Mediastinoscopy is the most reliable examination to determine the presence or absence of lymph node metastases in the preoperative evaluation of lung cancer extension. The sensitivity of Haemoccult for asymptomatic colorectal carcinoma has been estimated in a large randomized controlled trial of mass population screening, with a minimum follow-up of 2 years. A total of 111 cancers were diagnosed in those who completed the screening tests; of these, 36 appear to have been missed by Haemoccult and 75 were detected by the test, giving an overall sensitivity of 67.6 per cent. Haemoccult was shown to be significantly more sensitive for carcinoma of the sigmoid and descending colon than for rectal or right-sided cancers (81 versus 45 and 47 per cent, respectively)

Many surgeons, particularly in the UK, give inadequate primary treatment to patients with operable breast cancer. For spurious reasons they regard axillary clearance as unnecessarily extensive surgery and rely instead upon total mastectomy or tumour excision and node sampling, with or without postoperative radiotherapy. Between 1980 and 1990, 580 patients with known malignant lymphoma were referred for sonography for evaluation of abdominal masses as a part of the initial staging of the disease. Nine cases of inflammatory invasive carcinoma of the uterine cervix are reviewed. All appeared in women under less than fifty years old, quickly after normal PAP smears. The diagnosis and treatment of cancer of the major duodenal papilla in 83 patients are discussed. Radical operations were carried out in 71 (86%) patients; 60 were economical pancreatoduodenal resection (EPDR) and 11 were extended pancreatoduodenal ones (ExPDR). Postoperative mortality was 28%. The late-term results of radical operation were studied in 49 patients. Recurrent cancer developed in half (22) of the patients who underwent EPDR. Three patients who were treated by ExPDR also died from recurrent cancer and metastases. The authors believe EPDR to be expedient only in Stages I-II cancer

Neuroblastoma, a malignant tumor of neural crest origin, is the most common extracranial solid tumor in children. In 1971 Evans et al. introduced a clinical staging for neuroblastoma. Over sixty percent of patients present with neuroblastoma beyond stage I. Despite more aggressive therapy there has been only minimal improvement in survival. Since 1978, all patients with neuroblastoma have had CT scanning as part of their initial evaluation at our institution. We studied 347 patients with primary colorectal carcinoma treated by surgery during 1973-1983: 64 (18.2%) with right colon carcinoma, 158 (45.7%) with left colon carcinoma, 14 (4.2%) with transverse colon carcinoma, and 110 (31.9%) with rectal carcinoma. We compared the right-sided carcinoma (RSC) and left-sided carcinoma (LSC) groups. Obstruction was found in 17.2% of RSC compared with 20.3% LSC patients (P greater than 0.05). Advanced disease (Dukes stages C and D) was found in 34.4% on the right and 47.8% on the left side (P = 0.01). Crude 5-year survival rate, by life table method, was 71%

The histological degree of differentiation is a valuable determinant of the malignancy of colorectal carcinomas. It correlates well with the tumour stage and has a prognostic relevance similar to the Dukes stages. The sonographic evaluation of the prostate following rectal palpation is highly valuable and for the first time allows for a comparable documentation. The determination of prostatic size and the staging of prostate and bladder tumors can be realized endosonographically with high accuracy. The transrectal clarification of suspect palpationfindings with possibly following ultrasonically guided biopsy enables for safer diagnosis. The reaction to a therapeutic measure or the behavior in course of the carcinoma of the prostate can be observed and also well documented.

Transbronchial fine needle biopsy was performed in 52 patients with hilar enlargement on chest radiography or pulmonary carcinoma at bronchoscopy. The diagnostic success was high. Most (85%) of the specimens were adequate for cytological examination and malignant cells were found in 17 aspirates. One further patient with mediastinal metastasis was revealed by mediastinoscopy. No complications followed the fine needle biopsy.

Level V melanomas have been reported to have a poor prognosis, but in-depth analyses of prognostic factors and treatment have not been reported. The presence of estrogen receptors in breast cancer tissue has been reported to correlate with improved prognosis in women after mastectomy. Two hundred and three patients with upper tract urothelial tumours were reviewed. Eighty had pelvicaliceal tumours, 90 ureteric tumours, and 33 multifocal tumours of both pelvis and ureter. Two hundred patients had transitional cell carcinomas and 3 had squamous cell carcinomas. The overall corrected 5-year and 10-year survival rates were 59.7% and 41.1% respectively. Widespread multifocal occurrence poor differentiation, penetration of the muscle coat and conservative resection of tumours predisposed to a poorer prognosis.
