Platelet 5-HT2A receptor binding and tryptophan availability in depression are not associated with recent history of suicide attempts but with personality traits characteristic for suicidal behavior.

BACKGROUND: Abnormalities in the serotonergic (5-HT) system have been implicated in the pathogenesis of suicidal behavior. Studies on peripheral serotonergic parameters as a measure for central serotonergic function in suicidal patients appear to be promising, yet failed to show a clear association with suicidality. The objective of this study was to elucidate the role of serotonergic blood parameters in depressed suicidal patients and to examine their usefulness as a potential biological marker for suicidality. A number of personality traits were assessed in order to provide a basis for a psy

The use of the mini-mental state examination in recruitment for substance abuse research studies.

BACKGROUND: Substance abuse is associated with cognitive impairment. Participation in clinical addiction research can be cognitively demanding. Screening tools can identify cognitively impaired subjects. We examined the use of the mini-mental state examination (MMSE) as an entry criterion in three randomized controlled substance abuse clinical trials. METHODS: In each of the three studies, we calculated the proportion of subjects excluded due to MMSE scores (<21) suggestive of cognitive impairment. We estimated the potential impact on enrollment based on the number of excluded subjects. Separ

Intraoperative duplex scanning after carotid artery reconstruction: a valuable tool.

The operative management of extracranial cerebrovascular occlusive disease requires meticulous surgical technique. Intraoperative duplex imaging (10 MHz B-mode ultrasound combined with 7.5 MHz Doppler spectrum analysis) was used in an attempt to recognize technical problems that might result in serious neurologic injury. Eighty-four carotid arteries were studied in 76 consecutive patients undergoing carotid endarterectomy or reconstruction. Indications for operation included carotid territory transient ischemic attack (n = 36 [43%]), amaurosis fugax (n = 16 [19%]), stroke (n = 11 [13%]), asymp

Gender-stratified associations between resilience dimensions and big five personality traits among emerging adult college students in India.

BACKGROUND: Emerging adulthood is a developmental stage marked by identity exploration, psychosocial transitions, and heightened vulnerability to stress. Resilience and personality traits are important for adaptive functioning during this period; however, limited research has examined their multidimensional associations in Indian emerging adults, particularly in relation to how these traits influence coping strategies and overall mental health outcomes during this critical developmental stage. OBJECTIVE: This study examined the relationships between multidimensional resilience and Big Five pe

A review of the experimental evidence on the toxicokinetics of carbon monoxide: the potential role of pathophysiology among susceptible groups.

BACKGROUND: Acute high level carbon monoxide (CO) exposure can cause immediate cardio-respiratory arrest in anyone, but the effects of lower level exposures in susceptible persons are less well known. The percentage of CO-bound hemoglobin in blood (carboxyhemoglobin; COHb) is a marker of exposure and potential health outcomes. Indoor air quality guidelines developed by the World Health Organization and Health Canada, among others, are set so that CO exposure does not lead to COHb levels above 2.0%, a target based on experimental evidence on toxicodynamic relationships between COHb and cardiac

Intrapersonal dynamics of closeness versus distance: Qualities of the self-concept distinguish destructive overdependence, dysfunctional detachment, and healthy dependency.

INTRODUCTION: People vary in the degree to which they affiliate with others; exaggerated efforts maintain interpersonal closeness versus distance are codified in longstanding categorical models of personality pathology, and in contemporary dimensional frameworks as well. OBJECTIVE: To examine associations between destructive overdependence (DO), dysfuntional detachment (DD), and healthy dependency (HD) and qualities of the self-concept. METHOD: A mixed-sex sample of 229 adults completed the Relationship Profile Test to assess DO, DD, and HD, and the Object Relations Inventory (ORI) to assess

Digital Ophthalmology in the UK - Diabetic Retinopathy Screening and Virtual Glaucoma Clinics in the National Health Service.

The customary doctor and patient interactions are currently undergoing significant changes through technological advances in imaging and data processing and the need for reducing person-to person contacts during the COVID-19 crisis. There is a trend away from face-to-face examinations to virtual assessments and decision making. Ophthalmology is particularly amenable to such changes, as a high proportion of clinical decisions are based on routine tests and imaging results, which can be assessed remotely. The uptake of digital ophthalmology varies significantly between countries. Due to financia

Homelessness History Impacts on Health Outcomes and Economic and Risk Behavior Intermediaries: New Insights from Population Data.

Using data from the Washington Behavioral Risk Factor Surveillance System, this study assesses the relationship between homelessness history and physical and mental health outcomes as well as between homelessness history and health risk behaviors and economic precariousness as important intermediaries of subsequent health outcomes. Study results indicate that persons with a history of adult homelessness have consistently poorer health outcomes as compared to never homeless persons, and that even after accounting for childhood adversity and social assets, adult homelessness remains a consistent

Technology-driven interventions for caregivers of persons with dementia: a systematic review.

Caregiving for a person with dementia can lead to physical and psychological morbidity. Technology-driven interventions hold the promise of convenient, low-cost methods of delivering psychosocial interventions. This systematic review examined the efficacy of technology-driven interventions for caregivers of persons with dementia. A search of Ovid Medline, PsychInfo, and EBSCO from 1990 to May 2012 resulted in the identification of 295 articles. After removal of duplicates, 271 articles were reviewed, based on the abstract and title alone; 32 were relevant or could not be fully assessed without

Biomechanical evaluation of wrist-driven flexor hinge orthosis in persons with spinal cord injury.

The wrist-driven flexor hinge orthosis (WDFHO) is a device used to restore hand function in persons with tetraplegic spinal cord injury by furnishing three-point prehension. We assessed the effectiveness and biomechanical properties of the WDFHO in 24 persons with cervical 6 or 7 tetraplegia who have severely impaired hand function. This study introduces a mechanical operating model to assess the efficiency of the WDFHO. Experimental results showed that pinch force increased significantly (p < 0.001) after using the WDFHO and was found to positively correlate with the strength of wrist extenso

Using a webcast support service: experiences of in-person attendees of an implantable cardioverter defibrillator support group.

OBJECTIVE: Most patients with implantable cardioverter defibrillators (ICDs) adjust well to living with the device; however, some experience difficulties. Support groups assist in coping with the psychologic effects of living with an ICD. The study's aim was to examine acceptability of the in-person attendees of an ICD support group that was cast on the Internet. SAMPLE: A patient satisfaction survey describing the participants' experience was used as a measure of acceptability in this non-experimental, survey, pilot study. METHODS: The survey assessed reactions of the in-person participants

A Preliminary Evaluation of Daily and Momentary Variability in Parenting Practices During Adolescent Alcohol Use and Co-Occurring Disorders Treatment.

BACKGROUND: Adolescents with co-occurring alcohol use and mental health disorders have better treatment outcomes when caregivers are involved in their treatment. Currently, little is known about why caregiver involvement improves outcomes. OBJECTIVES: To facilitate caregiver-involved treatment process research, this study preliminarily examined within- and between-person variability in ecological momentary assessment (EMA) items assessing caregiver-child communication (eg, warmth, conflict), general (eg, monitoring, praise), and substance-specific parenting practices (eg, alcohol reactions an

When should the driver with a history of substance misuse be allowed to return to the wheel? A review of the substance misuse section of the Australian national guidelines.

Assessing fitness to drive in applicants with a historical or current substance use disorder presents a specific clinical challenge. The Australian guidelines require evidence of remission and absence of cognitive change when considering applications for re-licensing driver or individuals applying to reengage in safety-sensitive work. This paper reviews some of the clinical and biochemical indicators that determine whether a particular person is in 'remission' and meets the criteria for return to driving or other safety-sensitive occupation. It provides an overview of the challenges in establi

Advances in the Assessment and Treatment of Anti-Neutrophil Cytoplasmic Antibody-Associated Glomerulonephritis.

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a group of autoimmune diseases primarily cause inflammation of small blood vessels. Renal involvement occurs frequently and often leads to end-stage renal disease (ESRD), which significantly impacts patient health and survival. Early diagnosis and appropriate treatment are essential to improving patient outcomes. This review provides an overview of the latest advances in the assessment and treatment of ANCA-associated glomerulonephritis (AAGN). The assessment section covers diagnostic tools, disease activity assessment,

Incarceration as a Health Determinant for Sexual Orientation and Gender Minority Persons.

Incarceration is considerably more prevalent among sexual and gender minority persons (SGM) than among the general population. Once behind bars, they are at the greatest risk for health-related harms. Although a growing number of studies have assessed health disparities produced by mass incarceration, scholars are yet to systematically assess the health consequences of incarceration on the basis of sexual orientation and gender identity. We invite public health scholars to study the effects of incarceration on health in the SGM population and provide a roadmap to aid these research efforts. Fi

Ocular contributions to age-related loss in coarse stereopsis.

PURPOSE: To determine the basis for the dramatic decline in coarse stereopsis that occurs with age. METHODS: The Frisby test was used to assess stereopsis in a large sample of randomly selected persons over the age of 58 years. A number of other vision functions were also assessed in the same persons. The data were used to address the question of whether the decline in stereopsis reflects age-related alterations in cortical stereopsis mechanisms themselves (e.g., disparity detectors) or a degradation of the signal reaching the cortex as a result of alterations in earlier visual components. Behavioral assessment of feeding problems of individuals with severe disabilities.

As many as 80% of the individuals with severe or profound mental retardation exhibit feeding problems. Although behavioral interventions have been used to treat these problems, no assessment procedure for determining a functional relationship between a person's acceptance of food and the type and texture of that food has been reported. The purpose of this study was to test a behavioral assessment procedure for a feeding problem of limited intake. Five individuals with severe or profound mental retardation were fed 10 to 12 types of foods with one or more textures. Behavioral categories of acce

Applying the ADAPT guidance to implement a telemedicine and medication delivery service in a malaria-endemic setting: A prospective cohort study.

BACKGROUND: The ADAPT guidance proposes a process model for adapting evidence-informed interventions to novel contexts. Herein, we leveraged this guidance to adapt a paediatric nighttime telemedicine and medication delivery service from Haiti, a setting with low malaria prevalence, to Ghana, where malaria is a leading cause of paediatric mortality. METHODS: Core components of the intervention were defined and conserved. Discretionary components were identified and considered for adaptation. The service was defined by a workflow involving a call from a guardian of a sick child, a telemedicine
