[Noncompressive myelopathy and dysglobulinemia. A fortuitous association? Discussion of 3 new cases].

Three new cases of myelopathy without compression associated with paraproteinemia are reported. The neurologic picture was that of spinal muscular atrophy in one case and chronic spinal combined sclerosis in two. The nature of the paraproteinemia is discussed: all three patients seem to have had benign monoclonal dysglobulinemia. One patient died from cardiovascular disease five years after onset, and another from complications related to decubitus seven years after onset. Postmortem examination was not possible in either case. Spinal muscular atrophy has been reported in certain carcinomas (l

Cataracts and ketotic hypoglycemia.

Of 40 patients with ketotic hypoglycemia, 15 (nine boys and six girls) developed cataracts. The mean age at onset of the first hypoglycemic attack was 20 months, and the average age at the time the cataracts were discovered was 3 1/4 years. The average birth weight of 14 children was 2060 g. The cataracts were bilateral in all but one case. Seven patients (11 eyes, bilateral in four patients) developed complete cataracts. Despite aphakic correction and occlusion therapy, the major cause of visual loss after cataract surgery was stimulus deprivation amblyopia. Other ocular abnormalities include

Reversibility of human myopathy caused by vitamin E deficiency.

Although a neuromuscular syndrome has been induced experimentally by vitamin E deficiency, a human syndrome has not yet been documented. This report describes a 7-year-old boy with severe malabsorption since birth who presented with progressive external ophthalmoplegia, proximal muscle weakness, peripheral neuropathy, hyporeflexia, and bilateral Babinski signs. Abnormalities on neurologic examination included elevated creatine phosphokinase and aldolase, slowed distal sensory latencies, type II muscle fiber atrophy, and a plasma vitamin E level of 8 microgram per deciliter (normal, 550-1500 mi

Intracranial venous thrombosis complicating oral contraception.

Four days after the onset of a severe headache a 22-year-old woman who had been taking oral contraceptives for less than three weeks had a convulsion, followed by right hemiparesis. Other focal neurologic signs and evidence of raised intracranial pressure appeared, and she became comatose on the seventh day. A left craniotomy revealed extensive cerebral venous thrombosis. She died the next day. On postmortem examination extensive thrombosis of the superior sagittal sinus and draining cerebral veins, and multiple areas of cerebral hemorrhage and hemorrhagic infarction were seen. Some of the sup

Value of sequential postoperative brain scans in patients with anaplastic gliomas.

The authors analyze the postoperative course of 30 patients with anaplastic supratentorial gliomas to evaluate the usefulness of sequential brain scanning as an adjunct to clinical neurological examinations in the early detection of tumor recurrence. The correlation between sequential scanning and clinical evaluation was excellent; no examples of divergent resu;ts were seen. With the exception of scans made very early in the postoperative period or when postoperative scalp flap infections were present, initial postoperative scans were easily interpreted in terms of both the superficial (postcr

[An autopsy case with lower motor neuron disease showing a transient-appearance of anti-GM1 antibody and an improvement of conduction block after gamma-globulin administration].

We report a 63-year-old man who died of respiratory failure. He was well until 1992 (57 years of his age), when he had an onset of progressive weakness of the bilateral upper limbs. He showed no improvement with TRH administration in other hospital. On January 12, 1994, he admitted to our department because of the progressive muscle weakness. Neurologic examination revealed a muscular atrophy associated with severe weakness and hyporeflexia in both upper limbs, and fasciculation were seen in his tongue. Electrophysiological studies revealed mild conduction block in the left medial nerve, and F

Visual functions in relation with neonatal cerebral ultrasound, neurology and cognitive development in very-low-birthweight children.

In order to determine the relationship between visual functions and neonatal cerebral ultrasound, neurological examinations and cognitive development, a prospective longitudinal study was conducted in 69 high-risk very-low-birthweight children. Visual development was studied at 1 and 2.6 years of corrected age by assessment of visual acuity, binocular visual fields, optokinetic nystagmus and strabismus. Visual impairments were found in 33% at age 1 and in 28% at age 2.6. Visual impairments were related to intraparenchymal damage, as detected by neonatal cerebral ultrasound, as well as to abnor

Principles of vertebral fracture management.

Traumatic disruption of the spine and supporting soft tissue structures may result in vertebral fracture or luxation and subsequent spinal cord compromise. An understanding of the regional anatomy is important to the discussion of pathophysiology and treatment of traumatic disorders. Various traumatic forces result in disruption of specific anatomic structures and reflect inherent stability or instability of the vertebral column. The neurological examination, and sequential neurological examinations, reflect the degree of spinal cord damage and vertebral instability. Patients in whom radiograp

Nervous system involvement in Behçet's syndrome.

PURPOSE OF REVIEW: Neurological involvement in Behçet's syndrome is defined as 'the occurrence of neurological symptoms and signs in a patient who meets the International Diagnostic Criteria for BS not otherwise explained by any other known systemic or neurological disease or treatment, and in whom objective abnormalities consistent with neuro-Behçet's syndrome (NBS) are detected either on neurological examination, neuroimaging studies (magnetic resonance imaging [MRI]), and/or on cerebrospinal fluid (CSF) examination'. Given that the neurological involvement of Behçet's syndrome carries a poo

Macrocephaly with head growth parallel to normal growth pattern: neurological, developmental, and computerized tomography findings in full-term infants.

Fifteen infants with macrocephaly had a head growth pattern parallel to 2 SDs above normal. In eight patients with initial and subsequent normal findings on neurological and developmental examinations, computerized tomography (CT) showed slight ventricular dilation and increased width of the subarachnoid space anteriorly. The other seven patients had abnormal results from neurological or developmental examinations. The CT findings in two were similar to those in the first group; in four, other abnormalities were observed; and one had a normal scan. None of the 15 infants required shunting. In

Clinicopathologic mismatch in young-onset parkinsonism: Multiple system atrophy masquerading as a neurotransmitter synthesis disorder.

BACKGROUND: Multiple system atrophy (MSA) is a progressive adult-onset synucleinopathy that remains difficult to diagnose clinically, particularly in younger patients and during early disease stages. Brainstem nuclei degeneration resulting in reduced monoamines in cerebrospinal fluid (CSF) analysis may further complicate diagnostic interpretation. OBJECTIVES: To describe a clinicopathologic case of early-onset MSA found to have decreased CSF monoamines suggestive of a primary neurotransmitter synthesis disorder. MATERIALS AND METHODS: Clinical records, neuroimaging findings, CSF neurotransmi

Haemorrhagic myelopathy causing acute post-operative paralysis in a rabbit (Oryctolagus cuniculus) with a congenital lordosis.

A 7-month-old mixed-breed lionhead entire female rabbit, presented for ovariohysterectomy, displayed a mild bilateral hind limb paresis with external rotation of the hip joints, associated with grade 1/5 pododermatitis of the hind limbs. Investigations were declined by the owner prior to surgery. After a seemingly uncomplicated ovariohysterectomy, the rabbit displayed an acute rapidly progressive bilateral hind limb paralysis with retention of nociception and mild right forelimb paresis. A recheck complete neurologic examination on the following day showed paralysis of the hind limbs without n

Disorders of Puberty: Common Questions and Answers.

Clinicians often need to differentiate between benign, self-limited variations of pubertal development and more serious underlying causes. Precocious puberty should be considered when thelarche occurs in female patients before 8 years of age or when testicular enlargement occurs in male patients before 9 years of age. Delayed puberty should be considered in female patients who lack breast development by 13 years of age or do not experience menarche by age 15 and in male patients who lack testicular growth by age 14. Assessment should focus on clinical and family history, growth, and pubertal e

Dopamine-2 receptor antibody encephalitis presenting as pure tongue-biting in a tourette syndrome patient: a case report.

BACKGROUND: Tourette syndrome (TS) is a neuropsychiatric disorder characterized by repetitive and patterned tics. Its onset correlates with dysfunctions in immunological activation and neurotransmitters. Autoimmune movement disorders such as dopamine-2 receptor antibody encephalitis (D2R encephalitis) may go undiagnosed in TS patients seeking medical help for tic symptoms only. Here, we present a clinical case of D2R encephalitis in a TS patient. CASE PRESENTATION: A 13-year-old boy with a history of TS presented with acute tongue-biting without positive neurologic examination or auxiliary ex

Currarino syndrome as an incidental radiologic finding in a patient with acute flank pain: A case report.

Currarino syndrome is an autosomal dominant hereditary disease defined as a triad of anorectal abnormality, sacral dysgenesis, and a presacral mass, primarily an anterior sacral meningocele. It is often seen in children and considered rare in adults. It is mostly found as an incidental finding. We present a 21-year-old man who presented with acute flank pain. He had a history of Hirschsprung's disease and therefore had undergone surgery in his infancy. He also had a history of prolonged constipation and had an episode of admission due to suspected obstruction. On physical examination, he had a

Primary Brain Melanoma in a Pediatric Patient: A Case Report.

BACKGROUND Primary malignant melanoma of the brain is a challenging radiological diagnosis and a high index of suspicion is required about patients with the condition. In the pediatric population, only a few cases have been reported in the literature. The purpose of this report was to describe the expected imaging characteristics and the importance of a multidisciplinary approach in the diagnosis of this rare entity. CASE REPORT A 17-year-old Hispanic male who presented with new-onset tonic-clonic seizures had no focal neurologic deficits on physical examination. An initial computed tomography
