[Pneumococcal disease prevention from early infancy to old age].

The real disease burden due to Streptococcus pneumoniae infections are underrecognised worldwide both by the lay public and by the medical community in general. In infants and children pneumococcal otitis media is the most common reason for antimicrobial treatment, while the far less frequent, though potentially life threatening pneumococcal pleuropneumonia, sepsis and pneumococcal meningitis are high risk conditions even if the causative pneumococcus is not multiresistant. Asplenic patients, patients with chronic underlying conditions, and/or those receiving immunosuppressive therapy are at r

Pneumococcal protein PavA is important for nasopharyngeal carriage and development of sepsis.

Summary The pneumococcal cell surface protein PavA is a virulence factor associated with adherence and invasion in vitro. In this study we show in vivo that PavA is necessary for Streptococcus pneumoniae D39 colonization of the murine upper respiratory tract in a long-term carriage model, with PavA-deficient pneumococci being quickly cleared from nasopharyngeal tissue. In a pneumonia model, pavA mutants were not cleared from the lungs of infected mice and persisted to cause chronic infection, whereas wild-type pneumococci caused systemic infection. Hence, under the experimental conditions, Pav

Prospective study to determine clinical relevance of detection of pneumococcal DNA in sera of children by PCR.

We undertook a prospective study to evaluate the accuracy of PCR of serum (aimed at the pneumococcal pneumolysin gene) at detecting pneumococcal infections in infants and children. The assay was positive for all blood and cerebrospinal fluid culture-positive samples and for 38 and 44% of patients with lobar pneumonia and acute otitis media, respectively. It was positive for 17% of healthy controls. There was a marked effect of age on the rate of positivity among healthy controls, with the highest rate (33%) being in 2-year-old children, the age group with the highest rate of nasopharyngeal (NP

Comparative role of complement in pneumococcal and staphylococcal pneumonia.

To evaluate the role of complement in pneumococcal and staphylococcal pneumonia, we decomplemented rats with cobra venom factor and inoculated them intratracheally with Staphylococcus aureus or type 25 pneumococci. S. aureus produced a patchy bronchopneumonia in normal Sprague-Dawley or Lewis rats, and decomplementation did not increase the severity of staphylococcal infection in either rat strain as judged by quantitative cultures of the lungs and blood at 6, 24, and 48 h after inoculation. In contrast, decomplementation markedly increased the severity of pneumonia caused by type 25 pneumococ

Clinical Efficacy Protocol of Yinhuapinggan Granules: A Randomized, Double-Blind, Parallel, and Controlled Clinical Trial Program for the Intervention of Community-Acquired Drug-Resistant Bacterial Pneumonia as a Complementary Therapy.

Background: Community-acquired bacterial pneumonia (CABP) is an important health care concern in the worldwide, and is associated with significant morbidity, mortality, and health care expenditure. Streptococcus pneumoniae is the most frequent causative pathogen of CABP. Common treatment for hospitalized patients with CABP is empiric antibiotic therapy using β-lactams in combination with macrolides, respiratory fluoroquinolones, or tetracyclines. However, overuse of antibiotics has led to an increased incidence of drug-resistant S. pneumoniae, exacerbating the development of community-acquired

Comparison between broth microdilution and agar disk diffusion methods for antimicrobial susceptibility testing of bovine mastitis pathogens.

In order to counter the antibiotic resistance phenomenon, a prudent and rational use of antimicrobials should be driven by an accurate clinical diagnosis and, when possible, by the isolation of the etiological agent followed by susceptibility testing, with the aim to select the most suitable molecule for therapy. Cow mastitis is considered the main cause of antibiotic use in the cattle breeding sector. The purpose of this study was to compare the broth microdilution (BMD) method performed with Sensititre Custom Plates and the agar disk diffusion (ADD) method in determining antimicrobial suscep

Necrotizing pneumonia in children: a comprehensive narrative review of current challenges and future perspectives.

Necrotizing pneumonia (NP) represents an uncommon but severe complication of community-acquired pneumonia (CAP) in children, characterized by the destruction of lung parenchyma and formation of multiple thin-walled cavities. Despite its rarity, the incidence of pediatric NP has increased substantially over the past two decades, creating major diagnostic and treatment challenges. The COVID-19 pandemic has further altered the epidemiological landscape, with unique patterns of respiratory pathogen circulation. This narrative review synthesizes current evidence on pediatric necrotizing pneumonia t

Characterization of fluoroquinolone-resistant beta-hemolytic Streptococcus spp. isolated in North America and Europe including the first report of fluoroquinolone-resistant Streptococcus dysgalactiae subspecies equisimilis: report from the SENTRY Antimicrobial Surveillance Program (1997-2004).

Beta-hemolytic streptococci are common bacterial pathogens that can cause serious invasive disease, and although this group of species has remained susceptible to the fluoroquinolone class, resistant strains have been reported. This multicenter investigation determined the rate of fluoroquinolone-resistant beta-hemolytic streptococci using the SENTRY Antimicrobial Surveillance Program network data (1997-2004). Forty-seven surveillance culture isolates of beta-hemolytic streptococci from North America and Europe with elevated levofloxacin MIC results (2 to >32) microg/mL were tested for suscept

A Nonadjuvanted Whole-Inactivated Pneumococcal Vaccine Induces Multiserotype Opsonophagocytic Responses Mediated by Noncapsule-Specific Antibodies.

Streptococcus pneumoniae (Spn) remains a major cause of global mortality, with extensive antigenic diversity between capsular serotypes that poses an ongoing challenge for vaccine development. Widespread use of pneumococcal conjugate vaccines (PCVs) targeting Spn capsules has greatly reduced infections by vaccine-included serotypes but has led to increased infections by nonincluded serotypes. To date, high cost of PCVs has also limited their usefulness in low-income regions where disease burdens are highest. To overcome these limitations, serotype-independent vaccines are being actively resear

Clinical Comparison of Cefuroxime Axetil with Cefixime in the Treatment of Acute Bronchitis.

Acute bronchitis is the ninth most common outpatient illness seen by physicians in the United States. Oral antibiotic treatment is usually directed empirically against the most common bacterial pathogens associated with acute bronchitis, such as Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis. Although cefuroxime axetil and cefixime are both approved in the United States for treatment of acute bronchitis, currently they have not undergone direct clinical comparison for this indication. This randomized, investigator-blind, multicenter study was designed to compare th

Susceptibility of respiratory tract pathogens in Finland to cefixime and nine other antimicrobial agents.

The in vitro activity of cefixime and 9 other orally used antimicrobial agents against 545 selected clinical respiratory tract pathogens was tested using the plate-dilution method. Of the strains tested, 144 were Haemophilus influenzae strains, 151 group A streptococci, 105 pneumococci and 145 Moraxella catarrhalis isolates. Ciprofloxacin was the most active drug, with a minimal inhibitory concentration (MIC) of < 0.06 mg/l for more than 87% of the isolates. With cefixime, a MIC of 0.06 mg/l inhibited 55% of the strains tested and a MIC of 1.0 mg/l inhibited all except one of the 545 strains.

Type III group B streptococcal strain differences in susceptibility to opsonization with human serum.

Human serum opsonins to type III Group B streptococci (GBS) were studied in an in vitro opsonophagocytic assay. Two type III GBS test strains were susceptible (893 and IIINor) and two resistant (891 and 892) to opsonization by the majority of sera from 15 healthy adults. Four individuals with undetectable or low opsonic titers to the test strains were immunized with pneumococcal vaccine; immunization with pneumococcal vaccine induced a titer rise in all but one instance when susceptible GBS strains were tested. In contrast, only a single titer rise was detected when resistant GBS strains were

Co-carriage of respiratory tract bacterial pathogens among under-5 children with pneumococcal carriage in Peninsular Malaysia.

BACKGROUND: Streptococcus pneumoniae (SPN) is the commonest cause of bacterial childhood pneumonia, and adverse clinical outcomes have been associated with co-infections with other bacteria causing severe respiratory disease. We therefore conducted a study to look for evidence of other respiratory bacterial pathogens and to determine the prevalence and risk factors of co-carriage status among under-5 children in Peninsular Malaysia. METHODS: This retrospective case-control study is part of a paediatric pneumonia surveillance study “MY-Pneumo” (NCT04923035. Registered on June 11, 2021) in young

Safety and immunogenicity of 13-Valent pneumococcal conjugate vaccine in healthy infants in Shanghai during the COVID-19 pandemic: a prospective cohort study.

BACKGROUND: Streptococcus pneumoniae (S. pneumoniae) is a major cause of morbidity and mortality in infants, leading to diseases like pneumonia and otitis media. Although the World Health Organization (WHO) recommends inclusion of pneumococcal conjugate vaccines (PCVs) in national immunization programs, 13-Valent PCV (PCV13) remains optional in China, with suboptimal coverage, especially in urban settings like Shanghai. This study was performed to evaluate the safety, immunogenicity and impact on nasopharyngeal carriage of S. pneumoniae of PCV13 in healthy infants in Shanghai. MATERIAL AND ME
