Genetic polymorphism of the Rv3303c-Rv3304 intergenic region in Beijing and non-Beijing genotype Mycobacterium tuberculosis strains from China.

OBJECTIVE: To investigate the polymorphism of the Rv3303c-Rv3304 intergenic region (IR) in Beijing and non-Beijing genotype Mycobacterium tuberculosis (M. tuberculosis) strains isolated from China, and to explore its regulatory effects on downstream gene expression. METHODS: A total of 223 M. tuberculosis isolates from China were classified into Beijing, non-Beijing genotype and modern Beijing subgenotype based on single nucleotide polymorphisms (SNPs). The polymorphism of Rv3303c-Rv3304 IR was identified by DNA sequencing, and a lacZ reporter system was applied to assess its regulatory role

A Perfect Storm in Advanced AIDS: Severe Acute Respiratory Distress Syndrome From Pneumocystis Jirovecii and Mycobacterium Avium Complex Coinfection.

Advanced HIV infection is associated with profound immunosuppression, predisposing patients to opportunistic infections such as Pneumocystis jirovecii pneumonia (PJP) and Mycobacterium avium complex (MAC). PJP can cause severe respiratory disease, including acute respiratory distress syndrome (ARDS), while disseminated MAC may contribute to systemic illness and pulmonary complications. Concurrent infection with both pathogens is uncommon but may precipitate severe pulmonary failure. A man in his 40s presented with unintentional weight loss, fatigue, oral thrush, and progressive dyspnea. On arr

ESAT-6-derived lipopeptide-based mucosal vaccine elicits distinct humoral immune responses concomitant with a reduction in mycobacterium infection.

Respiratory mycobacterial infections remain a major global health threat, yet current vaccine strategies, which focus primarily on cell-mediated immunity, have met with limited success. We previously demonstrated that a vaccine composed of five lipopeptides (LPs) encoding immunodominant ESAT-6 epitopes elicits protective cellular immune responses against Mycobacterium tuberculosis infection. In this study, we enhanced the ESAT-LPs vaccine with a novel mucosal immune modulator, heat-killed Caulobacter crescentus (HKCC), and examined both cellular and humoral immunity in a mouse model (BALB/c) o

Frequently arising ESX-1-associated phase variants influence Mycobacterium tuberculosis fitness in the presence of host and antibiotic pressures.

UNLABELLED: Mycobacterium tuberculosis (Mtb) exhibits an impressive ability to adapt to rapidly changing environments, despite its genome's apparent stability. Recently, phase variation through indel formation in homopolymeric tracts (HT) has emerged as a potentially important mechanism promoting adaptation in Mtb. This study examines the impact of common phase variants associated with the ESX-1 type VII secretion system, focusing on a highly variable HT upstream of the ESX-1 regulatory factor, espR. By engineering this frequently observed indel into an isogenic background, we demonstrate that

IgG antibody response to Mycobacterium tuberculosis curli pili (MTP) in people from different geographical regions in Sub-Saharan Africa.

Previously, a slot blot or an indirect enzyme-linked immunosorbent assay (ELISA) using a synthetic or purified MTP antigen, conceptually demonstrated IgG antibody induction in pulmonary TB patients, albeit with small sample sizes and differing sensitivity. Therefore, we evaluated an IgG MTP ELISA in larger populations from The Gambia (n = 549), Uganda (n = 161), and South Africa (n = 193), comprising human immunodeficiency virus (HIV) positive and negative, with microbiologically confirmed active TB. The association between the IgG level and demographic characteristics was determined by multiv

In silico evaluation of snake venom proteins against multidrug-resistant Mycobacterium tuberculosis: A molecular dynamics study and simulation dynamic.

BACKGROUND: Mycobacterium tuberculosis (Mtb), a pathogen that belongs to the M. tuberculosis complex, causes tuberculosis (TB), an infectious bacterial disease. Although it usually affects the lungs and results in pulmonary TB, it can also lead to extra-pulmonary TB by affecting other regions of the body. TB, which ranks first on the list of infectious diseases that kill the most people, affects one-third of the world's population and has a high mortality and morbidity rate. The clinical treatment of active TB infection mainly relies on the use of Isoniazid (INH) in combination with three othe

Plasma-based ultrasensitive detection of Mycobacterium tuberculosis ESAT6/CFP10 fusion antigen using a CRISPR-driven aptamer fluorescence testing (CRAFT).

Tuberculosis (TB) screening in clinical diagnosis is challenging due to issues such as sputum dependence, time-consuming procedures, and high costs. In this study, we introduce a CRAFT (CRISPR-Driven Aptamer Fluorescence Testing), an aptamer-based CRISPR/Cas12a assay designed for the rapid and sensitive detection of Mycobacterium tuberculosis (Mtb) antigens from peripheral blood. Aptamer 3 (Ap3) and the aptamer-mediated probe (Aptamer-blocker 3-7) were selected through the Systematic Evolution of Ligands by Exponential Enrichment (SELEX). Ap3 demonstrated a dissociation constant (Kd) of 8.3E-7

Mycobacterium tuberculosis, Streptococcus pneumoniae, and Staphylococcus aureus Co-infection in a Two-Year-Old Immunocompetent Patient: A Case Report.

Co-infections in pulmonary tuberculosis are rare among immunocompetent children in settings with low tuberculosis prevalence. We present a case of a two-year-old immunocompetent child with necrotic pleuropneumonia caused by Mycobacterium tuberculosis, Streptococcus pneumoniae, and Staphylococcus aureus co-infection in a low tuberculosis prevalence setting. A 24-month-old boy presented with a five-day history of cough, followed by three days of high fever and dyspnea. Initial laboratory inflammatory markers were elevated. Chest ultrasound, radiography, and CT scan revealed necrotic changes in t

Loss of the PPE71-esxX-esxY-PPE38 locus drives adaptive transcriptional responses and hypervirulence of Mycobacterium tuberculosis lineage 2.

Mycobacterium tuberculosis (M.tb) is remarkable for its immense global disease burden and low mutation rate. Despite strong selective pressure, M.tb shows frequent deletions at the PPE71-38 locus, most notably in hypervirulent L2 Beijing strains. Here, we show that loss of the PPE71-38 locus causes increased stress response gene expression and increased triglyceride levels. In addition, we demonstrate that reintroduction of PPE71 into the L2 strain HN878 suppresses the baseline elevation of these transcripts, while overexpression of PPE71 increases the localization of PE_PGRS proteins and lipo

Toxoplasma gondii soluble antigen (STAg) induces resistance against Mycobacterium tuberculosis in a human monocyte cell line via P2X7 receptor.

BACKGROUND: The control of T. gondii in macrophages is mediated by the P2X7 receptor. The P2X7 receptor plays a crucial role in defense against Mycobacterium tuberculosis (M.tb), as well. Current study aimed to investigate the immunity induced by T. gondii soluble total antigen (STAg) against M.tb via P2X7R in human monocyte cell line. METHODS: The STAg was prepared from 1.6 × 107 of fresh T. gondii tachyzoites, RH strain. The siRNA targeting P2X7R was employed to knockdown P2X7R in M0 THP-1 macrophages. Prior to M.tb infection, the cells were treated by 80 µg/mL of STAg for 4 h. The cells we

A refined low-dose murine model of Mycobacterium ulcerans infection to assess integrated immune networks in Buruli ulcer pathogenesis.

Mycobacterium ulcerans, the causative agent of Buruli ulcer (BU), has a low infectious dose (<10 colony-forming units [CFU]). BU animal infection models typically use challenge doses orders of magnitude higher than natural infection. These doses are unrealistic for vaccine trials and studies of immunity. Here, we address this issue and describe a murine tail infection model in two genetically distinct mouse strains (BALB/c and C57BL/6) using quality-controlled M. ulcerans challenge doses (10-20 CFU and 100 CFU). Over 24 weeks, we measured >70 clinical, immunological, and microbiological parame

Luciferase reporter mycobacteriophage (TM4::GeNL) enables rapid assessment of drug susceptibilities and inducible macrolide resistance in Mycobacterium abscessus complex.

Mycobacterium abscessus (MAB) infections are challenging to treat due to high-level resistance to anti-tuberculosis drugs, carbapenems, fluoroquinolones, and tetracyclines. Clarithromycin and amikacin are considered cornerstone drugs for MAB treatment due to their superior efficacy; however, assessing clarithromycin susceptibility typically takes 7 to 14 days because of inducible macrolide resistance. We developed a 48 h luciferase reporter mycobacteriophage drug susceptibility testing (LRM-DST) assay using TM4::GeNL to evaluate MAB drug susceptibility. For this proof-of-principle study, we pe

Prevalence, Distribution, and Risk Factors of Mycobacterium Other Than Tuberculosis Among Tuberculosis Presumptive Patients in Karonga District in Malawi.

INTRODUCTION: Besides tuberculosis (TB), there are also other nontuberculous mycobacteria (NTM) that present with similar clinical signs and symptoms as TB. If not promptly found and treated, these organisms may affect the programming of the TB control and elimination campaign. The study sought to establish the prevalence, distribution, and factors contributing to MOTT infections among presumptive TB patients in the Karonga district. METHODS: A descriptive cross-sectional study research design was employed. A total of 196 participants were included in the study using a census approach. Data w

Differences in local immune response of bait Mycobacterium bovis heat-inactivated vaccinated badgers showing exacerbated tuberculous lesions after challenge.

European badgers (Meles meles) are reservoirs for animal tuberculosis (TB) in some European countries, complicating TB control in cattle. Badger vaccination and a deeper understanding of the subsequent protection mechanisms are necessary for effective TB control. In a previous study, two of eight badgers immunized with the heat-inactivated Mycobacterium bovis (HIMB) vaccine exhibited an unusual immune response (divergent), developing exacerbated lesions. The present study aimed to describe the local immune response in divergent badgers (those with severe disease progression), with respect to t

Association between immunity and antimicrobial treatment resistance in patients with Mycobacterium avium complex pulmonary disease: a multicenter observational study.

BACKGROUND: Mycobacterium avium complex pulmonary disease (MAC-PD) is a refractory infectious disease, with a low success rate of the antimicrobial therapy. T cells and macrophages play a critical role in regulating the mechanism of immunity against mycobacterial infections. Although T cell dysfunction may be associated with treatment failure in MAC-PD, supporting evidence is scarce. This study aimed to elucidate the immunological characteristics of patients with refractory MAC-PD, focusing on immunosuppression and T cell exhaustion. METHODS: Patients with MAC-PD who received standard antimic

MicroRNA-mediated regulatory mechanisms in cow's ileum and ileum lymph node in response to Mycobacterium avium subspecies paratuberculosis infection.

Mycobacterium avium subsp. paratuberculosis (MAP) causes Johne’s disease (JD), a chronic enteritis infection affecting ruminants with significant economic implications for the dairy industry. The molecular regulatory mechanisms governing host responses to MAP infection remain unclear. This study elucidated the roles of microRNAs (miRNAs) in the ileum (IL), ileal lymph node (ILLN), and liver of cows during subclinical MAP infection. Tissues (IL, ILLN, and liver) were collected from four JD-positive (JDP), five MAP-tolerant (MAPT), and five healthy control (HC) cows. MiRNA transcriptomes were an
