Splenic abscess is a rare disease whose diagnosis is difficult, though the use of modern imaging methods has increased diagnosed cases in recent years. We report a case of splenic abscess whose aetiology is unusual, namely, a gastric ulcer penetrating into the splenic artery and causing arterial thrombosis and septic embolism. Ultrasonography and ultrasound-guided puncture resolved any diagnostic doubt, and subsequent surgery confirmed the diagnosis. Pathogenesis, clinical picture, diagnostic methods, and treatment are discussed with reference to the literature.

Granuloma annulare, a relatively common cutaneous disorder of unknown origin, is named for its predominantly papular lesions, which tend to group and coalesce in an annular fashion. Clinically distinct subtypes are divided into localized, generalized, perforating, and subcutaneous forms. Only the superficial granulomata of the extremities in subcutaneous granuloma annulare have been radiographically depicted in the literature. We describe and illustrate what we believe is the first radiographic presentation of bone and joint involvement with advanced, generalized granuloma annulare.

CHARGE syndrome, first described by Pagon, was named for its six major clinical features. They are: coloboma of the eye, heart defects, atresia of the choanae, retarded growth and development including CNS anomalies, genital hypoplasia and/or urinary tract anomalies, and ear anomalies and/or hearing loss. We experienced three cases of CHARGE syndrome who displayed ocular coloboma, heart defects, retarded growth and development, and external ear anomalies, and we also review the previously reported literature concerning CHARGE syndrome.

The topical imidazole antimycotics are widely used and are an infrequent cause of contact allergy. We report on a woman, who developed an unusual clinical picture of allergic contact dermatitis, namely papulo-pustular reaction, evoked by an isoconazole nitrate-containing cream. Parkinsonism is a common complication of neuroleptic drug use; however, the pathophysiology of the persistence of parkinsonian symptoms after withdrawal of neuroleptic drugs is poorly understood. Twenty patients with neuroleptic induced parkinsonism were studied by high field MRI. Persistence of symptoms was associated with different findings depending on the age of the patients--namely, putaminal hypointensity in young patients and striatal hyperintensities in old patients. High field MRI may be useful in identifying patients at higher risk for neuroleptic induced parkinsonism.

Unlike other vitamins, Vitamin E probably has received less attention in the past because humans are not likely to suffer from deficiency disorders. This fat soluble vitamin is available in many foods, is easily stored, and is readily reused by the body. The original research carried out in the 1920s found that a Vitamin E deficiency in rats produced problems in their reproductive capacity. The name given to the substance at that time, "tocopherol," reflects this action as it is taken from the Greek word, "tocos," meaning "to give birth."

Occupational problems of the foot and ankle have received less attention than those of other body regions, namely the hand, neck, and lumbar spine. Functional requirements of the foot and ankle differ from other body regions, necessitating a different clinical approach to injuries than those indicated in the upper extremity or back. This article reviews the clinical characteristics of the foot and ankle pertinent to the worker and highlights the unique features of the management of foot and ankle problems encountered in occupational orthopedic practice.

Defining "emotional abuse" and "psychological abuse" is a task made difficult by uncertainty as to whether or not such terms are synonomous and/or interchangeable. There is an increasing tendency in child protection literature to regard them as synonomous, or at least, to make little distinction between them. The purpose of this paper is to explore the origins of this tendency, and to provide an alternative view, namely, that the terms are not synonomous, and there should be different definitions for each. A procedure for operative management of liver echinococcosis (hydatid disease) is implemented in practice, consisting in resection of the "protruding dome", resection of the altered and thinned surrounding liver tissue, and omentoplasty using tissue adhesive. A case of acute biphenotypic leukemia, in which blast cells coexpress antigens of both lymphoid and myeloid lineages, was seen in a 24-year-old female. The clonal karyotype was complex but did not include any of the abnormalities previously found in biphenotypic leukemia. Chromosome abnormalities included breakpoints seen in both acute lymphoblastic and acute myeloid leukemias as well as a number of known fragile sites, namely the rare heritable fragile site at 10q24, and the common, aphidicolin-induced sites on chromosome 7.

A new and simple technical method for insertion of a soft silastic nasogastric tube into the stomach in neonates born with esophageal atresia is described. The technique is simple, easy to perform, and enables feeding the baby through this tube a day after the operation. Our method is based on the principle used in insertion of Broviac-Hickman catheters, namely the use of a "peel-away sheath" through which the silastic feeding tube is threaded. We have used this technique in five patients with esophageal atresia without any complication.

A cross-sectional survey by standard questionnaire was conducted to determine the public's knowledge (n = 480) of diabetes and diabetes symptoms. Four hundred and sixty-two (96%) subjects had heard of diabetes, 350 (73%) could give a rudimentary definition of diabetes, but 231 (48%) were unable to name any symptom and only 20 (4%) knew of thirst and polyuria in combination. The public's knowledge of diabetes symptoms is poor. This may possibly contribute to delayed presentation of Type 2 diabetes.

Six patients with well-circumscribed, indurated, inflammatory plaques of the lower extremity are described. Incisional biopsy specimens in each patient revealed fat necrosis, sclerosis, and a lobular panniculitis. These patients are similar to those described under the designations hypodermitis sclerodermaformis or lipodermatosclerosis. The name sclerosing panniculitis is proposed to unify these conditions. Thirteen human proteins were found in plasma and cystic fluid of mice bearing human testicular yolk sac tumor. Six of them, alpha-fetoprotein, prealbumin, albumin, alpha 1-antitrypsin, hemopexin and transferrin, had been previously demonstrated to be produced by yolk sac tumors. The syntheses of the remaining seven, namely apolipoprotein A1, retinol-binding protein, alpha 2HS-glycoprotein, haptoglobin, apolipoprotein B, C5 component and anti-hemophilic factor, have been demonstrated for the first time.

Two surveys (one involving 650, the other 638 women) based upon interviews about the utilization of prenatal diagnosis by clients are reported. First it is demonstrated which factors obstruct a decision for prenatal diagnosis. Then, a contrast is made between the risk attitude of two groups, namely women who underwent amniocentesis or chorion villi biopsy and those respondents who decided against prenatal diagnosis. Finally, how the examination technique (amniocentesis or chorion villi biopsy) influences the experience of pregnancy is discussed.

Three patients are described with clinical features of connective tissue diseases, namely, dermatomyositis, progressive systemic scleroderma, and systemic lupus erythematosus. In two patients the symptoms of disease overlapped. The rare anti-Ku antibody was found in all of them. Anti-Ku antibody characterizes those patients, with polymyositis-scleroderma overlap syndromes who have a good prognosis. One of our patients, who also had severe anti-Sm-positive systemic lupus erythematosus, seems to be an exception.

Oral manifestations in AIDS, in AIDS-related complex, and in patients with antibodies to the AIDS-causing virus, human immunodeficiency virus (HIV), have been described previously. One of these manifestations, oral hairy leukoplakia, is apparently specifically associated with HIV infection and has until recently been reported in homosexual men only. This article demonstrates, among European patients, the occurrence of hairy leukoplakia in three patients belonging to another risk group for AIDS, namely HIV-infected hemophiliacs.

Authors present two cases of children with abnormal movements. One of them described slow and dystonic movements of neck and head due to an important gastroesophageal reflux. These movements were useful to decrease annoyances that reflux causes in the child. Appearance of these movements and reflux is what is named "Sandifer syndrome". The other case presented stereotyped movements of head, like a negation, continuous and produced by a long intake of neuroleptics, which had caused a blockade of central dopaminergic receivers. Suppression neuroleptics of cured the child.
