Third-party CMV- and EBV-specific T-cells for first viral reactivation after allogeneic stem cell transplant.

Virus-specific T-cells (VSTs) from third-party donors mediate short- and long-term antiviral effects in allogeneic hematopoietic stem cell transplant (HSCT) recipients with relapsed or refractory viral infections. We investigated early administration of third-party VSTs, together with antiviral therapy in patients requiring treatment for first cytomegalovirus (CMV) or Epstein-Barr virus (EBV) infection. Thirty HSCT patients were treated with 1 to 4 VST infusions (2 × 107 cells/m2; CMV n=27, EBV n=3) at a median of 4 days after initiation of antiviral treatment. The overall viral response rate

Understanding LSTM Network Behaviour of IMU-Based Locomotion Mode Recognition for Applications in Prostheses and Wearables.

Human Locomotion Mode Recognition (LMR) has the potential to be used as a control mechanism for lower-limb active prostheses. Active prostheses can assist and restore a more natural gait for amputees, but as a medical device it must minimize user risks, such as falls and trips. As such, any control system must have high accuracy and robustness, with a detailed understanding of its internal operation. Long Short-Term Memory (LSTM) machine-learning networks can perform LMR with high accuracy levels. However, the internal behavior during classification is unknown, and they struggle to generalize

Primary hypothalamic lymphoma with clinical findings mimicking pituitary apoplexy: a case report.

BACKGROUND: Primary central nervous system lymphoma (PCNSL) is a rare but well-known extra-nodal lymphoma, which usually presents with non-Hodgkin B-cell lymphomas. PCNSL is generally located around the ventricle and is often detected as multiple lesions. It is rarely seen in the area of the hypothalamus. CASE PRESENTATION: We report the case of a 48-year-old Caucasian woman with progressive short-term memory deterioration, headache, mental confusion, diabetes insipidus (DI) and hypopituitarism. Early findings were suggestive of a pituitary apoplexy. The results of tests performed during the

Occam or Hickham? The diagnostic dilemma of an inflammatory brain lesion in the setting of pulmonary aspergillosis and previous neurosurgery.

INTRODUCTION: Occam's Razor is a precept which invites one to consider the simplest and most unifying diagnosis as correct. In the modern era, clinical diagnosis remains critical in selecting appropriate therapies yet grows ever more complex with increased information from diagnostic technologies, but not always with sufficient granularity to answer the clinical question with certainty. We present an example of this dilemma in a patient with complex partial seizures on a background of pulmonary aspergillosis and historic clipping of a middle cerebral artery (MCA) aneurysm. CASE DESCRIPTION: A

Predicting individual differences in reading, spelling and maths in a sample of typically developing children: A study in the perspective of comorbidity.

We examined reading, spelling, and mathematical skills in an unselected group of 129 Italian fifth graders by testing various cognitive predictors for each behaviour. As dependent variables, we measured performance in behaviours with a clear functional value in everyday life, such as reading a text, spelling under dictation and doing mental and written computations. As predictors, we selected cognitive dimensions having an explicit relation with the target behaviour (called proximal predictors), and prepared various tests in order to select which task had the best predictive power on each beha

A novel method for identifying hotspots and forecasting air quality through an adaptive utilization of spatio-temporal information of multiple factors.

Air pollution exerts serious impacts on human health and sustainable development. The accurate forecasting of air quality can guide the formulation of mitigation strategies and reduce exposure to air pollution. It is beneficial to explicitly consider both spatial and temporal information of multiple factors, e.g., the meteorological data, in the forecasting of air pollutant concentrations. The temporal information of relevant factors collected at a location should be considered for forecasting. In addition, these factors recorded at other locations may also provide useful information. Buruli ulcer: The Efficacy of Innate Immune Defense May Be a Key Determinant for the Outcome of Infection With Mycobacterium ulcerans.

Buruli ulcer (BU) is a neglected, tropical infectious disease of the skin and the subcutaneous tissue caused by Mycobacterium ulcerans. This pathogen has emerged as a new species from a common ancestor with Mycobacterium marinum by acquisition of the virulence plasmid pMUM. The plasmid encodes enzymes required for the synthesis of the macrolide toxin mycolactone, which has cytotoxic and immunosuppressive activities. In advanced BU lesions, extracellular clusters of M. ulcerans reside in necrotic subcutaneous tissue and are protected from infiltrating leukocytes by the cytotoxic activity of sec

Factor structure of the Montreal Cognitive Assessment in Parkinson disease.

OBJECTIVES: The Montreal Cognitive Assessment (MoCA) is a common tool for screening mild cognitive impairment (MCI) and dementia. Studies in multiple clinical groups provide evidence for various factor structures mapping to different cognitive domains. We tested the factor structure of the MoCA in a large cohort of early Parkinson disease (PD). MATERIALS AND METHODS: Complete MoCA data were available from an observational cohort study for 1738 patients with recent-onset PD (64.6% male, mean age 67.6, SD 9.2). Confirmatory factor analysis (CFA) was applied to test previously defined two-factor

Trajectory of severity of postoperative delirium symptoms and its prospective association with cognitive function in patients with gastric cancer: results from a prospective observational study.

PURPOSE: Delirium is a common neurocognitive complication in cancer. Despite this, the studies examining the trajectory of the severity of delirium symptoms and its impact on health outcome in gastric cancer is rather limited. This study examined the trajectory of delirium symptom severity (DSS) following resection surgery for gastric cancer and its prospective association with cognitive function. METHODS: A three-wave prospective observational study was conducted with 242 gastric cancer patients admitted for resection surgery at a teaching hospital in South Korea from May 2016 to November 20

Innate immune memory through TLR2 and NOD2 contributes to the control of Leptospira interrogans infection.

Leptospira interrogans are pathogenic spirochetes responsible for leptospirosis, a worldwide reemerging zoonosis. Many Leptospira serovars have been described, and prophylaxis using inactivated bacteria provides only short-term serovar-specific protection. Therefore, alternative approaches to limit severe leptospirosis in humans and morbidity in cattle would be welcome. Innate immune cells, including macrophages, play a key role in fighting infection and pathogen clearance. Recently, it has been shown that functional reprograming of innate immune cells through the activation of pattern recogni

n-3 polyunsaturated fatty acids prevent d-galactose-induced cognitive deficits in prediabetic rats.

Nutritional deficit of n-3 polyunsaturated fatty acids (PUFAs) is closely related to cognitive impairment and depression in later life. Cognitive impairment and depression lead to comorbidities, such as metabolic syndrome, in elderly people. The aim of this study is to evaluate the effects of dietary n-3 PUFAs on cognition and depressive-like behavior in an accelerated senescence rat model with prediabetic status. Rats were cotreated with d-gal and sucrose solution for 7 months and then fed fish-oil- or flaxseed-oil-rich diets for 3 months. Cognitive impairment analysis and depressive-like beh

Atypical speech lateralization in adults with developmental coordination disorder demonstrated using functional transcranial Doppler ultrasound.

Research using clinical populations to explore the relationship between hemispheric speech lateralization and handedness has focused on individuals with speech and language disorders, such as dyslexia or specific language impairment (SLI). Such work reveals atypical patterns of cerebral lateralization and handedness in these groups compared to controls. There are few studies that examine this relationship in people with motor coordination impairments but without speech or reading deficits, which is a surprising omission given the prevalence of theories suggesting a common neural network underl
