Risk-free hospital at home: Strengthening safety in the patient's environment through hospital pharmacy.

Hospital at home has become an established care model capable of delivering high-complexity treatment in the patient's own environment; however, it shifts part of the medication circuit to a less controlled setting, introducing variability that may compromise pharmacotherapeutic safety. A descriptive quality-improvement study was conducted to evaluate the implementation of a multidimensional medication-safety programme in the Hospital at Home unit of a tertiary-care hospital. The programme was structured according to the Joint Commission International standards for home care and led by the Hos

Eating Behaviors, Appetite, and Childhood Obesity in Early Childhood: A Qualitative Study of Mothers' Perceptions and Prevention Approaches.

OBJECTIVE: In early childhood, packaged food consumption, screen-based sedentary behavior, and inconsistent parental feeding practices increase the risk of obesity. Mothers play a central role in shaping children's feeding environments. This study explored mothers' perceptions of eating and appetite behaviors in children aged 36 years, their feeding management strategies, and their evaluations of obesity risk within a cultural context. METHODS: A phenomenologically oriented qualitative design within an interpretiveconstructivist paradigm was employed. Fifteen mothers of children aged 36 years

Reimagining resuscitation ethics: a narrative review and emergent ethical pathways model.

BACKGROUND: Decision-making surrounding cardiopulmonary resuscitation (CPR) presents persistent ethical challenges in acute and critical care. Existing evidence points to ambiguous do-not-resuscitate pathways, moral distress, family-driven demands, and systemic pressures, yet these influences are often examined separately. This review synthesizes current evidence to clarify the ethical, relational, and organizational dynamics that influence CPR-related decisions. METHODS: We conducted a structured narrative review of four databases (Web of Science, PubMed, Scopus, Embase) for studies publishe

Generative AI and intelligent tutoring systems in nursing education: A systematic review of impacts on higher-order thinking skills and clinical competency.

AIMS: This review synthesizes the impact of Artificial Intelligence (AI) on nursing students' clinical competency, Higher-Order Thinking Skills (HOTS) and educational outcomes to differentiate between procedural efficiency and deep cognitive retention. BACKGROUND: Generative AI and Intelligent Tutoring Systems (ITS) offer personalized scaffolding. However, their comparative efficacy against conventional pedagogical approaches, such as face-to-face lectures and standard non-AI simulations, remains controversial, particularly regarding the potential trade-off between skill acquisition and cogni

Phosphorus Intake and Cancer Risk: A Theoretical-Conceptual Model and Hypothesis for Population-Study Replication.

Recent findings in nutritional epidemiology report an association between high dietary phosphorus intake and increased cancer risk. Building on the author's analysis of breast cancer incidence in the Study of Women's Health Across the Nation (SWAN), this paper presents a theoretical-conceptual model and a hypothesis to guide further population-study replication. To strengthen the initial SWAN analysis signal, a sensitivity analysis increased the number of controls in the nested case-control design from four to five per case. This adjustment modestly raised the relative risk (RR) of breast canc

Comparing Stapled and Hand-Sutured Colorectal Anastomoses: A Systematic Review and Meta-Analysis of Observational Cohorts Assessing Short- and Long-Term Complications.

This systematic review and meta-analysis was conducted to compare outcomes after stapled and hand-sewn colorectal anastomoses. The study was undertaken because complication patterns, such as anastomotic leak, surgical site infection (SSI), length of stay (LOS), and mortality, remain debated and may vary by technique. The objective was to evaluate whether either method showed a clear advantage across early and late outcomes. A systematic search of PubMed, Scopus, Web of Science, CINAHL (Cumulative Index to Nursing & Allied Health Literature), and Google Scholar was completed for studies publish

Cumulative excess weight exposure over time and cardiovascular risk: A prospective cohort study.

BACKGROUND: Obesity at a given point in time is a known cardiovascular risk factor. However, the contribution of long-term excess weight exposure to cardiovascular risk has not been well established. We therefore conducted a study to evaluate the relationship between long-term excess weight exposure and incidence of cardiovascular events. METHODS: This secondary analysis of a prospective cohort study analyzed data from adult participants in Nurses' Health Study and Health Professionals Follow-Up Study with a BMI > 25 kg/m2 between 01/01/1990 and 12/31/1999. We investigated the relationship be

Randomized Evaluation of Nutritional Risk Screening and HLA-DMB Gene Expression in Early Prediction of Sepsis.

BACKGROUND: Sepsis, a life-threatening syndrome with escalating mortality per treatment delay, requires prognostic tools beyond current Sepsis-3 criteria. This study investigated the dual predictive capacity of Nutrition Risk Screening 2002 (NRS-2002) and Human Leukocyte Antigen-DMB (HLA-DMB), proposing a novel early-warning framework for sepsis risk stratification. METHODS: This case-control study enrolled 90 patients with acute infections from the Department of Pulmonary and Critical Care Medicine at Hefei Second People's Hospital. Participants were stratified into sepsis (n = 45) and non-s

Improving end-of-life care in the cardiac intensive care unit: Navigating the challenge of timing palliative care intervention.

BACKGROUND: Integrating end-of-life (EoL) care in cardiac intensive care units (CICUs) is particularly complex because it requires a shift from a purely curative approach to one that emphasizes symptom management, emotional and spiritual support and patient-centered care. Moreover, this transition is challenging due to the need to balance life-sustaining treatments with the goals of comfort and dignity. The concept of EoL care varies across countries and is influenced by cultural, ethical and legal factors. METHODS: This narrative review examines palliative and EoL care in critically ill card

Factors influencing optimal postoperative pain management: a systematic scoping review.

Effective postoperative pain management is a critical component of surgical care, directly influencing patient recovery, satisfaction, and long-term outcomes. Despite recent advances in pain management, postoperative pain remains poorly managed worldwide, suggesting that postoperative pain outcomes are influenced by several factors beyond the inherent efficacy of available treatments. The aim of this scoping review was to map the factors influencing postoperative pain management and to generate interventions that support optimal, evidence-based pain management after surgery.
