PURPOSE: The sedimentation sign (SedSign) has been shown to discriminate well between selected patients with and without lumbar spinal stenosis (LSS). The purpose of this study was to compare the pressure values associated with LSS versus non-LSS and discuss whether a positive SedSign may be related to increased epidural pressure at the level of the stenosis.
METHODS: We measured the intraoperative epidural pressure in five patients without LSS and a negative SedSign, and in five patients with LSS and a positive SedSign using a Codman(™) catheter in prone position under radioscopy.
RESULTS:

OBJECTIVES: To define the characteristics of optimum implantation corridors in vertebral bodies C2 to C7 and T10 to S1, using computed tomography examination.
METHODS: Measurements were taken from 207 vertebrae from 35 different adult dogs.
RESULTS: Implantation corridors of the cervical vertebrae are narrow. The width preserving the transverse hole is less than 2.5 mm in 68.6 per cent of the 86 vertebrae C2 to C6. Dorsal implantation corridors of the last four thoracic vertebrae are narrow, and major anatomical structures are very close to their emergence point. In 63 per cent of the 40 tho

The authors report the use of percutaneous transpedicular vertebroplasty performed using polymethylmethacrylate (PMMA) in two patients. These men (53 and 57 years old) had previously undergone open surgery and posterior instrumentation to treat myelomatous compression fractures. Both patients presented with acute back pain that manifested after minor activities. Kyphotic wedge fractures were diagnosed at T-1 in one case and at L-1 in the other. Both patients were treated at other hospitals with laminectomy and instrumented fusion; multiple myeloma was diagnosed after surgery. The patients expe

Pedicular fixation has found great acceptance as a modality for spinal fusions. Being a "blind technique" it is associated with a potential high risk of neural and vascular morbidity. In an attempt to evaluate and/or establish a uniformly accepted concept of surgical management we designed a questionnaire composed of seventeen questions dealing with different pre-, intra- and postoperative procedures in transpedicular fixation. This was sent to 31 experienced surgeons organized in the working group "spine" of the german trauma association. Half of the answers to each question were similar. The

A case is presented in which a patient with rheumatoid damage to the cervical spine causing cervical cord and vertebral artery compression sustained transient cortical blindness with a partial left hemiparesis. Magnetic resonance imaging revealed anterior subluxation of middle cervical vertebrae, separation of the odontoid peg with resultant atlantoaxial subluxation, and proliferative pannus formation. The patient was almost symptom-free after transoral decompression and posterior cervical fusion.

Of 16 patients with anterior spinal tuberculosis treated in this unit during the period 1979-1984, five presented with neurological complications following laminectomy at another hospital. After the decompressive laminectomy all five patients developed an increasing kyphosis, and four of the five patients had deteriorated neurologically. They were transferred to our unit where the four with neurological deterioration underwent surgical stabilisation. This paper illustrates the dangers of posterior spinal surgery in the presence of anterior disease.

We studied 21 cases of diastematomyelia associated with myelo-meningocele (3 cases) or congenital spine deformities (18 cases). Analysis of bone and neuraxis abnormalities with special reference to the spinal canal, related to operative findings demonstrate in all cases an embarrassed cord by localized compression or global narrowing of the malformed spinal canal (11 cases), bone spur fixation (13 cases) and tethered cord associated lesions (5 cases). Mechanisms of neural damage and prophylactic value of laminectomy before the orthopedic treatment of the curve during the same operation is disc

A patient with grade II spondylolisthesis and back pain underwent Harrington rod instrumentation. Postoperatively, he developed pain and numbness of the lateral aspect of the left foot and loss of the left AChilles reflex. Preoperative electrodiagnostic findings of the left lower extremity were normal. Two postoperative studies revealed that, concomitant with evidence of denervation if the left S1-supplied muscles, his sural sensory nerve action potential (SNAP) had been abolished but the superficial peroneal SNAP, recorded from the intermediate branch, was normal and equal to that of the cont

Much of the current medical discussion for within centers for skeletal dysplasia and specifically patients with achondroplasia focuses on infancy and early childhood. Most neurosurgical concerns arise due to a defect in the endochondral ossification, resulting on early fusion of the synchondrosis. As patients age, the neurosurgical focus shifts from primarily cranial to spinal concerns. Often pediatric neurosurgeons may continue to follow their patients with skeletal dysplasia. However, general adult neurosurgeons and orthopedic surgeons may see these graduated adults in their practice. This a

We describe the case of a 46-year-old woman suffering from an hyperalgic sciatica caused by a lumbar synovial cyst. These are uncommon lesions associated with degenerative spine disease. They may be asymptomatic or may produce symptoms resulting from nervous structures compression. They are commonly found at the L4-L5 level, the site of maximum mobility. Their etiopathogeny is still unclear but degenerative spinal instability is the strongest factor for their growth. MRI is the most effective for the diagnosis. Conservative management is usually unsuccessful. Resection with or without fusion r

Evidence suggests that effective conservative treatment is available for chronic low back pain. The effectiveness of conservative treatment has recently received attention following publication of several randomized controlled trials (RCTs) that reported similar improvements in outcomes from cognitive intervention with exercise as from spinal fusion surgery. This paper will explore the conservative treatment arms of these RCTs with the goal of educating the reader about the principles of cognitive intervention with exercise. These principles can be incorporated into the care of chronic low bac

A 5-year-old male child who had undergone orthopedic surgery for foot deformities three years prior presented to the outpatient clinic with a complaint of flexion restriction in both hands. The radiological evaluation of the patient showed C3-4 spondyloptosis. Halo traction was placed and posterior and anterior spinal fusions were performed respectively to achieve realignment of the spine. The postoperative period was uneventful and the patient has been followed for 24 months. The etiology, diagnostic features and treatment options of childhood cervical spondyloptosis have been briefly reviewe

In vertebral surgery we frequently observe patients who complain of lumbar pain at intervals of time which vary after surgery. The causes that may involve the persistence or the recurrence of these symptoms are many, after both decompression surgery, and spinal fusion. In order to favor a more rapid and accurate diagnosis, in our clinical we use a diagnostic algorithm based on the time at which the symptoms occurred. The most modern radiodiagnostic methods allow for a more accurate preoperative diagnosis and a more accurate diagnosis of the pathologies capable of perpetuating or re-accentuatin

Four patients are described with a triad of neuroectodermal abnormalities consisting of lumbosacral kyphosis, tethered cord, and dyplomyelia. Of utmost importance is the recognition of the association between this type of kyphosis and the underlying spinal cord pathology and the progressive nature of the deformity. The patient presenting with lumbosacral kyphos and the presence of sacral hypoplasia should have a neuroradiographic evaluation (magnetic resonance imaging scanning or enhanced computed tomography) to rule out the presence of a tethered cord or other neural abnormalities. Early surg

The aims of internal fixation of thoracic and lumbar spinal fractures are immediate stability, preservation of spine mobility, and restoration of the anatomical form of the vertebra and of the canal diameter at acceptable risks. All these aims together can be achieved only by short posterior pedicle fixation systems, such as the external spine fixator, the various internal spine fixators, special dorsal Roy-Camille plates, Wolter plates, and the combined anterior and posterior unisegmental fusion with the USIS system. Isolated anterior procedures necessitate additional bracing. The indications

Spine surgery exposes to neurological complications. There were 170 immediate complications of spine surgery at Pitiè-Salpétrière Hospital out of 2,855 reviewed (5.95%) during 9 years. The nature of the complication (radicular or medullar), severity and evolution were quite different but less than 2.76% where permanent. Among them, 1.43% were major neurological complications. The "high risk" etiologies were cervical stenosis and primitive malignant tumors. The major cause for these complications was due to surgical procedure in 60% of the cases.

STUDY DESIGN: Retrospective cohort study.
OBJECTIVE: To determine the impact of screw density on outcomes in adult spinal deformity surgery.
SUMMARY OF BACKGROUND DATA: Previous studies have suggested that decreased screw density in long lumbar constructs does not compromise mechanical robustness or increase the risk of complications such as PJK, pseudoarthrosis, or reoperation.
METHODS: We retrospectively identified adults who underwent spinal fusions with an upper instrumented vertebra (UIV) of L2 or above with a minimum 2-year radiographic follow up. We collected data on UIV, Hounsfield

PURPOSE: A subset of adult spinal deformity (ASD) patients undergoing corrective surgery receive a disproportionate level of medical resources and incur greater costs. We examined the characteristics of such super-utilizers of health care resources among ASD patients.
METHODS: This prospective, multicenter study analyzed data from ASD patients with > 4 levels of spinal fusion and a minimum 2-year follow-up. Index and total episode-of-care (EOC) costs in 2022 US dollars were calculated using average itemized direct costs obtained from administrative hospital records. Patients with total 2-year

STUDY DESIGN: This is a consecutive study of patients having undergone surgical treatment of adult lumbar scoliosis. Follow-up ranged from 2 to 13 years (average 5 years).
OBJECTIVES: To assess the complications and outcomes of patients with long fusions to L4 (n=23), L5 (n=21), or the sacrum (n=15) and determine if a "deeply seated" L5 segment is protective.
SUMMARY OF BACKGROUND DATA: Few studies assess outcomes and complications in adults fused from the thoracic spine to L4, L5, or the sacrum with minimum 2-year follow-up.
METHODS: Fifty-eight patients (59 cases; average age 43 years; ra

INTRODUCTION AND IMPORTANCE: Spondylolysis is a pars interarticularis defect often associated with instability and pain. While commonly involving the inferior level, it can rarely present with disc degeneration or herniation at the superior level. Such coexistence poses unique biomechanical and clinical challenges, particularly in younger patients who require solutions that preserve spinal motion and minimize future degeneration. This report highlights a novel combined surgical approach while preserving spinal motion, addressing both pathologies.
CASE PRESENTATION: A 27-year-old male presente
