Reassessment of blood donor selection criteria for United States travelers to malarious areas.

In the United States (US), travelers who have had malaria or who have taken antimalarial chemoprophylaxis are deferred as blood donors for 3 years to prevent transfusion-transmitted malaria. To assess the impact of shortening this 3-year exclusion period, national malaria surveillance data from 1972 to 1988 were reviewed. The average annual rate of transfusion-transmitted malaria is 0.25 cases per million units of blood collected. Of 45 reported cases, 38 percent were caused by Plasmodium malariae, 29 percent by P. falciparum, 24 percent by P. vivax, and 9 percent by P. ovale. Thirty-two donor

High affinity dextromethorphan binding sites in guinea pig brain. Effect of sigma ligands and other agents.

Dextromethorphan (DM), a non-narcotic antitussive, binds in the guinea pig brain to specific high- and low-affinity sites with Kd values of 57 nM and 24 microM, respectively (Musacchio et al., 1988). The antitussives carbetapentane, caramiphen, butamirate and dimethoxanate competed with the high-affinity binding of [3H]DM at pH 7.4 with nanomolar Ki values. Sigma site ligands showed high affinity for [3H]DM binding sites. The rank order of potency was: haloperidol greater than (+)-pentazocine greater than (+)-cyclazocine greater than 3-(-3-hydroxyphenyl)-N-(1-propyl)piperidine greater than (+)

Effectiveness and safety of 7-day high-dose primaquine and single-dose tafenoquine versus 14-day low-dose primaquine in patients with Plasmodium vivax malaria (EFFORT): a multicentre, open-label, randomised, controlled, superiority trial.

BACKGROUND: Shorter courses of primaquine and single-dose tafenoquine have potential to improve the prevention of recurrent Plasmodium vivax infections, but there are few data on their comparative effectiveness when provided unsupervised. We aimed to assess the effectiveness and safety of these new treatment options. METHODS: We conducted a multicentre, open-label, randomised, controlled, superiority trial in Ethiopia, Pakistan, Indonesia, and Cambodia. Adult patients (aged ≥18 years, or aged ≥16 years in Indonesia) with uncomplicated P vivax infection and glucose-6-phosphate dehydrogenase (G

Synthesis and Standardization of Outcomes in Severe Malaria Treatment Trials: Protocol for the Development of a Core Outcome Set (the COSSMaT Study).

BACKGROUND: Severe malaria remains a major global health challenge, particularly in endemic regions, where it causes substantial morbidity and mortality, especially among children and pregnant women. Although numerous clinical trials have evaluated treatments for severe malaria, heterogeneity in outcome selection, definition, and measurement limits comparability across studies, hampers evidence synthesis, and contributes to research waste. There is currently no established core outcome set (COS) for trials specifically focused on severe malaria treatment. Developing a COS is therefore essentia

Protocol for a multicentric open-label randomized controlled trial comparing low-dose (0.25 mg/kg) and standard-dose (0.75 mg/kg) primaquine for gametocytocidal efficacy and safety in Plasmodium falciparum malaria.

INTRODUCTION: World Health Organization (WHO) in 2010, recommended 0.75 mg/kg single dose primaquine for killing gametocytes after ruling out glucose-6-phosphate dehydrogenase (G6PD) deficiency. However, considering the practical feasibility of G6PD testing, WHO revised the recommendation in 2012 to 0.25 mg/kg single dose primaquine on day-1. This study aims to investigate the efficacy and safety of single-low dose primaquine (0.25 mg/kg) administered on day-1 to intervention arm compared to standard regimen, high-dose primaquine (0.75 mg/kg) administered on day-2 to control arm. Assessment of long-term mental health effects of mefloquine use during pregnancy: a secondary analysis of a randomized clinical trial in Benin.

BACKGROUND: Mefloquine (MQ) was evaluated as intermittent preventive treatment in pregnancy (IPTp) in a randomized clinical trial, as part of the ongoing search for alternatives to sulfadoxine-pyrimethamine (SP). Although MQ was not recommended for IPTp due to poor tolerability, no psychiatric differences were found between trial arms. Nevertheless, widespread concerns about MQ's neuropsychiatric effects persist and remain clinically relevant - particularly for pregnant travelers to malaria-endemic areas for whom MQ prophylaxis is still indicated. Long-term data on MQ's effects on maternal men

Dissemination of trial results to study participants in Cambodia, Ethiopia and Pakistan: a qualitative study.

OBJECTIVES: Dissemination of study results to research participants is an ethical imperative, yet it is rarely implemented in resource-limited settings partly due to a lack of clear guidance. The purpose of this study was to evaluate and explore dissemination activities that were conducted as part of a multi-site clinical trial of Plasmodium vivax treatment options, informing future dissemination strategies in similar resource-limited settings. DESIGN: Qualitative study using focus group discussions (FGDs) and an inductive-deductive thematic analysis approach. SETTING: Clinical trial study s

Pregnant women as a sentinel population for genomic surveillance of malaria in the Democratic Republic of the Congo: a population-based study.

BACKGROUND: Genomic surveillance is a valuable tool for detecting changes in the drug susceptibility of malaria parasites, enabling timely adjustments to treatment strategies. However, implementation can be costly and challenging in high-burden countries, especially when targeting cohorts of children. To address these challenges, we investigated whether in the Democratic Republic of the Congo pregnant women attending antenatal care services could act as an effective sentinel population for children in the same area. METHODS: This population-based study aimed to target pregnant women in Kinsha

Plasmodium falciparum Kelch-13 artemisinin partial resistance markers in Fort Portal, Western Uganda, 2024.

In Fort Portal, Uganda, artemisinin resistance-associated mutations in Plasmodium falciparum Kelch13 (n = 126) were present in 4.8% (675V, 561H, and 441L). A mutation of unknown relevance, 490T, occurred in 9.5%. PfMDR1 variants suggested increased lumefantrine tolerance (N86, 100%). Mutation 500N was absent, and 199S occurred in 12.8%. The latter is of unknown relevance. These data indicate an incipient emergence of artemisinin resistance in a crucial location between Rwandan and Ugandan resistance hotspots and hardly affected DR Congo.

Safety and efficacy of single-dose primaquine to interrupt Plasmodium falciparum malaria transmission in children compared with adults: a systematic review and individual patient data meta-analysis.

BACKGROUND: Adding a single dose of primaquine to artemisinin-based combination therapy (ACT) for the treatment of falciparum malaria can reduce the transmission of Plasmodium falciparum and could limit the spread of artemisinin partial resistance, including in Africa, where the disease burden is greatest. We aimed to compare the safety and efficacy of single-dose primaquine plus ACT between young children (aged <5 years) and older children (aged 5 years to <15 years) and adults (aged ≥15 years), and between low and moderate-to-high transmission areas. METHODS: For this systematic review and

Congenital infection with Plasmodium malariae: a rare case of intrauterine transmission in Germany.

BACKGROUND: Malaria remains the leading parasitic disease worldwide with a significant global morbidity and mortality burden. Plasmodium malariae, the least prevalent of the five Plasmodium species that cause human malaria, has unique characteristics including prolonged prepatent periods and life-long persistance. In non-endemic countries and particular in neonates with coexisting diseases diagnosis and therapy pose challenges. CASE PRESENTATION: We report a rare case of severe congenital P. malariae malaria in a 2-month-old female infant born in Germany to a Nigerian mother. The infant prese

Within-host modeling of primaquine-induced hemolysis in hemizygote glucose-6-phosphate dehydrogenase deficient healthy volunteers.

Primaquine is the only widely available drug to prevent relapses of Plasmodium vivax malaria. Primaquine is underused because of concerns over oxidant hemolysis in glucose-6-phosphate dehydrogenase (G6PD) deficiency. A pharmacometric trial showed that ascending-dose radical cure primaquine regimens causing 'slow burn' hemolysis were safe in G6PD-deficient Thai and Burmese male volunteers. We developed and calibrated a within-host model of primaquine hemolysis in G6PD deficiency, using detailed serial hemoglobin and reticulocyte count data from 23 hemizygote deficient volunteers given ascending

Platelet-Derived Transforming Growth Factor-β1: A New Hope for Cerebral Malaria Treatment.

Cerebral malaria (CM), a fatal neurological complication arising from Plasmodium falciparum (P. falciparum) infection, remains a significant global health challenge due to the inadequacy of current drugs and vaccines. Consequently, novel therapeutic strategies for CM are urgently needed. Recent research identifies platelets as pivotal in CM pathogenesis, significantly contributing to immunopathological damage and vascular blockage. Platelet-derived transforming growth factor (TGF)-β1 induces apoptosis in endothelial cells, fostering microangiopathy and potentially compromising blood-brain barr

Decoding the multifaceted role of erythrocyte PMCA4b in oxidative stress-mediated malaria protection and artemisinin resistance.

The ATP2B4 gene, encoding the PMCA4b Ca²+-ATPase in erythrocytes, has been linked to malaria protection via genome-wide association studies, though the proposed dehydration mechanism remains unclear. This study evaluates ATP2B4 genotypes and PMCA4b expression in malaria susceptibility and artemisinin sensitivity. ATP2B4 genotypes were compared across severe malaria, uncomplicated malaria, and healthy controls in Indian population. PMCA4b expression, intra-erythrocytic calcium, oxidative stress markers, Gardos channel activity, and Plasmodium falciparum growth dynamics were analyzed. Artemisini

Prevalence of Malaria Infection in Pregnant Women Attending Antenatal Clinics in Southern Senegal.

Despite marked progress in Senegal, three regions in the southeast part continue to have a high burden of malaria, but there have been no recent studies assessing the prevalence of malaria associated with pregnancy. This study aimed to determine the prevalence of malaria infection in pregnant women attending antenatal clinics in Senegal. During the malaria transmission season of 2019, pregnant women attending 11 health care facilities for a scheduled visit and those presenting unwell with signs of malaria were invited to participate in a malaria screening study. A finger prick blood sample was

Delayed gametocyte clearance in Plasmodium vivax malaria is associated with polymorphisms in the cytochrome P450 reductase (CPR).

Primaquine (PQ) is the main drug used to eliminate dormant liver stages and prevent relapses in Plasmodium vivax malaria. It also has an effect on the gametocytes of Plasmodium falciparum; however, it is unclear to what extent PQ affects P. vivax gametocytes. PQ metabolism involves multiple enzymes, including the highly polymorphic CYP2D6 and the cytochrome P450 reductase (CPR). Since genetic variability can impact drug metabolism, we conducted an evaluation of the effect of CYP2D6 and CPR variants on PQ gametocytocidal activity in 100 subjects with P. vivax malaria. To determine gametocyte de

Assessing the Oxidative Stress Reducing Potential of Spilanthes filicaulis (Schumach & Thonn) Ethyl-Acetate Sub-fractions on Plasmodium berghei Infected Female Mice.

PURPOSE: Infections by Plasmodium parasite actuate oxidative stress. As malaria parasite actions overwhelm host antioxidant defense by producing excessive reactive species during haemoglobin degradation. This study aimed to evaluate the oxidative status by considering the antioxidant level of ethyl-acetate sub-fractions of Spilanthes filicaulis (ESSF) extract on Plasmodium berghei NK-65 parasitized mice. METHODS: Sixty female mice weighing 20.0 ± 3.0 g were inoculated intraperitoneally with 0.2 mL of parasitized erythrocytes randomly selected into five groups of 12 mice, Groups I and II were
