A woman with infectious endocarditis caused by Abiotrophia defectiva.

A 74-year-old woman with recurrent fever and multiple joint pain was admitted to Hokkaido University Hospital. Trans-esophageal echocardiogram revealed bacterial vegetation and destruction of the aortic valve. Although few bacteria grew in regular blood agar, Gram-positive coccobacillus was specifically grown in chocolate blood agar and Brucella agar, and it was identified to be Abiotrophia defectiva. Infectious endocarditis caused by Abiotrophia defectiva was diagnosed, she was treated with diuretics, penicillin G and gentamicin, and she immediately improved. Infectious diseases caused by Abi

HACEK endocarditis in infants and children: two cases and a literature review.

We report 2 cases of Haemophilus parainfluenzae endocarditis and review 34 cases of HACEK endocarditis from the literature. HACEK organisms are the most common cause of Gram-negative endocarditis in children. They have a propensity to form friable vegetations (especially H. parainfluenzae) that break off and cause symptomatic emboli. HACEK endocarditis (from a review of the 36 published cases) may involve previously normal hearts (33%), may be complicated by embolization (31%) and may require vegetectomy or other surgery (31%). Mortality with HACEK endocarditis was 14%. HACEK organisms may be

[Treatment of vocal fold nodules in singers].

Vocal nodules (singer's nodules) are a typical professional disease of the vocal apparatus. This disease occurs more frequently in 20- to 50-year-old singers. The nodules form because of vascular disorders secondary to overstrain of the vocal apparatus in phonation and disorders of vegetative innervation. 43 opera singers were treated whose age and singing history ranged from 22 to 57 years and 3 to 26 years, respectively. The treatment combined psychotherapy, medicines with phlogenzym as a basic drug, physiotherapy and phonopedia. Phlogenzym allows to avoid "silence regimen", surgical interve

Pyostomatitis vegetans in childhood.

UNLABELLED: Pyostomatitis vegetans is an oral eruption, characterized by small pustules, ulcers and erythematous vegetations of the labial and buccal mucosae as well as labial-attached gingivae. Its importance lies in its high correlation with inflammatory bowel disease. It is commonly associated with skin and inflammatory bowel disease and is rare in children. We here report a sister and brother with onset of the disease at the age of 5 and 7 years, respectively. It is the first report of familial pyostomatitis vegetans occurring in the youngest patients hitherto reported. CONCLUSION: The ob

[The neurodystrophic component of the structural changes in the hemispheric cerebellar cortex of patients with acoustic neurinomas].

Cerebellar specimens removed in surgical treatment of acoustic nerve neurinomas were studied morphologically in 16 patients. Histochemically, morphofunctional state of the vegetative adrenergic nervous fibers and endings of the vessels and connective tissue basis of the pia mater encephali in the region of the cerebellum indicates inhibited activity of the neuromediator contour of circulation and metabolism regulation in the brain. Light and electron microscopy in the cerebellar cortex detected disorders of circulation, nervous cell dystrophy, glyocytic proliferation. Correlation was found bet

Candida tropicalis multifocal endophthalmitis as the only initial manifestation of pacemaker endocarditis.

PURPOSE: To document a case of Candida tropicalis endophthalmitis as the only manifestation of pacemaker endocarditis. METHODS: We examined a 75-year-old man with diabetes mellitus who was initially examined for bilateral multifocal endophthalmitis complicating endocarditis 2 years after a permanent pacemaker for sick sinus syndrome was implanted. RESULTS: Transesophageal echocardiography showed a large vegetation with a 3-cm diameter attached to the pacing electrode in the right ventricle. Six consecutive blood cultures grew C tropicalis. CONCLUSIONS: Ocular involvement, including multifoc

Transport and fate of trifluoroacetate in upland forest and wetland ecosystems.

Although trifluoroacetate (TFA), a breakdown product of chlorofluorocarbon replacements, is being dispersed widely within the biosphere, its ecological fate is largely unknown. TFA was added experimentally to an upland, northern hardwood forest and to a small forest wetland ecosystem within the Hubbard Brook Experimental Forest in New Hampshire. Inputs of TFA were not transported conservatively through these ecosystems; instead, significant amounts of TFA were retained within the vegetation and soil compartments. More TFA was retained by the wetland ecosystem than by the upland forest ecosyste

[A case of infective aortic valve endocarditis with splenic abscess].

A 47-year-old woman was admitted to our hospital with high fever, exertional dyspnea and left flank pain. Echocardiography revealed vegetations attached to the aortic valve and moderate aortic regurgitation. Streptococcus anginosus was identified by blood culture examination. Abdominal CT scan showed a low density area in the spleen. The diagnosis of infective aortic valve endocarditis with splenic abscess was obtained. The splenectomy and the aortic valve replacement were performed simultaneously after the treatment with antibiotics. The postoperative clinical course was uneventful. In the ca

[Verrucous abacterial endocarditis and polymyositis. A possible association?].

We describe a case of a verrucous non-bacterial endocarditis on the native mitral valve in a patient with polymyositis. This case, to our knowledge, represents the first report in the literature we could get. The case reached our attention after an episode of acute limb ischemia which lead to an echocardiographic examination that showed vegetations on the valve. No cardiac signs or symptoms were evident. We discuss the possible relation between the two disorders. The possibility of autoimmune diseases, other than lupus or lupus-related disorders, to produce this kind of lesions should be confi

[Infectious endocarditis caused by Q fever].

A case of aortic valve endocarditis caused by Coxiella burnetii and operated on with success is reported. The patient is doing well at 18 months follow up. Diagnosis of Q-fever endocarditis was made by high antibodies against phase I Coxiella burnetii antigens titration and by demonstration of aortic valvular vegetations by bidimensional echocardiography. Our patient suffered emergency aortic valve substitution due to acute hemodynamic failure and started a long-term treatment with doxycycline and rifampicin. Some interesting aspects about the diagnosis and treatment of this patient are review

[The clinical picture, diagnosis and therapy of headaches in patients with a hypothalamic syndrome].

190 patients with hypothalamic insufficiency were investigated. The character, frequency of various forms of headache as well as its correlation with neuroendocrine emotional motivation and vegetative disorders were analysed. It was found that headache occurred more frequently in patients with hypothalamic insufficiency (66%) than in the whole population (5-20%). This confirms the role of diencephalic disfunction in its genesis. Psychovegetative disturbances were expressed most of all in tension-type headache (TTH). Very likely that some psychoendocrine mechanisms (particularly depression and

Etiology and management of chronic valve disease in antiphospholipid antibody syndrome and systemic lupus erythematosus.

Unlike the Libman-Sacks vegetations of acute systemic lupus erythematosus (SLE), which are usually asymptomatic, valve involvement in chronic SLE and primary antiphospholipid antibody syndrome (APLAS) is similar to that of chronic rheumatic disease. Typical findings include valve thickening and nodularity, poor coaptation, and regurgitation. Elevated levels of antiphospholipid antibodies have been associated with the development of these valvular abnormalities in some but not all reported cases, and there are undoubtedly other etiologic cofactors. When cardiac valvular replacement is required,

Out of 162 workers exposed to chemicals in the manufacture of construction plastics an outpatient and inpatient examination revealed gastrointestinal problems in 84.3%, vegetative nervous disorders in 62.6% of the examinees. In a random sample of 47 workers high acid production occurred in 63.3% and erosions, ulcers, cicatrices in the stomach and duodenum in 45.2% of them. No HLA genetic predisposition to duodenal ulcer was reported. Histological findings on gastroduodenal mucosa biopsies were indicative of the dystrophy in the chief and lining cells of the gastric glands, epithelium and strom

Occurrence and prevalence of bot flies, Metacuterebra apicalis (Diptera: Cuterebridae), in rodents of cerrado from central Brazil.

The occurrence and prevalence of Metacuterebra apicalis (Diptera: Cuterebridae) in natural populations of Oryzomys subflavus, Bolomys lasiurus, and Thalpomys cerradensis (Rodentia: Muridae) from August 1990 to July 1992 in the cerrado, a common savanna-like vegetation type of central Brazil, are reported. An increase in bot infection in the 3 rodent species between October 1991 and July 1992 without correlation to precipitation was detected. The prevalence was lower than in neotropical forest formations. Mean intensity was 1.3 bots (range 1-2) for T. cerradensis and 1 bot for B. lasiurus and O

[Polymicrobial infections in recurrent tonsillitis and adenoiditis in children].

Bacteriological examination focused on the aerobic and anaerobic flora from ectomized tonsils in 17 patients with recurrent tonsillitis, in 8 patients with hypertrophy of the tonsils and 122 children with an adenoid vegetation confirmed microbial aetiology. In ectomized tonsils and adenoids aerobic gram-positive bacteria comprised most frequently staphylococci, and gram-negative bacteria H. influenzae. As to anaerobic gram-positive bacteria, they isolated most frequently peptostreptococci, and as to gram-negative ones B. fragilis from tonsils and Bacteroides sp. from adenoids. In the treatment

[Epilepsies with complex focal seizures in childhood. Seizure semiology and routine EEG, localization, neuropathology and etiology, follow-up forms and prognosis with anticonvulsant therapy].

Focal epilepsies of childhood with complex partial seizures, with or without secondarily generalized seizures are reviewed. Particular emphasis is placed on the semiology of seizures, which are subdivided into: aura, disturbance of consciousness, speech disturbances, automatims, and other motor and vegetative seizure symptoms. The significance of routine ictal and interictal EGGs and their importance for the localization is discussed. Recent neuropathological findings and etiology are presented and the courses epilepsies can take (especially those involving febrile convulsions), prognosis and

[Olfactory and gustatory sweating--case report on reflex facial hyperhidrosis].

Localised hyperhidrosis is comparatively rare among the vegetative disorders. A case of pathological gustatory and olfactory sudation is reported, with the sequence of symptoms corresponding to that of the Frey syndrome. Working from the reflex nature of the phenomen, the paper puts forward a model to account for the delayed onset of the manifestation following a peripheral lesion. It is assumed that readjustment of the reflex excitation level takes place according to a modification in peripheral afference, and that a decompensation of the central control mechanisms results in a pathological e

Unreactive EEG: pattern in locked-in syndrome.

Locked-in syndrome (LIS) is a state of complete paralysis except for ocular movements in a conscious individual, normally resulting from brainstem lesions. We report three patients with acute brainstem strokes and LIS whose repeated EEG recordings exhibited an "alpha coma" pattern including an unreactive alpha rhythm to multimodal stimuli. This finding is of importance since preservation of consciousness is often difficult to ascertain in LIS and presence of unreactive alpha on unresponsive patients is normally equated with vegetative states and discards LIS. Failure to diagnose LIS results in

A study of 71 patients with atrioventricular blocks and 57 patients with the sick-sinus syndrome (SSS) revealed electrophysiologic evidence of sinus dysfunction in 38 (53.5%) patients with atrioventricular blocks. Sinus dysfunction was due to pathologic vegetative effects in 15 (21.1%) patients with AV blocks, and a pathology of the sinus node itself in 23 (32.4%). Impaired atrioventricular conductivity, was registered in 29 (38.7%) patients with the SSS. A disorder of atrioventricular automatism was detected in 38 patients, and was combined with impaired AV conductivity in 19 of those. Five c

Infective endocarditis in neonates.

Five patients with neonatal infective endocarditis were reviewed, two of whom survived. Infection was caused by Staphylococcus aureus in four and by Candida albicans in one. All cases of bacterial endocarditis had clinical signs of septicaemia, positive blood cultures, thrombocytopenia, microscopic haematuria, and heart murmurs. Three developed skin abscesses early in their illnesses. Three patients had two dimensional echocardiographic studies that showed bacterial vegetations. One of these studies was done before the heart murmur could be heard. We suggest that echocardiography in conjunctio
