Structure-function relationships in the rat brainstem subnucleus interpolaris: VI. Cervical convergence in cells deafferented at birth and a potential primary afferent substrate.

Possible substrates for peripheral injury-induced receptive field (RF) changes were assessed in the trigeminal (V) subnucleus interpolaris (SpVi). In adult rats with infraorbital nerve section at birth, 449 cells were studied ipsilateral to the lesion by using electrophysiological methods. Of these, 33 (7.4%) had RFs that included facial vibrissae, guard hairs, and skin, as well as ipsilateral regions normally innervated by cervical primary afferents (ear, neck, shoulder, arm, forepaw). Such non-V convergence was never seen in 373 normal SpVi cells or in 641 V ganglion cells ipsilateral to the

Non-invasive circulatory evaluation and electro-acupuncture & TES treatment of diseases difficult to treat in Western medicine.

Even in the presence of normal blood pressure (B.P.) in both arms in some individuals, abnormal B.P. and circulatory disturbances can be found in the brain and lower extremities. The author discovered the following five types of abnormal B.P. in the brain in the presence or absence of normal B.P. in the arms: unilateral cephalic hypertension; bilateral cephalic hypertension; unilateral cephalic hypotension; bilateral cephalic hypotension; mixed cephalic hypertension and hypotension. When the B.P. of the head exceeds about 160 mm Hg, patients experience sensation of increased pressure buildup i

Beyond Psychophysics: A Physiological Marker of Proprioceptive Function After Stroke.

Proprioceptive input is essential for motor control, yet clinical assessment of proprioceptive deficits relies largely on psychophysical tests that depend on self-report, are vulnerable to cognitive interference and bias, and probe conscious aspects of sensation. In this proof-of-concept study, we propose the automatic muscular response to passive limb displacement-the placing response-as a complementary, more objective marker of proprioceptive adequacy. Using surface electromyography (EMG) from the anterior and posterior deltoid during shoulder movements, we first confirm that placing is a ge

From Minor Knee Laceration to Multisystem Catastrophe: A Five-Year-Old Boy's Battle With Protein C Deficiency and Purpura Fulminans.

Purpura fulminans (PF) is a rapidly progressive thrombotic emergency. Trauma-induced acquired protein C deficiency is exceptionally rare in pediatric patients. This case adds to the limited literature describing PF triggered by minor traumatic injury, complicated by delayed recognition and resource limitations. A previously healthy five-year-old boy developed rapidly expanding purpura and skin necrosis after a knee laceration from flying debris in Gaza. Lesions progressed to involve the feet, groin, thighs, back, and upper arm, ultimately resulting in 14.5% total body surface area (TBSA) necro

The anatomy of concealed awareness: lessons from partial locked-in syndrome.

Partial locked-in syndrome (PLIS) is a rare stroke presentation in which preserved consciousness is masked by severe motor impairment, potentially leading to misinterpretation as encephalopathy. We report a patient in her early seventies with vascular risk factors and a prior pontine infarct who presented with acute quadriplegia and mutism. Initial clinical impressions raised concern for altered mental status. However, subsequent neurological examination demonstrated preserved awareness, with consistent eye tracking and command following through head movements. Examination showed bifacial and

Intrauterine high-dose intravenous immunoglobulin therapy during pregnancy for women with a history of pregnancy ending in documented neonatal haemochromatosis (NH001): study protocol.

INTRODUCTION: Neonatal haemochromatosis, considered to be a gestational alloimmune liver disease (NH-GALD), is a rare but serious disease that results in fulminant hepatic failure. The recurrence rate of NH-GALD in a subsequent infant of a mother with an affected infant is 70%-90%. Recently, antenatal maternal high-dose intravenous immunoglobulin (IVIG) therapy has been reported as being effective for preventing recurrence of NH-GALD in a subsequent infant. However, no clinical trial has been conducted to date. METHODS AND ANALYSIS: This is a multicentre open-label, single-arm study of antena

Assembly of Genetically Engineered Ionizable Protein Nanocage-based Nanozymes for Intracellular Superoxide Scavenging.

Nanozymes play a pivotal role in mitigating excessive oxidative stress, however, determining their specific enzyme-mimicking activities for intracellular free radical scavenging is challenging due to endo-lysosomal entrapment. In this study, we employ a genetic engineering strategy to generate ionizable ferritin nanocages (iFTn), enabling their escape from endo-lysosomes and entry into the cytoplasm. Specifically, ionizable repeated Histidine-Histidine-Glutamic acid (9H2E) sequences are genetically incorporated into the outer surface of human heavy chain FTn, followed by the assembly of variou

Left robotic assisted thoracic surgery (RATS) stellate ganglion ganglioneuroma resection with post-operative Harlequin syndrome.

Ganglioneuromas are rare, benign neurogenic tumours, most often located in the posterior mediastinum. We present an exceptional case of a stellate ganglion ganglioneuroma at the left cervicothoracic junction, abutting the subclavian artery, oesophagus, sympathetic chain and vertebral bodies. The patient presented with significant shortness of breath and paraesthesia in the left arm. Cross-sectional imaging confirmed a well-encapsulated paravertebral mass in the left posterior mediastinum. Given the intricate anatomy of the thoracic inlet and proximity to multiple critical structures, a robotic

Clickable Granular Hydrogel Scaffolds for Delivery of Neural Progenitor Cells to Sites of Spinal Cord Injury.

Spinal cord injury (SCI) is a serious condition with limited treatment options. Neural progenitor cell (NPC) transplantation is a promising treatment option, and the identification of novel biomaterial scaffolds that support NPC engraftment and therapeutic activity is a top research priority. The objective of this study is to evaluate in situ assembled poly (ethylene glycol) (PEG)-based granular hydrogels for NPC delivery in a murine model of SCI. Microgel precursors are synthesized by using thiol-norbornene click chemistry to react four-armed PEG-amide-norbornene with enzymatically degradable

A rare case of spinal involvement in hereditary hemorrhagic telangiectasia.

INTRODUCTION: Here, we describe a rare case of a spinal arteriovenous fistula in a patient with known hereditary hemorrhagic telangiectasia (HHT) and spontaneous intraspinal hemorrhage. Furthermore, we provide a brief review of the literature on the formation of spinal arteriovenous malformations (AVM) in relation to this disease. CASE PRESENTATION: The case involves a 54-year-old male with known HHT. At the age of 49, the patient experienced recurrent cystitis. Urological evaluation ruled the cause to be neurological and subsequent imaging revealed a thoracic AVM. Four years later, the patie

A novel robot-assisted method for implanting intracortical sensorimotor devices for brain-computer interface studies: principles, surgical techniques, and challenges.

Precise anatomical implantation of a microelectrode array is fundamental for successful brain-computer interface (BCI) surgery, ensuring high-quality, robust signal communication between the brain and the computer interface. Robotic neurosurgery can contribute to this goal, but its application in BCI surgery has been underexplored. Here, the authors present a novel robot-assisted surgical technique to implant rigid intracortical microelectrode arrays for the BCI. Using this technique, the authors performed surgery in a 31-year-old male with tetraplegia due to a traumatic C4 spinal cord injury

Ectopic intrauterine device in the bladder causing cystolithiasis: A case report.

BACKGROUND: An intrauterine device (IUD) is a commonly used contraceptive among women in China. It is widely used because it is safe, effective, simple, economic, and reversible. Among the possible complications, an ectopic IUD in the bladder is rare. It occurs insidiously, has a long course, is associated with a high risk for injury, and is difficult to treat. CASE SUMMARY: A 44-year-old woman was admitted for repeated episodes of urinary frequency, urgency, and dysuria over three months. Laboratory tests revealed significantly elevated urine leukocytes and bacteria. Urine culture suggested

From quail to earthquakes and human conflict: a historical perspective of rhabdomyolysis.

Rhabdomyolysis is a common cause of acute kidney injury, featuring muscle pain, weakness and dark urine and concurrent laboratory evidence of elevated muscle enzymes and myoglobinuria. Rhabdomyolysis is often seen in elderly and frail patients following prolonged immobilization, for example after a fall, but a variety of other causes are also well-described. What is unknown to most physicians dealing with such patients is the fascinating history of rhabdomyolysis. Cases of probable rhabdomyolysis have been reported since biblical times and during antiquity, often in the context of poisoning. E

Complement dysregulation in the central nervous system during development and disease.

The complement cascade is an important arm of the immune system that plays a key role in protecting the central nervous system (CNS) from infection. Recently, it has also become clear that complement proteins have fundamental roles in the developing and aging CNS that are distinct from their roles in immunity. During neurodevelopment, complement signalling is involved in diverse processes including neural tube closure, neural progenitor proliferation and differentiation, neuronal migration, and synaptic pruning. In acute neurotrauma and ischamic brain injury, complement drives inflammation and

Electrophysiological evidence for pre-attention information processing improvement in patients with central hemiplegic after peripheral nerve rewiring: a pilot study.

Central neurologic injury (CNI) causes dysfunctions not only in limbs but also in cognitive ability. We applied a novel peripheral nerve rewiring (PNR) surgical procedure to restore limb function. Here, we conducted a prospective study to develop estimates for the extent of preattentive processes to cognitive function changes in CNI patients after PNR. Auditory mismatch negativity (MMN) was measured in CNI patients who received the PNR surgery plus conventional rehabilitation treatment. During the 2-year follow-up, the MMN was enhanced with increased amplitude in the PNR plus rehabilitation gr
