Factors associated with major radiological progression of primary sclerosing cholangitis in patients with inflammatory bowel disease.

BACKGROUND/PURPOSE OF THE STUDY: Primary sclerosing cholangitis (PSC) is the most common hepatobiliary manifestation of inflammatory bowel disease (IBD). Magnetic resonance cholangiography (MRC) has become the primary diagnostic modality for PSC. However, data on radiological progression over time of large-duct PSC-IBD are limited. METHODS: We performed a nested case-control study to estimate the frequency of PSC in adult patients with IBD from a Middle Eastern population to assess the risk factors associated with major disease progression (formation of dominant strictures or cholangiocarcino

Osimertinib for Japanese patients with T790M-positive advanced non-small-cell lung cancer: A pooled subgroup analysis.

Epidermal growth factor receptor (EGFR)-tyrosine kinase inhibitors (TKIs) are the standard of care for non-small-cell lung cancer (NSCLC) patients harboring EGFR mutations. However, almost all patients develop resistance after approximately 1 y of treatment, with >50% of cases due to the T790M secondary mutation of the EGFR gene. A large global Phase III study (AURA3) demonstrated that osimertinib significantly prolonged progression-free survival (PFS) over platinum-doublet chemotherapy in patients with T790M-positive NSCLC who had progressed on previous EGFR-TKI therapy. However, it is not cl

Outcomes of combination therapy with tyrosine kinase inhibitors and immune checkpoint inhibitors in metastatic renal cell carcinoma - A retrospective study.

INTRODUCTION: Renal cell carcinoma (RCC) is a highly vascularized and immunogenic tumor. At the time of this study, there was limited published data on the combination of tyrosine kinase inhibitors and immune checkpoint inhibitors in patients who were heavily pretreated. At our institution, providers have used these combinations in heavily pretreated patients. METHODS: We conducted a retrospective review of patients receiving this combination with the primary objectives of assessing duration of therapy and toxicities and a secondary objective of disease progression at six months. We included

Allogeneic Hematopoietic Cell Transplantation as Curative Therapy for Patients with Non-Hodgkin Lymphoma: Increasingly Successful Application to Older Patients.

Non-Hodgkin lymphoma (NHL) constitutes a collection of lymphoproliferative disorders with widely varying biological, histological, and clinical features. For the B cell NHLs, great progress has been made due to the addition of monoclonal antibodies and, more recently, other novel agents including B cell receptor signaling inhibitors, immunomodulatory agents, and proteasome inhibitors. Autologous hematopoietic cell transplantation (auto-HCT) offers the promise of cure or prolonged remission in some NHL patients. For some patients, however, auto-HCT may never be a viable option, whereas in other

Twenty-seven cases of pineal parenchymal tumours of intermediate differentiation: mitotic count, Ki-67 labelling index and extent of resection predict prognosis.

OBJECTIVE: Optimal grading and treatment of pineal parenchymal tumours of intermediate differentiation (PPTID) have not been established due to their rarity. After careful review of more than 500 pineal region tumours treated in our centre, 27 patients with PPTID were identified. METHODS: Diagnoses were confirmed according to WHO classification and graded as suggested by Jouvet et al. The relationship between the WHO grade, histopathological characters, management and outcome was analysed. RESULTS: The WHO grade did not demonstrate significant correlation with outcome. Mitotic count and Ki-6

Rehabilitation Strategy Guided by Physiological Indicators in a Patient With Tetanus and Multiple Fractures: A Case Report.

Tetanus is a serious infectious disease characterized by muscle rigidity and autonomic storm. In severe cases, long-term deep sedation is required, resulting in a high risk of post-intensive care syndrome (PICS). However, criteria for determining the timing of early rehabilitation intervention have not yet been established. We report the case of a woman in her 70s who developed tetanus following a wood-splinter injury to her right forearm and subsequently suffered bilateral humeral fractures due to muscle rigidity and convulsions. She was admitted to the intensive care unit (ICU) with an Ablet

Expert Review of the Strategies to Optimize Long-Term Outcomes After Coronary Artery Bypass Grafting.

Coronary artery bypass grafting (CABG) remains a cornerstone in the treatment of advanced ischemic heart disease, offering durable and effective revascularization. Despite surgical success, long-term patient outcomes are often shaped by the progression of native coronary disease and the development of comorbid conditions. This narrative review explores seven critical domains in secondary prevention following CABG: Early recognition of postoperative complications, evidence-based pharmacotherapy, management of atrial fibrillation, lifestyle modification, psychological well-being, preservation of

Evolving Role of Lumbar Decompression: A Narrative Review.

Traditional open lumbar decompression techniques have long been used to relieve spinal canal pressure caused by lumbar spinal stenosis. However, these procedures are associated with significant postoperative pain and prolonged recovery. Over the past few decades, there has been a shift toward minimally invasive surgical (MIS) techniques designed to minimize tissue trauma, postoperative pain, and recovery time. These advancements represent a major step forward, offering smaller incisions and direct visualization of the spinal canal. Despite the clear benefits of MIS and endoscopic techniques, t

A collaborative and evolving response to the needs of frontline workers, patients and families during the COVID-19 pandemic at Tygerberg Hospital, Western Cape Province, South Africa.

The global devastation caused by the COVID-19 pandemic and its mental health impact is undeniable. The physical and psychological consequences are wide-ranging - affecting patients fighting the disease, frontline workers in the trenches with them, healthcare staff deployed in high-care settings, and families disconnected from their loved ones in their darkest hours. Within 6 weeks of the COVID-19 outbreak in South Africa, the Department of Psychiatry at Stellenbosch University established the TBH/SU COVID Resiliency Clinic to provide psychological support to frontline workers at Tygerberg Hosp

COVID-19 and assisted reproductive technology services: repercussions for patients and proposal for individualized clinical management.

The prolonged lockdown of health services providing high-complexity fertility treatments -as currently recommended by many reproductive medicine entities- is detrimental for society as a whole, and infertility patients in particular. Globally, approximately 0.3% of all infants born every year are conceived using assisted reproductive technology (ART) treatments. By contrast, the total number of COVID-19 deaths reported so far represents approximately 1.0% of the total deaths expected to occur worldwide over the first three months of the current year. It seems, therefore, that the number of inf

[Peutz-Jeghers syndrome manifested as massive melæna at CHU-JRA Madagascar hospital: a case report].

Peutz-Jeghers syndrome (SPJ) is characterized by intestinal hamartomatous polyps in association with mucocutaneous lentiginosis. Patients are exposed to mechanical and bleeding complications. It is a cancer predisposition syndrome. Our study highlights the diagnostic criteria for Peutz-Jeghers syndrome (SPJ), the complications and the therapeutic progresses in patient care. We report the case of a 32-year-old male presenting with a massive melaena. It was hospitalized in the surgical intensive care unit with hypovolemic shock difficult to control. This required surgical intervention to stop bl

[A case of misinsertion of the PEG tube into the abdominal cavity recovered on a referral to the outpatient by using simple endoscopy techniques].

We report a case of a 53-year-old man, who had been bed-ridden due to the progression of spinocerebellar degeneration and had been relying on percutaneous endoscopic gastrostomy (PEG) feeding for long-term nutritional support at home. The patient was referred to our clinic from his local GP because of suspected misinsertion of the PEG tube into the abdominal cavity on regular exchange of the tube. We performed emergent gastric endoscopy. First we induced the biopsy forceps through an endoscopic fiber, and pulled the forceps out through the injured fistula to the surface. Then the loop wire use

Posttraumatic acute cholecystitis. Relationship to the initial trauma.

Posttraumatic acute cholecystitis is an often unrecognized and potentially fatal complication seen among patients hospitalized for trauma, and differs in etiology from cholecystitis which develops de novo. The cause, although not yet clearly defined, is believed to be related to bile stasis, ischemia, bacterial infection, sepsis, the activation of factor XII, and the Shwarzman reaction. A case is described in which a 53-year-old man with pelvic fractures developed acute acalculous cholecystitis and died of multiple organ failure 3 weeks following cholecystectomy. The histopathological findings

Model-Based Prognostication and Symptom Distress in Patients with Advanced Hepatopancreaticobiliary Cancers Receiving Best Supportive Care: A Prospective Cohort Study.

PURPOSE: Metastatic hepatopancreaticobiliary (HPB) cancers often exhibit rapid and aggressive progression. Accurate prognostication is essential for aligning care objectives and preventing futile interventions. The Palliative Prognostic Index (PPI) is a simple, validated bedside tool for advanced cancers, but its efficacy in metastatic HPB malignancies remains unassessed. This study aimed to evaluate the predictive accuracy of the PPI and to describe the symptom burden in this patient population. METHODS: We conducted a prospective observational cohort study of adults with metastatic HPB cance

Early Enzyme-Alginogel Treatment Improves Healing and Reduces Complications, Scarring, and Healthcare Burden in Children with Mild-to-Moderate Burns: A Retrospective Real-World Study.

BACKGROUND/OBJECTIVES: Pediatric mild-to-moderate burns require careful management in the early phase, yet evidence guiding optimal initial dressing selection remains limited. Although topical medications and specialized dressings are routinely used, a universally accepted standard of care is lacking. METHODS: We conducted a 1-year, real-world, retrospective, comparative, single-center study evaluating whether treatment with enzyme alginogel (Flaminal®) or NaOCl 0.05% antiseptic in the first 24 h led to different outcomes in terms of healing time, lesion progression, complication occurrence,

Cost-Effectiveness of Lower Extremity Nerve Decompression Surgery in the Prevention of Ulcers and Amputations: A Markov Analysis.

BACKGROUND: The costs and health effects associated with lower extremity complications in diabetes mellitus are an increasing burden to society. In selected patients, lower extremity nerve decompression is able to reduce symptoms of neuropathy and the concomitant risks of diabetic foot ulcers and amputations. To estimate the health and economic effects of this type of surgery, the cost-effectiveness of this intervention compared to current nonsurgical care was studied. METHODS: To estimate the incremental cost-effectiveness of lower extremity nerve decompression over a 10-year period, a Marko

A scoping review of paediatric latent tuberculosis infection care cascades: initial steps are lacking.

BACKGROUND AND OBJECTIVES: Identifying and treating children with latent tuberculosis infection (TB infection) is critical to prevent progression to TB disease and to eliminate TB globally. Diagnosis and treatment of TB infection requires completion of a sequence of steps, collectively termed the TB infection care cascade. There has been no systematic attempt to comprehensively summarise literature on the paediatric TB infection care cascade. METHODS: We performed a scoping review of the paediatric TB infection care cascade. We systematically searched PubMed, Cumulative Index to Nursing and A
