One-stage posterior cruciate ligament inlay reconstruction combining anterior cruciate ligament reconstruction following knee dislocation.

Knee dislocation is a rare but severe injury that involves damage to the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), lateral or medial ligamentous structures, and other soft tissues or bony structures surrounding the knee joint. No consensus exists regarding the best treatment method. This work presents a method of 1-stage ACL and PCL reconstruction in which a contralateral quadriceps tendon-bone autograft with tibial inlay technique is used for the PCL and contralateral hamstring tendon autograft with suspension fixation is used for the ACL. After harvesting grafts, t

Various applications of titanium mesh screen implant to orbital wall fractures.

As the number of high velocity injuries increases, orbital wall fractures that involve other facial bone fractures, especially those showing multiple crushed fractures have become increasingly common. However, owing to its complex anatomic structure, our inability to visualize details and relatively thin orbital wall, corrective restorations and fixations are very difficult. Recently several reports have claimed good results using titanium implants to repair orbital fracture. Over a period of 36 months, Titanium mesh screen 1.0 (SYNTHES) were applied to the repair of orbital fracture in variou

Ipsilateral hip and distal femoral fractures.

We tried to find the trauma mechanism and treatment rationale of ipsilateral concomitant hip and distal femoral fractures involving the articular surface. Between 1988 and 1995, 15 cases of ipsilateral hip (confined to neck or trochanteric areas of the femur) and distal (confined to supra- and intercondylar area of the femur) femoral articular fractures were collected. The hip fractures consisted of 10 trochanteric fractures and five neck fractures, which were managed with reduction and fixation in 14 (Knowles' pin in eight, DHS in four and standard Gamma nail in two), and primary bipolar hemi

Posterior fixation of thoracic and lumbar spine fractures using DC plates and pedicle screws.

A prospective investigation of internal fixation of acute thoracic and lumbar spine fractures using pedicle screws and dynamic compression plates was performed. This article details the results of 23 patients who were followed for an average of 20 months. The indications for the procedure were an unstable fracture--dislocation below the eighth thoracic vertebra or low lumbar fractures that would be difficult to treat with conventional spinal implants. The patients were divided into three groups based on the fracture pattern and the level of the fracture. Group I consisted of fracture--dislocat

[Intra-articular fracture of the calcaneus. Classification, assessment and surgical procedures].

Our experience with open reduction of calcaneus fractures and internal stabilization without joint transfixation (n = 83) has shown us that the following features are mandatory in order to obtain normal postoperative foot statics and dynamics, normal joint mobility, and normal foot function: (1) restoration of the normal height, length and width of the calcaneus; (2) reduction of all joint surfaces and (3) stable screw- or H-plate osteosynthesis, allowing functional treatment later. Our own fracture classification (X-fragment/Y-joint fracture) allowed us to determine the type of fracture, its

Comparative histomorphometric analysis of maxillary sinus augmentation with and without bone grafting in rabbit.

PURPOSE: The aim of this animal study was to evaluate, by histomorphometric analysis, new bone formation in rabbit maxillary sinuses with, and without, bone grafting. MATERIALS AND METHODS: Bilateral sinus augmentation procedures were performed in 20 adult male rabbits. Bony windows were made by a saw insert connected with a piezoelectric surgical device in both groups. In the experimental group, the replaceable bony window was repositioned after careful elevation of the sinus mucosa without bone grafting. In the control group, anorganic bovine graft (Bio-Oss; Geistlich AG, Wolhusen, Switzerl

Surgical decompression in spinal Paget's disease: illustrative case.

BACKGROUND: Paget's disease of bone (PDB) is a common bone metabolic pathology in older adults, characterized by mixed osteolytic, osteoblastic, and quiescent periods. Surgical guidelines for PDB involving the spine are not well-defined and are reserved for cases refractory to medical treatments, typically bisphosphonates like zoledronic acid. This case study describes a 52-year-old male with PDB who presented with rapidly progressing myelopathy symptoms refractory to standard medical treatment, warranting surgical decompression. OBSERVATIONS: Surgical decompression and fusion, involving lami

Evaluation of Patient-Specific Cranial Implant Design Using Finite Element Analysis.

BACKGROUND: Various studies have investigated the load-bearing capacity of patient-specific cranial implants. However, little attention has been given to the evaluation of the design of ceramic-titanium (CeTi) implants. METHODS: A biomechanical evaluation of 3 patient-specific cranial implants was performed using finite element analysis. RESULTS: The results of the analyses allowed the identification of the implant regions as well as the magnitudes of the maximum stresses on, and displacements along, these regions after traumatic impact. The analyses also showed that polyether ether ketone c

Intraoperative Fractures of the Tibia and Femur in Knee Revision Surgery.

Intra-operative fractures in knee revision surgery are relatively rare and have not been well studied. They may occur during joint exposition, removal of the prosthesis or cement, or implantation of trial or original components. The fractures affect both the metaphyseal area and diaphysis of the tibia and femur. Tibial fractures are slightly more common than femur fractures. On the femur, the medial condyle is most frequently affected, followed by the femur diaphysis. The use of non-cemented stems is associated with a greater risk of intra-operative diaphyseal fractures than that of cemented s

Femur lengthening with monoplanar external fixator associated with locked intramedullary nail.

OBJECTIVE: This study aimed to demonstrate that the lengthening technique of an external fixator associated with locked intramedullary nail is an efficient method that decreases the duration of the external fixation and improves the rehabilitation period. METHODS: From January of 2005 to May of 2014, 31 patients with mean lower limb discrepancy of 5.31 cm were treated. The etiologies of the deformity were femur fracture sequelae, infection, hip development dysplasia, polio, and congenital short femur. RESULTS: The mean duration of external fixation was 2.47 months (external fixation index of

The shaft fractures of the radius and ulna in children: current concepts.

The incidence of forearm shaft fractures in children has increased in recent years. They are challenging to treat and they can result in several long-lasting complications. The treatment of children's fractures needs to be individualized to their needs. Nonoperative care will be satisfactory for young, preschool children and it is primarily treatment in stable fractures of children at every age. Injury mechanism must be understood to perform appropriate closed reduction. Immobilization using a long-arm cast needs to be focused against the deforming muscle forces - in particular those that rota

Arthrodesis of the Hallux Metatarsophalangeal Joint.

INTRODUCTION: Arthrodesis of the first metatarsophalangeal joint is the most reliable surgical option, with a low complication rate, for hallux rigidus from end-stage osteoarthritis. STEP 1 SURGICAL APPROACH: Make a medial approach, following the mid-axis of the joint. STEP 2 JOINT PREPARATION: Using a cup-cone configuration provides excellent bone exposure, construct stability, and metatarsophalangeal joint congruity. STEP 3 POSITIONING OF ARTHRODESIS: Fix the toe in 5° to 10° of valgus and elevated 5 mm from the floor to achieve desired dorsiflexion. STEP 4 APPLICATION OF IMPLANTS: Achie

Successful treatment of concomitant ipsilateral intracapsular and extracapsular hip fractures.

Femoral neck fractures and intertrochanteric fractures often occur in elderly patients, but simultaneous ipsilateral intra- and extracapsular hip fractures are rare. Either osteosynthesis or femoral head prosthesis is performed, but careful rehabilitation is necessary because of the instability of the fracture, even postoperatively. This article describes a 76-year-old man who fell and sustained concomitant ipsilateral intra- and extracapsular hip fractures. The patient was treated with a femoral head prosthesis with a polished cemented stem combined with locking plate osteosynthesis. Weight-b

Percutaneous internal fixation of selected scaphoid nonunions with an arthroscopically assisted dorsal approach.

BACKGROUND: Preliminary reports have indicated that selected scaphoid nonunions-i.e., those that are well aligned and without extensive sclerosis or bone resorption at the nonunion site-can be treated effectively with internal fixation alone. We examined the feasibility of percutaneous fixation in a series of such nonunions. METHODS: A consecutive series of fifteen patients with fibrous union or nonunion of a carpal scaphoid fracture with minimal sclerosis or resorption at the nonunion site were treated with rigid fixation alone (without bone graft) with a headless compression screw inserted

Hybrid fixation modular tibial prosthesis. Early clinical and radiographic results and retrieval analysis.

A prosthetic tibial component has been designed with features for fixation to bone using a combination of acrylic cement and ingrowth interfaces. This hybrid concept affords the component the immediate stability of cement fixation and the potential long-term stability of biologic fixation. The ingrowth interfaces (coupled with the central stem) are intended to shield the cement interface beneath the tibial tray from the tensile liftoff forces that result from eccentric loading, while avoiding the fretting and osteolysis associated with screw fixation. A disassembly capability allows the tray t

Guide-wire and K-wire readjustment jig for osteotomy and osteosynthesis: An instrument.

INTRODUCTION: Numerous orthopaedic procedures including dynamic hip screw plating and various osteotomies require placement of a reference guide pin or K wire to direct bone cuts or for drilling screw holes. Appropriate positioning of these wires is a critical component of surgery. Irrespective of whether one is a seasoned surgeon or an apprentice, these wires often need repositioning and readjustment. Parallel wire positioning is needed even during procedures like osteotomies or patellar fracture fixation. Though there are several tools available to help with accurate placement of wires and s

Mechanical strength of arthroscopic rotator cuff repair techniques: an in vitro study.

BACKGROUND: Retears after rotator cuff repairs occur relatively frequently and may compromise the functional result. The goal of this study was to analyze the mechanical properties following arthroscopic techniques for rotator cuff repair and to evaluate possible alternative techniques. METHODS: In the first part, five different bone anchors (the Revo screw; Mitek Rotator Cuff anchor, 5.0-mm Statak, PANALOK RC absorbable anchor, and 5.0-mm Bio-Statak) were tested in vitro under cyclic loading on five pairs of cadaveric shoulders. Then five types of arthroscopic tendon suturing instruments wer
