Reconstruction of chronic thumb metacarpophalangeal joint radial collateral ligament injuries with a half-slip of the abductor pollicis brevis tendon.

PURPOSE: To evaluate a reconstructive method for chronic radial collateral ligament (RCL) injuries of the thumb metacarpophalangeal (MCP) joint using a combination of RCL advancement and the transfer of a half-slip of the abductor pollicis brevis tendon. METHODS: Eight patients (4 male and 4 female; mean age, 25 y) with chronic RCL injury of the thumb MCP joint were enrolled. All patients were referred to our institution because of continuing pain and instability on the radial side of the MCP joint when grasping or pinching objects. The mechanism of the injury was adduction stress to the thum

[Transarterial periarticular embolization (TAPE)-From pain to silence: TAPE as a new local option for osteoarthritis and in the future for inflammatory rheumatic joint diseases].

Transarterial periarticular embolization (TAPE) is a novel interventional procedure for local pain therapy in osteoarthritis. The reduction of local hypervascularization is achieved by supraselective embolization of periarticular arteries using embolic agents. The procedure described herein results in a reduction in both pain and the use of analgesics. The drug was originally developed for the treatment of knee osteoarthritis and initial small studies and case reports indicated that it was effective for other joints, including the fingers and shoulders. In the domain of rheumatology TAPE opens

[Diabetic osteoarthropathy].

It is well-known that osteoarthropathy is one of common problems in diabetic patients. Limitation of joint mobility and painful disorder associated with inflammation of tendons and sheaths are more frequently observed on hands and shoulders. Most of these complications are not specific for diabetes mellitus, but more often in diabetic populations than in non-diabetic populations. On the other hand, destructive bone change due to diabetic polyneuropathy are commonly observed on lower extremities. To control neuroarthropathy is important for prevention of the following diabetic ulcer and gangren

Posterior dislocation of the sternoclavicular joint leading to mediastinal compression.

Dislocations of the sternoclavicular joint are uncommon, and the posterior variety have a potential for considerable morbidity. We report a case with compression of the vital structures within the superior mediastinum. It was a rugby player getting run over by the scrum. The mechanism was an indirect force exerted forward and laterally against the shoulder. The patient complained of pain and dysphagia. A systolic right cervical murmur was heard. Angiography was normal and esophagography showed extrinsic esophageal compression. Surgical reduction was performed because there was a slight pneumom

Comparison of Markerless and Conventional Marker-Based Shoulder Kinematics Models During Activities of Daily Living in Patients With Glenohumeral Osteoarthritis.

BACKGROUND: Markerless motion capture utilizes deep learning models to evaluate standard video from multiple cameras and is significantly more time-efficient than traditional marker-based systems in both setup and analysis. There has been increasing interest in validating markerless motion analysis in the clinical orthopaedic patient population. PURPOSE: To evaluate the concurrent validity of markerless shoulder analysis compared to traditional marker-based shoulder analysis during activities of daily living (ADLs) in patients with glenohumeral osteoarthritis. We hypothesize that the markerle

Biomechanical and physiological effects of a passive upper-body exoskeleton during stair ascent and descent.

The market has seen the emergence of various passive exoskeletons designed to assist with carrying tasks; however, evidence of their effects during stair negotiation remains limited. In this study fifteen female and fifteen male participants carried a 12-kg load with and without an upper-body passive exoskeleton (CarrySuitⓇ) while ascending and descending stairs. The impact of the CarrySuitⓇ was evaluated through measurements of heart rate, muscle activity, and joint range of motion during both stair ascent and descent. The results indicated that heart rate and muscle activity in the biceps br

Recurrent falls as the presentations of Gitelman syndrome in an octogenarian.

Gitelman syndrome (GS) is the most common hereditary renal tubular disorder, with a higher carrier frequency among Asians often overlooked in older adults. Electrolyte imbalances, such as those seen in GS, are crucial considerations for older adults experiencing recurrent falls. We described an 83-year-old diabetic female on metformin, who was admitted due to recurrent falls with the preceding dizziness and palpitations when standing. She had the history of chronic hypokalemia and hypomagnesemia on regular potassium (K+) and magnesium (Mg++) supplementation for 10 years and gout-like arthritis

Clinical outcome of wedged glenoid reconstruction in anatomic total shoulder arthroplasty for osteoarthritic retroverted glenoid: a minimum 2-year follow-up.

BACKGROUND: Glenoid retroversion and humeral head subluxation is a progressive disorder due to abnormal force coupling and increased contact force. In situ placement of anatomic total shoulder arthroplasty (TSA) components in this scenario results in edge loading, progressive subluxation, and early failure. Wedged glenoid components have been demonstrated to improve glenohumeral alignment, but have not been correlated with mid-term clinical outcomes. METHODS: Patients undergoing TSA using a wedged all-polyethylene glenoid component for retroverted glenoid deformity were identified from a pros

Risk of Iatrogenic Axillary Nerve Injury During Acromioclavicular Joint Reconstruction.

Anatomical reconstruction of the coracoclavicular ligaments involves drilling the base of the coracoid or looping a graft around it, placing the axillary nerve at risk for injury. Rockwood type V acromioclavicular (AC) separation injuries involve disruption of the AC joint capsule and coracoclavicular ligaments, resulting in inferomedial displacement of the scapulohumeral complex and alteration of the normal anatomical relations of the shoulder girdle structures. This study evaluated the effect of Rockwood type V AC separation on the anatomical relation of the axillary nerve to the coracoid ba

Treatment of the Upper Extremity Contracture/Deformities.

Patients with arthrogryposis multiplex congenita have a characteristic upper extremity resting posture consisting of internal rotation of the shoulders, elbow extension, flexed wrists, thumb-in palm deformities, and variable degrees of finger contractures. Treatment of these patients is aimed at improving independence and performance of activities of daily living. Although each area needs to be assessed independently for the most appropriate surgical procedure, often multiple areas can be addressed at the same operative setting. This limits the number of anesthetic exposures and cast immobiliz

Reactivated musculoskeletal tuberculosis with concomitant asymptomatic genitourinary infection.

A case is presented of local recurrence of tuberculosis after successful hip arthrodesis, in a patient subsequently found to have active asymptomatic renal tuberculosis. The possibility of hematogenous spread is raised, and the importance of thorough genitourinary evaluation in cases of reactivated skeletal tuberculosis is discussed.

The effects of sleeper stretch vs. crossbody stretch in overhead athletes with shoulder pain and glenohumeral internal rotation deficit: a randomized controlled trial.

BACKGROUND: Glenohumeral internal rotation deficit (GIRD) has been associated to shoulder injuries in overhead athletes. The glenohumeral posterior capsule stretching techniques, such as the sleeper stretch and cross-body stretch, are used to improve range of motion (ROM) and reduce pain. However, there are no studies comparing these techniques in symptomatic overhead athletes with GIRD. This study aimed to compare the effects of sleeper stretch and cross-body stretch on shoulder pain and shoulder ROM in overhead athletes with GIRD and shoulder pain. METHODS: This is a randomized, double-blin

Long-term (minimum 10 years) survival and outcomes of pyrocarbon interposition shoulder arthroplasty.

BACKGROUND: There are some major controversies surrounding the use and longevity of pyrocarbon interposition shoulder arthroplasty (PISA). The objective of this study was to investigate the long-term survival and outcomes (minimum 10-year) following PISA for osteoarthritis (OA) in young and active patients. METHODS: This was a retrospective review of prospectively collected data of patients who underwent PISA (InSpyre; Tornier-Stryker) for OA between 2009 and 2012. Arthroplasty survival was known for 71 patients followed longitudinally for a minimum of 10 years. The clinical and radiologic ou

Minimum 5-year outcomes of anatomic total shoulder arthroplasty with the Arthrosurface aspherical humerus and inlay glenoid.

BACKGROUND: Aseptic glenoid loosening is the most common cause of revision after anatomic total shoulder arthroplasty (aTSA). Inlay glenoid components have been shown to reduce edge loading and opposite-edge lift-off forces compared with onlay glenoids. Early clinical results of inlay glenoids have been promising, although there is a paucity of literature detailing mid-term to long-term follow-up. We report the minimum 5-year clinical and radiographic outcomes after aTSA with the Arthrosurface inlay glenoid component and a stemless, aspherical humeral head in an active, young patient populatio

Results of anatomic total shoulder arthroplasty with the Arthrex EclipseTM stemless humeral implant in patients over 70 years of age.

BACKGROUND: Anatomic total shoulder arthroplasty (aTSA) is a well-described technique for addressing glenohumeral osteoarthritis. Little has been written on outcomes for newer stemless humeral implants in older patients, with none looking specifically at an implant relying on screw fixation. The purpose of this study is to evaluate the clinical and radiographic outcomes for patients over 70 years of age undergoing aTSA with the Eclipse (Arthrex Inc., Naples, FL, USA) stemless humeral component. METHODS: A retrospective review using a multicenter shoulder arthroplasty registry was performed ev
