The impact of comorbidity on mortality following in-hospital diagnosis of tuberculosis.

STUDY OBJECTIVES: Despite the availability of curative chemotherapy, mortality remains high among patients hospitalized for tuberculosis. Although the elevated mortality rate is often attributed to the presence of multidrug resistant tuberculosis (MDRTB) or concomitant infection with the HIV, other factors must be contributory, especially among the HIV-negative population. Therefore, we performed a study to define the factors associated with mortality following the in-hospital diagnosis of tuberculosis in a region with low levels of MDRTB and coinfection with HIV. DESIGN: Retrospective cohort

Why do residents leave general surgery? The hidden problem in today's programs.

OBJECTIVE: Much has been written and discussed about the reasons for reduced interest in surgery, but few institutions have chosen to examine the loss or attrition of general surgery residents from their own programs. In preparation for an upcoming Residency Review Committee analysis of our program, we took the opportunity to examine the reasons for attrition in our own institution. DESIGN, SETTING, AND PARTICIPANTS: During the years 1990 to 2003, 120 categorical residents were admitted into our general surgery residency program. Residents who matched into preliminary positions or non-5-year

Experiences of children and adults having a sibling with profound intellectual and multiple disabilities: a qualitative systematic review.

BACKGROUND: Advances in medical care have enabled an increasing number of children with profound intellectual and multiple disabilities (PIMD) to live beyond early infancy and childhood. However, the burden of caregiving and restrictions on family life markedly impact the siblings of children with PIMD. To our knowledge, no qualitative systematic review has yet explored the experiences of these siblings. OBJECTIVE: This review aimed to synthesize the experiences of individuals, both children and adults, having a sibling with PIMD. INFORMATION SOURCES: Searches were conducted in MEDLINE (Ovid

Women Planned for Immediate Lymphatic Reconstruction During Axillary Lymph Node Dissection Should Be Reconstructable: Improving Intraoperative Team Collaboration.

Introduction: Immediate lymphatic reconstruction (ILR) during axillary lymph node dissection (ALND) has been shown to reduce breast cancer-related lymphedema (BCRL). However, some authors report many "non-reconstructable" patients, meaning that there were no suitable lymphatics or veins available in the axilla to complete the reconstructive procedure once the extirpative portion was complete. In contrast, almost all of our patients planned for ALND/ILR have had appropriate donor and recipient vessels. The purpose of our study was to contrast the incidence of "non-reconstructable" patients in t

Use of Incentive Spirometry to Prevent Acute Chest Syndrome (ACS) in Patients With Sickle Cell Disease (SCD): A Systematic Review.

Sickle cell disease (SCD) is a genetic hematological disorder that causes the production of sickle-shaped red blood cells. These abnormal cells reduce the oxygen-carrying ability around the body and obstruct blood flow, potentially resulting in devastating complications such as acute chest syndrome (ACS). The objective of this systematic review is to assess whether incentive spirometry is effective in reducing the incidence of ACS in patients with SCD. This review searched databases, including the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE, Embase, ClinicalTrials.gov, an

A Review of Smartphone Apps Used for Toric Intraocular Lens Calculation and Alignment.

Smartphone apps are becoming increasingly popular in ophthalmology, one specific area of their application being toric intraocular lens (IOL) surgery for astigmatism correction. Our objective was to identify, review and objectively score smartphone apps applicable to toric IOL calculation and/or axis alignment. This review was divided into three phases. A review was conducted on four major app databases (phase I): National Health Service (NHS) Apps Library, Google Play Store, Apple App Store and Amazon Appstore. A systematic literature review (phase II) was conducted to identify studies for in

Clinical outcomes following surgical management of coexistent cervical stenosis and multiple sclerosis: a cohort-controlled analysis.

BACKGROUND CONTEXT: The presentation of myelopathy in patients with the concomitant diagnosis of cervical stenosis (CS) and multiple sclerosis (MS) complicates both diagnosis and treatment because of the similarities of presentation and disease progression. There are only a few published case series that examine this unique patient population. PURPOSE: To define the demographic features and presenting symptoms of patients with both MS and CS and to investigate the immediate and long-term outcomes of surgery in patients with MS and CS. STUDY DESIGN/SETTING: Matched cohort-controlled retrospec

Secondary school teachers' experiences of supporting students with mental health issues: a systematic review and meta-aggregation of qualitative studies.

BACKGROUND: Mental health issues in adolescents is steadily increasing, mainly since the rise of social media, the concomitant effects of internet use and other problems associated with academic performance, familial abuse and bullying. Secondary school teachers are exposed to a wide range of adolescent behaviours, yet depression and anxiety appear to be those behaviours that are causing the greatest concern. AIM: This review aimed to appraise and synthesise qualitative studies examining secondary school teachers' experiences of supporting students experiencing mental health issues. METHODS:

Evidence for incentive-based strategies to promote breastfeeding: a systematic literature review of randomised controlled trials.

BACKGROUND: There is some evidence that incentive-based strategies effectively encourage smoking cessation in the perinatal period. Incentives could be part of policies aiming to increase breastfeeding rates. This systematic review aimed to summarise current evidence to guide researchers and policymakers towards potentially effective incentive-based strategies for increasing breastfeeding. METHODS: Searches of electronic literature databases were conducted (MEDLINE, Scopus, Embase, CINAHL, and PsycInfo) for evaluations of incentive-based strategies to promote breastfeeding published up to Aug

Flipping death-to-survival event coding shifted pooled risk ratios and GRADE imprecision ratings in 2,071 Cochrane meta-analyses.

OBJECTIVE: When meta-analyzing a dichotomous outcome such as death or survival, reviewers must decide which side of the outcome to code as the event. In a single study, death and survival are two sides of the same dichotomous outcome, and the choice does not change the result. However, in a meta-analysis using the risk ratio, this choice affects the weight given to each study. Pooling deaths and pooling survivors need not give equivalent results. We evaluated how flipping the event coding of death or survival outcomes affects pooled risk ratios, precision, and GRADE imprecision judgments acros

Prevalence, Risk Factors, and Clinical Profiles of Hepatitis D Virus in Nigeria: A Systematic Review, 2009-2024.

BACKGROUND: The World Health Organization (WHO) recommends hepatitis D virus (HDV) screening among hepatitis B virus (HBV) infected individuals, with a focus on priority populations in resource-limited settings like Nigeria. HDV infection is a growing public health challenge, particularly among individuals with chronic hepatitis B virus (HBV) infection. HDV accelerates liver disease progression and significantly increases the risk of cirrhosis and hepatocellular carcinoma. Despite this, the epidemiology of HDV in Nigeria remains inadequately documented. This scoping review critically evaluates

Emergency centre thoracotomy for penetrating trauma: Insights from 2 South African district-level emergency centres.

INTRODUCTION: Trauma is a major contributor to global disease burden, disproportionally affecting low- and middle-income countries, especially in the African Region. Emergency centre thoracotomy (ECT) is a potentially life-saving procedure for a sub-group of trauma patients in extremis. Most literature regarding ECT originated in high-income countries. This study aimed to describe patient, procedure and outcome characteristics of ECTs performed at two facilities in a resource-limited setting in South Africa. PATIENTS AND METHODS: A retrospective chart review was performed at two district-leve

Prevalence of latent tuberculosis infection in Malaysia: the first systematic review and meta-analysis.

BACKGROUND: Latent tuberculosis infection (LTBI) remains a major public health challenge in Malaysia, yet prevalence estimates across population groups and settings are fragmented. This systematic review and meta-analysis aimed to synthesize available evidence and identify sources of heterogeneity. METHODS: We searched major databases for studies reporting LTBI prevalence in Malaysia. Random-effects meta-analyses were performed, with subgroup and meta-regression analyses to explore heterogeneity. Publication bias was assessed using Egger's and Begg's tests. Meta-analysis was performed using O

The Value of Assessing Self-Reported and Biological Indicators of Outcomes in Evaluating HIV Programs.

PURPOSE OF REVIEW: In this manuscript, we present recent findings concerning concordance and discrepancy between biological measures and self-reports of these three outcomes of HIV programs: HIV status, adherence to antiretroviral medications (ARVs) and use of and adherence to pre-exposure prophylaxis medication (PrEP), and condom use/unprotected sex. RECENT FINDINGS: Recent studies suggest that three successive rapid HIV tests (for those whose first test in positive) might be reasonably inexpensive and valid biological data to collect to combine with self-reports of HIV status, dried blood s

Optimizing metabolic health through deep-phenotyping-based personalized nutrition.

With rising rates of chronic diseases, obesity and metabolic disorders, there is an urgent need for more effective lifestyle interventions. An increasing recognition of how an individual's diet affects their disease susceptibility has highlighted the limitations of traditional one-size-fits-all dietary guidelines, which often fail to account for the considerable metabolic heterogeneity among individuals. Precision nutrition tailors dietary advice and nutritional prescriptions by considering a broad range of factors, including genetics, age, sex, phenotype, lifestyle, preferences, clinical hist

A Narrative Review of HRV Metric Selection for Applied Monitoring: RMSSD as the Default Metric for Sport and Exercise.

Heart rate variability (HRV) monitoring is commonly used in sport and exercise science to assess recovery, training adaptation, and performance readiness. However, the routine reporting of numerous HRV metrics with substantial conceptual and physiological overlap has created inconsistency regarding which indices should be prioritized for applied monitoring. This narrative review synthesizes the contemporary literature to provide practical recommendations regarding HRV metric selection in applied monitoring, with particular emphasis on whether the root mean square of successive differences betw
