This house believes that: MARI/TAD is better than sentinel node biopsy after PST for cN+ patients.

The increasing use and effectiveness of primary systemic treatment (PST) enables tailored locoregional treatment. About one third of clinically node positive (cN+) breast cancer patients achieve pathologic complete response (pCR) of the axilla, with higher rates observed in Human Epidermal growth factor Receptor (HER)2-positive or triple negative (TN) breast cancer subtypes. Tailoring axillary treatment for patients with axillary pCR is necessary, as they are unlikely to benefit from axillary lymph node dissection (ALND), but may suffer complications and long-term morbidity such as lymphedema

Clinical and diagnosis characteristics of breast cancers in women with a history of radiotherapy in the first 30years of life: A French multicentre cohort study.

PURPOSE: Irradiation (>3Gy) to the breast or axillae before 30years of age increases the risk of secondary breast cancer (SBC). The purpose of this article is to describe the clinical characteristics of SBC and the way of diagnosis in young women (before the age of national screening) in France who had received previous radiotherapy for a childhood or a young adulthood cancer. PATIENTS AND METHODS: This retrospective, multicentre study reviewed the medical records of women with SBC before the age of the national screening who had received irradiation (≥3Gy) on part or all of the breast before

Can we consider always an esophageal carcinoma as radiation associated cancer after irradiation for breast cancer?

OBJECTIVE: To assess in a large-scale single-institution study whether radiotherapy (RT) for breast cancer increases the incidence of esophageal carcinoma as reported in a meta-analysis and in population studies, or does not increase incidence as suggested by smaller single-institution studies that take account of RT technique, dose, and possible confounding factors such as disease stage, smoking history, and the use of chemotherapy or hormone therapy. PATIENTS AND METHODS: We reviewed the cases of esophageal cancer after RT in our prospective institutional database of 16,705 patients treated

[Transpupillary thermotherapy for choroidal metastases].

PURPOSE: To present three cases with choroidal metastases treated with transpupillary thermotherapy (TTT). PATIENTS AND METHODS: Five choroidal metastases of three patients were treated with infrared laser thermotherapy (TTT). In all cases the distorted vision, caused by serous retinal detachment joining the posterior pole metastasis, led the patients to an ophthalmologist. In our first case an intraductal breast carcinoma led to bilateral multifocal choroidal metastases. In the second case a squamous cell carcinoma of the hypopharynx caused a rapidly growing choroidal metastasis. The primary

[Chromosome aberrations in patients with papillary thyroid cancer and other neoplasms].

Cancer is essentially a genetic disease resulting from congenital or acquired alterations in some cells of the patient. Such changes may occur in particular oncogens and are responsible for the tumour phenotype of the affected population of cells. In contrast, unaltered tumour-suppressor genes are responsible for suppressing the neoplastic phenotype, and their inactivation by deletion or mutation permits cancerous development in the affected cells. The genetic model of carcinogenesis is based on the idea mutations at the DNA level, what creates a functional imbalance between the oncogenes and

Brain-only metastatic breast cancer as a distinct clinical entity: evidence from national and institutional cohorts with implications for radiation therapy.

PURPOSE: Brain-only metastatic breast cancer (BO-MBC), defined as breast cancer with brain metastases (BrM) in the absence of extracranial metastatic disease (ECM), may represent a distinct metastatic phenotype. We characterized clinicopathologic features, treatment patterns, and outcomes of BO-MBC and evaluated associations between metastatic pattern, treatment, and survival. METHODS: Patients with isolated BrM at our institution were evaluated for intracranial progression-free survival (iPFS), extracranial progression-free survival (ePFS), leptomeningeal disease-free survival (LMD-PFS), and

Next-generation drug conjugates for breast cancer: Clinical prospects and mechanism-based strategies to overcome resistance.

Antibody-drug conjugates (ADCs) have revolutionized breast cancer (BC) therapy, yet their efficacy is limited by resistance. With advances in ADC technology, broader paradigms of next-generation drug conjugates have emerged. For BC treatment, emerging drug conjugates under current clinical trials include degrader-antibody conjugates, antibody-oligonucleotide conjugates, immune-stimulating antibody conjugates, small molecule-drug conjugates, and peptide-drug conjugates. These agents exhibit significant potential in the treatment of BC and in bypassing ADC common resistance pathways through nove

Prognostic Paradox of Breast Invasive Micropapillary Carcinoma.

Invasive micropapillary carcinoma of the breast is a rare subtype of breast cancer with a unique "inside out" polarity reversal pattern. Despite exhibiting extremely high rates of lymphatic vessel invasion (LVI) and regional lymph node metastasis, large clinical cohorts often observe long-term survival outcomes that are not inferior or even superior to those of ordinary non-specific invasive cancer (IBC-NST), leading to a long-standing controversy over the "clinical prognosis paradox." This article comprehensively summarizes the multidimensional factors that have led to this controversy, point

Proteomic signatures in triple-negative breast cancer.

An aggressive and heterogeneous malignancy, referred to as triple-negative breast cancer, is characterised by the absence of estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2. Treatment options remain limited, relying primarily on chemotherapy due to the lack of well-defined therapeutic targets, which is linked with poor prognosis and high recurrence rates. Proteomics and other high-throughput technologies have significantly advanced TNBC research by enabling the identification of protein-based biomarkers with potential applications in diagnosis, prognosis,

Molecular and Clinicopathological Biomarkers Predicting Brain Metastasis in Triple-Negative Breast Cancer: A Systematic Review.

Almost half of patients with triple-negative breast cancer (TNBC) develop brain metastasis (TNBCBM), a marker of poor prognosis. TNBC is a more aggressive breast cancer subtype which lacks ER, PR, and HER2 expression, and thus, exploring predictive biomarkers is crucial to improving TNBCBM outcomes through targeted therapy. To curate these biomarkers, peer-reviewed publications from 2010 to 2025 were extracted from PubMed, Scopus, Embase, Cochrane, and Web of Science if they evaluated clinicopathological biomarkers of TNBCBM. A total of 130 studies (60 clinical and 70 pre-clinical) were includ

Rethinking local therapy in de novo metastatic breast cancer: the emerging role of radiotherapy.

De novo metastatic breast cancer (dnMBC) accounts for 3-10% of breast cancer diagnoses and shows distinct biology and prognosis from recurrent metastatic disease. Systemic therapies remain the cornerstone of treatment; however, the role of locoregional treatment (LRT), including surgery and radiotherapy, is controversial. Primary tumor resection may modulate systemic immunity, reduce metastatic seeding, and improve systemic therapy efficacy, but survival benefits remain inconsistent and patient selection is critical. Radiotherapy, alone or combined with surgery, improves local control and may

Neuroglial-Breast Cancer Crosstalk Shapes the Brain Metastatic Niche.

Breast cancer brain metastasis (BCBM) affects up to 30% of patients with metastatic disease and carries a median survival of only 4-18 months. Emerging evidence reveals that BCBM cells are not passive survivors, but active participants that hijack core neurotransmitter networks, GABA (gamma-aminobutyric acid) and glutamate, to fuel their growth. BCBM, particularly triple-negative breast cancer (TNBC), frequently switch to a GABAergic mode utilizing brain-derived GABA as an oncometabolite. In parallel, BCBM cells can also form direct synapses with neurons, tapping into excitatory input through

Necroptosis triggers inflammatory interferon signatures in patient-derived metastatic breast cancer organoids.

Breast cancer (BC) is the most common type of cancer among women worldwide and underlies relapse, disease progression, and metastasis. Resistance to chemotherapy and programmed cell death (PCD), including apoptosis, strongly affects BC therapy success and remains a major challenge. Although necroptosis, a lytic, and via damage-associated molecular patterns (DAMP) release, highly immunogenic mode of PCD, might overcome apoptosis resistance, there is an urgent need for physiological and translational human models to model necroptosis and PCD resistance in BC. Here, we apply 3D patient-derived, m

End-to-end multimodal pathology foundation model with clinical dialogue.

Recent rapid progress in the field of computational pathology has been enabled by foundation models. These models are beginning to move beyond encoding image patches toward whole-slide understanding, but their clinical utility remains limited. Here we present PRISM2, a multimodal slide-level foundation model trained on 2.3 million whole-slide images and 14 million question-answer pairs derived from 700,000 pathology reports. Through clinical dialogue supervision, PRISM2 aligns histomorphology with diagnostic reasoning, yielding representations that support both prompt-based inference and trans

Does microtubule motor-mediated centrosomal dyshomeostasis confer chemoresistance in breast cancer?

Chemoresistance is a major challenge in effective chemotherapy for breast cancers. Unfortunately, the precise molecular mechanisms that confer resistance remain elusive so far. A remarkable feature in a multitude of breast cancers is the presence of clustered supernumerary centrosomes, which is a dysregulated condition that arises primarily through centrosome over-duplication. Normally, microtubule motor proteins (MiMos) maintain the integrity of centrosomes via regulating cohesion, separation and positioning of centrosomes. In the recent years, several MiMos have been reported to be different

Resistance to neoadjuvant chemotherapy in breast cancers: a metabolic perspective.

Neoadjuvant chemotherapy (NAC) is a cornerstone in the treatment of early-stage high-risk breast cancers (BC), particularly in triple-negative, HER2-positive, and selected hormone receptor-positive subtypes. However, its effectiveness is frequently hindered by intrinsic or acquired resistance, resulting in a significant residual cancer burden (RCB) in more than half of patients. Despite extensive genomic profiling, reliable predictive biomarkers for treatment response remain limited, impeding the development of personalized therapeutic strategies. Emerging evidence highlights tumor metabolic r

Feasibility and User Experience of Digital Patient Monitoring for Real-World Patients With Lung or Breast Cancer.

BACKGROUND: Digital patient monitoring (DPM) tools can facilitate early symptom management for patients with cancer through systematic symptom reporting; however, low adherence can be a challenge. We assessed patient/healthcare professional (HCP) use of DPM in routine clinical practice. MATERIALS AND METHODS: Patients with locally advanced/metastatic lung cancer or HER2-positive breast cancer received locally approved/reimbursed drugs alongside DPM, with elements tailored by F. Hoffmann-La Roche Ltd, on the Kaiku Health DPM platform. Patient access to the DPM tool was through their own device

A rare form of metastatic breast carcinoma mimicking lymphangioma circumscriptum.

Cutaneous metastasis is rare but may indicate an advanced internal malignancy or a recurrence of a previously treated one and is usually associated with a poor prognosis. They may also pose a diagnostic problem as the clinical manifestations are variable and non-specific, which could mimic other benign conditions. We report a case of a 48-year-old female who presented with a 4-year history of erythematous papules and vesicles on the trunk mimicking lymphangioma circumscriptum. Skin biopsy and immunohistochemistry were consistent with cutaneous metastasis from breast carcinoma. Cutaneous metast
