Sleep matters: sleep functioning and course of illness in bipolar disorder.

BACKGROUND: Few studies have prospectively examined the relationships of sleep with symptoms and functioning in bipolar disorder. METHODS: The present study examined concurrent and prospective associations between total sleep time (TST) and sleep variability (SV) with symptom severity and functioning in a cohort of DSM-IV bipolar patients (N = 468) participating in the National Institute of Mental Health Systematic Treatment Enhancement Program for Bipolar Disorder (STEP-BD), all of whom were recovered at study entry. RESULTS: Concurrent associations at study entry indicated that shorter TST

[Respiratory syntomatic prevalence, infection and tuberculosis disease and associated factors: population-based study].

A cross sectional survey on TB epidemiological characteristics was carried out in Mitú (Vaupes, Colombia) with the aim of measuring the prevalence of TB cases, the prevalence of TB suspected cases, the coverage with BCG vaccine and the prevalence of infection with Mycobacterium tuberculosis. One hundred and sixty five (165) households were included in the survey using a randomized cluster sampling design (n=20 clusters) which yielded a sample size of 972 subjects. The prevalence of TB suspect cases was 3.6% (C.I.95% 2.6-4.9%); coverage with BCG vaccine was 94%. Vaccinated people had a lower ch

Comparison of lifetime psychiatric diagnoses in Premenstrual Syndrome Clinic and community samples.

The purpose of this investigation was to obtain information about lifetime psychiatric diagnoses of women seeking treatment for premenstrual syndrome. The National Institute for Mental Health Diagnostic Interview Schedule (DIS) was administered to 223 women attending a premenstrual syndrome clinic. Rates of symptoms and psychiatric disorders were then compared with DIS data collected from an Epidemiologic Catchment Area (ECA) program community sample of 923 women in the same age group from the same geographic location. Women in the Premenstrual Syndrome Clinic sample met DIS/DSM-III criteria f

TB Data Improvement in Nkembo Health Treatment Center in Libreville, Gabon.

UNLABELLED: Although the estimated tuberculosis (TB) incidence in Gabon is declining, there have been challenges with treatment coverage, HIV status and treatment outcome documentation. Thus, the National TB Program (NTP) conducted an innovative data review at the Nkembo Health Treatment Center in Libreville, which manages more than 70% of Gabonese TB patients. Since our hypothesis was that the Nkembo treatment center was struggling with data mismanagement due to the workload, the objective was to perform a TB data quality review and triangulation exercise at the Nkembo health facility in Libr

Early Outcomes of Elective Anterior Cervical Diskectomy and Fusion for Degenerative Spine Disease Correlate With the Specialty of the Surgeon Performing the Procedure.

BACKGROUND: Comparative effectiveness research has a vital role in recent health reform and policies. Specialty training is one of these provider-side variables, and surgeons who were trained in different specialties may have different outcomes on performing the same procedure. OBJECTIVE: To investigate the impact of spine surgeon specialty (neurosurgery vs orthopedic surgery) on early perioperative outcome measures of elective anterior cervical diskectomy and fusion (ACDF) for degenerative spine diseases. METHODS: This was a retrospective, 1:1 propensity score-matched cohort study. In total

Liver disease burden and required treatment expenditures for hepatitis C virus (HCV) infection in Thailand: Implications for HCV elimination in the new therapeutic era, a population-based study.

The prevalence of hepatitis C virus (HCV) infection has been decreasing globally, but the growing effects of HCV-related morbidity and mortality remain of concern. Advances in curative medicine, involving direct-acting antivirals (DAAs), have led many countries to aim to eradicate HCV. Information on epidemiology and disease burden is essential for national policy development. Thus, this study aimed to determine the HCV-related hepatic disease burden in areas of Thailand with high and average HCV prevalence in order to extrapolate the viral burden across Thailand. Patients previously diagnosed

Potential epidemiologic, economic, and budgetary impacts of current rates of hepatitis C treatment in medicare and non-medicare populations.

We forecast the health and budgetary impact of hepatitis C (HCV) treatment on the Medicare program based on currently observed rates of treatment among Medicare and non-Medicare patients and identify the impact of higher rates of treatment among non-Medicare populations. We developed a computer microsimulation model to conduct an epidemiologic forecast, a budgetary impact analysis, and a cost-effectiveness analysis of the treatment of HCV based on three scenarios: 1) no treatment, 2) continuation of current-treatment rates, and 3) treatment rates among non-Medicare patients increased to match

Costs analysis of a population level rabies control programme in Tamil Nadu, India.

The study aimed to determine costs to the state government of implementing different interventions for controlling rabies among the entire human and animal populations of Tamil Nadu. This built upon an earlier assessment of Tamil Nadu's efforts to control rabies. Anti-rabies vaccines were made available at all health facilities. Costs were estimated for five different combinations of animal and human interventions using an activity-based costing approach from the provider perspective. Disease and population data were sourced from the state surveillance data, human census and livestock census.

Integration of leprosy elimination into primary health care in orissa, India.

BACKGROUND: Leprosy was eliminated as a public health problem (<1 case per 10,000) in India by December 2005. With this target in sight the need for a separate vertical programme was diminished. The second phase of the National Leprosy Eradication Programme was therefore initiated: decentralisation of the vertical programme, integration of leprosy services into the primary health care (PHC) system and development of a surveillance system to monitor programme performance. METHODOLOGY/PRINCIPAL FINDINGS: To study the process of integration a qualitative analysis of issues and perceptions of pat

Guidelines for school programs to prevent skin cancer.

Skin cancer is the most common type of cancer in the United States. Since 1973, new cases of the most serious form of skin cancer, melanoma, have increased approximately 150%. During the same period, deaths from melanoma have increased approximately 44%. Approximately 65%-90% of melanomas are caused by ultraviolet (UV) radiation. More than one half of a persons lifetime UV exposure occurs during childhood and adolescence because of more opportunities and time for exposure. Exposure to UV radiation during childhood plays a role in the future development of skin cancer. Persons with a history of

Displacement and kidney care disruption in Sudan: lessons from a fragile health crisis.

PURPOSE OF REVIEW: To examine the disruption of kidney care services in Sudan following the 2023 conflict, highlighting the compounded risks faced by dialysis and transplant patients during crises, and offering evidence-based strategies for future emergency preparedness. RECENT FINDINGS: The conflict in Sudan exposed deep systemic weaknesses in an already under-resourced nephrology infrastructure. Hemodialysis centers were damaged or shut down, supply chains collapsed, and patient displacement became widespread. Over 70% of patients experienced treatment interruption and many died due to miss

Prevalence, correlates, and treatment gap of generalized anxiety disorder among adults in Bangladesh: Results from a nationally representative survey.

This study presents nationally representative findings on the epidemiology of generalized anxiety disorder (GAD) among Bangladeshi adults, based on data from a nationwide household survey. Participants were initially screened using the Self-Reporting Questionnaire (SRQ), and those screening positive underwent face-to-face clinical interviews by trained psychiatrists. Diagnoses were established using DSM-5 criteria. A total of 7270 adults completed all measures used in this survey. The weighted lifetime prevalence of GAD was 3.5 % (95 % CI: 2.9-4.2), with higher rates observed among women (4.2

Building health research capacity in Africa: the Einstein-Rwanda research and capacity building program.

The growing demand for healthcare services and the burden of diseases such as cancer in sub-Saharan Africa (SSA) requires locally-led and setting-relevant evidence that should be driven by local investigators. However, there is a huge gap in the health research capacity to generate such evidence in most of SSA, particularly in Rwanda. With a changing focus and the willingness of investigators and funders from high-income countries (HICs) to support investigators and research from SSA, it is important to build strong, successful, and sustained partnerships. In this perspective, we describe the

Models of Governance of Disability Therapy Support Workers in Rural and Remote Settings: A Systematic Scoping Review.

The National Disability Insurance Scheme (NDIS) ushered in a transformative era in disability services in Australia, requiring new workforce models to meet evolving participant needs. Therapy Assistants are utilised to increase the capacity of therapy services in areas of workforce shortage. The governance arrangements required to support this emergent workforce have received limited attention in the literature. This review examined the key components and contextual factors of governance in rural settings, specifically focusing on therapy support workers under the guidance of allied health pro

The Association of Food Security With Hypertensive Disorders of Pregnancy: A National Health Interview Survey Analysis.

Food security is one of the most researched social determinants of health (SDoH), however, there is a lack of literature on the impact of food security on cardiovascular disease in pregnancy. The primary objective was to examine the association between food security with hypertensive disorders of pregnancy. We performed a cross-sectional analysis of 2019-2022 data from the National Health Interview Survey. The study population included women of childbearing age who were either pregnant or recently pregnant. Logistic regression models were developed to examine the association between food secur
