Improving Outcomes in People with Spinal Cord Injury: Encouraging Results from a Multidisciplinary Advanced Rehabilitation Pathway.

Spinal cord injury (SCI) consists of damage to any segment of the spinal cord extending to potential harm to nerves in the cauda equina. Rehabilitative efforts for SCI can involve conventional physiotherapy, innovative technologies, as well as cognitive treatment and psychological support. The aim of this study is to evaluate the feasibility of a dedicated, multidisciplinary, and integrated intervention path for SCI, encompassing both conventional and technological interventions, while observing their impact on cognitive, motor, and behavioral outcomes and the overall quality of life for indiv

Isolated Brainstem Congestion Caused By Craniocervical Junction Arteriovenous Fistula.

Although arteriovenous fistulas (AVFs) at the craniocervical junction (CCJAVFs) are rare, they often develop into a subarachnoid haemorrhage when they have an ascending venous drainage, or cause venous congestion of the spinal cord with descending venous drainage. Isolated brainstem lesions due to CCJAVF are extremely rare, and, to our knowledge, the vascular architectural features that could cause such lesions are unknown. We present a case of CCJAVF manifesting as isolated brainstem congestion and review the literature on the vessel architecture of these rare lesions. A 64-year-old man was a

Use of Sham Interventions in Randomized Controlled Trials in Neurosurgery.

BACKGROUND: The use of sham interventions in randomized controlled trials (RCTs) is essential to minimize bias. However, their use in surgical RCTs is rare and subject to ethical concerns. To date, no studies have looked at the use of sham interventions in RCTs in neurosurgery. METHODS: This study evaluated the frequency, type, and indication of sham interventions in RCTs in neurosurgery. RCTs using sham interventions were also characterized in terms of design and risk of bias. RESULTS: From a total of 1,102 identified RCTs in neurosurgery, 82 (7.4%) used sham interventions. The most common

[Percutaneous stabilization of the thoracic and lumbar spine with active intraoperative reduction].

OBJECTIVE: Operative treatment of vertebral fractures is focused on stabilization of the spine and decompression of the spinal cord and nerves. In German-speaking countries, it is common to restore the sagittal profile by fracture reduction. The use of percutaneous systems has been limited due to inferior reduction possibilities compared to open systems. The improved reduction options offered by newer percutaneous systems extend their application to highly deformed fractures. INDICATIONS: Unstable thoracic, lumbar and lumbosacral spine fractures; unacceptable spinal deformities. CONTRAINDICA

MRI of the spinal cord in neuromyelitis optica and recurrent longitudinal extensive myelitis.

BACKGROUND AND PURPOSE: Neuromyelitis optica (NMO) is a severe inflammatory and necrotizing disease that clinically affects the optic nerves and spinal cord in a relapsing course. We assessed the baseline and follow-up MRI characteristics of cord attacks in NMO and recurrent longitudinal extensive myelitis (RLEM). METHODS: We retrospectively reviewed MRI data of 20 Afro-Caribbean patients diagnosed with either NMO or RLEM. MRI data from 51 cord or mixed attacks were evaluated, and 65 follow-up MRI studies were available for 30 baseline acute examinations. RESULTS: The cervical cord was invol

Characterization of mouse lumbar splanchnic and pelvic nerve urinary bladder mechanosensory afferents.

Sensory information from the urinary bladder is conveyed via lumbar splanchnic (LSN) and sacral pelvic (PN) nerves to the spinal cord. In the present report we compared the mechanosensitive properties of single afferent fibers in these two pathways using an in vitro mouse bladder preparation. Mechanosensitive primary afferents were recorded from the LSN or PN and distinguished based on their response to receptive field stimulation with different mechanical stimuli: probing (160 mg to 2 g), stretch (1-25 g), and stroking of the urothelium (10-1,000 mg). Four different classes of afferent were r

Neurally augmented sexual function in human females: a preliminary investigation.

Objective.  Neurally augmented sexual function (NASF) is the production of pleasurable genital stimulation and subsequent orgasm through the application of electrical energy to provide stimulation of the spinal cord or peripheral nerves. The purpose of this paper is to demonstrate the reproducibility of this phenomenon. Materials and Methods.  Eleven otherwise healthy women, ages 32-60 years, were selected for this study. Through standard techniques, quadripolar (octopolar in the final patient) leads were placed in the epidural space percutaneuously. The lead was maneuvered initially to an L1-

Neonatal spinal muscular atrophy with multiple contractures, bone fractures, respiratory insufficiency and 5q13 deletion.

We present the case of a floppy neonate with marked and generalized weakness, respiratory insufficiency and fetal akinesia deformation sequence. The infant showed multiple joint contractures, two bone fractures and needed mechanical ventilation from birth to death at 16 days of age. Electrophysiological assessment showed electrically unexcitable motor and sensory nerves. Muscle biopsy showed diffuse atrophy of type I and type II fibers. Necropsy confirmed the diagnosis of infantile spinal muscular atrophy (SMA) with severe loss of motor neurons in anterior horns and motor nuclei of brainstem.

Myoelectric evoked potentials versus locomotor recovery in chronic spinal cord injured rats.

The purpose of this study was to determine the utility of descending evoked potentials in evaluating functional recovery in rats after spinal cord contusion injury. Rats received thoracic contusions at T9 using a controlled-displacement impactor. They were evaluated for 5 weeks postinjury using auditory startle responses (ASR) while alert, or by cerebellar motor evoked potentials (CMEP) while anesthetized. ASR and CMEP were recorded electromyographically from forelimb and hindlimb muscles. Open field locomotor performance was also assessed and recovered to almost normal levels by 3 weeks posti

Spinal, trigeminal, and cortical climbing fibre paths to the lateral vermis of the cerebellar anterior lobe in the cat.

The spinal, trigeminal and cortical climbing fibre projection to the lateral vermis of the cerebellar anterior lobe in the cat has been investigated. The b zone, which occupies a one mm wide band in the lateral vermis, receives a somatotopically arranged input. Successively more rostral segments (tail, hindlimb, thorax, forelimb and trigeminal nerves) project to successively more medial microzones. The b zone receives an input through each of three spino-olivocerebellar paths (SOCPs) ascending in the ventral, dorsal and dorsolateral funiculi (VF, DF and DLF). In the b zone, the hindlimb input

Hypothalamic Paraventricular Stimulation Inhibits Nociceptive Wide Dynamic Range Trigeminocervical Complex Cells via Oxytocinergic Transmission.

Oxytocinergic transmission blocks nociception at the peripheral, spinal, and supraspinal levels through the oxytocin receptor (OTR). Indeed, a neuronal pathway from the hypothalamic paraventricular nucleus (PVN) to the spinal cord and trigeminal nucleus caudalis (Sp5c) has been described. Hence, although the trigeminocervical complex (TCC), an anatomical area spanning the Sp5c, C1, and C2 regions, plays a role in some pain disorders associated with craniofacial structures (e.g., migraine), the role of oxytocinergic transmission in modulating nociception at this level has been poorly explored.

Thoracolumbar Dorsal Ramus Nerve Block Using Continuous Multiorifice Infusion Catheters: A Novel Technique for Postoperative Analgesia After Scoliosis Surgery.

BACKGROUND: This is a brief technical report about a novel regional anesthesia technique in which local anesthetic was deposited around the thoracolumbar dorsal rami nerves via 4 multiorifice pain catheters to obtain analgesia for posterior spinal fusion surgery on scoliosis patients. Scoliosis is the most common deformity of the spine. Currently, most surgeons prefer a dual rod, segmental spinal fixation system that allows multiple anchor points for attachment to the deformed spine. Scoliosis surgery is an extremely painful surgical procedure due to the large incision, surgical trauma to supe
