PURPOSE: Revision total hip arthroplasty (rTHA) with substantial femoral bone loss is technically demanding, requiring durable distal fixation, biomechanical restoration, and early functional recovery. Cementless modular long stems provide diaphyseal fixation with intraoperative adjustability, yet mid-term evidence across diverse revision indications remains limited. This prospective single-center study evaluated radiological stability, functional outcomes, and modifiable prognostic factors, particularly early weight bearing.
METHODS: Seventy-five patients underwent femoral rTHA with a cement

Endovascular aortic arch repair has seen increasing application for the treatment of complex aortic arch pathology in patients with suitable anatomy who are considered high risk for open surgical repair. Ongoing technical advancements have refined stent design, enabling total endovascular incorporation of the supra-aortic vessels. Despite these advances, the procedure remains highly complex and presents unique technical and physiologic challenges related to arch anatomy, device constraints, and cerebral protection. The risk of stroke remains high in several experiences and is the main limitati

BACKGROUND: Porous metal cones are commonly utilized in revision total knee arthroplasty (rTKA) to manage severe bone loss. Flexible metaphyseal cones (FMCs) allow for macrodeformation, more uniform stress distribution, and diminished peak stresses to the compromised cortical bone, potentially reducing fracture risk during impaction. This study aimed to evaluate the early outcomes of FMCs used to treat severe bone loss in rTKA.
METHODS: There were 241 FMCs inserted during 200 rTKAs. A combination of femoral and tibial FMCs was used in 41 knees. The most common reasons for revision were flexio

BACKGROUND: Bridging stent grafts (BSG) are a critical component of fenestrated and branched endovascular aortic repair (f/bEVAR) but remain prone to structural failure due to continuous biomechanical stress. Early detection of stent wire fractures is clinically relevant, yet remains challenging with conventional computed tomography (CT) because of limited effective spatial resolution and metal-related blooming artifacts. The aim of the study is to evaluate whether photon-counting detector CT (PCCT) improves the detectability of structural defects in BSG compared with dual-energy CT (DECT).

BACKGROUND: The Nexel total elbow arthroplasty (TEA) was introduced as an enhancement of the Coonrad-Morrey TEA, which through design modifications aimed to improve patient outcomes. However, high revision rates due to loosening have been reported for this implant.
METHODS: A review of 101 consecutive primary Nexel TEAs performed between 2015 and 2021 at a single institution was conducted using data from the Norwegian Arthroplasty Register, electronic medical records, and radiographic evaluations. Implant revision, other secondary surgery, and complications were investigated. Serial radiograp

Dissection morphology is important when considering landing zones for thoracic endovascular aortic repair. Landing proximally in a healthy aorta, free of dissection and intramural hematoma, is generally recommended. For the most common location of the proximal entry tear near the left subclavian artery, this generally requires landing in zone 2. However, zone 2 landing is associated with higher rates of stroke and need for left subclavian revascularization. Distal landing zone 5 is associated with much higher rates of spinal cord ischemia, without significant benefit over zone 4 or bare metal

BACKGROUND: The third-generation ceramic-on-ceramic total hip arthroplasty (THA) offers excellent wear resistance and biocompatibility but is associated with unique complications such as ceramic fracture and squeaking. While fourth-generation ceramics have become the standard, long-term outcomes of third-generation bearings remain clinically relevant, especially in patients still living with these implants.
METHODS: We retrospectively reviewed 533 hips in 417 patients who underwent primary alumina-on-alumina THA using ceramic bearings with a minimum 15-year follow-up (mean 17.4). After exclud

BACKGROUND: Cemented stems are currently recommended for the treatment of displaced femoral neck fractures (FNFs) due to their reduced risk of revision and periprosthetic fractures (PPF). However, cementless stems with a collar may enhance implant stability. This study assessed the effect of collar addition on revision rates and implant survival in conventional total hip arthroplasty (THA), double mobility THA (THA DM) or hemiarthroplasty (HA) for FNFs.
HYPOTHESIS: We hypothesized that the presence of a collar on cementless stems could play a protective role against both all-cause and peripro

BACKGROUND AND OBJECTIVE: Porous tantalum offers excellent osteointegration and bone growth over the implant, in addition to a physiological load transfer with reduced bone resorption. However, there is limited literature on the survival of tantalum-coated femoral stems. The objective of this study is to evaluate the outcomes in terms of survival of a tantalum-coated stem in the context of primary total hip arthroplasty (THA).
MATERIALS AND METHODS: A retrospective study was conducted on all patients who underwent primary THA between 2006 and 2009 at a single center, where a tantalum-coated f

AIMS: A range of metal-on-metal hip resurfacing (MoM-HR) implants have shown good functional outcomes, but some have been associated with adverse reactions to metal debris (ARMD) and early failure, requiring regular follow-up and monitoring of the blood metal ion levels. The aim of this study was to report the minimum 15-year survival of the Durom hip resurfacing system (Zimmer Biomet, USA), the functional outcome, and factors which were predictive of failure.
METHODS: A consecutive series of patients undergoing Durom MoM-HR at a single centre between January 2000 and December 2008 were inclu

OBJECTIVES: Mitral regurgitation (MR) represents the most common valvular heart disease (VHD) in the Western world. While transcatheter mitral valve repair (M-TEER) is the leading interventional treatment for surgically high-risk patients, transcatheter mitral valve implantation (TMVI) is reserved for selected patients with unsuitable anatomy for M-TEER. This review aims to summarize our institutional experience with transapical TMVI using the Tendyne valve (Abbott Vascular, CA, USA), focusing on interdisciplinary preoperative, intraoperative, and postoperative management strategies.

BACKGROUND: Congenital heart defects affect approximately 1% of births in the United States and Europe, with >1 million children in the United States living with congenital heart defects. Many experience abnormalities in the right ventricular outflow tract, often necessitating surgical intervention early in life. However, the initial repairs typically are temporary solutions as many patients will eventually need pulmonary valve replacement to address pulmonary valve regurgitation and prevent right ventricle failure. Addressing progressive pulmonary valve regurgitation, ideally in patients weig

OBJECTIVES: When tricuspid valve repair is unfeasible due to extensive damage or a complex congenital malformation, surgeons consider tricuspid valve replacement (TVR). However, it is still controversial and challenging to choose the best substitute. We aimed to introduce the innovative intraoperatively valve construction using the native right atrial appendage (RAA) tissue for TVR and investigate the short-term outcomes.
METHODS: This study recruited paediatric and adult patients with unrepairable severe tricuspid regurgitation (TR) who needed TVR. The patient's RAA tissue was harvested and
