Age of initiation, smoking patterns, and risk in a population of working adults.

BACKGROUND: Early age of initiation is a significant risk factor for long-term dependent smoking and may also relate to other unhealthy behaviors and increased likelihood of illness, independent of duration of smoking. METHODS: The current study assessed age of initiation in relation to cigarette dependence, interest in quitting, social environment pertaining to smoking, behavioral risk factors, and current health problems. Subjects were 2120 current daily smokers in 24 worksites in the Minneapolis/St. Paul, Minnesota, metropolitan area. RESULTS: Findings were surprisingly consistent with ea

The moral challenge of children at risk: protective policies and pediatrics. A report of the Children's Services, Inc. Task Force of Greater Cleveland.

Social workers and pediatricians are among the professionals who share a society-wide concern with current public policies regarding the placement of children at extreme risk. A healthcare professional may successfully treat a child, only to learn later that this same child was the victim of a tragic incident of domestic violence after returning home. Such events are not uncommon, create considerable frustration for pediatricians, and demand an integrated interprofessional and interdisciplinary response. This report emerged from 6 months of task force dialogue with leaders of children's servic

Suicide risk factors among veterans: risk management in the changing culture of the Department of Veterans Affairs.

Suicide risk management in the Department of Veterans Affairs (VA) health care system is particularly challenging because of both patient characteristics and aspects of the delivery system. The prototypical suicide-prone person is an older white male with alcoholism, depression, physical problems, and poor psychosocial support. This describes a large portion of the veteran patient population. Suicide risk factors that are common in VA patients include male gender, older age, diminished social environment support (exemplified by homelessness and unmarried status), availability and knowledge of

The need for research on dementia in developing countries.

Dementia is one of the commonest and most disabling late-life mental disorders. Its prevalence and incidence have been assessed in developed countries, and show little geographical variation between countries and regions. Although most older people live in developing countries, little research has been carried out in those settings. There is some evidence that age-specific prevalence rates for dementia in developing countries may be relatively low. More research is needed to allow developing countries to estimate the current extent, type and cost of medical and social service provision, and to

Childhood burns in south eastern Nigeria.

In a ten year retrospective study of burns in children in University of Calabar Teaching Hospital (UCTH), Calabar, the main causes were hot water, hot soup or oil (56.6%) involving children mostly in the one to three year age group. The relative safety of the home environment seen in other forms of paediatric trauma is not observed in burns in children. A changing pattern of burns in children has emerged within the region with naked flames/bush fire coming second and affecting 22.7% of the children. Chemical burns hitherto a rare occurrence is now frequent because of the storage of caustic sod

HIV infection among Brazilian transvestites in a prison population.

Eighty-two male transvestites imprisoned in Casa de Detenção (São Paulo, Brazil) were tested for HIV antibodies, and completed a questionnaire investigating their demographics, arrest and imprisonment records, sexual practices, and drug use. Data were then analyzed to evaluate the incidence of HIV infection and its association with various behavioral and other factors. Sixty-four of 82 (78%, 95% confidence interval [CI], 67-87%) transvestites were positive for HIV infection. The factors associated with significant differences in positivity among these individuals were the time spent in prison

Pain, quality of life, and coping in sickle cell disease.

This study examined the frequency and severity of sickle related pain, its impact on quality of life, and methods of coping for 25 children with sickle cell disease, aged 6-16 years. Subjects were matched with non-affected peers and asked to complete the Central Middlesex Hospital Children's Health Diary for four weeks. Results indicated that sickle pain occurred on average one in 14 days, and total summary pain scores indicated significantly greater pain than for controls. Children with sickle cell disease could discriminate sickle pain and did not adopt sick role responses to ordinary childh

Risk factors of psychosocial problems among children working in workshops in Alexandria.

This study was conducted to determine whether psychosocial problems occurring among working children are associated with other social or occupational factors. A sample of 34 working children ageing from 10-14 years was studied. They were classified into 2 groups according to the occurrence of psychosocial problems as anxiety, eating or sleeping problems, fatigue, nightmares etc... Interview data were examined to identify factors that might be considered as risk factors in psychosocial problems. Ten variables distinguished children with psychosocial problems. These variables were classified by

Medicine in tropical Australia.

In the unique environment of Australia's tropical north there are endemic diseases inherited from Gondwana, others introduced from the north and from Europe, and a wide range of particularly venomous animals. There is continuing disparity in morbidity and mortality between Aboriginal people and other Australians in tropical areas and elsewhere. This is being addressed by the National Aboriginal Health Strategy, which emphasises social, environmental and economic issues, as well as control and coordination of services by Aboriginal and Torres Strait Islander communities. While the re-introducti

Some ways of optimization of aphasics rehabilitation.

The main task of re-education in aphasia is the restoration of speech communicative function, which is characterized by activity, directionness and intentionality. These features can appear only in a society, with the best medium for its realization being a group or a society. Group lessons profit from the dynamics of the small social group in order to restore general and verbal communication. The group lesson also makes it possible to organize correctly, a verbal environment for aphasics. We have worked out methods of group rehabilitation based on some theoretical points of view, such as the

Environmental synchronizers of squirrel monkey circadian rhythms.

Various temporal signals in the environment were tested to determine if they could synchronize the circadian timing system of the squirrel monkey (Saimiri sciureus). The influence of cycles of light and dark, eating and fasting, water availability and deprivation, warm and cool temperature, sound and quiet, and social interaction and isolation was examined on the drinking and activity rhythms of unrestrained monkeys. In the absence of other time cues, 24-h cycles of each of these potential synchronizers were applied for up to 3 wk, and the periods of the monkey's circadian rhythms were examine

Evaluating an interprofessional team coaching program to promote a human-centered ICU environment.

BACKGROUND: Technology-driven ICU environments can be dehumanizing for critically ill patients, their families, and team members who care for them. Prior educational efforts to humanize the ICU have focused on training clinicians to provide human-centered care for patients and families, with relatively little attention on promoting human-centered environments for team members. OBJECTIVE: We used the framework of team coaching to develop an interprofessional educational program focused on humanizing the ICU. We sought to evaluate whether this program improves participants' perceived capability

The psychiatric ward environment and nursing observations at night: A qualitative study.

WHAT IS KNOWN ON THE SUBJECT?: Nursing observations at night are conducted on psychiatric wards to ensure the safety and well-being of patients as well as to reduce the risk of suicide or severe harm. To our knowledge, no studies have examined the lived experience of the psychiatric ward environment and of nursing observations at night. WHAT DOES THE PAPER ADD TO EXISTING KNOWLEDGE?: The main complaint from patients was the constant interruption of their sleep, as most of the patients in this study were observed 2-4 times an hour. Their sleep was interrupted by (a) the light from torches shone

The ReDiKid framework: a transdisciplinary systems approach to child resilience in the digital age.

Children's lives are increasingly shaped by digital environments that influence learning, social relationships, daily routines, and exposure to both opportunities and adversity. However, existing approaches often examine digital experiences, self-regulation, movement behaviors, and social context separately. This paper introduces ReDiKid (Resilient Kid in the Digital World), a developmental-systems framework for understanding resilience in late childhood, with a primary focus on children aged approximately 9-11 years. ReDiKid conceptualizes resilience as a higher-order, adversity-activated pro

Forensic insights into mortality and challenges in post-mortem investigations of homeless individuals: a 10-year study (2014-2024) in Milan, Italy.

Forensic medicine represents a critical socio-health observatory for socially marginalized populations. This study provides the first systematic retrospective forensic analysis of mortality among people experiencing homelessness in Italy. We examined 104 decedents undergoing autopsy in Milan between 2014 and 2024. The annual number of autopsied homeless decedents increased over the study period, peaking in 2023-2024. The majority of deaths occurred in indoor public spaces (45/104, 43.3%), with the remainder in hospitals or green areas, reflecting extreme social marginalization and environmenta

Anatomy of Antisemitism in Health Professions Education and Practice: A Narrative Review.

There has been an international resurgence in antisemitism in healthcare education and practice since October 7, 2023, prominently affecting the United States (US), United Kingdom (UK), Canada, and Australia. In the US, federal investigations by the US Department of Justice and the Congressional Subcommittee on Education and the Workforce were initiated into antisemitic harassment and hostile learning environments in medical schools. Healthcare organizations in the UK also came under investigation, and the US Congress opened an investigation into alleged antisemitism within a national psycholo

The Impact of Social Determinants on Posttraumatic Growth Among Young Adult Childhood Cancer Survivors.

BACKGROUND: Young adult childhood cancer survivors (YA CCSs) may experience posttraumatic growth (PTG), yet little is known about how neighborhood-level social determinants influence PTG. Posttraumatic stress may also shape PTG, but its mediating role in the relationship between neighborhood deprivation and PTG remains unclear. OBJECTIVE: To examine associations between neighborhood deprivation and PTG among YA CCSs and determine whether posttraumatic stress mediates these associations. METHODS: This cross-sectional study included 121 YA CCSs. Neighborhood deprivation was measured using the

Disrupted self-referential processing and empathy-based interventions in mental disorders: neural mechanisms, cognitive biases, and therapeutic integration.

Self-referential processing (SRP), the mental activity of linking experiences and stimuli to the self, is fundamental to shaping identity, regulating affect, and navigating complex social environments. Disruptions in SRP are increasingly recognized as a transdiagnostic hallmark across psychiatric disorders, including schizophrenia spectrum disorders, major depressive disorder, and anxiety disorders. Such disruptions often manifest as distorted self-perception, negative cognitive-affective biases, and dysfunctional connectivity within key neural networks, particularly the default mode network (

Towards medical pedagogical pragmatism: transforming medical students' Understanding of community health needs through the integrated longitudinal community clerkship in rural South Africa.

BACKGROUND: Community-oriented medical education seeks to align medical training with priority community health needs and their lived realities. While longitudinal community-based clerkships are increasingly adopted to promote socially accountable graduates, there is limited qualitative insights into how these experiences can transform students’ understanding of community health needs, particularly in resource-constrained settings. OBJECTIVE: This study explored how participation in a 20-week Integrated Longitudinal Community Clerkship at a South African medical school transformed medical stud

Decreased sound tolerance in a Canadian University Context: Associations with autistic traits, social competence, and gender in an undergraduate sample.

Disorders of decreased sound tolerance such as misophonia and hyperacusis cause significant distress through strong negative emotional and physiological reactions to everyday sounds. These conditions have been associated with poor mental and physical health as they impact day to day life. Prior to the recent development of consensus definitions of misophonia and hyperacusis, attempts to determine the prevalence of these conditions have been hindered by the ambiguity and inconsistency of their descriptions. Despite this, certain populations have been suggested to more frequently experience miso
