Acromioclavicular dislocations; open reduction with screw fixation.

Athletes with complete separation of the acromioclavicular joint were uniformly restored to full athletic competence by an operation consisting of direct repair of the involved ligamentous structures with temporary fixation by a metallic lag screw approximating the clavicle to the coracoid. The metallic fixation was left in place an average of eight weeks and the screw was then removed under local anesthesia. A number of professional and collegiate football players returned to the rigors of football with no discomfort or disability.

Anterior cervical discectomy and fusion with a zero-profile VA spacer device: a clinical and radiological study with two-year follow-up.

STUDY DESIGN: A retrospective study. OBJECTIVE: The aim of this study was to compare clinical and radiological outcomes of the anterior cervical discectomy and fusion (ACDF) with a novel zero-profile variable-angle (Zero-P VA) spacer and a traditional poly-ether-ether-ketone (PEEK) cage and plate system in cases pertaining to cervical radiculopathy/myelopathy. There are two conventional types of ACDF procedures aimed at treating symptomatic cervical spondylosis. The first one involves an uninstrumented "stand-alone" approach utilizing bone graft/cage, while the second incorporates bone graft/

Surgeon reported practice patterns related to full endoscopic cervical decompression procedures.

BACKGROUND: The microsurgical anterior approach to the cervical spine is commonplace. Fewer surgeons perform posterior cervical microsurgical procedures on a routine basis for lack of indication, more bleeding, persistent postoperative neck pain, and risk of progressive misalignment. In comparison, the endoscopic technique is preferentially performed through the posterior approach. Many spine surgeons and even surgeons versed in lumbar endoscopy are often reluctant to consider endoscopic procedures in the cervical spine. We report the results of a surgeon survey to find out why. METHODS: A qu

Bone morphogenetic protein-2 and bone marrow aspirate with allograft as alternatives to autograft in instrumented revision posterolateral lumbar spinal fusion: a minimum two-year follow-up study.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: Compare the efficacy of rhBMP-2 and bone marrow aspirate with allograft (BMAA) as alternatives to autograft in instrumented revision posterolateral lumbar fusion (PLF). SUMMARY OF BACKGROUND DATA: The use of autogenous bone graft is the current gold standard in revision PLF; however, this practice is associated with significant donor-site morbidity. Revision PLF pose the additional challenges of a less than ideal fusion environment and a limited quantity of autogenous bone graft. rhBMP-2 and BMAA have been shown to be acceptable bone graft

Minimally invasive surgery versus an arthroscopic procedure for the first metatarsophalangeal arthrodesis: A comparative study of the effectiveness and safety profile.

INTRODUCTION: First metatarsophalangeal (MTP) arthrodesis is a common procedure for treating degeneration in the MTP joint. Open arthrodesis is a conventional procedure that provides acceptable results but is more invasive than minimally invasive or arthroscopic arthrodesis. However, little is known about the different outcomes between minimally invasive surgery (MIS) and arthroscopic surgery (AS) for MTP arthrodesis. This study was performed to compare the outcomes of the first MTP joint preparation for arthrodesis regarding the effectiveness of articular surface preparation and safety betwee

Does intraoperative navigation assistance improve bone tumor resection and allograft reconstruction results?

BACKGROUND: Bone tumor resections for limb salvage have become standard treatment. Recently, computer-assisted navigation has been introduced to improve the accuracy of joint arthroplasty and possible tumor resection surgery; however, like with any new technology, its benefits and limitations need to be characterized for surgeons to make informed decisions about whether to use it. QUESTIONS/PURPOSES: We wanted to (1) assess the technical problems associated with computer-assisted navigation; (2) assess the accuracy of the registration technique; (3) define the time required to perform a navig

Functional outcomes of childhood dorsal rhizotomy in adults and adolescents with cerebral palsy.

OBJECT: In this descriptive study the authors evaluated medical outcomes, interventions, satisfaction with life, and subjective impressions about selective dorsal rhizotomy (SDR) in older adolescents and adults who had undergone the procedure as children. METHODS: A survey was administered to older adolescents (16-20 years old) and adults with CP who had undergone SDR between 1986 and 2000 at two academic centers. The patients or their caregivers participated in telephone or clinic interviews. Subjective impressions about the SDR and a history of post-SDR medical interventions were obtained.

[Late hematogenous infection of prosthetic joint].

The importance of prevention in late hematogenous infection is well understood but, because responsibility lies with general practitioners and other specialists, the orthopedic surgeon is usually not much interested. In both our and other countries, discussions are taking place on whether and to what extent antibiotic prevention should be carried out. Antibiotic prophylaxis of hematogenous infection is not indicated for all patients with joint arthroplasty, but only for a limited, defined group of patients at high risk. In these, however, the present state of knowledge suggests that prevention

Impact of Institutional Protocol on Urinary Catheter Outcome Measures in Orthopedic Children Treated With Epidural Analgesia.

BACKGROUND: Epidural analgesia is commonly used for pain control after major lower-limb orthopedic surgery in children, but it is associated with a risk of postoperative urinary retention. Consequently, urinary catheters are often placed and left in situ for the full duration of epidural analgesia, despite the potential risks of prolonged catheterization, including discomfort, ward reinsertion, and urinary tract infection. AIMS: To assess the impact of implementing a standardized institutional urinary catheter management protocol on postoperative urinary catheter outcomes in children less tha

Perfusion index as a predictor of hypotension after spinal anesthesia in lower extremity orthopedic surgery: a prospective observational trial.

BACKGROUND: Hypotension is the most common complication of spinal anesthesia, particularly in older patients, where the incidence and potential adverse effects are increasing. This study aims to investigate the role of the perfusion index (PI) in predicting spinal anesthesia-induced hypotension (SAIH) during orthopedic lower extremity surgery and its relationship with age. METHODS: We conducted a single-center, prospective, observational study of 120 elective patients over 18 years of age, classified as ASA I-II-III risk groups, undergoing lower extremity surgery in the supine position under s

Reducing Opioid Use in Orthopaedic Surgery.

The opioid crisis has been an issue in the United States since the mid-1990s, claiming numerous lives and presenting a significant challenge to health care clinicians. Various preoperative, intraoperative, and postoperative strategies aimed at reducing opioid consumption can be used by orthopaedic surgeons to help minimize this crisis. Preoperative screening tools can help identify patients at risk for prolonged opioid use, allowing for tailored interventions and counseling. Patient education initiatives, including multimedia presentations and handouts, have shown promising results in decreasi

Assessment of enflicoxib efficacy in the control of postoperative pain and inflammation in dogs undergoing orthopaedic surgery. A pilot randomised clinical trial.

Postoperative pain is most effectively managed pre-emptively and enflicoxib characteristics make it a good candidate to control it with a single administration. To assess the efficacy of enflicoxib in the control of postoperative pain and inflammation associated with orthopaedic surgery, twenty-eight dogs were randomised to receive 8 mg/kg enflicoxib (n = 14) orally the day before surgery, or 0.2 mg/kg meloxicam (n = 14) subcutaneously at induction and orally at 0.1 mg/kg daily every 24 h for seven days. Veterinarians assessed efficacy with the Glasgow Composite Pain Scale Short-Form (GCPS-SF)

Follow-up and complications rates in orthopedic trauma patients with substance use disorders.

BACKGROUND: Substance use disorders (SUD) are common and associated with trauma [1-5]. Despite the high frequency of patients with SUDs presenting with trauma and the ubiquitous concerns about compliance, follow-up, and complications amongst providers caring for these patients there has been little attempt to quantify outcomes in this everyday group of patients. The purpose of the current study was thus to document basic demographics, follow-up rates, and surgical outcomes in orthopedic trauma patients presenting with substance use disorders. METHODS: A retrospective review of an observationa

Selected Nutrients to Oppose Muscle Disuse Following Arthroscopic Orthopedic Surgery: A Narrative Review.

Background: Orthopedic surgery and the corresponding events (i.e., immobilization and muscle disuse) result in a cascade of biological events to promote healing but can come with the loss of skeletal muscle mass and strength. A good nutritional status of patients is associated with positive post-surgical outcomes, with macronutrients receiving the majority of emphasis in the research literature. However, beyond the surgical literature, there are other nutrients and nutritional supplements that have been established or postulated to improve skeletal muscle mass and strength. Objective: The purp

Normothermia to Decrease Surgical Site Infection Risk: Silver Bullet or Fool's Gold? A Retrospective Cohort Study.

PURPOSE: Surgical site infection (SSI) is the leading cause of nosocomial infections among surgical patients in the United States. Currently, there is compelling evidence suggesting that temperature dysregulation in surgical patients may be a risk factor for the development of SSI. We examined the relationship between perioperative hypothermia (PH) and SSI in a population of surgical patients with diabetes mellitus (DM). METHODS: This retrospective cohort review was conducted on patients with a history of DM undergoing orthopaedic surgery at our institution between May 1, 2018, and April 1, 2
