Diagnostic Performance of Artificial Intelligence in Chest Radiographs Referred from the Emergency Department.

BACKGROUND: The increasing integration of AI in chest X-ray evaluation holds promise for enhancing diagnostic accuracy and optimizing clinical workflows. However, understanding its performance in real-world clinical settings is essential. OBJECTIVES: In this study, we evaluated the sensitivity (Se) and specificity (Sp) of an AI-based software (Arterys MICA v29.4.0) alongside a radiology resident in interpreting chest X-rays referred from the emergency department (ED), using a senior radiologist's assessment as the gold standard (GS). We assessed the concordance between the AI system and the r

Outcomes with multi-disciplinary management of central lung tumors with CT-guided percutaneous high dose rate brachyablation.

BACKGROUND: Centrally located lung tumors present treatment challenges given their proximity to mediastinal structures including the central airway, esophagus, major vessels, and heart. Therapeutic options can be limited for medically inoperable patients, particularly if they have received previous thoracic radiotherapy. High dose rate (HDR) brachyablation was developed to improve the therapeutic ratio for patients with central lung tumors. The purpose of this study is to report initial safety and efficacy outcomes with this treatment for central lung malignancies. METHODS: From September 201

[CT-guided percutaneous polidocanol injection for giant pulmonary bulla: a report of 5 cases].

The clinical data of five patients with giant pulmonary bullae (GPB) who underwent computed tomography (CT)-guided percutaneous polidocanol injection were retrospectively analysed. Pre-and post-treatment comparisons were performed with respect to the diameter of the pulmonary bullae, arterial blood gas parameters, modified Medical Research Council (mMRC) dyspnoea score, Chronic Obstructive Pulmonary Disease Assessment Test (CAT) score, six-minute walk distance (6MWD), and complications. During a 1- to 12- month follow-up, 3 cases achieved complete bulla closure, and 2 showed stable shrinkage.

Minimally invasive thoracoscopic resection of mediastinal ectopic parathyroid adenomas using robotic and video-assisted approaches: a three-case series.

Mediastinal ectopic parathyroid adenomas are an uncommon cause of primary hyperparathyroidism and a frequent source of failed cervical exploration. We report three patients with deeply located mediastinal adenomas treated using minimally invasive thoracoscopic approaches-two robot-assisted (RATS) and one video-assisted (VATS). Preoperative localization was achieved with SPECT/CT and, when required, 18F-fluorocholine PET/CT, enabling accurate identification of lesions in para-aortic and aortopulmonary regions. Complete excision was confirmed in all cases by intraoperative parathyroid hormone mo

Tuberculosis presenting as dual immune dysregulation: Evans syndrome and haemophagocytic lymphohistiocytosis.

We report a case of a middle-aged man with no known comorbidities who presented with a 1-month history of fever, dry cough, weight loss and progressively worsening breathlessness.Initial investigations revealed declining haemoglobin, white blood cell and platelet levels, while a chest CT showed diffuse ground-glass opacities and a small pneumothorax. The patient was diagnosed with Evans syndrome (the co-occurrence of warm autoimmune haemolytic anaemia and bicytopenia) after a positive direct Coombs test and other haematological markers. Despite starting steroids, his fever persisted, leading t

Prone transradial approach for radiofrequency ablation of hepatocellular carcinoma in the caudate lobe following TACE: a technical case report.

Radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC) located in the caudate lobe is technically challenging because of the tumor's deep location and proximity to major vascular structures. A 71-year-old woman with a 2-cm HCC in segment I underwent conventional transcatheter arterial chemoembolization (TACE) via the femoral artery. Immediate RFA was not feasible because CT arterial portography revealed no safe puncture route due to surrounding vasculature. On the following day, angiography using a left transradial approach was performed with the patient in the prone position, enabli

Osteosarcoma Metastasis to the Thorax: A Pictorial Review of Chest Computed Tomography Findings.

Background: Osteosarcoma, a primary bone malignancy in children and adolescents, frequently metastasizes to the lungs, contributing significantly to morbidity and mortality. Lung Metastases: At diagnosis, 15-20% of patients present with detectable lung metastases. Chest computed tomography (CT) is vital for the early detection and monitoring of these metastases. Lung involvement typically presents as multiple nodules of varying sizes and can include atypical features such as cavitation, cystic lesions, ground-glass halos, intravascular tumor thrombi, and endobronchial disease. Pulmonary Complications of COVID-19: A case report.

Rapid evolution of pulmonary complications associated with severe COVID-19 pneumonia often pose a management challenge to clinicians especially in the critical care setting. Serial chest imaging enable clinicians to better monitor disease progression and identify potential complications early which may decrease the mortality and morbidity associated with COVID-19. We report a 69-year-old male patient with severe COVID-19 pneumonia who presented to a tertiary referral centre in Kuala Lumpur, Malaysia, in 2020 with multiple pulmonary complications including lung cavitation, bronchopleural fistul

Complications of invasive mechanical ventilation in critically Ill Covid-19 patients - A narrative review.

Critically ill COVID-19 patients have to undergo positive pressure ventilation, a non-physiological and invasive intervention that can be lifesaving in severe ARDS. Similar to any other intervention, it has its pros and cons. Despite following Lung Protective Ventilation (LPV), some of the complications are frequently reported in these critically ill patients and significantly impact overall mortality. The complications related to invasive mechanical ventilation (IMV) in critically ill COVID-19 patients can be broadly divided into pulmonary and non-pulmonary. Among pulmonary complications, the

Fatal Lung Toxicity After Intralesional Bleomycin Sclerotherapy of a Vascular Malformation.

Although there has been increased utilization of bleomycin in the treatment of low-flow vascular malformations in children, previous studies report minor adverse effects limited to skin changes/necrosis and flu-like symptoms (Horbach et al. in J Plast Reconstr Aesthet Surg 69(3):295-304, 2016). However, there have been rare reported cases of pulmonary injury observed in children after bleomycin intralesional administration. We report a case of fatal lung toxicity in a 15-month-old girl after injecting 7 units of bleomycin into a left cheek macrocystic lymphatic malformation. 1 week after thera
