Tourette syndrome: evolving concepts.

Tourette syndrome is a common childhood-onset neurobehavioral disorder characterized by multiple motor and phonic tics affecting boys more frequently than girls. Premonitory sensory urges prior to tic execution are common, and this phenomenon helps to distinguish tics from other hyperkinetic movement disorders. Tourette syndrome is commonly associated with attention deficit hyperactivity disorder, obsessive-compulsive disorder, learning difficulties, and impulse control disorder. The pathophysiology of this complex disorder is not well understood. Involvement of basal ganglia-related circuits

Sleep Disturbance in Parkinson's Disease: Consequences for the Brain and Disease Progression - A Narrative Review.

Sleep disturbance in Parkinson's disease (PD) is recognized to be one of the most common non-motor complications. It occurs before disease onset as a prodromal symptom, during and then throughout the disease course. Causes are multifactorial and can be multiple in the same patient. Specific sleep disorders that are known to occur in those with Parkinson's disease include REM sleep behavior disorder, sleep disordered breathing including obstructive sleep apnea, restless legs and periodic limb movements, nightmare disorder, insomnia alongside direct side-effects of the medication used for therap

[EXCORIATION (SKIN-PICKING) DISORDER].

Excoriation (Skin-Picking) disorder is a clinically recognized condition which was recently included in the Diagnostic and Statistical manual of the American Psychiatric Association (DSM) - fifth edition, as OCD (obsessive compulsive disorder) related disorder. The disorder's official status has been achieved due to its high frequency and unique clinical picture involving both mental and physical impairment. In this article, we would like to present a concise review of the literature together with an illustrative case. Epidemiological surveys show a prevalence of 3% to 5% for the general popul

High-frequency loading of lumbar ligaments increases proinflammatory cytokines expression in a feline model of repetitive musculoskeletal disorder.

BACKGROUND CONTEXT: Cumulative (repetitive) lumbar disorder is common in the workforce, and the associated epidemiology points out high risk for lifting heavy loads, performing many repetitions, and performing movements at high velocity. Experimental verification of viscoelastic tissue degradation and a neuromuscular disorder exist for cyclic work under heavy loads. Experimental validation for a disorder because of cyclic loads under high-velocity movement is missing. PURPOSE: Obtain experimental verification that high-velocity lumbar flexion-extension results in significant increase of proin

Oculometric Assessment of Sensorimotor Impairment Associated with TBI.

PURPOSE: Diffuse tissue damage from impact or blast traumatic brain injury (TBI) degrades information processing throughout the brain, often resulting in impairments in sensorimotor function. We have developed an eye-movement assessment test, consisting of a simple, appropriately randomized, radial tracking task together with a broad set of oculometric measures that can be combined to yield a sensitive overall indicator of sensorimotor functional status. We show here that this multidimensional method can be used to detect and characterize sensorimotor deficits associated with TBI. METHODS: To

Assessment of standing balance deficits in people who have undergone anterior cruciate ligament reconstruction using traditional and modern analysis methods.

Modern methods of assessing standing balance such as wavelet and entropy analysis could provide insight into postural control mechanisms in clinical populations. The aim of this study was to examine what effect anterior cruciate ligament reconstruction (ACLR) has on traditional and modern measures of balance. Ninety subjects, 45 who had undergone ACLR and 45 matched controls, performed single leg static standing balance tests on their surgical or matched limb on a Nintendo Wii Balance Board. Data were analysed in the anterior-posterior axis of movement, which is known to be affected by ACLR. T

Skill generalization relevant to robotic neuro-rehabilitation.

Upper limb extremity rehabilitation practices are increasingly involving robotic interaction for repetitive practice, and there is increasing skepticism whether such systems can provide the relevant practice that can be generalized (or transferred) to functional activities in the real world. Most importantly, will patients be able to generalize in three critical ways: (1) to unpracticed directions, (2) to unpracticed movement distances, and (3) to unpracticed weight-eliminated conditions? Rather than presuming that patients could generalize in three conditions, this study tested whether there

The linear vestibulo-ocular reflex in patients with skew deviation.

PURPOSE: The linear vestibulo-ocular reflex (LVOR) is mediated primarily by the otolith organs in the inner ear. Skew deviation is a vertical strabismus believed to be caused by imbalance of otolithic projections to ocular motor neurons (disynaptically through the medial longitudinal fasciculus in the brain stem or polysynaptically through the cerebellum). The authors postulated that if skew deviation is indeed caused by damage to these projections, patients with skew deviation would show abnormal LVOR responses. METHODS: Six patients with skew deviation caused by brain stem or cerebellar les

Eye Responsivity During Literacy Activities in Children With Reading Disabilities.

PURPOSE: This study evaluated eye movements (i.e., regressions, fixations, and saccades) in a variety of reading contexts in school-age children with and without reading disorders (RDs). METHOD: This study used an experimental design with two groups: peers with RDs and typically developing (TD) peers. A Tobii Pro Spectrum system was used to capture eye movement data for all participants during the reading tasks under the following conditions: (a) single words without cognitive demand, (b) single-word reading with cognitive demand, and (c) grade-level paragraph reading text. RESULTS: The resu

Short-term effects of vascular photobiomodulation on temporomandibular disorders in children: a randomized, controlled, single-blind pilot study.

Temporomandibular disorders (TMDs) are highly prevalent among children, the signs and symptoms of which include muscle pain, ringing in the ears, limited vertical movement of the jaw and deviated mouth opening. Recent studies have demonstrated that low level laser irradiation of the blood stream, which is denominated vascular photobiomodulation (VPBM), can diminish pain and inflammation and has sedative and systemic effects. The aim of the present study was to investigate the influence of VPBM on the reduction in pain and increase in the vertical movement of the jaw in children with TMDs. Chil

Effect of EMG-activated vibrotactile biofeedback on skill learning in children with genetic and acquired dystonia during point-to-point and cyclic task practice.

OBJECTIVE: Dystonia is a movement disorder that causes involuntary muscle contractions and abnormal movements. Often, repeated practice does not lead to motor improvement in children with acquired dystonia, likely because of sensory deficits, which may contribute to their impairment. Therefore, improvements in sensory function might improve motor performance. In this study, we propose that augmented vibrotactile biofeedback may improve motor learning in children with acquired dystonia but not in children with genetic dystonia who do not have associated sensory deficits. In vivo cervical vertebrae kinematic studies based on dual fluoroscopic imaging system measurement: A narrative review.

Understanding the motion characteristics of cervical spine through biomechanical analysis aids in the identification of abnormal joint movements. This knowledge is essential for the prevention, diagnosis, and treatment of related disorders. However, the anatomical structure of the cervical spine is complex, and traditional medical imaging techniques have certain limitations. Capturing the movement characteristics of various parts of the cervical spine in vivo during motion is challenging. The dual fluoroscopic imaging system (DFIS) is able to quantify the motion and motion patterns of individu
