Effects of electroacupuncture on intraoperative and postoperative analgesic requirement.

Acupuncture has been shown to be effective in experimental and clinical acute pain settings. This study aims to evaluate the effect of preoperative electroacupuncture (EA) on intraoperative and postoperative analgesic (alfentanil and morphine) requirement in patients scheduled for gynaecologic lower abdominal surgery. Ninety patients were randomly assigned to one of three groups: Group I (control group)--received placebo EA for 45 minutes before induction of general anaesthesia (GA); Group II--preoperative EA instituted 45 minutes before induction of GA; Group III--45 minutes of postoperative

Short-term outcomes of modified Lapidus procedure using the InCore® Lapidus System for hallux valgus: Case series of four cases.

INTRODUCTION: Hallux valgus is a common foot deformity often associated with first tarsometatarsal (TMT) joint instability. The Lapidus procedure, involving first TMT joint arthrodesis, is a well-established surgical procedure for severe cases, but the issue lies in the method of fixation. The InCore® Lapidus System is a novel intramedullary fixation system for modified Lapidus procedure. We here report the clinical experience of this system. CASE PRESENTATION: We retrospectively reviewed four cases who underwent a modified Lapidus procedure using the InCore® Lapidus System and followed up at

Needle size for thyroid fine needle aspiration cytology. A single institution experience.

BACKGROUND: Fine needle aspiration cytology (FNAC) is an invasive diagnostic technique which is widely used for the cytological diagnosis of thyroid nodules. This procedure is generally widely tolerated by patients, albeit often accompanied by local pain and discomfort. Despite various proposals of execution methods, no approach is universally accepted, especially regarding the size of the needle to be used for sampling. Needle gauge preferences vary across regions, with 25-gauge needles more common in Western countries and 22-gauge needles favored in Asian countries. Complications associated w

Removal of Thyroglossal Duct Cyst by a Submental Approach.

BACKGROUND: Thyroglossal duct cyst (TGDC) is the most common congenital cyst in the neck and is removed by the Sistrunk procedure. This surgery involves a horizontal skin incision over the cyst that may leave a noticeable scar on the front of the neck. Therefore, this study examined the clinical outcomes and cosmetic benefits of the Sistrunk procedure by an incision in the submental area that is not easily visible from the front. MATERIALS AND METHODS: This observational study was performed on 152 patients who underwent the Sistrunk procedure by a submental approach to remove TGDC at a univer

In the absence of evidence--why bother? A literature review of minimally invasive total hip replacement surgery.

Two new total hip replacement (THR) procedures have been termed minimally invasive. It is claimed that these smaller incision techniques reduce soft-tissue trauma, decrease postoperative pain and blood loss, speed recovery, and reduce the length of the hospital stay compared with hip replacement using the standard incision technique. These new procedures use either one small 6- to 10-cm incision through a posterior, lateral, or anterior approach or two very small (a 2.5 cm posterior incision for placement of the femoral component and a 5-cm anterior incision for placement of the acetabular com

Efficacy of virtual reality hypnosis versus conscious sedation with nitrous oxide in the management of dental anxiety in pediatric dentistry: protocol for a prospective randomized controlled trial.

BACKGROUND: Dental anxiety is defined as a persistent and excessive fear of dental treatment. It often leads to interruptions during procedures and, frequently, avoidance of dental care. For patients over the age of 7, nitrous oxide-oxygen inhaled sedation (NOIS) represents one of the most effective and well-established pharmacological approaches to reducing anxiety and pain during dental treatment. Meanwhile, medical hypnosis offers an interesting non-pharmacological alternative by inducing a hypnotic state, potentially serving as a means of sedation to alleviate anxiety or pain. The advancem

Assessment of Female Sexual Function Following Transobturator Midurethral Sling for Stress Urinary Incontinence 12 and 36 Months After Surgical Treatment in Postmenopausal Women.

Background/Objectives: Stress urinary incontinence (SUI) is a common condition affecting sexual function, exercise activities, and quality of life, accompanied by psychological distress. The treatment of SUI includes conservative and surgical treatment which comprises tensions-free vaginal tapes in the form of retropubic (TVT) and transobturator tape (TOT or TVT-O). The TVT procedure is considered the gold standard, but the TOT procedure is a safe alternative due to a lower rate of bladder and bowel complications. The aim of this study is to evaluate the long-term effects of the surgical treat

Same-day discharge for pediatric Nuss procedure; an analysis of the NSQIP-pediatric database from 2017-2022.

PURPOSE: Limited data exists evaluating same-day discharge for pediatric Nuss procedure; most being single-center studies. Our analysis, using the NSQIP-P registry, aimed to assess if same-day discharge for the Nuss procedure influenced post-operative outcomes. METHODS: The NSQIP-P database (2017-2022) identified patients who underwent the Nuss procedure. Patients discharged same-day postoperatively (SDD) were compared to those discharged 1-7 days postoperatively (non-SDD). RESULTS: Of 5486 patients identified, 91 (1.7%) were SDD. From 2018 to 2022, the annual SDD rate increased from 0.8% to

Caudal epidural steroid injections versus selective nerve root blocks for single-level lumbar spinal stenosis: a study protocol for a randomized controlled trial.

BACKGROUND: Lumbar spinal stenosis (LSS) is a common degenerative condition associated with old age. Its incidence continues to increase with the rapidly aging population in China. Treatment for LSS usually begins with conservative treatments, as some patients refuse surgical procedures or have surgery contraindications. Caudal epidural steroid injections (CESIs) and selective nerve root blocks (SNRBs) are two commonly used conservative treatments for LSS, which have proven to be effective at relieving LSS symptoms in many studies. However, there are no randomized controlled trials comparing t

[Palliative surgical treatment of tumors of pancreas and periampullary region].

BACKGROUNDS: Pancreatic cancer is an aggressive malignant disease with increasing incidence. Radical resection, the only potentially curative method, is possible in only 20-30% of patients. The main symptoms of advanced non-resectable pancreatic head tumors include obstructive jaundice, caused by stenosis of distal common bile duct, duodenal obstruction and pain, especially in the epigastric region and back. The aim of palliative treatment is to relieve these complaints. This paper evaluates our own palliative surgical treatment results in patients with pancreatic head and periampullary region

Functional and patient-reported outcomes in symptomatic lumbar spinal stenosis following percutaneous decompression.

BACKGROUND: Neurogenic claudication due to symptomatic lumbar spinal stenosis (LSS) is a painful condition causing significant functional disability. While the cause of LSS is multifactorial, thickened ligamentum flavum (LF) accounts for up to 85% of spinal canal narrowing. mild percutaneous lumbar decompression allows debulking of the hypertrophic LF while avoiding the morbidities frequently associated with more invasive surgical procedures. METHODS: In this prospective case series study, consecutive LSS patients presenting with neurogenic claudication were treated with percutaneous lumbar d

Functional surgery of the temporomandibular joint with conscious sedation for "closed lock" using eminectomy as a treatment: a case series.

PURPOSE: The purpose of this case series study was to evaluate the advantages and disadvantages of using local anesthesia and conscious sedation (CS) rather than general anesthesia (GA) for performing functional surgery on patients suffering from temporomandibular joint (TMJ) closed lock, using the eminectomy procedure. Performing this procedure using CS allows functional assessment of the TMJ meniscus at the time of surgery. The objectives of this study were to determine if this surgery is possible under CS, to determine the benefits of being able to see the meniscus in action, and to investi
