They underwent thoracicoabdominal triphasic enhancement CT scans with 64-slice MDCT. The data of the portal venous phase acquired were used for obtaining CT-MIP (maximum intensity projection) images of portosystematic collaterals such as

Complications associated with percutaneous placement of venous return cannula for venovenous bypass in adult orthotopic liver transplantation.

Percutaneous large bore cannula placement during orthotopic liver transplantation (OLT) for use in venovenous bypass (VVB) has been reported to be a rapid and simple technique. It is, however, a technique that carries its own risks. The aim of the study was to investigate the incidence of complications related to the placement of a percutaneous venous return cannula and subsequent VVB in OLT. A retrospective review of 360 consecutive adult OLT patients during a period of 18 months (January 1, 2003 to June 30, 2004) was performed. The percutaneous venous cannula (18 Fr) was placed by an attendi

Renal blood flow detection with Doppler ultrasonography in patients with hepatic cirrhosis.

BACKGROUND: Hepatorenal syndrome, a well-recognized complication of established liver disease, is characterized by early renal vasoconstriction before clinically recognized renal disease. Renal vasoconstriction causes increased renal vascular resistance, which can be detected noninvasively by Doppler ultrasonography. OBJECTIVE: To detect early renal hemodynamic changes in patients with hepatic cirrhosis who had clinically normal renal functions. PATIENTS: Twenty patients with hepatic cirrhosis and ascites, 11 patients with hepatic cirrhosis without ascites, and 23 healthy control subjects. A
